Purpose-Interface management is the process of managing communications,responsibilities and coordination of project parties,phases or physical entities which are dependent on one another.Interface management is a cruc...Purpose-Interface management is the process of managing communications,responsibilities and coordination of project parties,phases or physical entities which are dependent on one another.Interface management is a crucial part of managing any construction project-but particularly important for high-speed railway projects that often have several contractual parties and stakeholders,very long project timelines and huge upfront cost overlays.This paper discusses how various project interfaces were managed during the design and construction of the civil engineering infrastructure for the High Speed Two(HS2)project in the United Kingdom.Design/methodology/approach-The paper uses the case study methodology.Key interfaces on the HS2 project are grouped into various categories and the paper discusses how they were managed within the Area North Integrated Project Team(IPT)of the HS2 project made up of contractor Balfour Beatty VINCI(BBV),the Mott MacDonald SYSTRA Design Joint Venture(DJV)and client HS2 Ltd.3 different case studies drawn from across the IPT are used,each of them highlighting different interfaces and how these interfaces were managed.Findings-The paper shows how innovative technical designs and modern methods of construction were used to address some of the unique and peculiar challenges of designing a brand-new railway in the United Kingdom.Addressing the contrasting and often competing requirements of different stakeholders,coupled with challenging physical constraints of the very limited land available for the project and the use of a rarely used Act of Parliament in the delivery of the project required different approach to interface management.Collaboration and proactive stakeholder engagement are necessary for successful interface management on megaprojects.The authors posit that adopting an integrated approach to engineering and construction management is an essential ingredient for the successful delivery of high-speed railway projects.Originality/value-With many high-speed railway projects around the world coming up in the next few years,understanding the context and challenges for each country will help engineering and design managers adopt appropriate approaches for their projects.The lessons learned on the HS2 project are also transferable to other mega infrastructure projects with complex project interfaces.展开更多

Machine picking in cotton is an emerging practice in India,to solve the problems of labour shortages and production costs increasing.Cotton production has been declining in recent years;however,the high density planti...Machine picking in cotton is an emerging practice in India,to solve the problems of labour shortages and production costs increasing.Cotton production has been declining in recent years;however,the high density planting system(HDPS)offers a viable method to enhance productivity by increasing plant populations per unit area,optimizing resource utilization,and facilitating machine picking.Cotton is an indeterminate plant that produce excessive vegeta-tive growth in favorable soil fertility and moisture conditions,which posing challenges for efficient machine picking.To address this issue,the application of plant growth retardants(PGRs)is essential for controlling canopy architecture.PGRs reduce internode elongation,promote regulated branching,and increase plant compactness,making cotton plants better suited for machine picking.PGRs application also optimizes photosynthates distribution between veg-etative and reproductive growth,resulting in higher yields and improved fibre quality.The integration of HDPS and PGRs applications results in an optimal plant architecture for improving machine picking efficiency.However,the success of this integration is determined by some factors,including cotton variety,environmental conditions,and geographical variations.These approaches not only address yield stagnation and labour shortages but also help to establish more effective and sustainable cotton farming practices,resulting in higher cotton productivity.展开更多

Introduction Task-shifting and task-sharing strategies show promise for managing chronic diseases especially in low-income and middle-income countries(LMICs),though their effectiveness in multimorbidity management rem...Introduction Task-shifting and task-sharing strategies show promise for managing chronic diseases especially in low-income and middle-income countries(LMICs),though their effectiveness in multimorbidity management remains unclear.This study synthesised evidence on task-shifting and task-sharing strategies globally and assessed the impact on core health outcomes in multimorbidity management.Methods We conducted a systematic review and meta-analysis of global studies evaluating task-shifting and sharing interventions for individuals with multimorbidity.Six databases,including PubMed,Embase,Web of Science,Ovid(Medline),CINAHL and Cochrane Library,were searched for studies reporting the core outcomes of multimorbidity management in quality of life,mortality,hospitalisation,emergency department visits and symptoms of depression and anxiety.Random-effects models were used to calculate pooled effect sizes with heterogeneity assessed through subgroup and meta-regression analyses.Results From 8471 records,36 studies from 14 countries were included,with only 5 conducted in LMICs.Twenty-one studies,encompassing 20989 participants,were eligible for meta-analysis.More than half of the studies involved nurses as delegates,with some sharing the tasks with health professionals and about 10%of studies involved non-health professionals,including community healthcare workers as delegates to share the responsibility in caring for individuals with multimorbidity.Most studies were multicomponent,with 16.7%addressing all guideline-recommended aspects of multimorbidity management.By pooling the findings,task-shifting and task-sharing interventions were associated with a 27%reduction in mortality(OR:0.73,95%CI:0.55 to 0.97,I2=0%),a modest improvement in quality of life(standardised mean difference(SMD):0.1,95%CI:0.03 to 0.17,I2=47%)and reduced symptoms of depression(SMD:0.27,95%CI:−0.52 to–0.02,I2=90%),but showed no significant effect on hospitalisation,emergency visits or anxiety-related symptoms.展开更多

