Climate change and anthropogenic activities have profoundly affected coastal systems,making geomorphological research a critical focus for coastal protection and sustainable development.In this study,a comprehensive c...Climate change and anthropogenic activities have profoundly affected coastal systems,making geomorphological research a critical focus for coastal protection and sustainable development.In this study,a comprehensive classification of beach states around Hainan Island is conducted for the first time by utilizing theΩ-RTR model and geological control modes.Six distinct classic beach states ranging from dissipative to reflective are identified:barred dissipative beaches or no-barred dissipative beaches(BD or NBD),barred beaches(B),low-tide terrace or low-tide bar with rip(LTTR or LTBR),and reflective state(R).Among these,the BD and B types are predominant on Hainan Island.Notably,the beach states are subject to multiple factors,such as hydrodynamic forcings,geomorphic features and underlying substrates,and exhibit remarkable spatiotemporal variability.During extreme events,hydrodynamic forcings impact beach states more substantially than geological and geomorphic features do,leading to a more homogeneous distribution of beach states.Under normal circumstance,beach states are predominantly controlled by geological and geomorphic features.Coastal geological and geomorphic features have a pronounced influence on beach morphology and stability.For example,hard substrates underpin wide and stable dissipative beaches,whereas softer substrates lead to narrower,erosion-prone beaches.Three geological control modes are identified,namely,gently sloping hard substrates with dissipative beaches,moderately sloping hard substrates with seasonally variable reflective beaches,and steeply sloping soft substrates with dynamic sandbar-dominated beaches.These findings highlight the necessity of integrating geological settings in tandem with hydrodynamic forcings into coastal management practices.A dual-mode strategy is proposed:maintaining geomorphic self-organization on hard-substrate coasts under normal conditions and implementing hybrid engineering–ecological measures(e.g.,artificial sand replenishment and vegetation restoration)on erosion-prone soft substrates.展开更多

Background:Combined hepatocellular-cholangiocarcinoma(CHC)is a rare subtype of primary hepatic malignancies,with variably reported incidence between 0.4%–14.2%of primary liver cancer cases.This study aimed to systema...Background:Combined hepatocellular-cholangiocarcinoma(CHC)is a rare subtype of primary hepatic malignancies,with variably reported incidence between 0.4%–14.2%of primary liver cancer cases.This study aimed to systematically review the epidemiological,clinicopathological,diagnostic and therapeutic data for this rare entity.Data sources:We reviewed the literature of diagnostic approach of CHC with special reference to its clinical,molecular and histopathological characteristics.Additional analysis of the recent literature in order to evaluate the results of surgical and systemic treatment of this entity has been accomplished.Results:The median age at CHC’s diagnosis appears to be between 50 and 75 years.Evaluation of tumor markers[alpha fetoprotein(AFP),carbohydrate antigen 19–9(CA19–9)and carcinoembryonic antigen(CEA)]along with imaging patterns provides better opportunities for CHC’s preoperative diagnosis.Reported clinicopathologic prognostic parameters possibly correlated with increased tumor recurrence and grimmer survival odds include advanced age,tumor size,nodal and distal metastases,vascular and regional organ invasion,multifocality,decreased capsule formation,stem-cell features verification and increased GGT as well as CA19–9 and CEA levels.In case of inoperable or recurrent disease,combinations of cholangiocarcinoma-directed systemic agents display superior results over sorafenib.Liver-directed methods,such as transarterial chemoembolization(TACE),percutaneous ethanol injection(PEI),hepatic arterial infusion chemotherapy(HAIC),radioembolization and ablative therapies,demonstrate inferior efficacy than in cases of hepatocellular carcinoma(HCC)due to CHC’s common hypovascularity.Conclusions:CHC demonstrates an overlapping clinical and biological pattern between its malignant ingredients.Natural history of the disease seems to be determined by the predominant tumor element.Gold standard for diagnosis is histology of surgical specimens.Regarding therapeutic interventions,major hepatectomy is acknowledged as the cornerstone of treatment whereas minor hepatectomy and liver transplantation may be applied in patients with advanced cirrhosis.Despite all therapeutic attempts,prognosis of CHC remains dismal.展开更多
Diabetic foot ulceration is a devastating complication of diabetes that is associated with infection,amputation,and death,and is affecting increasing numbers of patients with diabetes mellitus.The pathogenesis of foot...Diabetic foot ulceration is a devastating complication of diabetes that is associated with infection,amputation,and death,and is affecting increasing numbers of patients with diabetes mellitus.The pathogenesis of foot ulcers is complex,and different factors play major roles in different stages.The refractory nature of foot ulcer is reflected in that even after healing there is still a high recurrence rate and amputation rate,which means that management and nursing plans need to be considered carefully.The importance of establishment of measures for prevention and management of DFU has been emphasized.Therefore,a validated and appropriate DFU classification matching the progression is necessary for clinical diagnosis and management.In the first part of this review,we list several commonly used classification systems and describe their application conditions,scope,strengths,and limitations;in the second part,we briefly introduce the common risk factors for DFU,such as neuropathy,peripheral artery disease,foot deformities,diabetes complications,and obesity.Focusing on the relationship between the risk factors and DFU progression may facilitate prevention and timely management;in the last part,we emphasize the importance of preventive education,characterize several of the most frequently used management approaches,including glycemic control,exercise,offloading,and infection control,and call for taking into account and weighing the quality of life during the formulation of treatment plans.Multidisciplinary intervention and management of diabetic foot ulcers(DFUs)based on the effective and systematic combination of these three components will contribute to the prevention and treatment of DFUs,and improve their prognosis.展开更多

