Background Increased backfat thickness of sows in early gestation is negative to reproductive performance.Endometrial receptivity is an important determinant of reproductive success,but it is unclear whether the effec...Background Increased backfat thickness of sows in early gestation is negative to reproductive performance.Endometrial receptivity is an important determinant of reproductive success,but it is unclear whether the effect of sow backfat thickness on litter size is associated with endometrial receptivity and whether melatonin treatment may have benefits.The present study seeks to answer these questions through in vitro and in vivo investigations.Results Excessive lipid deposition and lower melatonin levels in the uterus are detrimental to endometrial receptivity and embryo implantation in high backfat thickness sows.In cells treated with melatonin,the MT2/PI3K/LIF axis played a role in reducing lipid accumulation in porcine endometrial epithelium cells and improved endometrial receptivity.Furthermore,we found a reduction of lipids in the uterus after eight weeks of intraperitoneal administration of melatonin to HFD mice.Notably,melatonin treatment caused a significant reduction in the deposition of endometrial collagen,an increase in the number of glands,and repair of the pinopode structure,ultimately improving endometrial receptivity,promoting embryo implantation,and increasing the number of litter size of mice.Conclusions Collectively,the finding reveals the harmful effects of high backfat thickness sows on embryo implantation and highlight the role of melatonin and the MT2/PI3K/LIF axis in improving endometrial receptivity by enhancing metabolism and reducing the levels of uterine lipids in obese animals.展开更多
Background Exogenous gonadotropin-controlled ovarian stimulation is the critical step in animal reproductive management,such as pig,sheep,bovine and other species.It helps synchronize ovulation or stimulate multiple o...Background Exogenous gonadotropin-controlled ovarian stimulation is the critical step in animal reproductive management,such as pig,sheep,bovine and other species.It helps synchronize ovulation or stimulate multiple ovu-lations.However,a number of evidence indicated an unexpected decrease in pregnancy outcomes following ovarian stimulation.This study aimed to explore the underlying mechanism of the pregnancy defect and develop a practical rescue strategy.Results Compared with those in the control group,gilts that underwent ovarian stimulation showed a decrease in pregnancy rate,farrowing rate,and total number of piglets born.Stimulated gilts also showed an increase in estra-diol(E_(2))levels.The supraphysiologicalE_(2) level was correlated with the decrease in the number of piglets born.Furthermore,we found that high levels ofE_(2) impair uterine receptivity,as shown by the overproliferation of endo-metrial epithelial cells.In vitro mechanistic studies demonstrated that high levels ofE_(2) hyperactivate FGF-FGFR-ERK signaling cascade in the uterine endometrium,and in turn induces overproliferation of endometrial epithelial cells.Of note,N-acetyl-L-cysteine(NAC)supplementation effectively inhibits ERK hyperphosphorylation and ameliorates endometrial epithelial overproliferation.Importantly,in vivo experiments indicated that dietary NAC supplementa-tion,compared with ovarian stimulation group,improves the uterine receptivity in gilts,and significantly increases the pregnancy rate and total number of piglets born.Conclusions Ovarian stimulation-induced supraphysiological levels ofE_(2) impairs uterine receptivity by hyperactivat-ing FGF-FGFR-ERK signaling cascade,thereby reducing pregnancy rate and litter size.Supplementing NAC to a con-ventional diet for gilts ameliorates hyperactivated ERK signaling and improves uterine receptivity,thus rescuing adverse pregnancy outcomes following ovarian stimulation.展开更多

Objective:This study aimed to explore the clinical effect of acupuncture therapy for menstruation regu-lation and pregnancy promotion on thin endometrium in the real world.Design:This study is a single-center pragmati...Objective:This study aimed to explore the clinical effect of acupuncture therapy for menstruation regu-lation and pregnancy promotion on thin endometrium in the real world.Design:This study is a single-center pragmatic randomized controlled trial blinded to the statisticians.Using the“blockrand”software package,based on the age(≥35,<35),37 patients were randomized into an intervention group(19 cases)and a control group(18 cases).After reassignment regarding patient preference,21 patients were included in the intervention group and 16 in the control group.Setting:The trial was executed in the Specialty Outpatient Clinic,Acupuncture-Moxibustion Hospital of China Academy of Chinese Medical Sciences,from March 1,2019,to September 30,2020.Participants:The study included 37 patients with thin endometrium and without previous acupuncture treatment.Intervention:The intervention group was administered acupuncture for menstruation regulation and pregnancy promotion and a small-dose of progynova(2 mg daily),while the control group was adminis-tered a large-dose of progynova(4 mg daily).Interventions started from Day 5 of menstruation until the end of ovulation under B-ultrasound monitoring.The intervention lasted for three menstrual cycles.Measurements:Primary outcomes were changes in endometrial thickness between baseline and after intervention completion and the difference between the two groups after intervention.The secondary outcomes were endometrial and subendometrial blood flow,serum estradiol levels,menstrual conditions,and adverse reactions.Results:(1)Comparison of each indicator before and after intervention completion in the two groups:in the intervention group,the differences were significant in endometrial thickness,menstrual score,estra-diol(E2)level in ovulatory period,the pulsatility index(PI)and resistance index(RI)of uterine artery,the ratio of peak systolic velocity to end-diastolic velocity(S/D),the endometrial vascular index(VI),flow index(FI),and vascular flow index(VFI)and volume(P<0.01).In the control group,significant changes were observed in endometrial thickness,menstrual score,and E2 before and after the interven-tion(P<0.05),and no differences were observed in uterine artery PI,RI,S/D and endometrial VI,FI,VFI,and volume(P>0.05).Compared with the control group,the intervention group showed significant differences in endometrial thickness,menstrual score,E2,uterine artery PI,RI,S/D,and endometrial VI,FI,VFI,and volume after intervention(P<0.01).No adverse reactions were reported in the intervention group.In contrast,the control group had two cases of nausea and gastrointestinal discomfort after med-ication,eight cases of breast distention during medication,and one case of breast nodules enlarged by 1 cm after trial completion.展开更多