In this editorial,we discuss the article by Singh et al published in World Journal of Nephrology,stating the need for timely adjustments in inflammatory bowel disease(IBD)patients'long-term management plans.IBD is...In this editorial,we discuss the article by Singh et al published in World Journal of Nephrology,stating the need for timely adjustments in inflammatory bowel disease(IBD)patients'long-term management plans.IBD is chronic and lifelong,with recurrence and remission cycles,including ulcerative colitis and Crohn's disease.It's exact etiology is unknown but likely multifactorial.Related to gut flora and immune issues.Besides intestinal symptoms,IBD can also affect various extrain-testinal manifestations such as those involving the skin,joints,eyes and urinary system.The anatomical proximity of urinary system waste disposal to that of the alimentary canal makes early detection and the differentiation of such symptoms very difficult.Various studies show that IBD and it's first-line drugs have nephro-toxicity,impacting the patients'life quality.Existing guidelines give very few references for kidney lesion monitoring.Singh et al's plan aims to improve treatment management for IBD patients with glomerular filtration rate decline,specifically those at risk.Most of IBD patients are young and they need lifelong therapy.So early therapy cessation,taking into account drug side effects,can be helpful.Artificial intelligence-driven diagnosis and treatment has a big potential for management improvements in IBD and other chronic diseases.展开更多
BACKGROUND Children with autism spectrum disorder(ASD)face unique challenges in maintaining oral health due to sensory sensitivities,communication difficulties,and behavioral barriers.These factors,along with limited ...BACKGROUND Children with autism spectrum disorder(ASD)face unique challenges in maintaining oral health due to sensory sensitivities,communication difficulties,and behavioral barriers.These factors,along with limited access to ASD-trained dental professionals,increase their risk of dental caries,periodontal disease,bruxism,and other oral health issues.Despite growing awareness of these challenges,a comprehensive synthesis of evidence-based solutions remains lacking.AIM To review synthesizes existing research on dental problems in ASD,barriers to care,management strategies,and future directions for improved oral health outcomes.METHODS A systematic search of PubMed,Cochrane Library,and Scopus was conducted using predefined search terms.Related to ASD,dental health,and management strategies.Inclusion criteria encompassed studies focusing on children with ASD,dental health issues,and interventions.Data extraction included study design,participant characteristics,key findings,and intervention outcomes.The quality of studies was assessed using appropriate tools such as the Cochrane Risk of Bias Tool and the Newcastle-Ottawa Scale.A narrative synthesis approach,incorporating thematic analysis,was utilized to evaluate the findings.RESULTS A total of 165 studies met the inclusion criteria.Children with ASD exhibited a higher prevalence of dental caries,gingivitis,bruxism,and malocclusion compared to neurotypical peers.Barriers to dental care included sensory sensitivities,communication difficulties,financial constraints,and a shortage of ASD-trained dental professionals.Effective interventions included desensitization programs,behavioral therapy,digital applications,and interdiscip-linary collaboration.Parental education and professional training were crucial for improving oral health outcomes.CONCLUSION Tailored dental care strategies,including sensory adaptations,behavioral interventions,and interdisciplinary collaboration,are essential for children with ASD.Standardized guidelines and long-term studies are needed to refine evidence-based protocols.Future research should explore digital interventions and probiotic applications in ASD dental care.展开更多
BACKGROUND Non-alcoholic fatty liver disease(NAFLD)management requires sustainable lifestyle modifications.This study aimed to evaluate the effectiveness of the RESET care plan,a comprehensive program that is an integ...BACKGROUND Non-alcoholic fatty liver disease(NAFLD)management requires sustainable lifestyle modifications.This study aimed to evaluate the effectiveness of the RESET care plan,a comprehensive program that is an integrated personalized diet,exercise,and cognitive behavior therapy,delivered via MyTatva’s digital health application enabled through a body composition analyzer(BCA)and smartwatch.AIM To evaluates the effectiveness of the comprehensive program delivered via My-Tatva’s digital health app enabled through internet of thing devices.METHODS This retrospective observational study analyzed deidentified data from 22 par-ticipants enrolled in the MyTatva RESET care program.Participants were divided into three groups:Group A,diet plan;Group B,diet+exercise plan;and Group C,diet+exercise+cognitive behavioral therapy plan.Participants were provided with a BCA and smartwatch for continuous monitoring of anthropometric para-meters.Statistical analysis,including one-way ANOVA and post-hoc Tukey’s Honest Significant Difference test,was conducted to compare mean changes in anthropometric parameters across the groups.INTRODUCTION Non-alcoholic fatty liver disease(NAFLD)has emerged as a global health burden,affecting approximately 1 in 4 in-dividuals worldwide.NAFLD ranges from simple steatosis(fat accumulation)to non-alcoholic steatohepatitis(NASH),and its global prevalence in the general population is estimated between 6.3%and 33%,with NASH affecting 3-5%[1,2].Obesity is a major risk factor of NAFLD,with studies showing that the likelihood of developing NAFLD increases 5-fold at a body mass index(BMI)of 30-32.5 kg/m²and up to 14-fold at BMI of 37.5-40 kg/m²compared to a BMI of 20-22.5 kg/m²[3,4].Effective NAFLD management requires both dietary and physical activity modifications.Healthy weight loss with sustained muscle mass plays a pivotal role,with a reduction of 3%-5%decreasing hepatic steatosis,5%-7%improving NASH conditions,and 10%or more needed to reverse hepatic fibrosis[5].Management also normalizes elevated liver enzymes(aspartate aminotransferase and alanine aminotransferase),enhances insulin sensitivity,and thereby reduces cardiovascular risk by improving endothelial function and increasing cardiorespiratory fitness[6].However,diet or exercise alone is often not as effective as a combined approach.Integrating both balanced dietary changes and increased physical activity yields more sustainable improvements in NAFLD and overall metabolic health[1,7].Traditional intervention methods usually involve in-person consultations,which often lack real-time and continuous patient monitoring.The recommendation of drastic changes in diet and exercise can also be overwhelming for patients,leading to low adherence rates.Many patients struggle to maintain these changes in the long-term due to a lack of con-tinuous motivational support[8,9].In recent years,the health ecosystem has witnessed a significant shift toward digital health platforms,which complement pharmacological treatments in chronic disease management,and increase scala-bility.These platforms provide continuous monitoring and personalized support,helping to bridge the gap between health care setups and patients[10].Recent digital advances enable internet of things(IoT)devices to be integrated into such management plans to track health metrics,to address the limitations of traditional methods[11,12].The MyTatva digital health application offers the RESET plan,a novel comprehensive approach for NAFLD mana-gement by integrating personalized support from nutrition,physiotherapy,and cognitive behavioral therapy(CBT)coaches.We aimed to evaluate the effectiveness of the RESET plan by analyzing the reduction in anthropometric para-meters across three different digital intervention groups.展开更多
Objective:To investigate the application effect of intelligent empowerment standardized airway management process in patients receiving mechanical ventilation.Methods:A retrospective analysis was conducted on the clin...Objective:To investigate the application effect of intelligent empowerment standardized airway management process in patients receiving mechanical ventilation.Methods:A retrospective analysis was conducted on the clinical data of 79 EICU inpatients who underwent tracheal intubation and mechanical ventilation treatment at our hospital from January 2023 to May 2025.The patients were divided into a control group(conventional airway management process,n=40)and a study group(intelligent empowerment standardized airway management process,n=39)based on the intervention protocols they received.Oral health scores,dental plaque index,oral odor,serum inflammatory markers[C-reactive protein(CRP),procalcitonin(PCT)],clinical pulmonary infection score(CPIS),as well as the incidence of ventilator-associated pneumonia(VAP),duration of mechanical ventilation,and length of stay in the EICU were assessed before and after treatment.Results:The baseline values of all indicators were consistent between the two groups before intervention(p>0.05).After corresponding interventions,both groups showed significant improvements in Beck oral health scores,dental plaque index,and oral odor,with more pronounced improvements observed in the study group(p<0.05).After the intervention,the research group showed a significant decrease in serum CRP and PCT levels,as well as CPIS scores(p<0.05).In contrast,the control group experienced an increase in these three indicators to a certain extent(p<0.05).Moreover,the incidence of ventilator-associated pneumonia(VAP),duration of mechanical ventilation,and length of stay in the EICU were all lower in the research group compared to the control group,while the nurse’s compliance rate with the protocol was higher in the research group(p<0.05).Conclusion:The standardized airway management protocol empowered by intelligent technology can significantly improve nursing compliance,benefit oral health status,reduce the risk of pulmonary infection and systemic inflammation levels,and promote rapid patient recovery,demonstrating considerable potential for widespread adoption.展开更多