Method: The fractures were classified based on the age of the patient, unilateral/bilateral, location on the condyle, presence of displacement and dislocation, for those displaced, whether there was medial or lateral overlap, and features presented. Treatment done for each patient was documented. Both clinical and radiological assessments were done to ascertain the outcome of treatment. Result: 11 patients presented with 14 condyle fractures, 3 patients with bilateral and 8 with unilateral condyle fractures out of which 5 cases were on the right side. Age range of patients was between 13 and 44 years with a mean (SD) of 25.3 (10.7) years. Nine (81.8%) of the patients were males and 2 (18.2%) were females. Eight (72.7%) of the patients with condyle fracture had associated fractures affecting other sites of the mandible while 3 (27.3%) patients had isolated condyle fractures. Intracapusular fractures recorded were 2 (14.2%), while extracapsular accounted for 12 (85.8%) cases. Conservative treatment was not applied in any patient, 9 (81.8%) patients had IMF and 2 (18.2%) patients had ORIF. Conclusion: Most fractures of the condyle were extracapsular and, closed surgical treatment (IMF) was very useful to manage most of the cases.展开更多

In its 2023 global health statistics,the World Health Organization noted that noncommunicable diseases(NCDs)remain the leading cause of disease burden worldwide,with cardiovascular diseases(CVDs)resulting in more deat...In its 2023 global health statistics,the World Health Organization noted that noncommunicable diseases(NCDs)remain the leading cause of disease burden worldwide,with cardiovascular diseases(CVDs)resulting in more deaths than the three other major NCDs combined.In this study,we developed a method that can comprehensively detect which CVDs are present in a patient.Specifically,we propose a multi-label classification method that utilizes photoplethysmography(PPG)signals and physiological characteristics from public datasets to classify four types of CVDs and related conditions:hypertension,diabetes,cerebral infarction,and cerebrovascular disease.Our approach to multi-disease classification of cardiovascular diseases(CVDs)using PPG signals achieves the highest classification performance when encompassing the broadest range of disease categories,thereby offering a more comprehensive assessment of human health.We employ a multi-label classification strategy to simultaneously predict the presence or absence of multiple diseases.Specifically,we first apply the Savitzky-Golay(S-G)filter to the PPG signals to reduce noise and then transform into statistical features.We integrate processed PPG signals with individual physiological features as a multimodal input,thereby expanding the learned feature space.Notably,even with a simple machine learning method,this approach can achieve relatively high accuracy.The proposed method achieved a maximum F1-score of 0.91,minimum Hamming loss of 0.04,and an accuracy of 0.95.Thus,our method represents an effective and rapid solution for detecting multiple diseases simultaneously,which is beneficial for comprehensively managing CVDs.展开更多
Network traffic classification is a crucial research area aimed at improving quality of service,simplifying network management,and enhancing network security.To address the growing complexity of cryptography,researche...Network traffic classification is a crucial research area aimed at improving quality of service,simplifying network management,and enhancing network security.To address the growing complexity of cryptography,researchers have proposed various machine learning and deep learning approaches to tackle this challenge.However,existing mainstream methods face several general issues.On one hand,the widely used Transformer architecture exhibits high computational complexity,which negatively impacts its efficiency.On the other hand,traditional methods are often unreliable in traffic representation,frequently losing important byte information while retaining unnecessary biases.To address these problems,this paper introduces the Swin Transformer architecture into the domain of network traffic classification and proposes the NetST(Network Swin Transformer)model.This model improves the Swin Transformer to better accommodate the characteristics of network traffic,effectively addressing efficiency issues.Furthermore,this paper presents a traffic representation scheme designed to extract meaningful information from large volumes of traffic while minimizing bias.We integrate four datasets relevant to network traffic classification for our experiments,and the results demonstrate that NetST achieves a high accuracy rate while maintaining low memory usage.展开更多

