BACKGROUND Laparoscopic surgery has emerged as the preferred treatment for acute appendicitis,a common acute abdominal condition.Despite surgical advances,postoperative gastrointestinal dysfunction and delayed recover...BACKGROUND Laparoscopic surgery has emerged as the preferred treatment for acute appendicitis,a common acute abdominal condition.Despite surgical advances,postoperative gastrointestinal dysfunction and delayed recovery continue to negatively impact rehabilitation outcomes and patient quality of life.Laser acupuncture represents a promising emerging complementary therapy with potential benefits for promoting functional recovery.AIM To evaluate the clinical efficacy of laser acupuncture in enhancing gastrointestinal function recovery following laparoscopic appendectomy and to investigate its potential applications in postoperative rehabilitation.METHODS This retrospective controlled clinical study selected 120 acute appendicitis patients who underwent laparoscopic appendectomy from January 2022 to December 2023 at a tertiary hospital.Patients were randomly divided into two groups:Routine treatment group(n=60)and laser acupuncture intervention group(n=60).There were no significant statistical differences in baseline characteristics(age,gender,body mass index,appendicitis severity)between the two groups.The routine treatment group received standard postoperative care,while the laser acupuncture intervention group received additional laser acupuncture treatment on top of standard care.Key observation indicators included time to first exhaust,time to first bowel movement,time to gastrointestinal function recovery,and postoperative complication rates.RESULTS Patients in the laser acupuncture intervention group showed significantly faster gastrointestinal function recovery compared to the routine treatment group.Specifically,time to first exhaust was significantly shortened(intervention group:12.5±2.3 hours vs control group:18.7±3.1 hours,P<0.05),time to first bowel movement was earlier(intervention group:36.4±4.6 hours vs control group:48.2±5.7 hours,P<0.05),postoperative complication rate was significantly reduced(intervention group:8.3%vs control group:20.0%,P<0.05),postoperative pain score was significantly lower(intervention group:3.2±1.1 vs control group:4.7±1.5,P<0.05),and intestinal motility recovery was faster and more effective.CONCLUSION Laser acupuncture,as an auxiliary treatment method,can significantly promote gastrointestinal function recovery in patients after laparoscopic appendectomy,reduce complications,and improve postoperative comfort.This technique has advantages such as minimal invasiveness,rapid recovery,and few side effects,making it worthy of further clinical promotion and application.展开更多

In the present work, the guides to the expression of uncertainty in measurement(GUM) uncertainty framework(GUF) and its supplement, the Monte Carlo method, were used to estimate the uncertainty of task-specific laser tracker measurements. First, the sources of error in laser tracker measurement were analyzed in detail, including instruments, measuring network fusion, measurement strategies, measurement process factors(such as the operator), measurement environment, and task-specific data processing. Second, the GUM and Monte Carlo methods and their application to laser tracker measurement were presented. Finally, a case study involving the uncertainty estimation of a cylindricity measurement process using the GUF and Monte Carlo methods was illustrated. The expanded uncertainty results(at 95% confidence levels) obtained with the Monte Carlo method are 0.069 mm(least-squares criterion) and 0.062 mm(minimum zone criterion), respectively, while with the GUM uncertainty framework, none but the result of least-squares criterion can be got, which is 0.071 mm. Thus, the GUM uncertainty framework slightly underestimates the overall uncertainty by 10%. The results demonstrate that the two methods have different characteristics in task-specific uncertainty evaluations of laser tracker measurements. The results indicate that the Monte Carlo method is a practical tool for applying the principle of propagation of distributions and does not depend on the assumptions and limitations required by the law of propagation of uncertainties(GUF). These features of the Monte Carlo method reduce the risk of an unreliable measurement of uncertainty estimation, particularly in cases of complicated measurement models, without the need to evaluate partial derivatives. In addition, the impact of sampling strategy and evaluation method on the uncertainty of the measurement results can also be taken into account with Monte Carlo method, which plays a guiding role in measurement planning.展开更多
Background and Objective:Refractive surgery has evolved significantly,with artificial intelligence(AI)offering new possibilities for enhancing patient selection,surgical planning,and postoperative outcome prediction.W...Background and Objective:Refractive surgery has evolved significantly,with artificial intelligence(AI)offering new possibilities for enhancing patient selection,surgical planning,and postoperative outcome prediction.While AI has demonstrated promising applications,its integration into clinical practice remains inconsistent due to challenges in data standardization,model interpretability,and regulatory concerns.This review examines the current applications,limitations,and future directions of AI in refractive surgery,with a focus on its role in laser vision correction(LVC)and phakic intraocular lens(IOL)implantation.Methods:A literature review was conducted using peer-reviewed studies published between January 2010 and October 2024,sourced from databases including Google Scholar,PubMed,Embase,and Web of Science.Studies were selected based on predefined inclusion criteria,covering AI applications in refractive surgery.A total of 33 key studies(16 on LVC and 17 on phakic IOLs)were analyzed,focusing on machine learning and deep learning techniques used for patient selection,surgical optimization,and complication prediction.Only English-language studies were included.Key Content and Findings:AI models utilizing structured tabular data,imaging,and multimodal inputs have demonstrated superior performance in predicting surgical outcomes and refining patient selection compared to traditional methods.Machine learning approaches such as random forests,extreme gradient boosting,and ensemble techniques,alongside deep learning architectures like convolutional neural networks and generative models,have improved risk assessment and surgical planning.In LVC,AI enhances ectasia risk assessment,keratoconus detection,and myopic regression prediction.In phakic IOL implantation,AI improves postoperative vault prediction,lens sizing,and refractive error estimation.Multimodal AI systems integrating imaging,textual data,and clinical parameters hold promise for more comprehensive patient evaluations.However,challenges such as data heterogeneity,limited external validation,and regulatory barriers hinder widespread clinical adoption.Conclusions:AI is transforming refractive surgery by enhancing precision,personalization,and patient safety.Its integration into clinical workflows has the potential to improve surgical outcomes and patient satisfaction.Future efforts should focus on advancing multimodal AI,improving model generalizability,and addressing ethical and regulatory challenges to fully harness AI’s potential in refractive surgery.展开更多