However, the effectiveness of BMPs exhibits a complex nonlinear dependence on both configuratio...The spatial optimization of best management practices(BMPs) plays a critical role in precise watershed pollution control. However, the effectiveness of BMPs exhibits a complex nonlinear dependence on both configuration unit scale and rainfall intensity, often leading to widespread spatiotemporal mismatches during implementation. To fill this gap, this study proposes a new framework:(a) delineating configuration units based on the implementation scale differences between structural and nonstructural BMPs;(b) incorporating BMP reduction thresholds to enable dynamic adjustment of design scales according to inflow loads;and(c) developing a staged allocation strategy tailored to varying rainfall scenarios. The framework is exemplified by an agricultural catchment in the southeastern Liaohe watershed, China. The results showed that the framework could improve the assessment accuracy and cost-effectiveness of pollution control. Specifically, neglecting BMP reduction thresholds resulted in a 51.35% underestimation of treatment costs. Incorporating these thresholds and dynamically adjusting BMP design scales reduced treatment costs by 62.70%. Furthermore, the framework facilitated more precise localization of structural BMPs(1 km^(2)) and improved optimization efficiency by 95.91%. The proposed staged allocation strategy ensured water quality compliance under varying rainfall intensities. Structural BMPs primarily addressed pollution from light to moderate rainfall in the initial stage, while nonstructural BMPs targeted heavy rainfall pollution in the subsequent stage. The proposed framework may enhance the spatiotemporal adaptability of BMP configuration to respond to the threats posed by climate change and human activities. It can also be extended to other agriculture-dominated watersheds.展开更多

The proton exchange membrane fuel cell(PEMFC)and the hydrogen hybrid power system are studied by the fuzzy-PID(FPID)controlmethod and the fuzzy-PID controlmethod by Artificial Bee Colony algorithm(ABCFPID),respectivel...The proton exchange membrane fuel cell(PEMFC)and the hydrogen hybrid power system are studied by the fuzzy-PID(FPID)controlmethod and the fuzzy-PID controlmethod by Artificial Bee Colony algorithm(ABCFPID),respectively.The results reveal that compared with the FPID control method,the temperature overshoot of the PEMFC stack under the ABC-FPID control method is decreased by 0.6%.Moreover,the circulating water flow rate within the full operating envelope(about 3 min)is reduced by 19.46 L,which means the ABC-FPID control method is more effective in regulating the stack temperature.Then,the ABC-FPID control method is proposed to study the hydrogen hybrid power system,and the system output power matching,operating characteristic curve of the fuel cell,state of charge(SOC)of the lithium battery,system efficiency and hydrogen demand are obtained.The results indicate that the maximum system efficiency reaches 46.3%,the average system efficiency is 33.8%,and the average hydrogen demand is 0.192 kg/s.Overall,the ABC-FPID control method can efficiently ensure the stability of the fuel cell’s output power,and actively prompt the lithium battery to fulfill the function of“peak shaving and valley filling”under variable load power conditions.展开更多

Purpose-This study explores how managerial leadership and organizational innovation interact to enhance resilience and risk management in railway supply chains and how these capabilities contribute to sustained compet...Purpose-This study explores how managerial leadership and organizational innovation interact to enhance resilience and risk management in railway supply chains and how these capabilities contribute to sustained competitive advantage.It emphasizes the strategic importance of resilience in railway systems that face operational complexity,regulatory pressures and increasing exposure to systemic risks.Design/methodology/approach-A mixed-methods design was employed,integrating survey data from 186 railway organizations with six case studies involving railway operators,rolling stock manufacturers and supply chain partners across multiple regions.Constructs were measured using validated scales and hypothesized relationships were tested using Structural Equation Modeling(SEM).Case study interviews were analyzed thematically to provide contextual understanding of leadership practices and innovation strategies.Findings-The results confirm that transformational managerial leadership significantly predicts innovation adoption,which in turn strengthens resilience and risk management capabilities.Resilience emerged as a powerful driver of competitive advantage,reinforcing its role as a strategic capability rather than a reactive response to disruptions.Furthermore,innovation was shown to partially mediate the relationship between leadership and resilience,highlighting its function as the operational channel through which vision translates into capability.Originality/value-This study contributes to the literature by integrating the Resource-Based View(RBV)and Dynamic Capabilities(DC)framework into the context of railway supply chains.It is among the first to empirically validate the mediating role of innovation between leadership and resilience,offering both theoretical advancements and actionable strategies for building resilient and competitive railway systems.展开更多