OBJECTIVE:To evaluate the effectiveness and safety of the sequential therapy in treating infertility with polycystic ovary syndrome(PCOS)and luteal phase defects(LPD)by Yangxin Dianji decoction(养心奠基方,YXDJ-D)and N...OBJECTIVE:To evaluate the effectiveness and safety of the sequential therapy in treating infertility with polycystic ovary syndrome(PCOS)and luteal phase defects(LPD)by Yangxin Dianji decoction(养心奠基方,YXDJ-D)and Nuangong Tiaojing decoction(暖宫调经方,NGTJ-D).METHODS:This study was undertaken in the Jiangsu Province Hospital of Chinese Medicine.Altogether 90 eligible patients with PCOS and LPD were assigned to exposed group A(Chinese Medicine therapy,YXDJ-D and NGTJ-D),exposed group B(Chinese Medicine plus Western Medicine therapy),control group(Western Medicine therapy).The exposed group A adopted the sequential therapy that YXDJ-D is taken in the postmenstrual period(follicular phase)and NGTJ-D is taken in premenstrual period(luteal phase).Control group took letrozole,dydrogesterone and was given intramuscular injection of human menopausal gonadotropin,human chorionic gonadotropin.The exposed group B was treated with the above-mentioned therapy project of integrated Chinese Medicine and Western Medicine.This study lasted for 2 courses for 6 months.The primary outcomes were pregnancy rate and early abortion rate.The secondary outcomes were the Traditional Chinese Medicine(TCM)syndrome scores,estrogen(E2)and progesterone(P),endometrial volume(EV),vascularity index(VI),flow index(FI)and vascularization flow index(VFI).These outcomes will be assessed at baseline and post-intervention.RESULTS:The pregnancy rates of the exposed group A and B were higher than the control group(60.00%vs 60.00%vs 53.33%),while early abortion rates of exposed groups A and B were lower than the control group(33.33%vs 16.67%vs 43.75%,P>0.05).Total efficacy rates in exposed group A and B were better than the control group(93.30%vs 93.30%vs 53.30%,P<0.01).TCM symptom scores and endometrial receptivity indexes(EV,FI,VFI)were significantly lower in exposed groups compared to the control group(P<0.05).P increase in exposed group B was superior to the other two groups(P<0.01).No noticeable abnormalities in safety indicators in the three groups.CONCLUSION:The sequential therapy of YXDJ-D and NGTJ-D can effectively increase pregnancy rate,reduce the early abortion rate and alleviate the clinical symptoms of infertility in patients with PCOS and LPD by improving luteal function and promoting the endometrial receptivity.展开更多