Materials and Methods: A MEDLINE, a Co- chrane and an Embase search (three specified search- es) were conducted to identify randomized controlled trials (RCT) until June 2010, conducted on human teeth and published in English, German or French language, examining the root canal disinfection after the use of lasers with or without mechanical instru-mentation. Additionally, hand search was conducted and contact with authors, when needed. Results: The MEDLINE, the Cochrane and the EMBASE search identified 240, 28, and 35 published articles, respec-tively. Ten articles from the MEDLINE and 5 articles from the Cochrane search (that were also identified in the MEDLINE search) met the inclusion and va-lidity assessment criteria. In E. faecalis elimination, instrumentation of the root canal and diode laser/665 nanometer/1 Watt (diode laser/665 nm/1 W) irradia-tion with the combined effect of Methylene Blue (MB) as photosensitizing agent (logCFU/ml = 1636) seemed to be the best method. In P. aeruginosa and in A.naeslundii elimination, instrumentation of the root canal followed by irrigation with 5, 25% NaOCl (log-CFU/ml = 0) seemed to be the best method. In gen-eral, instrumentation of the root canal followed by irrigation with 5, 25% NaOCl (logCFU/ml = 0) and instrumentation of the root canal and Er: YAG laser/ 2940 nm/0.8 W irradiation (logCFU/ml = 1924) seemed to be the best (polymicrobial studies). Conclusions: There are treatment protocols with the assistance or not of laser irradiation that can eliminate E. faecalis, E. coli and S. aureus inside the root canal. However, there is a serious number of S. anginosus, F. nuclea-tum, A. naeslundii and P. aeruginosa that remain in-side the root canal even after laser irradiation. New research is needed in order to set a treatment proto-col effective in the root canal disinfection from all bac-teria that are related to endodontic origin pathology.展开更多
AIM:To assess the visual acuity and visual quality of cataract patients who received femtosecond laser-assisted cataract surgery(FLACS)and multifocal intraocular lens(MIOL)implantation with an improved polishing techn...AIM:To assess the visual acuity and visual quality of cataract patients who received femtosecond laser-assisted cataract surgery(FLACS)and multifocal intraocular lens(MIOL)implantation with an improved polishing technique during a 1-year follow-up period.METHODS:This study included 74 eyes from 37 patients,comprising 17 males and 20 females,with a mean age of 51.74±7.80 years.Using a coin toss method,one eye per patient was randomly selected for improved anterior capsular polishing,while the other eye received standard irrigation/aspiration(I/A)polishing.The polishing group consisted of 37 eyes,including 21 right and 16 left eyes,while the control group comprised the contralateral fellow eyes of the same individuals in the polishing group.Visual acuity and quality of the patients were evaluated before surgery and at 1 wk,1,6,and 12 mo after surgery.The OPD-Scan III was utilized to assess high-order aberrations,while the optical quality analysis system(OQAS)was employed to evaluate modulation transfer function(MTF cutoff),Strehl ratio(SR),and objective scatter index(OSI)for the purpose of visual quality assessment.Paired t-tests and repeated measures analysis of variance(ANOVA)were utilized to compare the results,and the SNK-q post hoc test was applied to identify significant differences.RESULTS:The polishing group’s uncorrected distant visual acuity(UDVA)and uncorrected near visual acuity(UNVA)significantly improved 1-week post-surgery(all P<0.05).At 12-months,UDVA,UNVA,and corrected distant visual acuity(CDVA)were better than the control group(P<0.05).The MTF cutoff,SR,OSI,and high-order aberrations in the polishing group also showed significant improvements compared to the control group at 12 mo(P<0.05).CONCLUSION:The improved capsular polishing method has been demonstrated to effectively maintain visual acuity and visual quality in patients with MIOL after FLACS within 1 a.展开更多

Methods: a total of 150 patients admitted to our hospital from December 2018 to December 2019 were selected as the research subjects. The above patients were all emergency surgical trauma patients. The above patients were divided into groups according to whether they had incision infection. Among them, 25 patients in the research group ( infection group) and 125 cases in the control group (uninfected group), the general data of the two groups of patients, the location of trauma, the use of antibiotics and other factors were statistically analyzed. Results: after treatment, among the 150 patients participating in the study, 25 patients had incision infection after surgery, and the infection rate was 16.7%. Factors include the location and number of trauma, the use of preoperative antibiotics, and age. The patient is older than 60 years old, has multiple trauma or head trauma, and has not been treated with anti-inflammatory drugs before surgery. The above factors are related to postoperative wound infection. There was a statistically significant difference between the factors (P0.05). Conclusion: in the treatment of trauma patients in emergency surgery, some patients will have incision infection. Evaluation and preventive measures for patients, and disinfection treatment in strict accordance with the standards can reduce the probability of incision infection and better improve the treatment effect. We recently reported the primary results. Here we introduce this procedure in detail. A 70-W, 2-um (thulium) laser was used in continuous-wave mode. We joined the incision by making a transverse cut from the level of the verumontanum to the bladder neck, making the resection sufficiently deep to reach the surgical capsule, and resected the prostate into small pieces, just like peeling a tangerine. As we resected the prostate, the pieces were vaporized, sufficiently small to be evacuated through the reseetoscope sheath, and the use of the mechanical tissue morcellator was not required. The excellent hemostasis of the thulium laser ensured the safety of TmLRP-TT. No patient required blood transfusion. Saline irrigation was used intraoperatively, and no case of transurethral resection syndrome was observed. The bladder outlet obstruction had clearly resolved after catheter removal in all cases. We designed the tangerine technique and proved it to be the most suitable procedure for the use of thulium laser in the treatment of benign prostatic hyperplasia (BPH). This procedure, which takes less operative time than standard techniques, is safe and combines efficient cutting and rapid organic vaporization, thereby showing the great superiority of the thulium fiber laser in the treatment of BPH. It has been proven to be as safe and efficient as transurethral resection of the prostate (TURP) during the 1-year follow-up.展开更多

