Poland syndrome complicated by macromastia in an adolescent female:A case report and literature review

Poland syndrome(PS)is a rare congenital condition characterized by unilateral absence or underdevelopment of the pectoralis major muscle,frequently accompanied by breast and chest wall anomalies.We report the case of an 18-year-old Chinese female diagnosed with PS complicated by contralateral macromastia who presented with significant breast asymmetry.Magnetic resonance imaging revealed a left breast volume of 618.90 mL and a right breast volume of 1137.04 mL.The patient underwent a staged reconstruction that included a right breast reduction mammaplasty and three sessions of autologous fat grafting to the left breast.Postoperative imaging 3 months after the final procedure confirmed near symmetry,with volumes measuring 831.37 mL(left)and 841.07 mL(right).The clinical follow-up demonstrated stable results and no complications.This case underscores the importance of individualized surgical planning,precise volumetric assessment,and the role of fat grafting in achieving satisfactory aesthetic outcomes in patients with PS and complex breast deformities. 展开更多

乳房肥大下垂矫正术后继发畸形的临床分型及个体化修复策略研究

目的 系统总结乳房肥大下垂矫正术后常见继发畸形的临床特征与成因,建立科学的临床分型体系,并提出针对性个体化修复策略。方法 收集分析河南省人民医院整形外科2022年1月至2025年1月收治的30例乳房肥大下垂矫正术后继发畸形患者的完整... 目的 系统总结乳房肥大下垂矫正术后常见继发畸形的临床特征与成因,建立科学的临床分型体系,并提出针对性个体化修复策略。方法 收集分析河南省人民医院整形外科2022年1月至2025年1月收治的30例乳房肥大下垂矫正术后继发畸形患者的完整临床资料,由两名正高级整形外科医师采用双人盲法进行畸形评估,结合畸形特征与成因进行临床分型,针对不同分型制定并实施个体化修复手术方案。通过术后12个月随访,通过温哥华瘢痕量表、并发症发生率、Breast-Q量表和Likert5级满意度量表综合评估修复效果。结果 3300例患者的乳房肥大下垂矫正术后继发畸形被分为3种类型:Ⅰ型“假性矫正型”(双泡畸形)8例(26.7%),其核心成因是初次手术术式选择错误;Ⅱ型“扁平外扩型”(饼状畸形)12例(40.0%),主要由术式与畸形严重程度不匹配及腺体处理不充分导致;Ⅲ型“复发伴局部畸形型”(猫耳伴下垂)10例(33.3%),主要由手术技术执行偏差引起。对应各分型采用“假体调整/取出联合确切乳房上提术”(Ⅰ型)、“切口转换联合腺体重塑术”(Ⅱ型)及“局部修整联合腺体再固定术”(Ⅲ型)进行修复后,术后随访发现所有患者的畸形均得到显著改善,患者乳房手术切口的瘢痕情况得到显著改善(P<0.05);并发症发生率为13.3%,仅出现3例切口局部延迟愈合、1例血清肿,经对症处理后均痊愈,无血肿、感染、乳头坏死等严重并发症发生;30名患者修复手术后3个月,对乳房外观的满意度,心理健康评分,社会健康评分,性健康评分,身体健康评分相较于手术前均有显著提升(P<0.05);患者总体满意度达93.3%(非常满意60.0%,满意33.3%)。结论 基于畸形特征与成因的三型分类法能全面覆盖乳房肥大下垂矫正术后常见继发畸形,据此制定的个体化修复策略具有逻辑严谨、靶向性强的特点,可有效提升修复手术的精准度与临床疗效,提高患者满意度,为临床规范化处理此类复杂修复手术提供可靠的参考依据与标准化临床路径。 展开更多

采用气相色谱-质谱法(GC/MS)、傅立叶变换红外光谱法(FTIR)和热裂解气质联用仪(Py-GC/MS)等先进表征仪器,探究了不同溶剂(CS_(2)、丙酮和等体积丙酮/CS_(2)混合溶剂)萃取焦油渣(coal tar residue,CTR)后,萃取物的组成特征及萃余物(分别... 采用气相色谱-质谱法(GC/MS)、傅立叶变换红外光谱法(FTIR)和热裂解气质联用仪(Py-GC/MS)等先进表征仪器,探究了不同溶剂(CS_(2)、丙酮和等体积丙酮/CS_(2)混合溶剂)萃取焦油渣(coal tar residue,CTR)后,萃取物的组成特征及萃余物(分别记为RCTR1、RCTR2和RCTR3)的热解产物分布规律。进一步结合热重分析(TG-DTG)与Coats-Redfern动力学模型,探究CTR萃余物热解过程中的潜在反应路径。结果表明:相比于CTR,三种萃余物中苯环类官能团含量减少且具有较好的热稳定性。萃取物中可检测的化合物以芳烃为主,相对含量分别为37.06%、32.86%和33.61%。表明以上三种溶剂可选择性破坏芳环间的π—π作用、强氢键作用和氢键/π—π复合作用,从而削弱分子量较小的多环芳烃(1~5环)与CTR中较大分子(如稠环芳烃,≥6环)间的作用力,使得分子量较小的芳烃类化合物更易被提取。此外,三种萃余物在460°C下的热解产物以烷烃为主,其中RCTR1和RCTR3热解产物中主要是C_(21)~C_(30)的正构烷烃,而RCTR2热解产物中的烷烃以C11~C20为主。最后,动力学模型随温度升高呈现从D1型向F2型的渐进转变,表明萃余物热解过程中存在物理扩散与化学键断裂的协同作用机制。 展开更多