A cluster...Due to global warming, extreme weather and climate events are becoming more frequent, highlighting the need to explore the changing characteristics of precipitation in China, including extreme precipitation. A clustering algorithm was developed to classify summer(June, July, and August) daily precipitation in China from 1961 to 2020, considering spatial distribution, standard deviations, and frequency of extreme precipitation events. The results reveal six distinct precipitation climate zones, a classification that differs from previous divisions. While overall precipitation has decreased in most regions, the frequency of extreme precipitation events has increased across all clusters, indicating a shift in precipitation distribution patterns. Analysis shows that the weakened Lake Baikal blocking high and strengthened Mongolian cyclone influence the arid region in northwest China(Cluster 1), which is characterized by the lowest precipitation.The transition zone between the monsoon and arid region(Cluster 2) is affected by the Mongolian cyclone, water vapor transport from the Indian Ocean, and shifts in the monsoon boundary. Clusters 3 and 4 represent areas associated with advancement and retreat of the summer monsoon. In the Meiyu region, two distinct subregions have been identified exist.Cluster 4 is primarily influenced by the East Asia-Pacific wave train. Despite sharing similar climate drivers and proximity,Clusters 4 and 5 differ significantly due to topographic variations and disparate levels of urbanization. Cluster 5 exhibits a higher average precipitation, greater variability, and more frequent extreme events. Cluster 6 exhibits the highest overall precipitation in the coastal areas of Guangdong and Guangxi, where abundant water vapor contributes to a higher frequency of extreme precipitation. In addition, anthropogenic activities and urbanization significantly influence precipitation in Beijing-Tianjin-Hebei and Yangtze River Delta regions. This research proposes a precipitation classification scheme integrating multiple precipitation parameters, providing support for risk management and mitigation strategies in the face of increasing extreme precipitation events.展开更多
Invasive alien species(IAS)significantly threaten global biodiversity and ecosystem stability.Despite increasing management efforts,a critical knowledge gap existed in understanding commonalities and disparities among...Invasive alien species(IAS)significantly threaten global biodiversity and ecosystem stability.Despite increasing management efforts,a critical knowledge gap existed in understanding commonalities and disparities among national strategies.We analyzed several IAS management lists from 23 countries and the European Union,focusing specifically on vascular plant species within these lists.List composition,characteristics,and associated management measures were analyzed.Key patterns in species prioritization across national lists and intercontinental exchange of invasive alien plants(IAPs)were identified.Pistia stratiotes,Pontederia crassipes,Salvinia molesta,Cabomba caroliniana,Ulex europaeus were identified as globally recognized threats,being listed by at least 33.3%of analyzed countries and invading five or more continents.Aquatic plants were found to be more frequently included in management lists.A significant directional invasion pattern between the Eastern and Western Hemispheres was identified.Species native to Asia were observed to dominate as significant donors of IAPs across continents.The analysis of list management strategies highlighted substantial gaps in achieving Target 6 of the Kunming-Montreal Global Biodiversity Framework,particularly in species prioritization and inclusion of potential IAPs.In response to these challenges,a tiered classification system for invasive alien species list was proposed,encompassing High-Priority,Watchlist,Potential,and Priority Site categories,which aimed at enhancing management effectiveness by tailoring strategies to different invasion stages and ecological contexts.This study could contribute to understanding global IAPs management strategies and serve as a reference for policymakers and conservation managers to identify priority IAPs and refine management approaches.展开更多
With the increasing integration of emerging source-load types such as distributed photovoltaics,electric vehicles,and energy storage into distribution networks,the operational characteristics of these networks have ev...With the increasing integration of emerging source-load types such as distributed photovoltaics,electric vehicles,and energy storage into distribution networks,the operational characteristics of these networks have evolved from traditional single-load centers to complex multi-source,multi-load systems.This transition not only increases the difficulty of effectively classifying distribution networks due to their heightened complexity but also renders traditional energy management approaches-primarily focused on economic objectives-insufficient to meet the growing demands for flexible scheduling and dynamic response.To address these challenges,this paper proposes an adaptive multi-objective energy management strategy that accounts for the distinct operational requirements of distribution networks with a high penetration of new-type source-loads.The goal is to establish a comprehensive energy management framework that optimally balances energy efficiency,carbon reduction,and economic performance in modern distribution networks.To enhance classification accuracy,the strategy constructs amulti-dimensional scenario classification model that integrates environmental and climatic factors by analyzing the operational characteristics of new-type distribution networks and incorporating expert knowledge.An improved split-coupling K-means preclustering algorithm is employed to classify distribution networks effectively.Based on the classification results,fuzzy logic control is then utilized to dynamically optimize the weighting of each objective,allowing for an adaptive adjustment of priorities to achieve a flexible and responsivemulti-objective energy management strategy.The effectiveness of the proposed approach is validated through practical case studies.Simulation results indicate that the proposed method improves classification accuracy by 18.18%compared to traditional classification methods and enhances energy savings and carbon reduction by 4.34%and 20.94%,respectively,compared to the fixed-weight strategy.展开更多