The first 273 consecutive eyes receiving FLACS and 553 eyes receiving traditional phacoemulsification were examined.All surgeries were performed at a single surgical center in Hawaii. The presence of intraoperative complications was used as the main outcome measure. Approval was obtained from the institutional review board of the University of Hawaii.RESULTS: The overall complication rate for FLACS was 1.8%, while that of the traditional procedure was5.8%(P 【0.05). A majority of the surgeons(80%) had a lower complication rate while using FLACS.CONCLUSION: FLACS is comparable in safety, if not safer, than traditional cataract surgery when performed by qualified cataract surgeons on carefully selected patients.展开更多

Firstly, by the help of laser scanning technique, the three-dimensional model of the surface of rock discontinuity was established. Secondly, a light source was simulated, and there would be some shadows produced on the model surface. Thirdly, to obtain the value of SAP of each specimen, the shadow detection technique was introduced for use. Fourthly, compared with the result from direct shear testing and based on statistics, an empirical for- mula was found among SAP, normal stress, and shear strength. Data of Yujian (~ River were used as an example, and the following conclusions have been made. (1) In the case of equal normal stress, the peak shear stress is positively proportional to the SAP. (2) The formula for estimating was derived, and the predictions of peak-shear strength made with this equation well agreed with the experimental re- suits obtained in laboratory tests.展开更多

In this paper, the type 304 austenitic stainless steel is welded by a 5 kW high-power fiber laser and a high-speed camera is employed to capture the topside images of weld pools. Then we propose a robust visual-detection approach for the molten pool based on the supervised descent method. It provides an elegant framework for representing the outline of a weld pool and is especially efficient for weld pool detection in the presence of strong uncertainties and disturbances. Finally, welding experimental results verified that the proposed approach can extract the weld pool boundary accurately, which will lay a solid foundation for controlling the weld quality of fiber laser welding process.展开更多

RESULTS The efficacy of this device, even at very low power(1W, CW), allows us to obtain very high intra and postoperative comfort for the patient, even with just topical anaesthesia and without needing suture. The healing process was completed in one week and, during the follow-up, the patient did not report any problems, pain or discomfort even without the consumption of any kind of drugs, such as painkillers and antibiotics. The histological examination performed by the pathologist showed a large area of fibrous connective tissue with some portions of epithelium-connective detachments and a regular incision with very scanty areas of carbonization.CONCLUSION The 450 nm diode laser proved of being very efficient in the oral soft tissue surgical procedures, with no side effects for the patients.展开更多

METHODS: Sixty cataract patients with N2 to N3 stage according to the LOCS III were enrolled in this study and divided into three groups randomly: FLACSl group (cataract surgery by FLACS with LenSx), FLACS2 group (cataract surgery by FLACS with LensAR) and manual group (cataract surgery by phacoemulsification). Patients in two FLACS groups performed anterior capsulotomy by LenSx or LensAR laser system. Patients in the manual group were performed continuous curvilinear capsulorrhexis (CCC) manually. The anterior capsules were fixed right after moved out of eye. Hematoxylin-eosine staining, immunofluorescence staining and real-time PCR were performed in order to observe human lens epithelium cells changes after cataract surgery. RESULTS: The capsule cutting edge was shown irregularity and roughness in two FLACS groups and smooth edge in manual capsulotomy by pathologic staining. Irregularities of the cell configuration with partly swollen and destroyed nuclei were observed in two FLACS groups. Femtosecond laser could induce a significantly higher cell apoptosis in human lens epithelium cell than manually performed CCC (P〈0.05). Lens epithelium cells apoptosis were correlated with femtosecond laser duration according to Pearson correlation analysis. Decreased N-cadherin expression, alpha-SMA and FSP-1 level in two FLACS groups showed the inhibition of cell EMT. CONCLUSION: Femtosecond laser may affect the apoptosis and EMT of lens epithelium cells which are under the peeled central lens capsule.展开更多

AIM:To evaluate corneal astigmatic outcomes of femtosecond laser-assisted arcuate keratotomies(FAKs)combined with femtosecond-laser assisted cataract surgery(FLACS)over 12mo follow-up.METHODS:Totally 145 patients with...AIM:To evaluate corneal astigmatic outcomes of femtosecond laser-assisted arcuate keratotomies(FAKs)combined with femtosecond-laser assisted cataract surgery(FLACS)over 12mo follow-up.METHODS:Totally 145 patients with bilateral cataracts and no ocular co-morbidities were recruited to a singlecentre,single-masked,prospective randomized controlled trial(RCT)comparing two monofocal hydrophobic acrylic intraocular lenses.Eyes with corneal astigmatism(CA)of>0.8 dioptres(D)received unpaired,unopened,surface penetrating FAKs at the time of FLACS.Visual acuity,subjective refraction and Scheimpflug tomography were recorded at 1,6,and 12mo.Alpins vectoral analyses were performed.RESULTS:Fifty-one patients(61 eyes),mean age 68.2±9.6y[standard deviation(SD)],received FAKs.Sixty eyes were available for analysis,except at 12mo when 59 attended.There were no complications due to FAKs.Mean pre-operative CA was 1.13±0.20 D.There was a reduction of astigmatism at all post-operative visits(residual CA 1mo:0.85±0.42 D,P=0.0001;6mo:0.86±0.35 D,P=0001;and 12mo:0.90±0.39,P=0.0001).Alpins indices remained stable over 12mo.Overall,the cohort was under-corrected at all time points.At 12mo,61%of eyes were within±15 degrees of pre-operative astigmatic meridian.CONCLUSION:Unpaired unopened penetrating FAKs combined with on-axis phacoemulsification are safe but minimally effective.CA is largely under-corrected in this cohort using an existing unmodified nomogram.The effect of arcuate keratotomies on CA remained stable over 12mo.展开更多