BACKGROUND Despite advances in the use of in vitro fertilization-embryo transfer(ET)in couples with infertility,recurrent implantation failure(RIF)after ET is still a major problem affecting women,with great psycholog...BACKGROUND Despite advances in the use of in vitro fertilization-embryo transfer(ET)in couples with infertility,recurrent implantation failure(RIF)after ET is still a major problem affecting women,with great psychological and economic burdens for the patient and his/her family.AIM To investigate the psychological burden(anxiety and depression)and evaluate the clinical benefits of endometrial receptivity testing(ERT)in patients experiencing RIF following frozen-thawed ET.METHODS A retrospective cohort study analyzed 371 patients experiencing RIF after assisted reproductive treatment at the First People’s Hospital of Changde City between January 2021 and June 2024.Demographic and clinical data were systematically collected through standardized questionnaires.Psychological assessment utilized validated instruments:The Self-Rating Depression Scale for depression evaluation and the Self-Rating Anxiety Scale for anxiety assessment.Participants were stratified by psychological status(anxiety/non-anxiety and depression/nondepression)to analyze influencing factors for anxiety and depression.The cohort was further categorized into the ERT and non-ERT groups based on ERT implementation to comparatively analyze their clinical outcomes.Additionally,they were divided into clinical and nonclinical pregnancy groups to identify factors affecting clinical pregnancy using univariate and multivariate logistic regression models.Compared with the non-ERT group,the 226 patients who underwent ERT-guided ET achieved a higher clinical pregnancy rate,thicker endometrium on transfer day,fewer embryos transferred,and a lower miscarriage rate.RESULTS The study identified a substantial psychological burden,with anxiety prevalence at 55.0%(mean Self-Rating Anxiety Scale score,50.89±9.34)and depression at 61.2%(mean Self-Rating Depression Scale score,55.55±9.48).Multivariate analysis identified annual household income>100000 yuan as protective factors against both anxiety and depression,whereas advanced maternal age(>35 years)and multiple implantation failures(≥3)served as risk factors.Additionally,anxiety-specific risk factors included prolonged infertility treatment(>5 years)and the spouse’s status as an only child.As to depression-specific risks,chronic infertility(>3 years)and higher educational attainment(college/bachelor’s degree or higher)were key determinants,whereas urban residence was a protective factor.Age>35 years was a risk factor for clinical pregnancy in patients experiencing RIF,whereas blastocyst-stage ET,a higher number of embryos transferred,and thicker endometrium were protective factors.CONCLUSION Patients experiencing RIF are particularly susceptible to anxiety and depression,and advanced maternal age and multiple implantation failures represent salient risk factors.Clinicians should implement proactive and evidencebased interventions to mitigate these psychological burdens.For patients experiencing RIF,ERT-guided ET demonstrates significant potential to improve assisted reproductive outcomes.展开更多
With the continuous development of artificial intelligence and machine learning techniques,there have been effective methods supporting the work of dermatologist in the field of skin cancer detection.However,object si...With the continuous development of artificial intelligence and machine learning techniques,there have been effective methods supporting the work of dermatologist in the field of skin cancer detection.However,object significant challenges have been presented in accurately segmenting melanomas in dermoscopic images due to the objects that could interfere human observations,such as bubbles and scales.To address these challenges,we propose a dual U-Net network framework for skin melanoma segmentation.In our proposed architecture,we introduce several innovative components that aim to enhance the performance and capabilities of the traditional U-Net.First,we establish a novel framework that links two simplified U-Nets,enabling more comprehensive information exchange and feature integration throughout the network.Second,after cascading the second U-Net,we introduce a skip connection between the decoder and encoder networks,and incorporate a modified receptive field block(MRFB),which is designed to capture multi-scale spatial information.Third,to further enhance the feature representation capabilities,we add a multi-path convolution block attention module(MCBAM)to the first two layers of the first U-Net encoding,and integrate a new squeeze-and-excitation(SE)mechanism with residual connections in the second U-Net.To illustrate the performance of our proposed model,we conducted comprehensive experiments on widely recognized skin datasets.On the ISIC-2017 dataset,the IoU value of our proposed model increased from 0.6406 to 0.6819 and the Dice coefficient increased from 0.7625 to 0.8023.On the ISIC-2018 dataset,the IoU value of proposed model also improved from 0.7138 to 0.7709,while the Dice coefficient increased from 0.8285 to 0.8665.Furthermore,the generalization experiments conducted on the jaw cyst dataset from Quzhou People’s Hospital further verified the outstanding segmentation performance of the proposed model.These findings collectively affirm the potential of our approach as a valuable tool in supporting clinical decision-making in the field of skin cancer detection,as well as advancing research in medical image analysis.展开更多

Drone-based small object detection is of great significance in practical applications such as military actions, disaster rescue, transportation, etc. However, the severe scale differences in objects captured by drones...Drone-based small object detection is of great significance in practical applications such as military actions, disaster rescue, transportation, etc. However, the severe scale differences in objects captured by drones and lack of detail information for small-scale objects make drone-based small object detection a formidable challenge. To address these issues, we first develop a mathematical model to explore how changing receptive fields impacts the polynomial fitting results. Subsequently, based on the obtained conclusions, we propose a simple but effective Hybrid Receptive Field Network (HRFNet), whose modules include Hybrid Feature Augmentation (HFA), Hybrid Feature Pyramid (HFP) and Dual Scale Head (DSH). Specifically, HFA employs parallel dilated convolution kernels of different sizes to extend shallow features with different receptive fields, committed to improving the multi-scale adaptability of the network;HFP enhances the perception of small objects by capturing contextual information across layers, while DSH reconstructs the original prediction head utilizing a set of high-resolution features and ultrahigh-resolution features. In addition, in order to train HRFNet, the corresponding dual-scale loss function is designed. Finally, comprehensive evaluation results on public benchmarks such as VisDrone-DET and TinyPerson demonstrate the robustness of the proposed method. Most impressively, the proposed HRFNet achieves a mAP of 51.0 on VisDrone-DET with 29.3 M parameters, which outperforms the extant state-of-the-art detectors. HRFNet also performs excellently in complex scenarios captured by drones, achieving the best performance on the CS-Drone dataset we built.展开更多