The incidence of ERCP-related complications is 5%-10%, most commonly involving post-ERCP pancreatitis and clinically significant post-endoscopic sphincterotomy bleeding. Although ERCP-related perforation has a relatively lower incidence of 0.14%-1.6%, this complication is associated with a high mortality rate of 4.2%-29.6%. A classification of perforation type based on the instrument that caused the perforation was recently described that we postulated could affect the implementation of perforation management. In the present article, an algorithm for management and prevention of ERCP-related perforations is proposed that is based on the perforation type and delay of diagnosis. Available evidence demonstrates that a delayed diagnosis and/or treatment of perforation re-sults in a poorer prognosis, and thus should be at the forefront of procedural consideration. Furthermore, this review provides steps and recommendations from the pre-procedural stage through the post-procedural evaluation with consideration of contributing factors in order to minimize ERCP-related complication risk and improve patient outcome. To avoid perforation, endoscopists must evaluate the risks related to the individual patient and the procedure and perform the procedure gently. Once a perforation occurs, immediate diagnosis and early management are key factors to minimize mortality.展开更多
Machine learning algorithms are an important measure with which to perform landslide susceptibility assessments, but most studies use GIS-based classification methods to conduct susceptibility zonation.This study pres...Machine learning algorithms are an important measure with which to perform landslide susceptibility assessments, but most studies use GIS-based classification methods to conduct susceptibility zonation.This study presents a machine learning approach based on the C5.0 decision tree(DT) model and the K-means cluster algorithm to produce a regional landslide susceptibility map. Yanchang County, a typical landslide-prone area located in northwestern China, was taken as the area of interest to introduce the proposed application procedure. A landslide inventory containing 82 landslides was prepared and subsequently randomly partitioned into two subsets: training data(70% landslide pixels) and validation data(30% landslide pixels). Fourteen landslide influencing factors were considered in the input dataset and were used to calculate the landslide occurrence probability based on the C5.0 decision tree model.Susceptibility zonation was implemented according to the cut-off values calculated by the K-means cluster algorithm. The validation results of the model performance analysis showed that the AUC(area under the receiver operating characteristic(ROC) curve) of the proposed model was the highest, reaching 0.88,compared with traditional models(support vector machine(SVM) = 0.85, Bayesian network(BN) = 0.81,frequency ratio(FR) = 0.75, weight of evidence(WOE) = 0.76). The landslide frequency ratio and frequency density of the high susceptibility zones were 6.76/km^(2) and 0.88/km^(2), respectively, which were much higher than those of the low susceptibility zones. The top 20% interval of landslide occurrence probability contained 89% of the historical landslides but only accounted for 10.3% of the total area.Our results indicate that the distribution of high susceptibility zones was more focused without containing more " stable" pixels. Therefore, the obtained susceptibility map is suitable for application to landslide risk management practices.展开更多
Internet of Things(IoT)defines a network of devices connected to the internet and sharing a massive amount of data between each other and a central location.These IoT devices are connected to a network therefore prone...Internet of Things(IoT)defines a network of devices connected to the internet and sharing a massive amount of data between each other and a central location.These IoT devices are connected to a network therefore prone to attacks.Various management tasks and network operations such as security,intrusion detection,Quality-of-Service provisioning,performance monitoring,resource provisioning,and traffic engineering require traffic classification.Due to the ineffectiveness of traditional classification schemes,such as port-based and payload-based methods,researchers proposed machine learning-based traffic classification systems based on shallow neural networks.Furthermore,machine learning-based models incline to misclassify internet traffic due to improper feature selection.In this research,an efficient multilayer deep learning based classification system is presented to overcome these challenges that can classify internet traffic.To examine the performance of the proposed technique,Moore-dataset is used for training the classifier.The proposed scheme takes the pre-processed data and extracts the flow features using a deep neural network(DNN).In particular,the maximum entropy classifier is used to classify the internet traffic.The experimental results show that the proposed hybrid deep learning algorithm is effective and achieved high accuracy for internet traffic classification,i.e.,99.23%.Furthermore,the proposed algorithm achieved the highest accuracy compared to the support vector machine(SVM)based classification technique and k-nearest neighbours(KNNs)based classification technique.展开更多
Classification management is one of nature reserves management system in China. But state nature reserves and local administration nature reserves under the regulations are only the approval system and embody the concept of protection. Management pattern of nature reserves can be divided into nine types. There are big differences in the nine patterns in management foundation and coordination ability of management because different management pattern occupies different resources of administration,personnel,financial and law enforcement. By analyzing management pattern,thirty-eight indexes in thirteen categories were selected to evaluate the management effectiveness of national nature reserves subordinate to the State Forestry Administration (SFA) . Results show that the management effectiveness of national nature reserves is good as a whole,and the management effectiveness is direct proportional to administration level. Provincial administration has the higher efficiency than municipal and county administration. Direct administration by governments at all levels has the higher efficiency than departments' administration at the same level.展开更多

文摘Climate change and anthropogenic activities have profoundly affected coastal systems,making geomorphological research a critical focus for coastal protection and sustainable development.In this study,a comprehensive classification of beach states around Hainan Island is conducted for the first time by utilizing theΩ-RTR model and geological control modes.Six distinct classic beach states ranging from dissipative to reflective are identified:barred dissipative beaches or no-barred dissipative beaches(BD or NBD),barred beaches(B),low-tide terrace or low-tide bar with rip(LTTR or LTBR),and reflective state(R).Among these,the BD and B types are predominant on Hainan Island.Notably,the beach states are subject to multiple factors,such as hydrodynamic forcings,geomorphic features and underlying substrates,and exhibit remarkable spatiotemporal variability.During extreme events,hydrodynamic forcings impact beach states more substantially than geological and geomorphic features do,leading to a more homogeneous distribution of beach states.Under normal circumstance,beach states are predominantly controlled by geological and geomorphic features.Coastal geological and geomorphic features have a pronounced influence on beach morphology and stability.For example,hard substrates underpin wide and stable dissipative beaches,whereas softer substrates lead to narrower,erosion-prone beaches.Three geological control modes are identified,namely,gently sloping hard substrates with dissipative beaches,moderately sloping hard substrates with seasonally variable reflective beaches,and steeply sloping soft substrates with dynamic sandbar-dominated beaches.These findings highlight the necessity of integrating geological settings in tandem with hydrodynamic forcings into coastal management practices.A dual-mode strategy is proposed:maintaining geomorphic self-organization on hard-substrate coasts under normal conditions and implementing hybrid engineering–ecological measures(e.g.,artificial sand replenishment and vegetation restoration)on erosion-prone soft substrates.

