Co-designing Approaches to Sustainable Exercise Care for People with Metabolic Dysfunction-associated Steatotic Liver Disease

Background and Aims:Regular exercise is fundamental for people with metabolic dysfunction-associated steatotic liver disease(MASLD),yet exercise maintenance is generally poor.This generative co-design process aimed to embed the voices and opinions of people with lived experience of MASLD and their care stakeholders to(i)frame barriers and enablers to exercise maintenance and(ii)highlight priorities for exercise-focused research agendas in MASLD.Methods:A generative co-design framework was applied.Two virtual co-design sessions were undertaken:Session 1–Framing the issue,where initial discovery was conducted with people with lived experience of MASLD;and Session 2–Generative design and sharing ideas with lived experience partners and healthcare stakeholders.Sessions were audio-recorded and transcribed,and key determinants and considerations were discerned by two independent researchers.Results:Lived experience partners(n=5,53±16 years,40%male)ranked five equally important barriers to exercise maintenance:musculoskeletal and pain issues,lack of access to exercise equipment/facilities,cost,competing priorities,and low energy levels,which influenced core positive and negative determinants.Alongside lived experience partners,healthcare stakeholders(hepatologists[n=3],exercise professionals[n=3],67%male)identified three core needs with eight considerations.Some disconnects in priorities were observed.Lived experience partners emphasized affordability,accessibility,and considerations for comorbidities,while healthcare partners advocated for research on natural history,prevention,behavior change,cost-effectiveness,and health system change.Conclusions:This co-design methodology highlights unique consumer-informed research questions.Exercise interventions and their associated implementation trials will benefit from being co-designed with both people with MASLD and care stakeholders. 展开更多

This paper deals with a case study of a context-sensitive solution of the Jamestown Corridor, located along James City County and the City of Williamsburg in Virginia. The corridor is an important passageway to many tourist attractions in the Historic Triangle of Hampton Roads. During the improvement process, the project should minimize the congestions impacts, maximize convenience, safety and reliability of the surrounding transportation systems, minimize social impact to the local community and address long-term capacity issues. Through the project, the agencies involved learned that projects near any of the historic resources must be context-sensitive and should include all stakeholders early and often to make sure that a comprehensive schedule was developed. They also learned that time should be considered for review and input from key regional stakeholders for a variety of issues. 展开更多

This article aims to share an innovative experience of organizing and funding research involving those most directly affected:patients.The“ECLAIR”working group of the Canceropole Lyon Auvergne-Rhone-Alpes(CLARA)was created at the end of 2020 with the goal of contributing to the development of a call for projects on the patient experience in oncology,which was launched in January 2021.Initially composed of 8 members,including 7 patients,coordinated by a project manager from CLARA,the ECLAIR working group actively participated in drafting the specifications of the call for projects,developing the eligibility criteria for applications,revising the evaluation and selection criteria for projects,and monitoring the selected projects.This experience was repeated twice.With significant freedom of action,the working group made two decisions that strongly demonstrate the commitment to supporting research partnerships and the active involvement of those affected:firstly,by making partnership a mandatory requirement for the eligibility of applications,and secondly,by conducting the selection of projects themselves,after an independent scientific evaluation phase.Seeking to shed light on the“black box”of partnership,the article also presents the concrete modalities of interaction among the working group members,the adjustments made between different editions of the call for projects,and the relationships maintained with CLARA. 展开更多

A questionnaire survey of residents’risk perceptions related to Taiwan nuclear power plant in China was carried out to explore the determining factors that affect individual risk perception.This study proposed to pursue a more comprehensive understanding of factors that affected individual risk perception to nuclear power plants.Covariance structure analysis was conducted using risk perceptions of nuclear power as dependent variable and including interest and knowledge levels of nuclear power,acceptability,benefit perception,trust in nuclear power operation,and trust in government as independent variables.The use of the hypothesis of Elaboration likelihood model(ELM)was also proposed.The results showed that persons with higher levels of interest and knowledge of nuclear power had their own perceptions of risk closely associated with acceptability and potential benefits of nuclear power.In contrast,persons with no interest in and knowledge of nuclear power would have risk perceptions related to their trust in nuclear operation and the government,which partially supported the ELM hypothesis.All these results indicated that the government in China plays an important role in rational risk perceptions,and well-designed communication of risks will help the public to be involved in risk management and improve people’s rational acceptance of risk. 展开更多