Methods eGOS rat model ...Objective To observe acupuncture effects on endometrial receptivity of the rats with polycystic ovary syndrome (PCOS) treated by ovarian stimulation with Clomiphene to explore its mechanisms. Methods eGOS rat model was experimentally induced by subcutaneous injection of oil solution of dehydroepiandrosterone (DHEA) in immature (24-day-old) female rats for continuous 20 days. The same dose of oil was given for an equivalent length of time to animals in a control group (Group Con). The rats with PCOS were randomly divided into a model group (group M), a Clomiphene treatment group (group C), a Clomiphene plus acupuncture group (group C+ A). The treatments were given for 5 days since 80 days after birth. Half of the rats were killed by decapitation after treatment finished, and the serum, uteri and endometria were obtained. The other half mated with male rats, and they were killed at the 8th day of pregnancy. We observed the implantation number and condition of pregnant uteri. Results Compared with group C, the animals in group C+A had higher implantation rate and more average implanted embryos (P〈0. 01 ), higher serum estradiol (E2) level (P〈0.01 ), better development of endometrium, and increased mRNA and protein expressions of molecular markers of endometrial receptivity (P〈0.01), which were closer to those in the control group. Conclusion Acupuncture could improve poor endometrial receptivity of PCOS rats with ovarian stimulation by citrated Clomiphene and promote implantation. The effects may be related with its up-regulation of serum E2 level, mRNA and protein expressions of molecular markers of endometrial receptivity.展开更多

[Result] The results showed that three varieties of Prunus domestica×armeniaca were in blossom in early April, the florescence were relatively late for 8-12 d comparing with the varieties of apricot, the florescence were relatively similar to the P. domestica L. Morphology characteristics of floral organs of three varieties of Prunus domestica×armeniaca had few differences, were relatively similar to two varieties of plum, and they all had high percent of middle-style and long-style flower.The pollen amount of three varieties of Prunus domestica ×armeniaca had a little differences comparing with two varieties of plum, but were lower than three varieties of apricot. Stigma receptivity was detected with benzidine-hydrogen peroxide method and in vivo pol en germina-tion method. [Result] The pistil ate flower development process of E. pulcherrima was consisted of columnar stigma phase, stigma lobe slightly opening phase, Y-shape stigma lobe phase, inverse V-shaped stigma lobe phase and stigma curling phase. Pistil ate flower development and stigma receptivity of E. pulcherrima exhibit-ed certain correlation. The stigma receptive period lasted from stigma lobe slightly opening phase to inverse V-shaped stigma lobe phase; inverse V-shaped stigma lobe phase was appropriate for pol ination, lasting for 3-5 d, with V-shaped stigma lobe and a large amount of exudates on stigma surface according to SEM obser-vation. BACKGROUND: The tumor necrosis factor recepter associated factor (TRAF) 6 is an important intracellular adapter protein that plays a pivotal role in activating multiple inflammatory and immune related processes ind...BACKGROUND: The tumor necrosis factor recepter associated factor (TRAF) 6 is an important intracellular adapter protein that plays a pivotal role in activating multiple inflammatory and immune related processes induced by cytokines. TRAF6 represents a strong candidate susceptibility factor for sepsis. We investigated whether polymorphisms at the TRAF6 gene are associated with the susceptibility to and severity of sepsis.METHODS: A hospital-based case-control study was conducted with 255 patients with sepsis and 260 controls who were recruited from Zhengzhou, China. Haplotype tagging single nucleotide polymorphisms (htSNPs) were selected from the HapMap database and genotyped using the SNPstream genotyping platform. The associations with the susceptibility and disease severity of sepsis were estimated by logistic regression, and adjusted for age, sex, smoking, drinking, chronic diseases status, APACHEII score and critical illness status.RESULTS: A total of 13 TRAF6 SNPs were tagged by 7 htSNPs. Five htSNPs (rs5030490, rs5030411, rs5030416, rs5030445 and rs3740961) were genotyped in the case control study. Genotype frequencies of the htSNPs were conformed to the Hardy-Weinberg equilibrium in both patients and controls. No significant association was found between the 5 htSNPs and the susceptibility to and severity of sepsis. Compared with the main haplotype -11120A/-10688T/-9423A/805G/12967G, no certain haplotype was associated with the signi? cantly susceptibility to or severity of sepsis.CONCLUSION: TRAF6 gene polymorphisms might not play a major role in mediating the susceptibility to and severity of sepsis in the Chinese population. A larger population-based case-control study is warranted.展开更多

文摘Background Increased backfat thickness of sows in early gestation is negative to reproductive performance.Endometrial receptivity is an important determinant of reproductive success,but it is unclear whether the effect of sow backfat thickness on litter size is associated with endometrial receptivity and whether melatonin treatment may have benefits.The present study seeks to answer these questions through in vitro and in vivo investigations.Results Excessive lipid deposition and lower melatonin levels in the uterus are detrimental to endometrial receptivity and embryo implantation in high backfat thickness sows.In cells treated with melatonin,the MT2/PI3K/LIF axis played a role in reducing lipid accumulation in porcine endometrial epithelium cells and improved endometrial receptivity.Furthermore,we found a reduction of lipids in the uterus after eight weeks of intraperitoneal administration of melatonin to HFD mice.Notably,melatonin treatment caused a significant reduction in the deposition of endometrial collagen,an increase in the number of glands,and repair of the pinopode structure,ultimately improving endometrial receptivity,promoting embryo implantation,and increasing the number of litter size of mice.Conclusions Collectively,the finding reveals the harmful effects of high backfat thickness sows on embryo implantation and highlight the role of melatonin and the MT2/PI3K/LIF axis in improving endometrial receptivity by enhancing metabolism and reducing the levels of uterine lipids in obese animals.