文摘Objective expertise evaluation of individuals,as a prerequisite stage for team formation,has been a long-term desideratum in large software development companies.With the rapid advancements in machine learning methods,based on reliable existing data stored in project management tools’datasets,automating this evaluation process becomes a natural step forward.In this context,our approach focuses on quantifying software developer expertise by using metadata from the task-tracking systems.For this,we mathematically formalize two categories of expertise:technology-specific expertise,which denotes the skills required for a particular technology,and general expertise,which encapsulates overall knowledge in the software industry.Afterward,we automatically classify the zones of expertise associated with each task a developer has worked on using Bidirectional Encoder Representations from Transformers(BERT)-like transformers to handle the unique characteristics of project tool datasets effectively.Finally,our method evaluates the proficiency of each software specialist across already completed projects from both technology-specific and general perspectives.The method was experimentally validated,yielding promising results.
文摘Background:Combined hepatocellular-cholangiocarcinoma(CHC)is a rare subtype of primary hepatic malignancies,with variably reported incidence between 0.4%–14.2%of primary liver cancer cases.This study aimed to systematically review the epidemiological,clinicopathological,diagnostic and therapeutic data for this rare entity.Data sources:We reviewed the literature of diagnostic approach of CHC with special reference to its clinical,molecular and histopathological characteristics.Additional analysis of the recent literature in order to evaluate the results of surgical and systemic treatment of this entity has been accomplished.Results:The median age at CHC’s diagnosis appears to be between 50 and 75 years.Evaluation of tumor markers[alpha fetoprotein(AFP),carbohydrate antigen 19–9(CA19–9)and carcinoembryonic antigen(CEA)]along with imaging patterns provides better opportunities for CHC’s preoperative diagnosis.Reported clinicopathologic prognostic parameters possibly correlated with increased tumor recurrence and grimmer survival odds include advanced age,tumor size,nodal and distal metastases,vascular and regional organ invasion,multifocality,decreased capsule formation,stem-cell features verification and increased GGT as well as CA19–9 and CEA levels.In case of inoperable or recurrent disease,combinations of cholangiocarcinoma-directed systemic agents display superior results over sorafenib.Liver-directed methods,such as transarterial chemoembolization(TACE),percutaneous ethanol injection(PEI),hepatic arterial infusion chemotherapy(HAIC),radioembolization and ablative therapies,demonstrate inferior efficacy than in cases of hepatocellular carcinoma(HCC)due to CHC’s common hypovascularity.Conclusions:CHC demonstrates an overlapping clinical and biological pattern between its malignant ingredients.Natural history of the disease seems to be determined by the predominant tumor element.Gold standard for diagnosis is histology of surgical specimens.Regarding therapeutic interventions,major hepatectomy is acknowledged as the cornerstone of treatment whereas minor hepatectomy and liver transplantation may be applied in patients with advanced cirrhosis.Despite all therapeutic attempts,prognosis of CHC remains dismal.

文摘Diabetic foot ulceration is a devastating complication of diabetes that is associated with infection,amputation,and death,and is affecting increasing numbers of patients with diabetes mellitus.The pathogenesis of foot ulcers is complex,and different factors play major roles in different stages.The refractory nature of foot ulcer is reflected in that even after healing there is still a high recurrence rate and amputation rate,which means that management and nursing plans need to be considered carefully.The importance of establishment of measures for prevention and management of DFU has been emphasized.Therefore,a validated and appropriate DFU classification matching the progression is necessary for clinical diagnosis and management.In the first part of this review,we list several commonly used classification systems and describe their application conditions,scope,strengths,and limitations;in the second part,we briefly introduce the common risk factors for DFU,such as neuropathy,peripheral artery disease,foot deformities,diabetes complications,and obesity.Focusing on the relationship between the risk factors and DFU progression may facilitate prevention and timely management;in the last part,we emphasize the importance of preventive education,characterize several of the most frequently used management approaches,including glycemic control,exercise,offloading,and infection control,and call for taking into account and weighing the quality of life during the formulation of treatment plans.Multidisciplinary intervention and management of diabetic foot ulcers(DFUs)based on the effective and systematic combination of these three components will contribute to the prevention and treatment of DFUs,and improve their prognosis.
文摘While the incidence of gastric cancer(GC)in general has decreased worldwide in recent decades,the incidence of diffuse cancer historically comprising poorly cohesive cells-GC(PCC-GC)and including signet ring cell cancer is rising.Literature concerning PCC-GC is scarce and unclear,mostly due to a large variety of historically used definitions and classifications.Compared to other histological subtypes of GC,PCC-GC is nevertheless characterized by a distinct set of epidemiological,histological and clinical features which require a specific diagnostic and therapeutic approach.The aim of this review was to provide an update on the definition,classification and therapeutic strategies of PCC-GC.We focus on the updated histological definition of PCC-GC,along with its implications on future treatment strategies and study design.Also,specific considerations in the diagnostic management are discussed.Finally,the impact of some recent developments in the therapeutic management of GC in general such as the recently validated taxane-based regimens(5-Fluorouracil,leucovorin,oxaliplatin and docetaxel),the use of hyperthermic intraperitoneal chemotherapy as well as pressurized intraperitoneal aerosol chemotherapy and targeted therapy have been reviewed in depth for their relative importance for PCC-GC in particular.
文摘Background: Condyle fractures are not common but could lead to detrimental effects of growth disturbance of the mandible, ankylosis of temporomandibular joint and facial asymmetry especially in children, if not promptly and adequately managed, the aim of this study was to document our experience in the management of mandibular condyle fractures. Method: The fractures were classified based on the age of the patient, unilateral/bilateral, location on the condyle, presence of displacement and dislocation, for those displaced, whether there was medial or lateral overlap, and features presented. Treatment done for each patient was documented. Both clinical and radiological assessments were done to ascertain the outcome of treatment. Result: 11 patients presented with 14 condyle fractures, 3 patients with bilateral and 8 with unilateral condyle fractures out of which 5 cases were on the right side. Age range of patients was between 13 and 44 years with a mean (SD) of 25.3 (10.7) years. Nine (81.8%) of the patients were males and 2 (18.2%) were females. Eight (72.7%) of the patients with condyle fracture had associated fractures affecting other sites of the mandible while 3 (27.3%) patients had isolated condyle fractures. Intracapusular fractures recorded were 2 (14.2%), while extracapsular accounted for 12 (85.8%) cases. Conservative treatment was not applied in any patient, 9 (81.8%) patients had IMF and 2 (18.2%) patients had ORIF. Conclusion: Most fractures of the condyle were extracapsular and, closed surgical treatment (IMF) was very useful to manage most of the cases.

