This study presented a simulation-based two-stage interval-stochastic programming (STIP) model to support water resources management in the Kaidu-Konqi watershed in Northwest China. The modeling system coupled a distributed hydrological model with an interval two-stage stochastic programing (ITSP). The distributed hydrological model was used for establishing a rainfall-runoff forecast system, while random parameters were pro- vided by the statistical analysis of simulation outcomes water resources management planning in Kaidu-Konqi The developed STIP model was applied to a real case of watershed, where three scenarios with different water re- sources management policies were analyzed. The results indicated that water shortage mainly occurred in agri- culture, ecology and forestry sectors. In comparison, the water demand from municipality, industry and stock- breeding sectors can be satisfied due to their lower consumptions and higher economic values. Different policies for ecological water allocation can result in varied system benefits, and can help to identify desired water allocation plans with a maximum economic benefit and a minimum risk of system disruption under uncertainty. 展开更多

Due to complicated kinetics of polymerization,differential/algebraic equations(DAEs) always cause great computational burden and system non-linearity usually makes GTTO non-convex bearing multiple optima. Therefore, coupled with the three-stage decomposition model, a three-section algorithm of dynamic programming(TSDP) is proposed based on the general iteration mechanism of iterative programming(IDP) and incorporated with adaptivegrid allocation scheme and heuristic modifications. The algorithm iteratively performs dynamic programming with heuristic modifications under constant calculation loads and adaptively allocates the valued computational resources to the regions that can further improve the optimality under the guidance of local error estimates. TSDP is finally compared with IDP and interior point method(IP) to verify its efficiency of computation. 展开更多

Consideration of the Self-Care Supporting Program Including Simple Exercise for Patients with Early Stages of Secondary Lower-Limb Lymphedema

Aim: We devised a self-care supporting program targeting patients with early stages of secondary lower-limb lymphedema. The program incorporates “Simple exercises to replace lymphatic drainage” based on lymphatic flow. The purpose of this study was to consider the feasibility of continuing this program. Methods: The participants were patients in the early stages of secondary lower-limb lymphedema after gynecological cancer surgery and lymphedema therapists with more than five years of experience. Patients continued self-care at home after being briefed on the program, and they were analyzed on their self-care continuity status one month later based on a self-administered questionnaire survey and self-care notes. We interviewed the lymphedema therapists about this program to discuss the feasibility of continuing it and obtain feedback. Results: The patients who participated in the study were six women who underwent surgery with lymph node dissection for gynecological cancer. The therapists were five nurses and one occupational therapist. The patient understood the importance of all items in the self-care. “Observation,” “Touching,” and “Skin care” were relatively easy to continue. “Lymph drainage” and “Exercise” were continued with “Simple exercises to replace lymphatic drainage”. “Other exercises” were able to continue by adding distance and time to daily activities. Furthermore, “Measurement” and “Recording” became a burden and were difficult to continue. Nonetheless, this program was generally approved by the lymphedema therapists. They also pointed out the content and format of the self-care notes as improvements. Conclusions: We found that the self-care supporting program that incorporated “Simple exercises to replace lymphatic drainage” might be feasible to continue self-care for patients with early stages of secondary lower-limb lymphedema. Furthermore, we found that we needed to improve the “Measurement” and “Recording” sections of this program. 展开更多

目的:探讨基于信息-知识-信念-行为(IKAP)理论引领的定制化健康管理方案对腹膜透析(PD)患者自我认知与希望感重塑的实证研究。方法:选取2023年3月至2024年3月郑州大学第一附属医院肾脏内科收治的90例PD患者作为研究对象。根据管理方法... 目的:探讨基于信息-知识-信念-行为(IKAP)理论引领的定制化健康管理方案对腹膜透析(PD)患者自我认知与希望感重塑的实证研究。方法:选取2023年3月至2024年3月郑州大学第一附属医院肾脏内科收治的90例PD患者作为研究对象。根据管理方法的不同,分为观察组45例和对照组45例。对照组给予常规管理,观察组加用IKAP理论引领的定制化健康管理方案。比较两组患者遵医行为、自我认知能力、希望水平及并发症发生情况。结果:干预后,观察组患者按时透析、容量控制、正确护理腹膜透析管路及维持血压正常例数显著高于对照组(χ^(2)=6.049、5.414、3.920、8.459,P<0.05)。观察组患者健康知识水平、自我概念、自护技能及自护责任感显著高于对照组(t=8.172、6.581、9.644、7.575,P<0.05)。观察组希望水平评分显著高于对照组(t=9.437、9.526、11.196、11.985,P<0.05)。观察组患者总并发率6.67%显著低于对照组22.22%(χ^(2)=4.406,P<0.05)。结论:IKAP理论引领的定制化健康管理方案对PD患者能够提高患者遵医行为和自我管理能力,提升患者希望水平,降低并发症风险。 展开更多