文摘Background Exogenous gonadotropin-controlled ovarian stimulation is the critical step in animal reproductive management,such as pig,sheep,bovine and other species.It helps synchronize ovulation or stimulate multiple ovu-lations.However,a number of evidence indicated an unexpected decrease in pregnancy outcomes following ovarian stimulation.This study aimed to explore the underlying mechanism of the pregnancy defect and develop a practical rescue strategy.Results Compared with those in the control group,gilts that underwent ovarian stimulation showed a decrease in pregnancy rate,farrowing rate,and total number of piglets born.Stimulated gilts also showed an increase in estra-diol(E_(2))levels.The supraphysiologicalE_(2) level was correlated with the decrease in the number of piglets born.Furthermore,we found that high levels ofE_(2) impair uterine receptivity,as shown by the overproliferation of endo-metrial epithelial cells.In vitro mechanistic studies demonstrated that high levels ofE_(2) hyperactivate FGF-FGFR-ERK signaling cascade in the uterine endometrium,and in turn induces overproliferation of endometrial epithelial cells.Of note,N-acetyl-L-cysteine(NAC)supplementation effectively inhibits ERK hyperphosphorylation and ameliorates endometrial epithelial overproliferation.Importantly,in vivo experiments indicated that dietary NAC supplementa-tion,compared with ovarian stimulation group,improves the uterine receptivity in gilts,and significantly increases the pregnancy rate and total number of piglets born.Conclusions Ovarian stimulation-induced supraphysiological levels ofE_(2) impairs uterine receptivity by hyperactivat-ing FGF-FGFR-ERK signaling cascade,thereby reducing pregnancy rate and litter size.Supplementing NAC to a con-ventional diet for gilts ameliorates hyperactivated ERK signaling and improves uterine receptivity,thus rescuing adverse pregnancy outcomes following ovarian stimulation.

文摘Hypersonic boundary-layer receptivity to freestream entropy and vorticity waves is investigated using direct numerical simula-tions for a Mach 6 flow over a 5.08 mm nose radius cone.Two frequencies of 33 kHz and 150 kHz are considered to be rep-resentative of the first and second instability modes,respectively.For the first mode,wall pressure fluctuations for both entropy and vorticity wave cases exhibit a strong modulation yet without a growing trend,indicating that the first mode is not generated despite its instability predicted by linear stability theory.The potential reason for this is the absence of postshock slow acoustic waves capable of synchronizing with the first mode.By contrast,for the second mode,a typical three-stage boundary-layer response is observed,consistent with that to slow acoustic waves studied previously.Furthermore,the postshock disturbances outside the boundary layer can be decomposed into the entropy(density/temperature fluctuations)and vorticity components(ve-locity fluctuations),and the latter is shown to play a leading role in generating the second mode,even for the case with entropy waves where the density/temperature fluctuations dominate the postshock regions.

文摘Objective:This study aimed to explore the clinical effect of acupuncture therapy for menstruation regu-lation and pregnancy promotion on thin endometrium in the real world.Design:This study is a single-center pragmatic randomized controlled trial blinded to the statisticians.Using the“blockrand”software package,based on the age(≥35,<35),37 patients were randomized into an intervention group(19 cases)and a control group(18 cases).After reassignment regarding patient preference,21 patients were included in the intervention group and 16 in the control group.Setting:The trial was executed in the Specialty Outpatient Clinic,Acupuncture-Moxibustion Hospital of China Academy of Chinese Medical Sciences,from March 1,2019,to September 30,2020.Participants:The study included 37 patients with thin endometrium and without previous acupuncture treatment.Intervention:The intervention group was administered acupuncture for menstruation regulation and pregnancy promotion and a small-dose of progynova(2 mg daily),while the control group was adminis-tered a large-dose of progynova(4 mg daily).Interventions started from Day 5 of menstruation until the end of ovulation under B-ultrasound monitoring.The intervention lasted for three menstrual cycles.Measurements:Primary outcomes were changes in endometrial thickness between baseline and after intervention completion and the difference between the two groups after intervention.The secondary outcomes were endometrial and subendometrial blood flow,serum estradiol levels,menstrual conditions,and adverse reactions.Results:(1)Comparison of each indicator before and after intervention completion in the two groups:in the intervention group,the differences were significant in endometrial thickness,menstrual score,estra-diol(E2)level in ovulatory period,the pulsatility index(PI)and resistance index(RI)of uterine artery,the ratio of peak systolic velocity to end-diastolic velocity(S/D),the endometrial vascular index(VI),flow index(FI),and vascular flow index(VFI)and volume(P<0.01).In the control group,significant changes were observed in endometrial thickness,menstrual score,and E2 before and after the interven-tion(P<0.05),and no differences were observed in uterine artery PI,RI,S/D and endometrial VI,FI,VFI,and volume(P>0.05).Compared with the control group,the intervention group showed significant differences in endometrial thickness,menstrual score,E2,uterine artery PI,RI,S/D,and endometrial VI,FI,VFI,and volume after intervention(P<0.01).No adverse reactions were reported in the intervention group.In contrast,the control group had two cases of nausea and gastrointestinal discomfort after med-ication,eight cases of breast distention during medication,and one case of breast nodules enlarged by 1 cm after trial completion.

