Patch testing and cross sensitivity study of adverse cutaneous drug reactions due to anticonvulsants: A preliminary report 被引量:1

AIM To evaluate the utility of patch test and cross-sensitivity patterns in patients with adverse cutaneous drug reactions(ACDR) from common anticonvulsants. METHODS Twenty-four(M:F = 13:11) patients aged 18-75 years with ACDR from anticonvulsants were patch tested 3-27 mo after complete recovery using carbamazepine, phenytoin, phenobarbitone, lamotrigine, and sodium valproate in 10%, 20% and 30% conc. in pet. after informed consent. Positive reactions persisting on D3 and D4 were considered significant. RESULTS Clinical patterns were exanthematous drug rash with or without systemic involvement(DRESS) in 18(75%), Stevens-Johnsons syndrome/toxic epidermal necrolysis(SJS/TEN) overlap and TEN in 2(8.3%) patients each, SJS and lichenoid drug eruption in 1(4.2%) patient each, respectively. The implicated drugs were phenytoin in 14(58.3%), carbamazepine in 9(37.5%), phenobarbitone in 2(8.3%), and lamotrigine in 1(4.7%) patients,respectively. Twelve(50%) patients elicited positive reactions to implicated drugs; carbamazepine in 6(50%), phenytoin alone in 4(33.3%), phenobarbitone alone in 1(8.3%), and both phenytoin and phenobarbitone in 1(8.33%) patients, respectively. Cross-reactions occurred in 11(92%) patients. Six patients with carbamazepine positive patch test reaction showed cross sensitivity with phenobarbitone, sodium valproate and/or lamotrigine. Three(75%) patients among positive phenytoin patch test reactions had cross reactions with phenobarbitone, lamotrigine, and/or valproate. CONCLUSION Carbamazepine remains the commonest anticonvulsant causing ACDRs and cross-reactions with other anticonvulsants are possible. Drug patch testing appears useful in DRESS for drug imputability and cross-reactions established clinically. 展开更多

操作系统测试是操作系统质量保障及自主可控的前提和基础,而集成现有各种开源测试工具是构建操作系统测试平台的现实可行路线。首先,重点分析了Linux测试集成平台LKP-tests的测试工具集成组织结构及其应用于openEuler、Debian和CentOS... 操作系统测试是操作系统质量保障及自主可控的前提和基础,而集成现有各种开源测试工具是构建操作系统测试平台的现实可行路线。首先,重点分析了Linux测试集成平台LKP-tests的测试工具集成组织结构及其应用于openEuler、Debian和CentOS可能遭遇的软件包缺失问题,并设计实现了软件包缺失自动识别与修复系统;其次,该系统通过正则表达式来分类描述源自测试运行结果的错误信息,提取其中的缺失组件名称;最后,利用对应操作系统的包管理器,解析确定有关组件所属的软件包,并完成LKP-tests中相应软件包依赖的添加与整合。实验表明,该模型对软件包缺失问题进行自动修复的比例均高于91%,可有效减少LKP-tests应用过程中的人工干预并改善其运营维护,对其他类似的大型开源软件集成项目亦有借鉴意义。 展开更多

Theoretically, the constant stress patch test is not rigorous. Also, either the patch test of non-zero constant shear for Mindlin plate problem or non-zero strain gradient curvature of the microstructures cannot be performed. To improve the theory of the patch test, in this paper, based on the variational principle with relaxed continuity requirement of nonconforming element for homogeneous differential equations, the author proposed the individual element condition for passing the patch test and the convergence condition of the element: besides passing the patch test, the element function should include the rigid body modes and constant strain modes and satisfy the weak continuity condition, and no extra zero energy modes occur. Moreover, the author further established a variational principle with relaxed continuity requirement of nonconforming element for inhomogene-ous differential equations, the enhanced patch test condition and the individual element condition. To assure the convergence of the element that should pass the enhanced patch test, the element function should include the rigid body modes and non-zero strain modes which satisfied the equilibrium equations, and no spurious zero energy modes occur and should satisfy new weak continuity condition. The theory of the enhanced patch test pro-posed in this paper can be applied to both homogeneous and inhomogeneous differential equations. Based on this theory, the patch test of the non-zero constant shear stress for Mindlin plate and the C0-1 patch test of the non-zero constant curvature for the couple stress/strain gradient theory were established. 展开更多

In FEM,it is called C0 and C1 theory. The convergence criteria of finite elements for microstructures are less mature than those for the conventional thin plate bending problem. In this paper,the patch test functions for assessing convergence of the C0 and C1 finite elements in microstructures is established based on the enhanced patch test theory. The author has further explored the C0 and C1 finite element theories and investigated the difference and correlation between their finite element formulations. Newly proposed finite element theories for microstructures are as follows:(1) the displacement-rotation dependent C1 element that requires the element function satisfying both C0 and C1 continuity;(2) the displacement-rotation independent C0 element which requires new convergence criteria,such as non-zero constant shear stress patch test and zero constant shear stress patch test for approximating C1 element. 展开更多