文摘The primary intent of the current research is to provide insights regarding the management of spare parts within the supply chain,in conjunction with offering some methods for enhancing forecasting and inventory management.In particular,to use classical forecasting methods,the use of weak and unstable demand is not recommended.Furthermore,statistical performance measures are not involved in this particular context.Furthermore,it is expected that maintenance contracts will be aligned with different levels.In addition to the examination of some literature reviews,some tools will guide us through this process.The article proposes new performance analysis methods that will help integrate inventory management and statistical performance while considering decision maker priorities through the use of different methodologies and parts age segmentation.The study will also identify critical level policies by comparing different types of spenders according to the inventory management model,also with separate and common inventory policies.Each process of the study is combined with a comparative analysis of different forecasting methods and inventory management models based on N.A.C.C.parts supply chain data,allowing us to identify a set of methodologies and parameter recommendations based on parts segmentation and supply chain prioritization.
文摘With the widespread use of upper gastrointestinal endoscopy,more and more gastric polyps(GPs)are being detected.Traditional management strategies often rely on histopathologic examination,which can be time-consuming and may not guide immediate clinical decisions.This paper aims to introduce a novel classification system for GPs based on their potential risk of malignant transformation,categorizing them as"good","bad",and"ugly".A review of the literature and clinical case analysis were conducted to explore the clinical implications,management strategies,and the system's application in endoscopic practice.Good polyps,mainly including fundic gland polyps and inflammatory fibrous polyps,have a low risk of malignancy and typically require minimal or no intervention.Bad polyps,mainly including hyperplastic polyps and adenomas,pose an intermediate risk of malignancy,necessitating closer monitoring or removal.Ugly polyps,mainly including type 3 neuroendocrine tumors and early gastric cancer,indicate a high potential for malignancy and require urgent and comprehensive treatment.The new classification system provides a simplified and practical framework for diagnosing and managing GPs,improving diagnostic accuracy,guiding individualized treatment,and promoting advancements in endoscopic techniques.Despite some challenges,such as the risk of misclassification due to similar endoscopic appearances,this system is essential for the standardized management of GPs.It also lays the foundation for future research into biomarkers and the development of personalized medicine.

文摘In its 2023 global health statistics,the World Health Organization noted that noncommunicable diseases(NCDs)remain the leading cause of disease burden worldwide,with cardiovascular diseases(CVDs)resulting in more deaths than the three other major NCDs combined.In this study,we developed a method that can comprehensively detect which CVDs are present in a patient.Specifically,we propose a multi-label classification method that utilizes photoplethysmography(PPG)signals and physiological characteristics from public datasets to classify four types of CVDs and related conditions:hypertension,diabetes,cerebral infarction,and cerebrovascular disease.Our approach to multi-disease classification of cardiovascular diseases(CVDs)using PPG signals achieves the highest classification performance when encompassing the broadest range of disease categories,thereby offering a more comprehensive assessment of human health.We employ a multi-label classification strategy to simultaneously predict the presence or absence of multiple diseases.Specifically,we first apply the Savitzky-Golay(S-G)filter to the PPG signals to reduce noise and then transform into statistical features.We integrate processed PPG signals with individual physiological features as a multimodal input,thereby expanding the learned feature space.Notably,even with a simple machine learning method,this approach can achieve relatively high accuracy.The proposed method achieved a maximum F1-score of 0.91,minimum Hamming loss of 0.04,and an accuracy of 0.95.Thus,our method represents an effective and rapid solution for detecting multiple diseases simultaneously,which is beneficial for comprehensively managing CVDs.