文摘OBJECTIVE:To evaluate the effectiveness and safety of the sequential therapy in treating infertility with polycystic ovary syndrome(PCOS)and luteal phase defects(LPD)by Yangxin Dianji decoction(养心奠基方,YXDJ-D)and Nuangong Tiaojing decoction(暖宫调经方,NGTJ-D).METHODS:This study was undertaken in the Jiangsu Province Hospital of Chinese Medicine.Altogether 90 eligible patients with PCOS and LPD were assigned to exposed group A(Chinese Medicine therapy,YXDJ-D and NGTJ-D),exposed group B(Chinese Medicine plus Western Medicine therapy),control group(Western Medicine therapy).The exposed group A adopted the sequential therapy that YXDJ-D is taken in the postmenstrual period(follicular phase)and NGTJ-D is taken in premenstrual period(luteal phase).Control group took letrozole,dydrogesterone and was given intramuscular injection of human menopausal gonadotropin,human chorionic gonadotropin.The exposed group B was treated with the above-mentioned therapy project of integrated Chinese Medicine and Western Medicine.This study lasted for 2 courses for 6 months.The primary outcomes were pregnancy rate and early abortion rate.The secondary outcomes were the Traditional Chinese Medicine(TCM)syndrome scores,estrogen(E2)and progesterone(P),endometrial volume(EV),vascularity index(VI),flow index(FI)and vascularization flow index(VFI).These outcomes will be assessed at baseline and post-intervention.RESULTS:The pregnancy rates of the exposed group A and B were higher than the control group(60.00%vs 60.00%vs 53.33%),while early abortion rates of exposed groups A and B were lower than the control group(33.33%vs 16.67%vs 43.75%,P>0.05).Total efficacy rates in exposed group A and B were better than the control group(93.30%vs 93.30%vs 53.30%,P<0.01).TCM symptom scores and endometrial receptivity indexes(EV,FI,VFI)were significantly lower in exposed groups compared to the control group(P<0.05).P increase in exposed group B was superior to the other two groups(P<0.01).No noticeable abnormalities in safety indicators in the three groups.CONCLUSION:The sequential therapy of YXDJ-D and NGTJ-D can effectively increase pregnancy rate,reduce the early abortion rate and alleviate the clinical symptoms of infertility in patients with PCOS and LPD by improving luteal function and promoting the endometrial receptivity.

文摘Based on BERTopic Model,the paper combines qualitative and quantitative methods to explore the reception of Can Xue’s translated works by analyzing readers’book reviews posted on Goodreads and Lovereading.We first collected book reviews from these two well-known websites by Python.Through topic analysis of these reviews,we identified recurring topics,including details of her translated works and appreciation of their translation quality.Then,employing sentiment and content analysis methods,the paper explored the emotional attitudes and the specific thoughts of readers toward Can Xue and her translated works.The fingdings revealed that,among the 408 reviews,though the reception of Can Xue’s translated works was relatively positive,the current level of attention and recognition remains insufficient.However,based on the research results,the paper can derive valuable insights into the translation and dissemination processes such as adjusting translation and dissemination strategies,so that the global reach of Chinese literature and culture can be better facilitated.
文摘BACKGROUND Despite advances in the use of in vitro fertilization-embryo transfer(ET)in couples with infertility,recurrent implantation failure(RIF)after ET is still a major problem affecting women,with great psychological and economic burdens for the patient and his/her family.AIM To investigate the psychological burden(anxiety and depression)and evaluate the clinical benefits of endometrial receptivity testing(ERT)in patients experiencing RIF following frozen-thawed ET.METHODS A retrospective cohort study analyzed 371 patients experiencing RIF after assisted reproductive treatment at the First People’s Hospital of Changde City between January 2021 and June 2024.Demographic and clinical data were systematically collected through standardized questionnaires.Psychological assessment utilized validated instruments:The Self-Rating Depression Scale for depression evaluation and the Self-Rating Anxiety Scale for anxiety assessment.Participants were stratified by psychological status(anxiety/non-anxiety and depression/nondepression)to analyze influencing factors for anxiety and depression.The cohort was further categorized into the ERT and non-ERT groups based on ERT implementation to comparatively analyze their clinical outcomes.Additionally,they were divided into clinical and nonclinical pregnancy groups to identify factors affecting clinical pregnancy using univariate and multivariate logistic regression models.Compared with the non-ERT group,the 226 patients who underwent ERT-guided ET achieved a higher clinical pregnancy rate,thicker endometrium on transfer day,fewer embryos transferred,and a lower miscarriage rate.RESULTS The study identified a substantial psychological burden,with anxiety prevalence at 55.0%(mean Self-Rating Anxiety Scale score,50.89±9.34)and depression at 61.2%(mean Self-Rating Depression Scale score,55.55±9.48).Multivariate analysis identified annual household income>100000 yuan as protective factors against both anxiety and depression,whereas advanced maternal age(>35 years)and multiple implantation failures(≥3)served as risk factors.Additionally,anxiety-specific risk factors included prolonged infertility treatment(>5 years)and the spouse’s status as an only child.As to depression-specific risks,chronic infertility(>3 years)and higher educational attainment(college/bachelor’s degree or higher)were key determinants,whereas urban residence was a protective factor.Age>35 years was a risk factor for clinical pregnancy in patients experiencing RIF,whereas blastocyst-stage ET,a higher number of embryos transferred,and thicker endometrium were protective factors.CONCLUSION Patients experiencing RIF are particularly susceptible to anxiety and depression,and advanced maternal age and multiple implantation failures represent salient risk factors.Clinicians should implement proactive and evidencebased interventions to mitigate these psychological burdens.For patients experiencing RIF,ERT-guided ET demonstrates significant potential to improve assisted reproductive outcomes.