Cellulose frameworks have emerged as promising materials for light management due to their exceptional light-scattering capabilities and sustainable nature.Conventional biomass-derived cellulose frameworks face a fund...Cellulose frameworks have emerged as promising materials for light management due to their exceptional light-scattering capabilities and sustainable nature.Conventional biomass-derived cellulose frameworks face a fundamental trade-off between haze and transparency,coupled with impractical thicknesses(≥1 mm).Inspired by squid’s skin-peeling mechanism,this work develops a peroxyformic acid(HCOOOH)-enabled precision peeling strategy to isolate intact 10-μm-thick bamboo green(BG)frameworks—100×thinner than wood-based counterparts while achieving an unprecedented optical performance(88%haze with 80%transparency).This performance surpasses delignified biomass(transparency<40%at 1 mm)and matches engineered cellulose composites,yet requires no energy-intensive nanofibrillation.The preserved native cellulose I crystalline structure(64.76%crystallinity)and wax-coated uniaxial fibril alignment(Hermans factor:0.23)contribute to high mechanical strength(903 MPa modulus)and broadband light scattering.As a light-management layer in polycrystalline silicon solar cells,the BG framework boosts photoelectric conversion efficiency by 0.41%absolute(18.74%→19.15%),outperforming synthetic anti-reflective coatings.The work establishes a scalable,waste-to-wealth route for optical-grade cellulose materials in next-generation optoelectronics.展开更多

Immersion cooling is known for high efficiency and excellent temperature unifor...An effective lithium-ion battery thermal management system(BTMS) ensures the safety of electric vehicles(EVs) and energy storage systems. Immersion cooling is known for high efficiency and excellent temperature uniformity. To address the high energy consumption of secondary cooling loops in vehicles for cooling immersion oil, this paper proposes a method of immersion coupled cooling tubes. Battery heat is directly absorbed by the immersion liquid and rapidly dissipated via cooling water in the S-type cooling tube(SCT). This work investigated the effects of structural parameters, immersion fluid medium, and operating parameters on the cooling performance of the BTMS. The results showed that the fin height(h_(f)) has no significant effect on the cooling performance. Both Transformer Oil and HFE-7100 delivered outstanding thermal management, reaching a maximum of 36.73℃ and 41.39℃ while maintaining narrow temperature differences of only 3.70℃ and 2.09℃ apart, respectively. When the ambient temperature reached 40℃ and HFE-7100 was used as the immersion liquid, the maximum temperature difference remained consistently below 3℃. Subsequently, a sensitivity analysis was conducted on the respective influential parameters. It was found that inlet water temperature(T_(in)) and immersion fluids exert the most significant influence on the system performance. Finally, different immersion cooling schemes were compared, which demonstrated the advantages of the system proposed in this study under harsh condition, thereby supporting the application of different schemes under various scenarios. It can be directly integrated with the vehicle-mounted cooling circuit system, thereby reducing energy consumption and weight, and offering new insights for research on thermal management systems for EVs.展开更多
Perovskite solar cells(PSCs)have emerged as promising photovoltaic technologies owing to their remarkable power conversion efficiency(PCE).However,heat accumulation under continuous illumination remains a critical bot...Perovskite solar cells(PSCs)have emerged as promising photovoltaic technologies owing to their remarkable power conversion efficiency(PCE).However,heat accumulation under continuous illumination remains a critical bottleneck,severely affecting device stability and long-term operational performance.Herein,we present a multifunctional strategy by incorporating highly thermally conductive Ti_(3)C_(2)T_(X) MXene nanosheets into the perovskite layer to simultaneously enhance thermal management and optoelectronic properties.The Ti_(3)C_(2)T_(X) nanosheets,embedded at perovskite grain boundaries,construct efficient thermal conduction pathways,significantly improving the thermal conductivity and diffusivity of the film.This leads to a notable reduction in the device’s steady-state operating temperature from 42.96 to 39.97 under 100 mW cm^(−2) illumination,thereby alleviating heat-induced performance degradation.Beyond thermal regulation,Ti_(3)C_(2)T_(X),with high conductivity and negatively charged surface terminations,also serves as an effective defect passivation agent,reducing trap-assisted recombination,while simultaneously facilitating charge extraction and transport by optimizing interfacial energy alignment.As a result,the Ti_(3)C_(2)T_(X)-modified PSC achieve a champion PCE of 25.13%and exhibit outstanding thermal stability,retaining 80%of the initial PCE after 500 h of thermal aging at 85 and 30±5%relative humidity.(In contrast,control PSC retain only 58%after 200 h.)Moreover,under continuous maximum power point tracking in N2 atmosphere,Ti_(3)C_(2)T_(X)-modified PSC retained 70%of the initial PCE after 500 h,whereas the control PSC drop sharply to 20%.These findings highlight the synergistic role of Ti_(3)C_(2)T_(X) in thermal management and optoelectronic performance,paving the way for the development of high-efficiency and heat-resistant perovskite photovoltaics.展开更多
文摘Purpose-Interface management is the process of managing communications,responsibilities and coordination of project parties,phases or physical entities which are dependent on one another.Interface management is a crucial part of managing any construction project-but particularly important for high-speed railway projects that often have several contractual parties and stakeholders,very long project timelines and huge upfront cost overlays.This paper discusses how various project interfaces were managed during the design and construction of the civil engineering infrastructure for the High Speed Two(HS2)project in the United Kingdom.Design/methodology/approach-The paper uses the case study methodology.Key interfaces on the HS2 project are grouped into various categories and the paper discusses how they were managed within the Area North Integrated Project Team(IPT)of the HS2 project made up of contractor Balfour Beatty VINCI(BBV),the Mott MacDonald SYSTRA Design Joint Venture(DJV)and client HS2 Ltd.3 different case studies drawn from across the IPT are used,each of them highlighting different interfaces and how these interfaces were managed.Findings-The paper shows how innovative technical designs and modern methods of construction were used to address some of the unique and peculiar challenges of designing a brand-new railway in the United Kingdom.Addressing the contrasting and often competing requirements of different stakeholders,coupled with challenging physical constraints of the very limited land available for the project and the use of a rarely used Act of Parliament in the delivery of the project required different approach to interface management.Collaboration and proactive stakeholder engagement are necessary for successful interface management on megaprojects.The authors posit that adopting an integrated approach to engineering and construction management is an essential ingredient for the successful delivery of high-speed railway projects.Originality/value-With many high-speed railway projects around the world coming up in the next few years,understanding the context and challenges for each country will help engineering and design managers adopt appropriate approaches for their projects.The lessons learned on the HS2 project are also transferable to other mega infrastructure projects with complex project interfaces.
文摘Colonoscopy represents a safe procedure that is widely used in medical practice either to diagnose or treat various gastrointestinal diseases.During the last few years,the incidence rate of perforations in colonoscopic procedures has increased,especially in therapeutic colonoscopies.The recent advancements in endoscopic techniques and gastrointestinal tumoral resection procedures such as endoscopic mucosal resection,endoscopic full-thickness resection,and endoscopic submucosal dissection(ESD)could be a risk factor for this increased risk.The incidence rate of mortality of serious colonoscopic perforations is 7.1%.The management plan for these perforations starts with conservative treatment in mild cases,endoscopic closure,and surgical management in severe cases.Recently,endoluminal vacuum therapy was found to be effective in the management of colorectal perforations and this has been reported in multiple case reports.This editorial provides an overview of the current guidelines for the management of iatrogenic colorectal perforations.These insights are from the perspectives of endoscopists and gastroenterologists.We also present a management algorithm based on the guidelines of the European Society of Gastrointestinal Endoscopy,the American Gastroenterological Association,and the World Society of Emergency Surgery.We also discussed in brief the use of endoluminal vacuum therapy in colorectal perforations.
文摘Machine picking in cotton is an emerging practice in India,to solve the problems of labour shortages and production costs increasing.Cotton production has been declining in recent years;however,the high density planting system(HDPS)offers a viable method to enhance productivity by increasing plant populations per unit area,optimizing resource utilization,and facilitating machine picking.Cotton is an indeterminate plant that produce excessive vegeta-tive growth in favorable soil fertility and moisture conditions,which posing challenges for efficient machine picking.To address this issue,the application of plant growth retardants(PGRs)is essential for controlling canopy architecture.PGRs reduce internode elongation,promote regulated branching,and increase plant compactness,making cotton plants better suited for machine picking.PGRs application also optimizes photosynthates distribution between veg-etative and reproductive growth,resulting in higher yields and improved fibre quality.The integration of HDPS and PGRs applications results in an optimal plant architecture for improving machine picking efficiency.However,the success of this integration is determined by some factors,including cotton variety,environmental conditions,and geographical variations.These approaches not only address yield stagnation and labour shortages but also help to establish more effective and sustainable cotton farming practices,resulting in higher cotton productivity.