目的:探究在高脂血症中,丹蒌片(Dan-Lou tablets,DLT)调控阿托伐他汀(atorvastatin,ATV)对大鼠体内药动学及药效学的影响,为异常血脂中西药联用提供理论依据。方法:采用高脂模型,按临床剂量折算单次/多次灌胃DLT,联合ATV干预后,通过UPLC... 目的:探究在高脂血症中,丹蒌片(Dan-Lou tablets,DLT)调控阿托伐他汀(atorvastatin,ATV)对大鼠体内药动学及药效学的影响,为异常血脂中西药联用提供理论依据。方法:采用高脂模型,按临床剂量折算单次/多次灌胃DLT,联合ATV干预后,通过UPLC-MS/MS多离子检测法定量检测大鼠血浆中ATV及其活性代谢物浓度,计算药动参数;同时评估高脂模型血脂指标如总胆固醇(total Cholesterol,TC)、甘油三酯(triglyceride,TG)、高密度脂蛋白胆固醇(highdensity lipoprotein cholesterol,HDL-C)和低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C),并观察肝组织病理形态的变化。结果:药动学方面,正常组与模型组中,与单独灌服ATV相比,联合用药后ATV的AUC0-24h分别上升160.63%、50.96%,Cmax分别上升27.95%、7.93%。药效学方面,ATV组与联合给药组分别使高脂大鼠的体质量下降12.48%和29.89%,联合给药组的TC、TG和LDL-C均降低,HDL-C升高,差异具有统计学意义(P<0.01)。肝组织切片显示,联合用药组脂滴分布显著减少,改善效果最为明显。结论:在高脂异常体质中,DLT能影响ATV在大鼠体内的药动学过程,显著增加ATV的体内暴露量。临床联合用药时,应密切监测疗效及药物不良反应,调整给药方案,避免潜在的药物相互作用风险。 展开更多

为了研究氢燃料适应性改进对涡桨发动机特性以及不同热力循环参数下涡桨发动机性能的影响,建立了与试验数据相匹配的某型燃油涡桨发动机及其氢燃料适应性改进后发动机的总体性能仿真模型,计算分析了氢燃料适应性改进前后发动机特性以及... 为了研究氢燃料适应性改进对涡桨发动机特性以及不同热力循环参数下涡桨发动机性能的影响,建立了与试验数据相匹配的某型燃油涡桨发动机及其氢燃料适应性改进后发动机的总体性能仿真模型,计算分析了氢燃料适应性改进前后发动机特性以及不同热力循环参数下的发动机性能。结果表明,氢燃料适应性改进会使发动机涡轮流通能力减弱、涡轮做功能力增强、燃料消耗率大幅下降;采用等输出功率控制规律和等燃气涡轮物理转速控制规律时,能使涡桨发动机拥有更好的高空、高速和高温天性能;采用等动力涡轮进口总温控制规律时,能有效地挖掘涡桨发动机的性能潜力,发动机的输出功率最大可提高约19%,热效率最大可提高约11%;设计点的燃烧室出口总温的变化对氢燃料适应性改进前后性能参数变化量影响显著,并且总温每增加100 K,起飞状态的压气机喘振裕度减小幅度会扩大约0.6%,热效率增大幅度会扩大约0.2%,燃烧室出口总温减小幅度会扩大5~10 K,而设计点的发动机进口换算流量和压气机增压比的变化对氢燃料适应性改进前后性能参数变化量基本无影响或影响有限。 展开更多

AIM:To determine the efficacy and safety of rapid induction therapy with oral tacrolimus without a meal in steroid-refractory ulcerative colitis(UC)patients.METHODS:This was a prospective,multicenter,observational study.Between May 2010 and August 2012,49 steroid-refractory UC patients(55 flare-ups)were consecutively enrolled.All patients were treated with oral tacrolimus without a meal at an initial dose of 0.1mg/kg per day.The dose was adjusted to maintain trough whole-blood levels of 10-15 ng/m L for the first 2 wk.Induction of remission at 2 and 4 wk after tacrolimus treatment initiation was evaluated using Lichtiger’s clinical activity index(CAI).RESULTS:The mean CAI was 12.6±3.6 at onset.Within the first 7 d,93.5%of patients maintained high trough levels(10-15 ng/m L).The CAI significantly decreased beginning 2 d after treatment initiation.At 2wk,73.1%of patients experienced clinical responses.After tacrolimus initiation,31.4%and 75.6%of patients achieved clinical remission at 2 and 4 wk,respectively.Treatment was well tolerated.CONCLUSION:Rapid induction therapy with oral tacrolimus shortened the time to achievement of appropriate trough levels and demonstrated a high remission rate 28 d after treatment initiation.Rapid induction therapy with oral tacrolimus appears to be a useful therapy for the treatment of refractory UC. 展开更多