Micro holes and lines are firstly etched for parameter study. Grating profile is theoretically optimized to form high quality moire patterns. For a demonstration, a parallel grating is fabricated on a specimen of quartz glass. The minimum line width and the distance between two adjacent lines are both set to be 1 μm, and the frequency of grating is 500 lines/ram. The experimental results indicate that the quality of gratings is good and the relative error of the gratings pitch is about 1.5%. Based on molte method, scanning electron microscope (SEM) moire patterns are observed clearly, which manifests that gratings fabricated with the femtosecond laser exposure is suitable for micro scale deformation measurement.展开更多

The experimentally observed line profiles of neutral aluminum have been used to extract the excitation temperature using the Boltzmann plot method, whereas the electron number density has been determined from the Stark broadened as well as using the Saha-Boltzmann equation (SBE). Each approach was also carried out by using the AI emission line and Mg emission lines. It was observed that the,SBE method generated a little higher electron number density value than the Stark broadening, method, but within the experimental uncertainty range. Comparisons of Ne determined by the two methods show the presence of a linear relation which is independent of laser energy or laser wavelength. These results show the applicability of the SBE method for Are determination, especially when the system does not have any pure emission lines whose electron impact factor is known, Also use of Mg lines gives superior results than Al lines.展开更多
A fully non-contact experimental platform for ultrasonic Lamb wave damage detection was constructed,where laser exciting and the scanning laser Doppler vibrometer were used to realize the high-resolution pickup of the...A fully non-contact experimental platform for ultrasonic Lamb wave damage detection was constructed,where laser exciting and the scanning laser Doppler vibrometer were used to realize the high-resolution pickup of the Lamb wave field in the structure,which has overcome the disadvantages of low spatial resolution caused by the conventional contact Lamb wave transducer.In order to suppress the dispersion effect of broadband laser-ultrasonic signal,we proposed time-domain filtering in multi-band method based on wavelet analysis to decompose the broadband signal into multiple narrowband ones and separate the scattering signals effectively without reference signal.On this basis,the total focusing method(TFM)was used for damage imaging.However,when the traditional TFM was applied to image based on ultrasonic Lamb wave,the inherent dispersion characteristic of ultrasonic Lamb wave could lead to the miscalculation of time delay,thus reducing the imaging precision.Therefore,the frequency-domain TFM was developed by applying phase delay in the frequency domain.The logical AND was introduced to synthesize the damage imaging results of multiple narrowband signals to obtain high-precision damage imaging.Our study has shown that the method of time-domain filtering in multi-band combining with frequency-domain TFM can realize non-contact and accurate damage detection in isotropic plate structures,and it is a potential effective method for application in engineering practice.展开更多
AIM:To investigate the clinical efficacy and safety of femtosecond laser-assisted steepest-meridian clear corneal incisions for correcting preexisting corneal astigmatism performed at the time of cataract surgery.METH...AIM:To investigate the clinical efficacy and safety of femtosecond laser-assisted steepest-meridian clear corneal incisions for correcting preexisting corneal astigmatism performed at the time of cataract surgery.METHODS:This prospective case series study comprised consecutive age-related cataract patients with corneal regular astigmatism(range:+0.75 to+2.50 D)who had femtosecond laser-assisted steepest-meridian clear corneal incisions(single or paired).Corneal astigmatism was performed with the Pentacam preoperatively and 3 mo postoperatively.Total corneal astigmatism and steepestmeridian measured in the 3-mm central zone were used to guide the location,size and number of clear corneal incision.The vector analysis of astigmatic change was performed using the Alpins method.RESULTS:Totally 138 eyes of 138 patients were included.The mean preoperative corneal astigmatism was 1.31±0.41 D,and was significantly reduced to 0.69±0.34 D(equivalent to difference vector)after surgery(P<0.01).The surgically-induced astigmatism was 1.02±0.54 D.The correction index(ratio of target induced astigmatism and surgically-induced astigmatism:0.72±0.36)as well as the magnitude of error(difference between surgically-induced astigmatism and target induced astigmatism:-0.29±0.51)represented a slight under correction.For angle of error,the arithmetic mean was 1.11±13.70,indicating no significant systematic alignment errors.CONCLUSION:Femtosecond-assisted steepest-meridian clear corneal incision is a fast,customizable,adjustable,precise,and safe technique for the reduction of low to moderate corneal astigmatism during cataract surgery.展开更多
文摘BACKGROUND Laparoscopic surgery has emerged as the preferred treatment for acute appendicitis,a common acute abdominal condition.Despite surgical advances,postoperative gastrointestinal dysfunction and delayed recovery continue to negatively impact rehabilitation outcomes and patient quality of life.Laser acupuncture represents a promising emerging complementary therapy with potential benefits for promoting functional recovery.AIM To evaluate the clinical efficacy of laser acupuncture in enhancing gastrointestinal function recovery following laparoscopic appendectomy and to investigate its potential applications in postoperative rehabilitation.METHODS This retrospective controlled clinical study selected 120 acute appendicitis patients who underwent laparoscopic appendectomy from January 2022 to December 2023 at a tertiary hospital.Patients were randomly divided into two groups:Routine treatment group(n=60)and laser acupuncture intervention group(n=60).There were no significant statistical differences in baseline characteristics(age,gender,body mass index,appendicitis severity)between the two groups.The routine treatment group received standard postoperative care,while the laser acupuncture intervention group received additional laser acupuncture treatment on top of standard care.Key observation indicators included time to first exhaust,time to first bowel movement,time to gastrointestinal function recovery,and postoperative complication rates.RESULTS Patients in the laser acupuncture intervention group showed significantly faster gastrointestinal function recovery compared to the routine treatment group.Specifically,time to first exhaust was significantly shortened(intervention group:12.5±2.3 hours vs control group:18.7±3.1 hours,P<0.05),time to first bowel movement was earlier(intervention group:36.4±4.6 hours vs control group:48.2±5.7 hours,P<0.05),postoperative complication rate was significantly reduced(intervention group:8.3%vs control group:20.0%,P<0.05),postoperative pain score was significantly lower(intervention group:3.2±1.1 vs control group:4.7±1.5,P<0.05),and intestinal motility recovery was faster and more effective.CONCLUSION Laser acupuncture,as an auxiliary treatment method,can significantly promote gastrointestinal function recovery in patients after laparoscopic appendectomy,reduce complications,and improve postoperative comfort.This technique has advantages such as minimal invasiveness,rapid recovery,and few side effects,making it worthy of further clinical promotion and application.