文摘Introduction Task-shifting and task-sharing strategies show promise for managing chronic diseases especially in low-income and middle-income countries(LMICs),though their effectiveness in multimorbidity management remains unclear.This study synthesised evidence on task-shifting and task-sharing strategies globally and assessed the impact on core health outcomes in multimorbidity management.Methods We conducted a systematic review and meta-analysis of global studies evaluating task-shifting and sharing interventions for individuals with multimorbidity.Six databases,including PubMed,Embase,Web of Science,Ovid(Medline),CINAHL and Cochrane Library,were searched for studies reporting the core outcomes of multimorbidity management in quality of life,mortality,hospitalisation,emergency department visits and symptoms of depression and anxiety.Random-effects models were used to calculate pooled effect sizes with heterogeneity assessed through subgroup and meta-regression analyses.Results From 8471 records,36 studies from 14 countries were included,with only 5 conducted in LMICs.Twenty-one studies,encompassing 20989 participants,were eligible for meta-analysis.More than half of the studies involved nurses as delegates,with some sharing the tasks with health professionals and about 10%of studies involved non-health professionals,including community healthcare workers as delegates to share the responsibility in caring for individuals with multimorbidity.Most studies were multicomponent,with 16.7%addressing all guideline-recommended aspects of multimorbidity management.By pooling the findings,task-shifting and task-sharing interventions were associated with a 27%reduction in mortality(OR:0.73,95%CI:0.55 to 0.97,I2=0%),a modest improvement in quality of life(standardised mean difference(SMD):0.1,95%CI:0.03 to 0.17,I2=47%)and reduced symptoms of depression(SMD:0.27,95%CI:−0.52 to–0.02,I2=90%),but showed no significant effect on hospitalisation,emergency visits or anxiety-related symptoms.
文摘Artificial intelligence(AI)is transforming gastroenterology by enhancing diagnostic accuracy,enabling personalized treatment,and improving disease management efficiency.This review explored the evolution and application of core AI technologies,including machine learning,deep learning,and neural networks,that underpin modern computational advancements in the field.These tools have demonstrated significant success in detecting premalignant and malignant lesions and in managing gastrointestinal bleeding,colorectal cancer,and Helicobacter pylori infection.AI also supports the diagnosis and treatment of liver and pancreatic diseases.Its use is expanding in functional gastrointestinal disorders such as irritable bowel syndrome with emerging applications in pediatric gastroenterology.In addition AI enables advanced risk stratification and addresses persistent challenges in conventional diagnostic and therapeutic approaches,including interobserver variability and inefficiencies in care delivery.However,integration into routine clinical practice faces several barriers,including data privacy concerns,algorithmic bias,limited model interpretability,regulatory gaps,and interoperability issues with existing healthcare infrastructure.Future directions include real-time procedural guidance,multi-omic prediction models,minimally invasive surgical automation,and drug discovery.Achieving the full potential of AI will require ethical governance,regulatory clarity,and sustained interdisciplinary collaboration.
文摘In this editorial,we discuss the article by Singh et al published in World Journal of Nephrology,stating the need for timely adjustments in inflammatory bowel disease(IBD)patients'long-term management plans.IBD is chronic and lifelong,with recurrence and remission cycles,including ulcerative colitis and Crohn's disease.It's exact etiology is unknown but likely multifactorial.Related to gut flora and immune issues.Besides intestinal symptoms,IBD can also affect various extrain-testinal manifestations such as those involving the skin,joints,eyes and urinary system.The anatomical proximity of urinary system waste disposal to that of the alimentary canal makes early detection and the differentiation of such symptoms very difficult.Various studies show that IBD and it's first-line drugs have nephro-toxicity,impacting the patients'life quality.Existing guidelines give very few references for kidney lesion monitoring.Singh et al's plan aims to improve treatment management for IBD patients with glomerular filtration rate decline,specifically those at risk.Most of IBD patients are young and they need lifelong therapy.So early therapy cessation,taking into account drug side effects,can be helpful.Artificial intelligence-driven diagnosis and treatment has a big potential for management improvements in IBD and other chronic diseases.
文摘BACKGROUND Children with autism spectrum disorder(ASD)face unique challenges in maintaining oral health due to sensory sensitivities,communication difficulties,and behavioral barriers.These factors,along with limited access to ASD-trained dental professionals,increase their risk of dental caries,periodontal disease,bruxism,and other oral health issues.Despite growing awareness of these challenges,a comprehensive synthesis of evidence-based solutions remains lacking.AIM To review synthesizes existing research on dental problems in ASD,barriers to care,management strategies,and future directions for improved oral health outcomes.METHODS A systematic search of PubMed,Cochrane Library,and Scopus was conducted using predefined search terms.Related to ASD,dental health,and management strategies.Inclusion criteria encompassed studies focusing on children with ASD,dental health issues,and interventions.Data extraction included study design,participant characteristics,key findings,and intervention outcomes.The quality of studies was assessed using appropriate tools such as the Cochrane Risk of Bias Tool and the Newcastle-Ottawa Scale.A narrative synthesis approach,incorporating thematic analysis,was utilized to evaluate the findings.RESULTS A total of 165 studies met the inclusion criteria.Children with ASD exhibited a higher prevalence of dental caries,gingivitis,bruxism,and malocclusion compared to neurotypical peers.Barriers to dental care included sensory sensitivities,communication difficulties,financial constraints,and a shortage of ASD-trained dental professionals.Effective interventions included desensitization programs,behavioral therapy,digital applications,and interdiscip-linary collaboration.Parental education and professional training were crucial for improving oral health outcomes.CONCLUSION Tailored dental care strategies,including sensory adaptations,behavioral interventions,and interdisciplinary collaboration,are essential for children with ASD.Standardized guidelines and long-term studies are needed to refine evidence-based protocols.Future research should explore digital interventions and probiotic applications in ASD dental care.