Oil and gas pipeline networks are a key link in the coordinated development of oil and gas both upstream and downstream.To improve the reliability and safety of the oil and gas pipeline network, inspections are implemented to minimize the risk of leakage, spill and theft, as well as documenting actual incidents. In recent years, unmanned aerial vehicles have been recognized as a promising option for inspection due to their high efficiency. However, the integrated optimization of unmanned aerial vehicle inspection for oil and gas pipeline networks, including physical feasibility, the performance of mission, cooperation, real-time implementation and three-dimensional(3-D) space, is a strategic problem due to its large-scale,complexity as well as the need for efficiency. In this work, a novel mixed-integer nonlinear programming model is proposed that takes into account the constraints of the mission scenario and the safety performance of unmanned aerial vehicles. To minimize the total length of the inspection path, the model is solved by a two-stage solution method. Finally, a virtual pipeline network and a practical pipeline network are set as two examples to demonstrate the performance of the optimization schemes. Moreover, compared with the traditional genetic algorithm and simulated annealing algorithm, the self-adaptive genetic simulated annealing algorithm proposed in this paper provides strong stability. 展开更多

We present an approximation-exact penalty function method for solving the single stage stochastic programming problem with continuous random variable. The original problem is transformed into a determinate nonlinear programming problem with a discrete random variable sequence, which is obtained by some discrete method. We construct an exact penalty function and obtain an unconstrained optimization. It avoids the difficulty in solution by the rapid growing of the number of constraints for discrete precision. Under lenient conditions, we prove the equivalence of the minimum solution of penalty function and the solution of the determinate programming, and prove that the solution sequences of the discrete problem converge to a solution to the original problem. 展开更多

In this study, an interval probability-based inexact two-stage stochastic (IP-ITSP) model is developed for environmental pollutants control and greenhouse gas (GHG) emissions reduction management in regional energy system under uncertainties. In the IP-ITSP model, methods of interval probability, interval-parameter programming (IPP) and two-stage stochastic programming (TSP) are introduced into an integer programming framework;the developed model can tackle uncertainties described in terms of interval values and interval probability distributions. The developed model is applied to a case of planning GHG -emission mitigation in a regional electricity system, demonstrating that IP-ITSP is applicable to reflecting complexities of multi-uncertainty, and capable of addressing the problem of GHG-emission reduction. 4 scenarios corresponding to different GHG -emission mitigation levels are examined;the results indicates that the model could help decision makers identify desired GHG mitigation policies under various economic costs and environmental requirements. 展开更多