文摘Measurement uncertainty plays an important role in laser tracking measurement analyses. In the present work, the guides to the expression of uncertainty in measurement(GUM) uncertainty framework(GUF) and its supplement, the Monte Carlo method, were used to estimate the uncertainty of task-specific laser tracker measurements. First, the sources of error in laser tracker measurement were analyzed in detail, including instruments, measuring network fusion, measurement strategies, measurement process factors(such as the operator), measurement environment, and task-specific data processing. Second, the GUM and Monte Carlo methods and their application to laser tracker measurement were presented. Finally, a case study involving the uncertainty estimation of a cylindricity measurement process using the GUF and Monte Carlo methods was illustrated. The expanded uncertainty results(at 95% confidence levels) obtained with the Monte Carlo method are 0.069 mm(least-squares criterion) and 0.062 mm(minimum zone criterion), respectively, while with the GUM uncertainty framework, none but the result of least-squares criterion can be got, which is 0.071 mm. Thus, the GUM uncertainty framework slightly underestimates the overall uncertainty by 10%. The results demonstrate that the two methods have different characteristics in task-specific uncertainty evaluations of laser tracker measurements. The results indicate that the Monte Carlo method is a practical tool for applying the principle of propagation of distributions and does not depend on the assumptions and limitations required by the law of propagation of uncertainties(GUF). These features of the Monte Carlo method reduce the risk of an unreliable measurement of uncertainty estimation, particularly in cases of complicated measurement models, without the need to evaluate partial derivatives. In addition, the impact of sampling strategy and evaluation method on the uncertainty of the measurement results can also be taken into account with Monte Carlo method, which plays a guiding role in measurement planning.
文摘Background and Objective:Refractive surgery has evolved significantly,with artificial intelligence(AI)offering new possibilities for enhancing patient selection,surgical planning,and postoperative outcome prediction.While AI has demonstrated promising applications,its integration into clinical practice remains inconsistent due to challenges in data standardization,model interpretability,and regulatory concerns.This review examines the current applications,limitations,and future directions of AI in refractive surgery,with a focus on its role in laser vision correction(LVC)and phakic intraocular lens(IOL)implantation.Methods:A literature review was conducted using peer-reviewed studies published between January 2010 and October 2024,sourced from databases including Google Scholar,PubMed,Embase,and Web of Science.Studies were selected based on predefined inclusion criteria,covering AI applications in refractive surgery.A total of 33 key studies(16 on LVC and 17 on phakic IOLs)were analyzed,focusing on machine learning and deep learning techniques used for patient selection,surgical optimization,and complication prediction.Only English-language studies were included.Key Content and Findings:AI models utilizing structured tabular data,imaging,and multimodal inputs have demonstrated superior performance in predicting surgical outcomes and refining patient selection compared to traditional methods.Machine learning approaches such as random forests,extreme gradient boosting,and ensemble techniques,alongside deep learning architectures like convolutional neural networks and generative models,have improved risk assessment and surgical planning.In LVC,AI enhances ectasia risk assessment,keratoconus detection,and myopic regression prediction.In phakic IOL implantation,AI improves postoperative vault prediction,lens sizing,and refractive error estimation.Multimodal AI systems integrating imaging,textual data,and clinical parameters hold promise for more comprehensive patient evaluations.However,challenges such as data heterogeneity,limited external validation,and regulatory barriers hinder widespread clinical adoption.Conclusions:AI is transforming refractive surgery by enhancing precision,personalization,and patient safety.Its integration into clinical workflows has the potential to improve surgical outcomes and patient satisfaction.Future efforts should focus on advancing multimodal AI,improving model generalizability,and addressing ethical and regulatory challenges to fully harness AI’s potential in refractive surgery.
文摘Purpose: The aim of this systematic review was to answer the question “Which treatment protocol, am- ong classical methods and/or various laser applica- tions is the most effective in root canal disinfection, in vitro”. Materials and Methods: A MEDLINE, a Co- chrane and an Embase search (three specified search- es) were conducted to identify randomized controlled trials (RCT) until June 2010, conducted on human teeth and published in English, German or French language, examining the root canal disinfection after the use of lasers with or without mechanical instru-mentation. Additionally, hand search was conducted and contact with authors, when needed. Results: The MEDLINE, the Cochrane and the EMBASE search identified 240, 28, and 35 published articles, respec-tively. Ten articles from the MEDLINE and 5 articles from the Cochrane search (that were also identified in the MEDLINE search) met the inclusion and va-lidity assessment criteria. In E. faecalis elimination, instrumentation of the root canal and diode laser/665 nanometer/1 Watt (diode laser/665 nm/1 W) irradia-tion with the combined effect of Methylene Blue (MB) as photosensitizing agent (logCFU/ml = 1636) seemed to be the best method. In P. aeruginosa and in A.naeslundii elimination, instrumentation of the root canal followed by irrigation with 5, 25% NaOCl (log-CFU/ml = 0) seemed to be the best method. In gen-eral, instrumentation of the root canal followed by irrigation with 5, 25% NaOCl (logCFU/ml = 0) and instrumentation of the root canal and Er: YAG laser/ 2940 nm/0.8 W irradiation (logCFU/ml = 1924) seemed to be the best (polymicrobial studies). Conclusions: There are treatment protocols with the assistance or not of laser irradiation that can eliminate E. faecalis, E. coli and S. aureus inside the root canal. However, there is a serious number of S. anginosus, F. nuclea-tum, A. naeslundii and P. aeruginosa that remain in-side the root canal even after laser irradiation. New research is needed in order to set a treatment proto-col effective in the root canal disinfection from all bac-teria that are related to endodontic origin pathology.
文摘AIM:To assess the visual acuity and visual quality of cataract patients who received femtosecond laser-assisted cataract surgery(FLACS)and multifocal intraocular lens(MIOL)implantation with an improved polishing technique during a 1-year follow-up period.METHODS:This study included 74 eyes from 37 patients,comprising 17 males and 20 females,with a mean age of 51.74±7.80 years.Using a coin toss method,one eye per patient was randomly selected for improved anterior capsular polishing,while the other eye received standard irrigation/aspiration(I/A)polishing.The polishing group consisted of 37 eyes,including 21 right and 16 left eyes,while the control group comprised the contralateral fellow eyes of the same individuals in the polishing group.Visual acuity and quality of the patients were evaluated before surgery and at 1 wk,1,6,and 12 mo after surgery.The OPD-Scan III was utilized to assess high-order aberrations,while the optical quality analysis system(OQAS)was employed to evaluate modulation transfer function(MTF cutoff),Strehl ratio(SR),and objective scatter index(OSI)for the purpose of visual quality assessment.Paired t-tests and repeated measures analysis of variance(ANOVA)were utilized to compare the results,and the SNK-q post hoc test was applied to identify significant differences.RESULTS:The polishing group’s uncorrected distant visual acuity(UDVA)and uncorrected near visual acuity(UNVA)significantly improved 1-week post-surgery(all P<0.05).At 12-months,UDVA,UNVA,and corrected distant visual acuity(CDVA)were better than the control group(P<0.05).The MTF cutoff,SR,OSI,and high-order aberrations in the polishing group also showed significant improvements compared to the control group at 12 mo(P<0.05).CONCLUSION:The improved capsular polishing method has been demonstrated to effectively maintain visual acuity and visual quality in patients with MIOL after FLACS within 1 a.
文摘Objective: to investigate the risk factors and prevention methods of wound infection in emergency surgical trauma patients. Methods: a total of 150 patients admitted to our hospital from December 2018 to December 2019 were selected as the research subjects. The above patients were all emergency surgical trauma patients. The above patients were divided into groups according to whether they had incision infection. Among them, 25 patients in the research group ( infection group) and 125 cases in the control group (uninfected group), the general data of the two groups of patients, the location of trauma, the use of antibiotics and other factors were statistically analyzed. Results: after treatment, among the 150 patients participating in the study, 25 patients had incision infection after surgery, and the infection rate was 16.7%. Factors include the location and number of trauma, the use of preoperative antibiotics, and age. The patient is older than 60 years old, has multiple trauma or head trauma, and has not been treated with anti-inflammatory drugs before surgery. The above factors are related to postoperative wound infection. There was a statistically significant difference between the factors (P0.05). Conclusion: in the treatment of trauma patients in emergency surgery, some patients will have incision infection. Evaluation and preventive measures for patients, and disinfection treatment in strict accordance with the standards can reduce the probability of incision infection and better improve the treatment effect. Promote and apply in emergency surgery.