文摘BACKGROUND Non-alcoholic fatty liver disease(NAFLD)management requires sustainable lifestyle modifications.This study aimed to evaluate the effectiveness of the RESET care plan,a comprehensive program that is an integrated personalized diet,exercise,and cognitive behavior therapy,delivered via MyTatva’s digital health application enabled through a body composition analyzer(BCA)and smartwatch.AIM To evaluates the effectiveness of the comprehensive program delivered via My-Tatva’s digital health app enabled through internet of thing devices.METHODS This retrospective observational study analyzed deidentified data from 22 par-ticipants enrolled in the MyTatva RESET care program.Participants were divided into three groups:Group A,diet plan;Group B,diet+exercise plan;and Group C,diet+exercise+cognitive behavioral therapy plan.Participants were provided with a BCA and smartwatch for continuous monitoring of anthropometric para-meters.Statistical analysis,including one-way ANOVA and post-hoc Tukey’s Honest Significant Difference test,was conducted to compare mean changes in anthropometric parameters across the groups.INTRODUCTION Non-alcoholic fatty liver disease(NAFLD)has emerged as a global health burden,affecting approximately 1 in 4 in-dividuals worldwide.NAFLD ranges from simple steatosis(fat accumulation)to non-alcoholic steatohepatitis(NASH),and its global prevalence in the general population is estimated between 6.3%and 33%,with NASH affecting 3-5%[1,2].Obesity is a major risk factor of NAFLD,with studies showing that the likelihood of developing NAFLD increases 5-fold at a body mass index(BMI)of 30-32.5 kg/m²and up to 14-fold at BMI of 37.5-40 kg/m²compared to a BMI of 20-22.5 kg/m²[3,4].Effective NAFLD management requires both dietary and physical activity modifications.Healthy weight loss with sustained muscle mass plays a pivotal role,with a reduction of 3%-5%decreasing hepatic steatosis,5%-7%improving NASH conditions,and 10%or more needed to reverse hepatic fibrosis[5].Management also normalizes elevated liver enzymes(aspartate aminotransferase and alanine aminotransferase),enhances insulin sensitivity,and thereby reduces cardiovascular risk by improving endothelial function and increasing cardiorespiratory fitness[6].However,diet or exercise alone is often not as effective as a combined approach.Integrating both balanced dietary changes and increased physical activity yields more sustainable improvements in NAFLD and overall metabolic health[1,7].Traditional intervention methods usually involve in-person consultations,which often lack real-time and continuous patient monitoring.The recommendation of drastic changes in diet and exercise can also be overwhelming for patients,leading to low adherence rates.Many patients struggle to maintain these changes in the long-term due to a lack of con-tinuous motivational support[8,9].In recent years,the health ecosystem has witnessed a significant shift toward digital health platforms,which complement pharmacological treatments in chronic disease management,and increase scala-bility.These platforms provide continuous monitoring and personalized support,helping to bridge the gap between health care setups and patients[10].Recent digital advances enable internet of things(IoT)devices to be integrated into such management plans to track health metrics,to address the limitations of traditional methods[11,12].The MyTatva digital health application offers the RESET plan,a novel comprehensive approach for NAFLD mana-gement by integrating personalized support from nutrition,physiotherapy,and cognitive behavioral therapy(CBT)coaches.We aimed to evaluate the effectiveness of the RESET plan by analyzing the reduction in anthropometric para-meters across three different digital intervention groups.
文摘Objective:To investigate the application effect of intelligent empowerment standardized airway management process in patients receiving mechanical ventilation.Methods:A retrospective analysis was conducted on the clinical data of 79 EICU inpatients who underwent tracheal intubation and mechanical ventilation treatment at our hospital from January 2023 to May 2025.The patients were divided into a control group(conventional airway management process,n=40)and a study group(intelligent empowerment standardized airway management process,n=39)based on the intervention protocols they received.Oral health scores,dental plaque index,oral odor,serum inflammatory markers[C-reactive protein(CRP),procalcitonin(PCT)],clinical pulmonary infection score(CPIS),as well as the incidence of ventilator-associated pneumonia(VAP),duration of mechanical ventilation,and length of stay in the EICU were assessed before and after treatment.Results:The baseline values of all indicators were consistent between the two groups before intervention(p>0.05).After corresponding interventions,both groups showed significant improvements in Beck oral health scores,dental plaque index,and oral odor,with more pronounced improvements observed in the study group(p<0.05).After the intervention,the research group showed a significant decrease in serum CRP and PCT levels,as well as CPIS scores(p<0.05).In contrast,the control group experienced an increase in these three indicators to a certain extent(p<0.05).Moreover,the incidence of ventilator-associated pneumonia(VAP),duration of mechanical ventilation,and length of stay in the EICU were all lower in the research group compared to the control group,while the nurse’s compliance rate with the protocol was higher in the research group(p<0.05).Conclusion:The standardized airway management protocol empowered by intelligent technology can significantly improve nursing compliance,benefit oral health status,reduce the risk of pulmonary infection and systemic inflammation levels,and promote rapid patient recovery,demonstrating considerable potential for widespread adoption.