文摘With the continuous development of artificial intelligence and machine learning techniques,there have been effective methods supporting the work of dermatologist in the field of skin cancer detection.However,object significant challenges have been presented in accurately segmenting melanomas in dermoscopic images due to the objects that could interfere human observations,such as bubbles and scales.To address these challenges,we propose a dual U-Net network framework for skin melanoma segmentation.In our proposed architecture,we introduce several innovative components that aim to enhance the performance and capabilities of the traditional U-Net.First,we establish a novel framework that links two simplified U-Nets,enabling more comprehensive information exchange and feature integration throughout the network.Second,after cascading the second U-Net,we introduce a skip connection between the decoder and encoder networks,and incorporate a modified receptive field block(MRFB),which is designed to capture multi-scale spatial information.Third,to further enhance the feature representation capabilities,we add a multi-path convolution block attention module(MCBAM)to the first two layers of the first U-Net encoding,and integrate a new squeeze-and-excitation(SE)mechanism with residual connections in the second U-Net.To illustrate the performance of our proposed model,we conducted comprehensive experiments on widely recognized skin datasets.On the ISIC-2017 dataset,the IoU value of our proposed model increased from 0.6406 to 0.6819 and the Dice coefficient increased from 0.7625 to 0.8023.On the ISIC-2018 dataset,the IoU value of proposed model also improved from 0.7138 to 0.7709,while the Dice coefficient increased from 0.8285 to 0.8665.Furthermore,the generalization experiments conducted on the jaw cyst dataset from Quzhou People’s Hospital further verified the outstanding segmentation performance of the proposed model.These findings collectively affirm the potential of our approach as a valuable tool in supporting clinical decision-making in the field of skin cancer detection,as well as advancing research in medical image analysis.

文摘The thalamic reticular nucleus(TRN)plays a crucial role in regulating sensory encoding,even at the earliest stages of visual processing,as evidenced by numerous studies.Orientation selectivity,a vital neural response,is essential for detecting objects through edge perception.Here,we demonstrate that somatostatin(SOM)-expressing and parvalbumin(PV)-expressing neurons in the TRN project to the dorsal lateral geniculate nucleus and modulate orientation selectivity and the capacity for visual information processing in the primary visual cortex(V1).These findings show that SOM-positive and PV-positive neurons in the TRN are powerful modulators of visual information encoding in V1,revealing a novel role for this thalamic nucleus in influencing visual processing.

文摘Drone-based small object detection is of great significance in practical applications such as military actions, disaster rescue, transportation, etc. However, the severe scale differences in objects captured by drones and lack of detail information for small-scale objects make drone-based small object detection a formidable challenge. To address these issues, we first develop a mathematical model to explore how changing receptive fields impacts the polynomial fitting results. Subsequently, based on the obtained conclusions, we propose a simple but effective Hybrid Receptive Field Network (HRFNet), whose modules include Hybrid Feature Augmentation (HFA), Hybrid Feature Pyramid (HFP) and Dual Scale Head (DSH). Specifically, HFA employs parallel dilated convolution kernels of different sizes to extend shallow features with different receptive fields, committed to improving the multi-scale adaptability of the network;HFP enhances the perception of small objects by capturing contextual information across layers, while DSH reconstructs the original prediction head utilizing a set of high-resolution features and ultrahigh-resolution features. In addition, in order to train HRFNet, the corresponding dual-scale loss function is designed. Finally, comprehensive evaluation results on public benchmarks such as VisDrone-DET and TinyPerson demonstrate the robustness of the proposed method. Most impressively, the proposed HRFNet achieves a mAP of 51.0 on VisDrone-DET with 29.3 M parameters, which outperforms the extant state-of-the-art detectors. HRFNet also performs excellently in complex scenarios captured by drones, achieving the best performance on the CS-Drone dataset we built.

文摘Objective To observe acupuncture effects on endometrial receptivity of the rats with polycystic ovary syndrome (PCOS) treated by ovarian stimulation with Clomiphene to explore its mechanisms. Methods eGOS rat model was experimentally induced by subcutaneous injection of oil solution of dehydroepiandrosterone (DHEA) in immature (24-day-old) female rats for continuous 20 days. The same dose of oil was given for an equivalent length of time to animals in a control group (Group Con). The rats with PCOS were randomly divided into a model group (group M), a Clomiphene treatment group (group C), a Clomiphene plus acupuncture group (group C+ A). The treatments were given for 5 days since 80 days after birth. Half of the rats were killed by decapitation after treatment finished, and the serum, uteri and endometria were obtained. The other half mated with male rats, and they were killed at the 8th day of pregnancy. We observed the implantation number and condition of pregnant uteri. Results Compared with group C, the animals in group C+A had higher implantation rate and more average implanted embryos (P〈0. 01 ), higher serum estradiol (E2) level (P〈0.01 ), better development of endometrium, and increased mRNA and protein expressions of molecular markers of endometrial receptivity (P〈0.01), which were closer to those in the control group. Conclusion Acupuncture could improve poor endometrial receptivity of PCOS rats with ovarian stimulation by citrated Clomiphene and promote implantation. The effects may be related with its up-regulation of serum E2 level, mRNA and protein expressions of molecular markers of endometrial receptivity.