The paper investigates the foaming and mildness properties of trehalolipids surfactant in rinse-off formulations,comparing it with the traditional nonionic surfactant alkyl-polyglycolide(APG).Both single surfactant and their surfactant complexes are evaluated.And the results show that trehalolipids have significant advantages in reducing surface tension,improving foam performance,and enhancing mildness.For mildness evaluation,multiple methods are employed in the study,including in vitro cell toxicity,inflammatory markers,colorimetric index of mildness CIM,and in vivo method of human skin patch test.The results show that the addition of trehalolipids increases IC50 and reduces the release of inflammatory markers of the binary surfactant system.However,the incorporation of trehalolipids has some negative effects on the skin’s lipid barrier function.Skin patch test results indicate that the addition of trehalolipids improves the mildness of surfactant system,particularly in sulfate-free system.Overall,trehalolipids,as nonionic surfactants,presents great potential to replace traditional surfactants in rinse off formulations with enhanced mildness and foam performance. 展开更多

目的:以利斯的明透皮贴剂为模型药物,研究体外透皮实验(IVPT)中皮肤完整性的2个关键指标:皮肤电阻(TEER)和经皮水分流失(TEWL)的变化,以及这2个指标与体外透皮实验累积渗透量(AMT)和渗透速率的关系,探讨其与药物渗透行为的相关性。方法... 目的:以利斯的明透皮贴剂为模型药物,研究体外透皮实验(IVPT)中皮肤完整性的2个关键指标:皮肤电阻(TEER)和经皮水分流失(TEWL)的变化,以及这2个指标与体外透皮实验累积渗透量(AMT)和渗透速率的关系,探讨其与药物渗透行为的相关性。方法:选取皮肤厚度相近及TEWL满足要求,不同TEER倍数的皮肤,进行利斯的明透皮贴剂IVPT;通过胶带剥离法破坏角质层,比较剥离前后皮肤的TEWL、TEER和AMT。结果:使用厚度均一、TEWL<15 g·m^(-2)·h^(-1)的皮肤,利斯的明透皮贴剂的AMT和时间t^(1/2)呈线性相关,r>0.99;且TEER倍数和渗透速率呈线性相关,r≥0.99。角质层遭到破坏的皮肤,TEWL增加了约2~3倍,TEER倍数降低了约10倍,药物的AMT明显增加。结论:TEER倍数与利斯的明透皮贴剂的渗透速率具有一定相关性。TEWL、TEER倍数都可以用来评估皮肤完整性,初步来看TEER检测法的敏感性更高。 展开更多

以乌拉特前旗矿山废弃地为研究区域,基于人工冲刷试验,探究了植被恢复格局(空白对照、棋盘状、横条状与斜条状)与冲刷流量(3,4.5,6L/min)对坡面土壤侵蚀特征及植被水土保持效益的影响.结果表明:径流量与泥沙量在植被恢复坡面降低,但随... 以乌拉特前旗矿山废弃地为研究区域,基于人工冲刷试验,探究了植被恢复格局(空白对照、棋盘状、横条状与斜条状)与冲刷流量(3,4.5,6L/min)对坡面土壤侵蚀特征及植被水土保持效益的影响.结果表明:径流量与泥沙量在植被恢复坡面降低,但随冲刷流量提高而增加.横条坡面径流量与泥沙量随冲刷流量提高增加幅度最小,总体坡面减流率(34.36%)与坡面减沙率(79.50%)最大,总体植被减沙量[35.09g/(min·m^(2))]最高.随冲刷进行,坡面产流率整体上升,不同格局坡面产沙率具有差异.植被斑块产沙率均显著低于坡面平均产沙率,且多数情况下为负值.横条坡面产沙率较为稳定,植被斑块产沙率最低[-13.02g/(min·m^(2))].除横条坡面外,坡面产沙率与产流率呈线性正相关.冲刷流量对径流量与产流率具有决定性作用,贡献率分别达79.37%与66.21%.植被斑块格局对侵蚀的影响高于径流.综上,横条格局是最适宜矿山废弃地水土保持的植被斑块格局,设计排水对该地土壤侵蚀防治极为重要. 展开更多

探寻不良反应发生的特点和规律,为化妆品监管和不良反应监测提供借鉴和参考。采用回顾性研究法,对曲靖市799例化妆品不良反应监测报告数据进行统计与分析。结果表明,化妆品不良反应多见于中青年女性,主要表现为化妆品接触性皮炎,皮损部... 探寻不良反应发生的特点和规律,为化妆品监管和不良反应监测提供借鉴和参考。采用回顾性研究法,对曲靖市799例化妆品不良反应监测报告数据进行统计与分析。结果表明,化妆品不良反应多见于中青年女性,主要表现为化妆品接触性皮炎,皮损部位以面部和头皮为主;瘙痒和灼热感是常见自觉症状;红斑和丘疹为主要皮损形态且皮损常于7天内出现。网购化妆品常出现不良反应;选择斑贴试验患者较少。建议监管部门加强法规宣贯,规范化妆品销售渠道;化妆品注册人、备案人落实主体责任,建立并完善监测和评价体系;医疗机构继续发挥报告主渠道作用;监测部门持续提升监测评价和风险预警能力,加强科学护肤和安全用妆宣传教育,携手守护消费者用妆安全。 展开更多