文摘Network traffic classification is a crucial research area aimed at improving quality of service,simplifying network management,and enhancing network security.To address the growing complexity of cryptography,researchers have proposed various machine learning and deep learning approaches to tackle this challenge.However,existing mainstream methods face several general issues.On one hand,the widely used Transformer architecture exhibits high computational complexity,which negatively impacts its efficiency.On the other hand,traditional methods are often unreliable in traffic representation,frequently losing important byte information while retaining unnecessary biases.To address these problems,this paper introduces the Swin Transformer architecture into the domain of network traffic classification and proposes the NetST(Network Swin Transformer)model.This model improves the Swin Transformer to better accommodate the characteristics of network traffic,effectively addressing efficiency issues.Furthermore,this paper presents a traffic representation scheme designed to extract meaningful information from large volumes of traffic while minimizing bias.We integrate four datasets relevant to network traffic classification for our experiments,and the results demonstrate that NetST achieves a high accuracy rate while maintaining low memory usage.

文摘Due to global warming, extreme weather and climate events are becoming more frequent, highlighting the need to explore the changing characteristics of precipitation in China, including extreme precipitation. A clustering algorithm was developed to classify summer(June, July, and August) daily precipitation in China from 1961 to 2020, considering spatial distribution, standard deviations, and frequency of extreme precipitation events. The results reveal six distinct precipitation climate zones, a classification that differs from previous divisions. While overall precipitation has decreased in most regions, the frequency of extreme precipitation events has increased across all clusters, indicating a shift in precipitation distribution patterns. Analysis shows that the weakened Lake Baikal blocking high and strengthened Mongolian cyclone influence the arid region in northwest China(Cluster 1), which is characterized by the lowest precipitation.The transition zone between the monsoon and arid region(Cluster 2) is affected by the Mongolian cyclone, water vapor transport from the Indian Ocean, and shifts in the monsoon boundary. Clusters 3 and 4 represent areas associated with advancement and retreat of the summer monsoon. In the Meiyu region, two distinct subregions have been identified exist.Cluster 4 is primarily influenced by the East Asia-Pacific wave train. Despite sharing similar climate drivers and proximity,Clusters 4 and 5 differ significantly due to topographic variations and disparate levels of urbanization. Cluster 5 exhibits a higher average precipitation, greater variability, and more frequent extreme events. Cluster 6 exhibits the highest overall precipitation in the coastal areas of Guangdong and Guangxi, where abundant water vapor contributes to a higher frequency of extreme precipitation. In addition, anthropogenic activities and urbanization significantly influence precipitation in Beijing-Tianjin-Hebei and Yangtze River Delta regions. This research proposes a precipitation classification scheme integrating multiple precipitation parameters, providing support for risk management and mitigation strategies in the face of increasing extreme precipitation events.

文摘Invasive alien species(IAS)significantly threaten global biodiversity and ecosystem stability.Despite increasing management efforts,a critical knowledge gap existed in understanding commonalities and disparities among national strategies.We analyzed several IAS management lists from 23 countries and the European Union,focusing specifically on vascular plant species within these lists.List composition,characteristics,and associated management measures were analyzed.Key patterns in species prioritization across national lists and intercontinental exchange of invasive alien plants(IAPs)were identified.Pistia stratiotes,Pontederia crassipes,Salvinia molesta,Cabomba caroliniana,Ulex europaeus were identified as globally recognized threats,being listed by at least 33.3%of analyzed countries and invading five or more continents.Aquatic plants were found to be more frequently included in management lists.A significant directional invasion pattern between the Eastern and Western Hemispheres was identified.Species native to Asia were observed to dominate as significant donors of IAPs across continents.The analysis of list management strategies highlighted substantial gaps in achieving Target 6 of the Kunming-Montreal Global Biodiversity Framework,particularly in species prioritization and inclusion of potential IAPs.In response to these challenges,a tiered classification system for invasive alien species list was proposed,encompassing High-Priority,Watchlist,Potential,and Priority Site categories,which aimed at enhancing management effectiveness by tailoring strategies to different invasion stages and ecological contexts.This study could contribute to understanding global IAPs management strategies and serve as a reference for policymakers and conservation managers to identify priority IAPs and refine management approaches.

文摘With the increasing integration of emerging source-load types such as distributed photovoltaics,electric vehicles,and energy storage into distribution networks,the operational characteristics of these networks have evolved from traditional single-load centers to complex multi-source,multi-load systems.This transition not only increases the difficulty of effectively classifying distribution networks due to their heightened complexity but also renders traditional energy management approaches-primarily focused on economic objectives-insufficient to meet the growing demands for flexible scheduling and dynamic response.To address these challenges,this paper proposes an adaptive multi-objective energy management strategy that accounts for the distinct operational requirements of distribution networks with a high penetration of new-type source-loads.The goal is to establish a comprehensive energy management framework that optimally balances energy efficiency,carbon reduction,and economic performance in modern distribution networks.To enhance classification accuracy,the strategy constructs amulti-dimensional scenario classification model that integrates environmental and climatic factors by analyzing the operational characteristics of new-type distribution networks and incorporating expert knowledge.An improved split-coupling K-means preclustering algorithm is employed to classify distribution networks effectively.Based on the classification results,fuzzy logic control is then utilized to dynamically optimize the weighting of each objective,allowing for an adaptive adjustment of priorities to achieve a flexible and responsivemulti-objective energy management strategy.The effectiveness of the proposed approach is validated through practical case studies.Simulation results indicate that the proposed method improves classification accuracy by 18.18%compared to traditional classification methods and enhances energy savings and carbon reduction by 4.34%and 20.94%,respectively,compared to the fixed-weight strategy.