文摘[Objective] The aims of this study were to understand the flowering biological characteristics of Prunus domestica ×armeniaca and analyze its differences with other apricot varieties and plum varieties. [Method] Taking three varieties of Prunus domestica ×armeniaca as the research object, the phenophase of blooming,morphology characteristics of floral organs and development characteristics of floral organs were observed, pollen amount, pollen viability and stigma receptivity were measured. [Result] The results showed that three varieties of Prunus domestica×armeniaca were in blossom in early April, the florescence were relatively late for 8-12 d comparing with the varieties of apricot, the florescence were relatively similar to the P. domestica L. Morphology characteristics of floral organs of three varieties of Prunus domestica×armeniaca had few differences, were relatively similar to two varieties of plum, and they all had high percent of middle-style and long-style flower.The pollen amount of three varieties of Prunus domestica ×armeniaca had a little differences comparing with two varieties of plum, but were lower than three varieties of apricot. There were some differences in pollen viability, the germination percentage of pollen of ‘Konglongdan' was relatively high, was 51.6%; The pollen amount of ‘Fengweihuanghou' was relatively high, was 880.8 per anther; ‘Weidi' had fewer pollen amount, and the pollen viability was just 2.1%; Three varieties of Prunus domestica ×armeniaca all had strong stigma receptivity on the bloom day, and the stigma receptivity continued exist until the day that 5 days after blooming. [Conclusion] The florescence of three varieties of Prunus domestica ×armeniaca were relatively concentrated, and the rate of pistil abortion was lowest, the variation intensity of stigma receptivity and sustainable time of three varieties of Prunus domestica×armeniaca almost had a little differences comparing with a variety of plum, but slightly more than three varieties of apricot.

文摘Objective] This study aimed to investigate the relationship between pistil-late flower developmental morphology and stigma receptivity in Euphorbia pulcherri-ma. [Method] One-year-old adult plants of E. pulcherrima cultivar ‘Mil enium’ were used as experimental materials to analyze pistil ate flower development process by macroscopic and scanning electron microscope (SEM) observation. Stigma receptivity was detected with benzidine-hydrogen peroxide method and in vivo pol en germina-tion method. [Result] The pistil ate flower development process of E. pulcherrima was consisted of columnar stigma phase, stigma lobe slightly opening phase, Y-shape stigma lobe phase, inverse V-shaped stigma lobe phase and stigma curling phase. Pistil ate flower development and stigma receptivity of E. pulcherrima exhibit-ed certain correlation. The stigma receptive period lasted from stigma lobe slightly opening phase to inverse V-shaped stigma lobe phase; inverse V-shaped stigma lobe phase was appropriate for pol ination, lasting for 3-5 d, with V-shaped stigma lobe and a large amount of exudates on stigma surface according to SEM obser-vation. Stigma receptivity of E. pulcherrima detected with benzidine-hydrogen perox-ide method was consistent with that detected in vivo pol en germination method. [Conclusion] ln breeding practice, the optimal pol ination period of E. pulcherrima can be determined based on the developmental morphology of pistil ate flower.
文摘BACKGROUND: The tumor necrosis factor recepter associated factor (TRAF) 6 is an important intracellular adapter protein that plays a pivotal role in activating multiple inflammatory and immune related processes induced by cytokines. TRAF6 represents a strong candidate susceptibility factor for sepsis. We investigated whether polymorphisms at the TRAF6 gene are associated with the susceptibility to and severity of sepsis.METHODS: A hospital-based case-control study was conducted with 255 patients with sepsis and 260 controls who were recruited from Zhengzhou, China. Haplotype tagging single nucleotide polymorphisms (htSNPs) were selected from the HapMap database and genotyped using the SNPstream genotyping platform. The associations with the susceptibility and disease severity of sepsis were estimated by logistic regression, and adjusted for age, sex, smoking, drinking, chronic diseases status, APACHEII score and critical illness status.RESULTS: A total of 13 TRAF6 SNPs were tagged by 7 htSNPs. Five htSNPs (rs5030490, rs5030411, rs5030416, rs5030445 and rs3740961) were genotyped in the case control study. Genotype frequencies of the htSNPs were conformed to the Hardy-Weinberg equilibrium in both patients and controls. No significant association was found between the 5 htSNPs and the susceptibility to and severity of sepsis. Compared with the main haplotype -11120A/-10688T/-9423A/805G/12967G, no certain haplotype was associated with the signi? cantly susceptibility to or severity of sepsis.CONCLUSION: TRAF6 gene polymorphisms might not play a major role in mediating the susceptibility to and severity of sepsis in the Chinese population. A larger population-based case-control study is warranted.