目的:回顾性研究Z市化妆品功效实验室进行的单次皮肤封闭型斑贴试验结果,探讨影响因素,为化妆品不良反应监测及安全性质控提供数据参考。方法:收集2022年1月1日至2024年6月30日的4093例样品测试结果,分析其与季节、人群、剂型和产品类... 目的:回顾性研究Z市化妆品功效实验室进行的单次皮肤封闭型斑贴试验结果,探讨影响因素,为化妆品不良反应监测及安全性质控提供数据参考。方法:收集2022年1月1日至2024年6月30日的4093例样品测试结果,分析其与季节、人群、剂型和产品类型的关系。结果:季节方面,冬季斑贴阴性率为80.29%,低于春夏秋季。人群方面,非敏感性皮肤阴性率81.94%,高于敏感性皮肤的79.63%。剂型方面,块状、粉状和膜类阴性率较高,分别为95.59%、94.00%和88.05%,乳液和冻干剂型较低,为76.29%和75.86%。产品类型方面,淋洗类产品阴性率偏低,其中洗发水最低为62.04%。结论:单次皮肤封闭型斑贴试验结果受季节、人群皮肤特征影响,不同产品类型及剂型的斑贴试验结果存在差异。 展开更多

随着临床相关学科的发展,国内外相关标准/指南的更新和建立,GBZ 185—2006《职业性三氯乙烯药疹样皮炎诊断标准》(以下简称“GBZ 185—2006”)已不能满足临床需求。基于循证医学原则,依据有关法律法规和相关标准/指南,系统梳理国内外的... 随着临床相关学科的发展,国内外相关标准/指南的更新和建立,GBZ 185—2006《职业性三氯乙烯药疹样皮炎诊断标准》(以下简称“GBZ 185—2006”)已不能满足临床需求。基于循证医学原则,依据有关法律法规和相关标准/指南,系统梳理国内外的职业性三氯乙烯药疹样皮炎(OMDT)的研究资料,并结合前期临床实践和临床相关学科的发展,对GBZ 185—2006进行修订。主要修改内容包括:增加了OMDT的术语与定义,调整了诊断原则中关于临床表现的表述,调整了潜伏期,删除了发病概率的诊断要求,增加了特异性变应原斑贴试验为病因学诊断指标,规范了特异性变应原斑贴试验的适用范围、操作方法与注意事项。此外,亦完善了附录A的“三氯乙烯药疹样皮炎皮肤损害的基本特点和临床类型”相关内容,修订处理原则,增加了治疗技术新进展——人工肝应用的内容。修订后的GBZ 185—2024《职业性三氯乙烯药疹样皮炎诊断标准》具有较强的科学性和可操作性,可为医疗卫生机构规范诊断和处理OMDT提供技术依据。 展开更多

目的:检测女性玫瑰痤疮患者化妆品接触性过敏原情况。方法:对2018年1月—2021年12月在我院皮肤科就诊的93例女性玫瑰痤疮患者进行化妆品变应原斑贴试验。结果:93例患者中有68例(73.12%)呈阳性反应,其中阳性率排前4位的变应原种类分别是... 目的:检测女性玫瑰痤疮患者化妆品接触性过敏原情况。方法:对2018年1月—2021年12月在我院皮肤科就诊的93例女性玫瑰痤疮患者进行化妆品变应原斑贴试验。结果:93例患者中有68例(73.12%)呈阳性反应,其中阳性率排前4位的变应原种类分别是金属盐类(41例)、表面活性剂类(37例)、防腐剂类(30例)和芳香剂类(24例),阳性率排前9位的具体变应原成分分别是氯化镍(36例)、氯化钴(23例)、十二烷基硫酸钠(16例)、戊基肉桂醇(14例)、烷基醇聚氧乙烯醚(12例)、硫酸镍(12例)、硫柳汞(11例)、硫酸钠(11例)和橡苔浸膏(10例)。结论:化妆品接触性过敏原可能与玫瑰痤疮的发病和复发有关,金属盐类、表面活性剂类、防腐剂类和芳香剂类是最常见的化妆品变应原种类。 展开更多

The interfacial debonding in fiber-reinforced plastic(FRP)strengthened repair material will affect the bonding strength and lead to failure of the repair without warning.Unfortunately the interfacial damage is normally invisible and often in the form of a patch rather than a through-width crack.Therefore,a debonding patch detection technique based on fiber optic interferometry is proposed.A quasi-impulse loading is applied with a rubberhead hammer and the total elongation of a surface-mounted optical fiber along the length of the repair material is measured as a function of load position.When a debonding patch is present,the induced sudden slope or sign change on the plot of fiber integral strain v.s.load position will reveal the extent and the location of the debonded area.The results of the study indicate that the proposed technique is applicable for debonding patch detection in repaired members under various support conditions. 展开更多