文摘A mathematical energy coupling model was developed to analyze the light transmission in the keyhole and energy distribution on the keyhole wall.The main characteristics of the model include:1) a prototype of the keyhole and the inverse Bremsstrahlung absorption coefficient in the keyhole plasma are obtained from the experiments;2) instead of using a parallel incident beam,a focused laser beam with real Gaussian intensity distribution is implemented;3) both Fresnel absorption and inverse Bremsstrahlung absorption during multiple reflections are considered.The calculation results show that the distribution of absorbed laser intensity by the keyhole wall is not uniform.The maximum laser energy is absorbed by the bottom of the keyhole,although no rays irradiate directly onto the bottom.According to analysis of beam focusing characteristics,the location of the focal plane plays a more important role in the laser energy absorption by the front wall than by the rear wall.
文摘Two-micron (thulium) laser resection of the prostate-tangerine technique (TmLRP-TT) is a transurethral procedure that uses a thulium laser fiber to dissect whole prostatic lobes off the surgical capsule, similar to peeling a tangerine. We recently reported the primary results. Here we introduce this procedure in detail. A 70-W, 2-um (thulium) laser was used in continuous-wave mode. We joined the incision by making a transverse cut from the level of the verumontanum to the bladder neck, making the resection sufficiently deep to reach the surgical capsule, and resected the prostate into small pieces, just like peeling a tangerine. As we resected the prostate, the pieces were vaporized, sufficiently small to be evacuated through the reseetoscope sheath, and the use of the mechanical tissue morcellator was not required. The excellent hemostasis of the thulium laser ensured the safety of TmLRP-TT. No patient required blood transfusion. Saline irrigation was used intraoperatively, and no case of transurethral resection syndrome was observed. The bladder outlet obstruction had clearly resolved after catheter removal in all cases. We designed the tangerine technique and proved it to be the most suitable procedure for the use of thulium laser in the treatment of benign prostatic hyperplasia (BPH). This procedure, which takes less operative time than standard techniques, is safe and combines efficient cutting and rapid organic vaporization, thereby showing the great superiority of the thulium fiber laser in the treatment of BPH. It has been proven to be as safe and efficient as transurethral resection of the prostate (TURP) during the 1-year follow-up.
文摘AIM: To compare the complication rate of femtosecond laser-assisted cataract surgery(FLACS) and traditional phacoemulsification for the first 18 mo of FLACS use at a private surgical center in Hawaii.METHODS: A retrospective chart review was conducted from January 2012 to June 2013. The first 273 consecutive eyes receiving FLACS and 553 eyes receiving traditional phacoemulsification were examined.All surgeries were performed at a single surgical center in Hawaii. The presence of intraoperative complications was used as the main outcome measure. Approval was obtained from the institutional review board of the University of Hawaii.RESULTS: The overall complication rate for FLACS was 1.8%, while that of the traditional procedure was5.8%(P 【0.05). A majority of the surgeons(80%) had a lower complication rate while using FLACS.CONCLUSION: FLACS is comparable in safety, if not safer, than traditional cataract surgery when performed by qualified cataract surgeons on carefully selected patients.

