AIM:To find out whether a newly designed big cup nitinol stent is suitable for treatment of patients with gastric outlet obstruction resulting from gastric cancer.METHODS:The new stent is composed of a proximal big cup segment(20 mm in length and 48-55 mm in diameter),a middle part(60 mm in length and 20 mm in diameter)covered by a polyethylene membrane and a distal sphericity(20 mm in length and 28 mm in diameter).Half of the proximal big cup segment is also covered by a polyethlene membrane,which is adjacent to the middle part of the stent.The stent is preloaded in a 6.0-mm-diameter introducer system.Thirteen patients with gastric outlet obstruction resulting from gastric cancer received the new stents under endoscopic and fluoroscopic guidance.RESULTS:Technical success was achieved in 12 of 13(92.3%)patients.Among the 12 patients in whom endoscopic stent was placed successfully,the clinical success rate was 91.7%during a follow-up of average 6.5 mo.During the first month follow-up,the migration rate was 0%,recurrent obstruction 0%and gastric bleeding 8.3%.During the follow-up between 2-12 mo,no migration,recurrent obstruction and gastric bleeding occurred.CONCLUSION:The proximal big cup segment seems to be effective and promising for technical efficacy,clinical outcome,and preventing migration and tumor ingrowth and increasing the emptying rate of sinus ventriculi. 展开更多

One-stage human acellular nerve allograft reconstruction for digital nerve defects

Human acellular nerve allografts have a wide range of donor origin and can effectively avoid nerve injury in the donor area. Very little is known about one-stage reconstruction of digital nerve defects. The present study observed the feasibility and effectiveness of human acellular nerve allograft in the reconstruction of 〈 5-cm digital nerve defects within 6 hours after injury. A total of 15 cases of nerve injury, combined with nerve defects in 18 digits from the Department of Emergency were enrolled in this study. After dehridement, digital nerves were reconstructed using human acellular nerve allografts. The patients were followed up for 6-24 months after reconstruction. Mackinnon-Dellon static two-point discrimination results showed excellent and good rates of 89%. Semmes-Weinstein monofilament test demonstrated that light touch was normal, with an obvious improvement rate of 78%. These findings confirmed that human acellular nerve allograft for one-stage reconstruction of digital nerve defect after hand injury is feasible, which provides a novel trend for peripheral nerve reconstruction. 展开更多

Destruction resistance such as start-up wear resistance characteristics of vitamin E(dl-α-tocopherol)blended ultrahigh molecular weight polyethylene(UHMWPE)were evaluated using the destruction energy index(DEI).The DEI is used to evaluate wear-like destruction,by minimising the effects of viscosity at the sliding interface,and is estimated using newly designed friction testers.In this experiment,silicone oils of different viscosities were used to determine micro displacement up to a start-up point,and the DEI of the UHMWPE was changed from 17.48 to 1.84μJ by adding vitamin E.The results suggest that the blending with vitamin E reduces destruction of UHMWPE at start-up friction in silicone oils. 展开更多

Industry effect of job hopping: an agent-based simulation of Chinese construction workers

Job hopping affects the development of industries in terms of efficiency and quality of work. It is a problem for the Chinese construction industry, where excessive job hopping is detrimental to meeting the current daunting challenges involved in the industry's transformation and efficiency improvement. To provide an exhaustive analysis of this effect, game theory is combined with social relationship networks to create an agent-based simulation model. Simulation results indicate that the frequent job moves of Chinese construction workers have a negative effect on their skill development, employment, and worker relationships, as well as results in sharp increase in employer labor costs. The findings point to the need to act for the benefit of workers and employers and maintain the development of the industry. 展开更多

高压电脉冲致裂煤岩增透技术研究进展与展望

我国大部分煤层低透气性的特点严重制约了煤层气高效开采,高压电脉冲作为一种安全、环保且重复可控的煤岩致裂增透技术,在煤层增透领域展现了广阔的应用前景。结合业内多年研究成果,回顾了高压电脉冲技术的基本概念及发展历程,剖析了该... 我国大部分煤层低透气性的特点严重制约了煤层气高效开采,高压电脉冲作为一种安全、环保且重复可控的煤岩致裂增透技术,在煤层增透领域展现了广阔的应用前景。结合业内多年研究成果,回顾了高压电脉冲技术的基本概念及发展历程,剖析了该技术的致裂增透机制,并围绕放电参数、煤岩电学特性、煤岩矿物分布及孔裂隙特征、赋存条件4个方面系统梳理了高压电脉冲煤岩致裂增透理论的研究进展。研究表明:2015年以来,高压电脉冲技术研究热度显著上升,其典型作用机制包括液电效应和电破碎效应;高压电脉冲击穿煤岩过程可分为电场加载、极化、击穿和稳定4个阶段,煤岩孔隙结构在应力波作用下经历了扩孔、破裂、贯穿和扩展裂隙的演化模式,从而提高了渗透性;放电参数决定裂隙网络的优化程度,煤岩电学特性影响击穿难度与能量传递,矿物分布及孔裂隙特征控制等离子体通道的形成与裂纹扩展,地应力与气体压力的耦合作用则共同调控煤岩裂隙演化与渗透性变化。基于上述分析,指出了该领域发展存在的主要问题,包括信息获取困难、致裂增透机制尚不明确及击穿场强较高等,展望“十五五”乃至更长时期,认为该领域未来发展的着力点在于:①发展高压电脉冲致裂煤岩的实时监测技术;②构建高精度的热流固化电多物理场耦合模型;③探索高压电脉冲协同多技术致裂煤岩增透。以期突破现有瓶颈,推动该技术的进一步发展。 展开更多

降水预报机器学习订正及其在分类型降水测试的效果研究?

强降水是对我国国计民生影响最为广泛的灾害性天气之一,其精准预报也是最具挑战的科学问题之一,湿物理过程的多尺度、非线性特征使数值预报和客观订正处理降水问题比处理一般气象要素困难得多。文章以京津冀地区3 h累计降水量为对象,基... 强降水是对我国国计民生影响最为广泛的灾害性天气之一,其精准预报也是最具挑战的科学问题之一,湿物理过程的多尺度、非线性特征使数值预报和客观订正处理降水问题比处理一般气象要素困难得多。文章以京津冀地区3 h累计降水量为对象,基于站点观测和数值模式预报,通过降水样本构建和采样、降水相关物理特征输入、使用残差训练等策略,探索了机器学习算法LightGBM应用于降水预报订正的效果。结果显示,面对降水样本的长尾分布难题,构建数据集时综合考虑保持降水的真实分布,同时适度增大较强量级降水样本的比例,是提升强量级降水订正效果的关键一环。独立测试集的统计检验表明,LightGBM订正方案在所有阈值降水的客观评分均较原模式预报显著提升,且提升率随降水阈值增大而增加。分类型降水的统计检验和个例评估显示,LightGBM订正方案在不同类型降水预报均体现出强度和落区的综合调整,其中:强天气系统强迫类型降水样本数量相对占优,在各降水阈值订正效果均较显著;而弱天气系统强迫类型降水订正在≥15 mm阈值时较难获得提升,落区的调整也更具挑战。这说明降水样本的不平衡分布仍是机器学习订正面临的挑战,但机器学习体现出对模式预报准确率更低的较大量级降水的订正潜力,即模式预报能力越低,机器学习订正提升的空间和幅度越大。特征重要性分析表明,输入与降水密切相关的动力、热力和水汽等物理特征,对提升LightGBM订正评分具有正贡献。 展开更多