文摘Endoscopic retrograde cholangiopancreatography(ERCP) is a procedure that can result in serious complications, and thus should be handled by a skilled endoscopist to minimize the risk of complications and to enhance the success rate. The incidence of ERCP-related complications is 5%-10%, most commonly involving post-ERCP pancreatitis and clinically significant post-endoscopic sphincterotomy bleeding. Although ERCP-related perforation has a relatively lower incidence of 0.14%-1.6%, this complication is associated with a high mortality rate of 4.2%-29.6%. A classification of perforation type based on the instrument that caused the perforation was recently described that we postulated could affect the implementation of perforation management. In the present article, an algorithm for management and prevention of ERCP-related perforations is proposed that is based on the perforation type and delay of diagnosis. Available evidence demonstrates that a delayed diagnosis and/or treatment of perforation re-sults in a poorer prognosis, and thus should be at the forefront of procedural consideration. Furthermore, this review provides steps and recommendations from the pre-procedural stage through the post-procedural evaluation with consideration of contributing factors in order to minimize ERCP-related complication risk and improve patient outcome. To avoid perforation, endoscopists must evaluate the risks related to the individual patient and the procedure and perform the procedure gently. Once a perforation occurs, immediate diagnosis and early management are key factors to minimize mortality.

文摘Machine learning algorithms are an important measure with which to perform landslide susceptibility assessments, but most studies use GIS-based classification methods to conduct susceptibility zonation.This study presents a machine learning approach based on the C5.0 decision tree(DT) model and the K-means cluster algorithm to produce a regional landslide susceptibility map. Yanchang County, a typical landslide-prone area located in northwestern China, was taken as the area of interest to introduce the proposed application procedure. A landslide inventory containing 82 landslides was prepared and subsequently randomly partitioned into two subsets: training data(70% landslide pixels) and validation data(30% landslide pixels). Fourteen landslide influencing factors were considered in the input dataset and were used to calculate the landslide occurrence probability based on the C5.0 decision tree model.Susceptibility zonation was implemented according to the cut-off values calculated by the K-means cluster algorithm. The validation results of the model performance analysis showed that the AUC(area under the receiver operating characteristic(ROC) curve) of the proposed model was the highest, reaching 0.88,compared with traditional models(support vector machine(SVM) = 0.85, Bayesian network(BN) = 0.81,frequency ratio(FR) = 0.75, weight of evidence(WOE) = 0.76). The landslide frequency ratio and frequency density of the high susceptibility zones were 6.76/km^(2) and 0.88/km^(2), respectively, which were much higher than those of the low susceptibility zones. The top 20% interval of landslide occurrence probability contained 89% of the historical landslides but only accounted for 10.3% of the total area.Our results indicate that the distribution of high susceptibility zones was more focused without containing more " stable" pixels. Therefore, the obtained susceptibility map is suitable for application to landslide risk management practices.

文摘Internet of Things(IoT)defines a network of devices connected to the internet and sharing a massive amount of data between each other and a central location.These IoT devices are connected to a network therefore prone to attacks.Various management tasks and network operations such as security,intrusion detection,Quality-of-Service provisioning,performance monitoring,resource provisioning,and traffic engineering require traffic classification.Due to the ineffectiveness of traditional classification schemes,such as port-based and payload-based methods,researchers proposed machine learning-based traffic classification systems based on shallow neural networks.Furthermore,machine learning-based models incline to misclassify internet traffic due to improper feature selection.In this research,an efficient multilayer deep learning based classification system is presented to overcome these challenges that can classify internet traffic.To examine the performance of the proposed technique,Moore-dataset is used for training the classifier.The proposed scheme takes the pre-processed data and extracts the flow features using a deep neural network(DNN).In particular,the maximum entropy classifier is used to classify the internet traffic.The experimental results show that the proposed hybrid deep learning algorithm is effective and achieved high accuracy for internet traffic classification,i.e.,99.23%.Furthermore,the proposed algorithm achieved the highest accuracy compared to the support vector machine(SVM)based classification technique and k-nearest neighbours(KNNs)based classification technique.
文摘Classification management is one of nature reserves management system in China. But state nature reserves and local administration nature reserves under the regulations are only the approval system and embody the concept of protection. Management pattern of nature reserves can be divided into nine types. There are big differences in the nine patterns in management foundation and coordination ability of management because different management pattern occupies different resources of administration,personnel,financial and law enforcement. By analyzing management pattern,thirty-eight indexes in thirteen categories were selected to evaluate the management effectiveness of national nature reserves subordinate to the State Forestry Administration (SFA) . Results show that the management effectiveness of national nature reserves is good as a whole,and the management effectiveness is direct proportional to administration level. Provincial administration has the higher efficiency than municipal and county administration. Direct administration by governments at all levels has the higher efficiency than departments' administration at the same level.