文摘The estimation of shear strength of rock mass discontinuity is always a focal, but difficult, problem in the field of geotechnical engineering. Considering the disadvantages and limitation of exist- ing estimation methods, a new approach based on the shadow area percentage (SAP) that can be used to quantify surface roughness is proposed in this article. Firstly, by the help of laser scanning technique, the three-dimensional model of the surface of rock discontinuity was established. Secondly, a light source was simulated, and there would be some shadows produced on the model surface. Thirdly, to obtain the value of SAP of each specimen, the shadow detection technique was introduced for use. Fourthly, compared with the result from direct shear testing and based on statistics, an empirical for- mula was found among SAP, normal stress, and shear strength. Data of Yujian (~ River were used as an example, and the following conclusions have been made. (1) In the case of equal normal stress, the peak shear stress is positively proportional to the SAP. (2) The formula for estimating was derived, and the predictions of peak-shear strength made with this equation well agreed with the experimental re- suits obtained in laboratory tests.

文摘In order to obtain a high-quality weld during the laser welding process, extracting the characteristic parameters of weld pool is an important issue for automated welding. In this paper, the type 304 austenitic stainless steel is welded by a 5 kW high-power fiber laser and a high-speed camera is employed to capture the topside images of weld pools. Then we propose a robust visual-detection approach for the molten pool based on the supervised descent method. It provides an elegant framework for representing the outline of a weld pool and is especially efficient for weld pool detection in the presence of strong uncertainties and disturbances. Finally, welding experimental results verified that the proposed approach can extract the weld pool boundary accurately, which will lay a solid foundation for controlling the weld quality of fiber laser welding process.
文摘Laser refractive surgery is one of the most commonly performed procedures worldwide.In laser refractive surgery,Femtosecond Laser in Situ Keratomileusis and Refractive Lenticule Extraction have emerged as promising alternatives to microkeratome Laser in Situ Keratomileusis and Photorefractive Keratectomy.Following laser refractive surgery,the corneal nerves,epithelial and stromal cells release neuromediators,including neurotrophins,neuropeptides and neurotransmitters.Notably,nerve growth factor,substance P,calcitonin gene-related peptide and various cytokines are important mediators of neurogenic inflammation and corneal nerve regeneration.Alterations in neuromediator profiles and ocular surface parameters following laser refractive surgery are attributed to the surgical techniques and the severity of tissue insult induced.In this review,we will discuss the(1)Functions of neuromediators and their physiological and clinical significance;(2)Changes in the neuromediators following various laser refractive surgeries;(3)Correlation between neuromediators,ocular surface health and corneal nerve status;and(4)Future directions,including the use of neuromediators as potential biomarkers for ocular surface health following laser refractive surgery,and as adjuncts to aid in corneal regeneration after laser refractive surgery.
文摘AIM To describe the performance of 450 nm diode laser in oral surgery procedures.METHODS The case described consisted of the removal of a lower lip fibroma through a blue diode laser(λ = 450 nm). RESULTS The efficacy of this device, even at very low power(1W, CW), allows us to obtain very high intra and postoperative comfort for the patient, even with just topical anaesthesia and without needing suture. The healing process was completed in one week and, during the follow-up, the patient did not report any problems, pain or discomfort even without the consumption of any kind of drugs, such as painkillers and antibiotics. The histological examination performed by the pathologist showed a large area of fibrous connective tissue with some portions of epithelium-connective detachments and a regular incision with very scanty areas of carbonization.CONCLUSION The 450 nm diode laser proved of being very efficient in the oral soft tissue surgical procedures, with no side effects for the patients.
文摘AIM: To evaluate human lens epithelium cell apoptosis and epithelial to mesenchymal transition (EMT) induced by femtosecond laser in femtosecond laser assisted cataract surgery (FLACS). METHODS: Sixty cataract patients with N2 to N3 stage according to the LOCS III were enrolled in this study and divided into three groups randomly: FLACSl group (cataract surgery by FLACS with LenSx), FLACS2 group (cataract surgery by FLACS with LensAR) and manual group (cataract surgery by phacoemulsification). Patients in two FLACS groups performed anterior capsulotomy by LenSx or LensAR laser system. Patients in the manual group were performed continuous curvilinear capsulorrhexis (CCC) manually. The anterior capsules were fixed right after moved out of eye. Hematoxylin-eosine staining, immunofluorescence staining and real-time PCR were performed in order to observe human lens epithelium cells changes after cataract surgery. RESULTS: The capsule cutting edge was shown irregularity and roughness in two FLACS groups and smooth edge in manual capsulotomy by pathologic staining. Irregularities of the cell configuration with partly swollen and destroyed nuclei were observed in two FLACS groups. Femtosecond laser could induce a significantly higher cell apoptosis in human lens epithelium cell than manually performed CCC (P〈0.05). Lens epithelium cells apoptosis were correlated with femtosecond laser duration according to Pearson correlation analysis. Decreased N-cadherin expression, alpha-SMA and FSP-1 level in two FLACS groups showed the inhibition of cell EMT. CONCLUSION: Femtosecond laser may affect the apoptosis and EMT of lens epithelium cells which are under the peeled central lens capsule.
文摘In this paper, the generalized nonlinear Schrodinger equation (GNLSE) is solved by an adaptive split-step Fourier method (ASSFM). It is found that ASSFM must be used to solve GNLSE to ensure precision when the soliton selffrequency shift is remarkable and the photonic crystal fibre (PCF) parameters vary with the frequency considerably. The precision of numerical simulation by using ASSFM is higher than that by using split-step Fourier method in the process of laser pulse propagation in PCFs due to the fact that the variation of fibre parameters with the peak frequency in the pulse spectrum can be taken into account fully.

