文摘目的探讨性别、年龄及糖脂代谢指标对血小板指标(platelet indices,PI)的影响。方法纳入上海市崇明区9403名4070岁的居民,收集其基本信息(包括性别、年龄、吸烟史、饮酒史),生化指标[包括糖化血红蛋白(haemoglobin A1c,HbA1c)、空腹血糖(fasting blood glucose,FBG)、餐后2 h血糖(2 h postprandial blood glucose,2hPG)、总胆固醇(total cholesterol,TC)、低密度脂蛋白胆固醇(low density lipoprotein-cholesterol,LDL-C)、甘油三酯(triglyceride,TG)、高密度脂蛋白胆固醇(high density lipoprotein-cholesterol,HDL-C)]以及PI[包括血小板计数(platelet count,PLT)、平均血小板体积(mean platelet volume,MPV)、血小板分布宽度(platelet distribution width,PDW)],通过Spearman相关分析评估各个变量与PI的相关性,使用线性回归分析确定PI的影响因素。结果Spearman相关分析显示,性别(女性)与PLT、MPV和PDW均呈正相关,年龄与PLT呈负相关;HbA1c、LDL-C、TC、TG与PLT呈正相关;MPV与HbA1c、LDL-C、TC、TG呈负相关;PDW与HbA1c、LDL-C、TG呈负相关。逐步回归分析和岭回归分析结果显示:女性(β=0.054,P<0.001)、HbA1c(β=0.030,P<0.001)、TC(β=0.033,P<0.001)和LDL-C(β=0.078,P<0.001)均是PLT升高的独立危险因素,而年龄与PLT呈负相关。岭回归分析显示,TG(β=-0.024,P<0.001)、LDL-C(β=-0.033,P<0.001)与MPV独立负相关,LDL-C(β=-0.037,P<0.001)与PDW独立负相关。结论女性、HbA1c、LDL-C、TC是PLT升高的独立危险因素,其中LDL-C的影响最大,提示临床应加强血糖及血脂检测与管理。