文摘The spatial optimization of best management practices(BMPs) plays a critical role in precise watershed pollution control. However, the effectiveness of BMPs exhibits a complex nonlinear dependence on both configuration unit scale and rainfall intensity, often leading to widespread spatiotemporal mismatches during implementation. To fill this gap, this study proposes a new framework:(a) delineating configuration units based on the implementation scale differences between structural and nonstructural BMPs;(b) incorporating BMP reduction thresholds to enable dynamic adjustment of design scales according to inflow loads;and(c) developing a staged allocation strategy tailored to varying rainfall scenarios. The framework is exemplified by an agricultural catchment in the southeastern Liaohe watershed, China. The results showed that the framework could improve the assessment accuracy and cost-effectiveness of pollution control. Specifically, neglecting BMP reduction thresholds resulted in a 51.35% underestimation of treatment costs. Incorporating these thresholds and dynamically adjusting BMP design scales reduced treatment costs by 62.70%. Furthermore, the framework facilitated more precise localization of structural BMPs(1 km^(2)) and improved optimization efficiency by 95.91%. The proposed staged allocation strategy ensured water quality compliance under varying rainfall intensities. Structural BMPs primarily addressed pollution from light to moderate rainfall in the initial stage, while nonstructural BMPs targeted heavy rainfall pollution in the subsequent stage. The proposed framework may enhance the spatiotemporal adaptability of BMP configuration to respond to the threats posed by climate change and human activities. It can also be extended to other agriculture-dominated watersheds.