文摘AIM:To evaluate corneal astigmatic outcomes of femtosecond laser-assisted arcuate keratotomies(FAKs)combined with femtosecond-laser assisted cataract surgery(FLACS)over 12mo follow-up.METHODS:Totally 145 patients with bilateral cataracts and no ocular co-morbidities were recruited to a singlecentre,single-masked,prospective randomized controlled trial(RCT)comparing two monofocal hydrophobic acrylic intraocular lenses.Eyes with corneal astigmatism(CA)of>0.8 dioptres(D)received unpaired,unopened,surface penetrating FAKs at the time of FLACS.Visual acuity,subjective refraction and Scheimpflug tomography were recorded at 1,6,and 12mo.Alpins vectoral analyses were performed.RESULTS:Fifty-one patients(61 eyes),mean age 68.2±9.6y[standard deviation(SD)],received FAKs.Sixty eyes were available for analysis,except at 12mo when 59 attended.There were no complications due to FAKs.Mean pre-operative CA was 1.13±0.20 D.There was a reduction of astigmatism at all post-operative visits(residual CA 1mo:0.85±0.42 D,P=0.0001;6mo:0.86±0.35 D,P=0001;and 12mo:0.90±0.39,P=0.0001).Alpins indices remained stable over 12mo.Overall,the cohort was under-corrected at all time points.At 12mo,61%of eyes were within±15 degrees of pre-operative astigmatic meridian.CONCLUSION:Unpaired unopened penetrating FAKs combined with on-axis phacoemulsification are safe but minimally effective.CA is largely under-corrected in this cohort using an existing unmodified nomogram.The effect of arcuate keratotomies on CA remained stable over 12mo.

文摘Fabrication of micro gratings using a femtosecond laser exposure system is experimentally investigated for the electron moire method. Micro holes and lines are firstly etched for parameter study. Grating profile is theoretically optimized to form high quality moire patterns. For a demonstration, a parallel grating is fabricated on a specimen of quartz glass. The minimum line width and the distance between two adjacent lines are both set to be 1 μm, and the frequency of grating is 500 lines/ram. The experimental results indicate that the quality of gratings is good and the relative error of the gratings pitch is about 1.5%. Based on molte method, scanning electron microscope (SEM) moire patterns are observed clearly, which manifests that gratings fabricated with the femtosecond laser exposure is suitable for micro scale deformation measurement.
文摘We report spectroscopic studies on plasma electron number density of laser-induced plasma produced by ns-Nd:YAG laser light pulses on an aluminum sample in air at atmospheric pressure. The effect of different laser energy and the effect of different laser wavelengths were compared. The experimentally observed line profiles of neutral aluminum have been used to extract the excitation temperature using the Boltzmann plot method, whereas the electron number density has been determined from the Stark broadened as well as using the Saha-Boltzmann equation (SBE). Each approach was also carried out by using the AI emission line and Mg emission lines. It was observed that the,SBE method generated a little higher electron number density value than the Stark broadening, method, but within the experimental uncertainty range. Comparisons of Ne determined by the two methods show the presence of a linear relation which is independent of laser energy or laser wavelength. These results show the applicability of the SBE method for Are determination, especially when the system does not have any pure emission lines whose electron impact factor is known, Also use of Mg lines gives superior results than Al lines.

文摘A fully non-contact experimental platform for ultrasonic Lamb wave damage detection was constructed,where laser exciting and the scanning laser Doppler vibrometer were used to realize the high-resolution pickup of the Lamb wave field in the structure,which has overcome the disadvantages of low spatial resolution caused by the conventional contact Lamb wave transducer.In order to suppress the dispersion effect of broadband laser-ultrasonic signal,we proposed time-domain filtering in multi-band method based on wavelet analysis to decompose the broadband signal into multiple narrowband ones and separate the scattering signals effectively without reference signal.On this basis,the total focusing method(TFM)was used for damage imaging.However,when the traditional TFM was applied to image based on ultrasonic Lamb wave,the inherent dispersion characteristic of ultrasonic Lamb wave could lead to the miscalculation of time delay,thus reducing the imaging precision.Therefore,the frequency-domain TFM was developed by applying phase delay in the frequency domain.The logical AND was introduced to synthesize the damage imaging results of multiple narrowband signals to obtain high-precision damage imaging.Our study has shown that the method of time-domain filtering in multi-band combining with frequency-domain TFM can realize non-contact and accurate damage detection in isotropic plate structures,and it is a potential effective method for application in engineering practice.
文摘AIM:To investigate the clinical efficacy and safety of femtosecond laser-assisted steepest-meridian clear corneal incisions for correcting preexisting corneal astigmatism performed at the time of cataract surgery.METHODS:This prospective case series study comprised consecutive age-related cataract patients with corneal regular astigmatism(range:+0.75 to+2.50 D)who had femtosecond laser-assisted steepest-meridian clear corneal incisions(single or paired).Corneal astigmatism was performed with the Pentacam preoperatively and 3 mo postoperatively.Total corneal astigmatism and steepestmeridian measured in the 3-mm central zone were used to guide the location,size and number of clear corneal incision.The vector analysis of astigmatic change was performed using the Alpins method.RESULTS:Totally 138 eyes of 138 patients were included.The mean preoperative corneal astigmatism was 1.31±0.41 D,and was significantly reduced to 0.69±0.34 D(equivalent to difference vector)after surgery(P<0.01).The surgically-induced astigmatism was 1.02±0.54 D.The correction index(ratio of target induced astigmatism and surgically-induced astigmatism:0.72±0.36)as well as the magnitude of error(difference between surgically-induced astigmatism and target induced astigmatism:-0.29±0.51)represented a slight under correction.For angle of error,the arithmetic mean was 1.11±13.70,indicating no significant systematic alignment errors.CONCLUSION:Femtosecond-assisted steepest-meridian clear corneal incision is a fast,customizable,adjustable,precise,and safe technique for the reduction of low to moderate corneal astigmatism during cataract surgery.