目的比较瑞戈非尼联合程序性死亡受体-1(PD-1)抑制剂、瑞戈非尼单药二线治疗中晚期肝细胞癌(HCC)的效果和安全性。方法2018年1月—2023年4月新疆医科大学第一附属医院行二线治疗的中晚期HCC患者61例,31例应用瑞戈非尼联合PD-1抑制剂者... 目的比较瑞戈非尼联合程序性死亡受体-1(PD-1)抑制剂、瑞戈非尼单药二线治疗中晚期肝细胞癌(HCC)的效果和安全性。方法2018年1月—2023年4月新疆医科大学第一附属医院行二线治疗的中晚期HCC患者61例,31例应用瑞戈非尼联合PD-1抑制剂者为联合组,30例应用瑞戈非尼者为单药组,治疗期间酌情行经动脉化疗栓塞(TACE)治疗。随访至2024年4月,比较2组客观缓解率(ORR)、疾病控制率(DCR),记录治疗期间不良事件(TRAEs)发生情况;绘制Kaplan-Meier生存曲线,采用log-rank法比较分析2组总生存期(OS)、无进展生存期(PFS)。结果(1)联合组瑞戈非尼起始剂量80 mg(45.20%)、TACE辅助治疗比率(25.80%)及瑞戈非尼治疗周期[(7.258±6.287)个]与单药组[46.70%、16.70%、(4.733±3.903)个]比较差异均无统计学意义(χ^(2)=7.136,P=0.028;χ^(2)=0.759,P=0.384;t=-1.877,P=0.065)。(2)末次随访时,联合组DCR(64.50%)高于单药组(40.00%)(χ^(2)=4.731,P=0.030),ORR(29.00%)与单药组(13.30%)比较差异无统计学意义(χ^(2)=2.241,P=0.134)。(3)联合组中位PFS[7个月(95%CI:5.58~8.43)]长于单药组[4个月(95%CI:1.35~6.65)](P=0.012),中位OS[15个月(95%CI:7.79~22.21)]与单药组[11个月(95%CI:7.85~14.15)]比较差异无统计学意义(P=0.134)。(4)2组均未发生Ⅳ级TRAEs,联合组治疗期间总TRAEs发生率(77.42%)、Ⅲ级TRAEs发生率(6.45%)与单药组(60.00%、3.33%)比较差异均无统计学意义(χ^(2)=2.157,P=0.142;χ^(2)=0.317,P=0.573)。结论与瑞戈非尼单药治疗相比,中晚期HCC患者二线治疗应用瑞戈非尼联合PD-1抑制剂方案可提高ORR,延长PFS,TRAEs可耐受。 展开更多

The Usefulness of Dynamic Programming in Course Allocation in the Nigerian Universities

Having lectured in some universities and polytechnics in Nigeria, the researchers observed problems in course allocations. There are no lay-down techniques on how courses should be allocated with respect to the minimum and maximum credit a lecturer should carry in a semester. Many lecturers were overloaded while others were under-loaded. For this reason, dynamic programming model was developed for allocating courses among lecturers in the Nigerian universities using the Department of Statistics, Federal University of Technology Owerri, as a case study. From our analysis, we observed that among all the optimal allocations discovered in the study, the best optimal allocation policy was achieved at the point (1, 2, 1, 2). Allocation of courses in this order will yield an optimal credit hour of 12 per lecturer per semester. 展开更多

AIM:To investigate if and how programmed death type-1(PD-1)expression affects the natural course of hepatitis B virus(HBV)infection. METHODS:Sixty-four patients in different natural stages of chronic HBV infection were enrolled in this study.PD-1 expression in total T cells was detected by flow cytometry.Levels of total CD8+T cell responses and proliferation in relation to PD-1 expression levels were analyzed with intracellular staining and PD-1/ PD-L1 blockage. RESULTS:The PD-1 expression in T cells was dynamically changed during the natural course of chronic HBV infection,did not significantly increase in the immune tolerance phase,and returned to normal in the inactive virus carrier stage.Blockage of the PD-1/PD-L1 pathway could not affect the T-cell response in the immune tolerance and inactive virus carrier stages of chronic HBV infection.However,it could significantly restore the T-cell response in the immune clearance stage of chronic HBV infection.Furthermore,the PD-1 expression level in T cells was associated with the alanine aminotransferase level during the immune clearance stage of chronic HBV infection. CONCLUSION:The PD-l/PD-L1 pathway plays a different role in T-cell response during the natural course of chronic HBV infection. 展开更多