文摘The proton exchange membrane fuel cell(PEMFC)and the hydrogen hybrid power system are studied by the fuzzy-PID(FPID)controlmethod and the fuzzy-PID controlmethod by Artificial Bee Colony algorithm(ABCFPID),respectively.The results reveal that compared with the FPID control method,the temperature overshoot of the PEMFC stack under the ABC-FPID control method is decreased by 0.6%.Moreover,the circulating water flow rate within the full operating envelope(about 3 min)is reduced by 19.46 L,which means the ABC-FPID control method is more effective in regulating the stack temperature.Then,the ABC-FPID control method is proposed to study the hydrogen hybrid power system,and the system output power matching,operating characteristic curve of the fuel cell,state of charge(SOC)of the lithium battery,system efficiency and hydrogen demand are obtained.The results indicate that the maximum system efficiency reaches 46.3%,the average system efficiency is 33.8%,and the average hydrogen demand is 0.192 kg/s.Overall,the ABC-FPID control method can efficiently ensure the stability of the fuel cell’s output power,and actively prompt the lithium battery to fulfill the function of“peak shaving and valley filling”under variable load power conditions.

文摘Purpose-This study explores how managerial leadership and organizational innovation interact to enhance resilience and risk management in railway supply chains and how these capabilities contribute to sustained competitive advantage.It emphasizes the strategic importance of resilience in railway systems that face operational complexity,regulatory pressures and increasing exposure to systemic risks.Design/methodology/approach-A mixed-methods design was employed,integrating survey data from 186 railway organizations with six case studies involving railway operators,rolling stock manufacturers and supply chain partners across multiple regions.Constructs were measured using validated scales and hypothesized relationships were tested using Structural Equation Modeling(SEM).Case study interviews were analyzed thematically to provide contextual understanding of leadership practices and innovation strategies.Findings-The results confirm that transformational managerial leadership significantly predicts innovation adoption,which in turn strengthens resilience and risk management capabilities.Resilience emerged as a powerful driver of competitive advantage,reinforcing its role as a strategic capability rather than a reactive response to disruptions.Furthermore,innovation was shown to partially mediate the relationship between leadership and resilience,highlighting its function as the operational channel through which vision translates into capability.Originality/value-This study contributes to the literature by integrating the Resource-Based View(RBV)and Dynamic Capabilities(DC)framework into the context of railway supply chains.It is among the first to empirically validate the mediating role of innovation between leadership and resilience,offering both theoretical advancements and actionable strategies for building resilient and competitive railway systems.
文摘Objective:To systematically sort out the application forms and effects of digital health intervention technologies in oral health management,and provide references for the digital development of stomatology.Methods:By reviewing relevant domestic and foreign studies and clinical practices,this paper summarizes and analyzes the main application forms of digital health interventions,including digital health education,intelligent detection equipment,telemedicine platforms,oral health big data platforms,and school-hospital collaborative screening robots.Results:Studies have shown that digital health interventions can effectively improve the public’s oral health knowledge level,optimize personal health behaviors,enhance clinical diagnosis efficiency,reduce overall medical costs,and promote the innovation and upgrading of oral health management models.Conclusion:Digital health intervention represents an inevitable trend in the future development of stomatology.In the future,it is still necessary to improve data security and privacy protection,technology adaptability and popularity,as well as relevant policies and norms,to give full play to its potential value.

文摘Cellulose frameworks have emerged as promising materials for light management due to their exceptional light-scattering capabilities and sustainable nature.Conventional biomass-derived cellulose frameworks face a fundamental trade-off between haze and transparency,coupled with impractical thicknesses(≥1 mm).Inspired by squid’s skin-peeling mechanism,this work develops a peroxyformic acid(HCOOOH)-enabled precision peeling strategy to isolate intact 10-μm-thick bamboo green(BG)frameworks—100×thinner than wood-based counterparts while achieving an unprecedented optical performance(88%haze with 80%transparency).This performance surpasses delignified biomass(transparency<40%at 1 mm)and matches engineered cellulose composites,yet requires no energy-intensive nanofibrillation.The preserved native cellulose I crystalline structure(64.76%crystallinity)and wax-coated uniaxial fibril alignment(Hermans factor:0.23)contribute to high mechanical strength(903 MPa modulus)and broadband light scattering.As a light-management layer in polycrystalline silicon solar cells,the BG framework boosts photoelectric conversion efficiency by 0.41%absolute(18.74%→19.15%),outperforming synthetic anti-reflective coatings.The work establishes a scalable,waste-to-wealth route for optical-grade cellulose materials in next-generation optoelectronics.

文摘An effective lithium-ion battery thermal management system(BTMS) ensures the safety of electric vehicles(EVs) and energy storage systems. Immersion cooling is known for high efficiency and excellent temperature uniformity. To address the high energy consumption of secondary cooling loops in vehicles for cooling immersion oil, this paper proposes a method of immersion coupled cooling tubes. Battery heat is directly absorbed by the immersion liquid and rapidly dissipated via cooling water in the S-type cooling tube(SCT). This work investigated the effects of structural parameters, immersion fluid medium, and operating parameters on the cooling performance of the BTMS. The results showed that the fin height(h_(f)) has no significant effect on the cooling performance. Both Transformer Oil and HFE-7100 delivered outstanding thermal management, reaching a maximum of 36.73℃ and 41.39℃ while maintaining narrow temperature differences of only 3.70℃ and 2.09℃ apart, respectively. When the ambient temperature reached 40℃ and HFE-7100 was used as the immersion liquid, the maximum temperature difference remained consistently below 3℃. Subsequently, a sensitivity analysis was conducted on the respective influential parameters. It was found that inlet water temperature(T_(in)) and immersion fluids exert the most significant influence on the system performance. Finally, different immersion cooling schemes were compared, which demonstrated the advantages of the system proposed in this study under harsh condition, thereby supporting the application of different schemes under various scenarios. It can be directly integrated with the vehicle-mounted cooling circuit system, thereby reducing energy consumption and weight, and offering new insights for research on thermal management systems for EVs.

文摘Perovskite solar cells(PSCs)have emerged as promising photovoltaic technologies owing to their remarkable power conversion efficiency(PCE).However,heat accumulation under continuous illumination remains a critical bottleneck,severely affecting device stability and long-term operational performance.Herein,we present a multifunctional strategy by incorporating highly thermally conductive Ti_(3)C_(2)T_(X) MXene nanosheets into the perovskite layer to simultaneously enhance thermal management and optoelectronic properties.The Ti_(3)C_(2)T_(X) nanosheets,embedded at perovskite grain boundaries,construct efficient thermal conduction pathways,significantly improving the thermal conductivity and diffusivity of the film.This leads to a notable reduction in the device’s steady-state operating temperature from 42.96 to 39.97 under 100 mW cm^(−2) illumination,thereby alleviating heat-induced performance degradation.Beyond thermal regulation,Ti_(3)C_(2)T_(X),with high conductivity and negatively charged surface terminations,also serves as an effective defect passivation agent,reducing trap-assisted recombination,while simultaneously facilitating charge extraction and transport by optimizing interfacial energy alignment.As a result,the Ti_(3)C_(2)T_(X)-modified PSC achieve a champion PCE of 25.13%and exhibit outstanding thermal stability,retaining 80%of the initial PCE after 500 h of thermal aging at 85 and 30±5%relative humidity.(In contrast,control PSC retain only 58%after 200 h.)Moreover,under continuous maximum power point tracking in N2 atmosphere,Ti_(3)C_(2)T_(X)-modified PSC retained 70%of the initial PCE after 500 h,whereas the control PSC drop sharply to 20%.These findings highlight the synergistic role of Ti_(3)C_(2)T_(X) in thermal management and optoelectronic performance,paving the way for the development of high-efficiency and heat-resistant perovskite photovoltaics.