These are Geoffrion and Graves [1], Sharma [2], Sharma [3] and Sharma and Berry [4]. In this paper we give a “vertical decomposition” approach to solve SSCWLP that uses Lagrangian relaxation. This way SSCWLP is broken into two versions of capacitated plant location problem (the CPLP_L and CPLP_R) by relaxing the flow balance constraints. For CPLP_R, we use well known Lagrangian relaxations given in literature (Christofides and Beasley [5] and Nauss [6]);and adopt them suitably for solving CPLP_L. We show theoretically in this paper that SSCWLP can be more efficiently solved by techniques of vertical decomposition developed in this paper than the method available in literature (Sharma and Berry [4]). 目的:探讨程序性死亡受体1(PD-1)抑制剂联合减剂量化疗对晚期肺癌患者免疫功能及血小板的影响。方法:选取77例晚期肺癌患者为研究对象,根据化疗方案不同分为对照组(n=38)和观察组(n=39)。对照组患者给予紫杉醇+顺铂常规剂量化疗;观察组... 目的:探讨程序性死亡受体1(PD-1)抑制剂联合减剂量化疗对晚期肺癌患者免疫功能及血小板的影响。方法:选取77例晚期肺癌患者为研究对象,根据化疗方案不同分为对照组(n=38)和观察组(n=39)。对照组患者给予紫杉醇+顺铂常规剂量化疗;观察组患者给予PD-1抑制剂(特瑞普利单抗)联合紫杉醇+顺铂减剂量化疗,1个周期为21 d,均治疗3个周期。比较两组患者临床疗效[客观有效率(ORR)、疾病控制率(DCR)]、治疗前后免疫细胞水平[CD3^(+)细胞、CD8^(+)细胞毒性T细胞(CD8^(+)CTL)、自然杀伤细胞(NK)]和血小板(PLT)水平,并比较不良反应发生情况及预后情况[无进展生存期(PFS)和总生存期(OS)]。结果:两组患者ORR、DCR比较,差异无统计学意义(P>0.05)。治疗后,对照组患者CD3^(+)、CD8^(+)CTL、NK细胞水平与治疗前比较,差异无统计学意义(P>0.05);观察组患者CD3^(+)细胞、CD8^(+)CTL、NK细胞水平均较治疗前升高(P<0.05),且高于对照组(P<0.05);两组患者PLT水平均降低(P<0.05),但观察组高于对照组(P<0.05)。两组患者白细胞减少、神经毒性、肝损害、脱发发生率比较,差异无统计学意义(P>0.05);观察组患者Ⅰ~Ⅱ级胃肠道反应、Ⅲ~Ⅳ级贫血发生率低于对照组(P<0.05)。观察组患者PFS、OS长于对照组(P<0.05)。结论:PD-1抑制剂联合减剂量化疗可增强晚期肺癌患者免疫功能,稳定PLT水平,改善预后。 展开更多

目的探究仑伐替尼、程序性死亡受体1(programmed death-1,PD-1)抑制剂联合经肝动脉化疗栓塞术用于中晚期肝癌患者治疗的效果。方法选取高州市人民医院2022年7月至2024年5月收治的40例中晚期肝癌患者,采用双盲法分为对照组和观察组,每组2... 目的探究仑伐替尼、程序性死亡受体1(programmed death-1,PD-1)抑制剂联合经肝动脉化疗栓塞术用于中晚期肝癌患者治疗的效果。方法选取高州市人民医院2022年7月至2024年5月收治的40例中晚期肝癌患者,采用双盲法分为对照组和观察组,每组20例。对照组给予经肝动脉化疗栓塞术治疗,观察组在对照组基础上加用仑伐替尼、PD-1抑制剂联合治疗,比较两组临床疗效、肝功能指标及不良反应情况。结果治疗后,观察组的有效率为75.00%,高于对照组(40.00%),差异有显著性(P<0.05)。疗前后,两组患者的丙氨酸氨基转移酶、总胆红素、天冬氨酸氨基转移酶及甲胎蛋白水平比较均无显著差异(P>0.05)。治疗后,观察组患者的甲胎蛋白水平低于对照组,差异有显著性(P<0.001)。治疗后,观察组患者的皮疹、腹泻发生率高于对照组,差异有显著性(P<0.05),但两组患者的消化道出血、高血压、蛋白尿发生率比较无显著差异(P>0.05)。结论经肝动脉化疗栓塞术联合仑伐替尼、PD-1抑制剂治疗有助于提升中晚期肝癌患者疾病治疗效果,降低患者甲胎蛋白水平,且治疗安全性较高。 展开更多