AIM: To evaluate the efficacy of sequential vs hybrid therapy in patients with Helicobacter pylori(H. pylori) infection.METHODS: From March 2013 to May 2014,one hundred and seventy-five H. pylori infected patients who had not been treated for H. pylori before wererandomized to receive either sequential therapy(rabeprazole 20 mg and amoxicillin 1 g twice daily for 5 d,followed by rabeprazole 20 mg,clarithromycin 500 mg and metronidazole 500 mg twice daily for 5 d) or hybrid therapy(rabeprazole 20 mg and amoxicillin 1 g for 7 d,followed by rabeprazole 20 mg,amoxicillin 1 g,clarithromycin 500 mg and metronidazole 500 mg twice daily for 7 d). H. pylori status was confirmed by positive results of both rapid urease test and histology examination or a positive result of culture. Eradication efficacy was assessed by follow-up endoscopy with rapid urease test and histological examination 8 wk after the end of anti-H. pylori therapy,or 13C-urea breath test at least 4 wk after completion of treatment. The primary outcome was H. pylori eradication by intension-to-treat(ITT) and per-protocol(PP) analyses.RESULTS: One hundred and sixty-seven patients(83 patients in the sequential group and 84 patients in the hybrid group) completed the study. The compliance rates were 97.6% and 97.7% for the two groups,respectively. The eradication rate was 78.2% for the sequential group and 92% for the hybrid group by ITT analysis(P = 0.01). The eradication rate was 81.9% for the sequential group and 96.4% for the hybrid group by PP analysis(P = 0.01). Univariate analysis for the clinical and bacterial factors did not identify any risk factors associated with treatment failure. Severe adverse events were observed in 2.3% of patients in the sequential group and 2.4% of those in the hybrid group.CONCLUSION: Due to a grade A(> 95%) success rate for H. pylori eradication by PP analysis,similar compliance and adverse events,hybrid therapy seems to be an appropriate eradication regimen in Taiwan. 展开更多

A thermal fatigue behaviour of Co-based alloy coating obtained by laser surface meltcasting on the high temperature alloy GH33 was studied.The results show that after each time of thermal cycling,the final residual stress was formed in the melt-casting layer which is attributed to the thermal stress and structural stress.Through the first 50 times of thermal cycling,the morphology of coating still inherits the laser casting one,but the dendrites get bigger;After the second 50 times of thermal cycling,corrosion pits emerge from coating,and mostly in the places where coating and substrate meet.The fatigue damage type of coating belongs to stress corrosion. 展开更多

Several nc-TiN/a-TiB2 thin films comprised of nanocrystalline (nc-) TiN and amor phous (a-) TiB2 phases were deposited on Si(100) at room temperature by reactive unbalanced dc magnetron sputtering, followed by vacuum annealed at 400, 600, 80 0 and 1000℃ for 1h, respectively. Effects of B content on microstructure, mecha nical behaviors and thermal microstructure stability have been investigated by X -ray diffraction (XRD), scanning electron microscopy (SEM), X-ray photoelectron spectroscopy (XPS) and nanoindentation measurements. The results indicated that B addition greatly affected both microstructure and mechanical behavior of nc-Ti N/a-TiB2 thin films. With increasing B content the grain size decreased. A maxim um hardness value of about 33GPa was obtained at B content of about 19at.%. The improved mechanical properties of nc-TiN/a-TiB2 films with the addition of B int o TiN were attributed to their densified microstructure with development of fine grain size. Only addition of sufficient B could restrain grain growth during an nealing. High B content resulted in high microstructure stability. The crystalli zation of amorphous matrix occurred at about 800℃, forming TiB or TiB2 crystall ite, depending on B content. Before that no change in bonding configuration was found. 展开更多

Gastric cancer is the fourth most common cancer,and the second-highest cause of cancer-related deaths worldwide.Despite extensive research to identify novel diagnostic and therapeutic agents,patients with advanced gastric cancer suffer from a poor quality of life and poor prognosis,and treatment is dependent mainly on conventional cytotoxic chemotherapy.To improve the quality of life and survival of gastric cancer patients,a better understanding of the underlying molecular pathologies,and their application towards the development of novel targeted therapies,is urgently needed.Chemokines are a group of small proteins associated with cytoskeletal rearrangements,the directional migration of several cell types during development and physiology,and the host immune response via interactions with G-protein coupled receptors.There is also growing evidence to suggest that chemokines not only play a role in the immune system,but are also involved in the development and progression of tumors.In gastric cancer,CXC chemokines and chemokine receptors regulate the trafficking of cells in and out of the tumor microenvironment.CXC chemokines and their receptors can also directly influence tumorigenesis by modulating tumor transformation,survival,growth,invasion and metastasis,as well as indirectly by regulating angiogenesis,and tumor-leukocyte interactions.In this review,we will focus on the roles of CXC chemokines and their receptors in the development,progression,and metastasis of gastric tumors,and discuss their therapeutic potential for gastric cancer. 展开更多

Based on vibration analysis, single-layered graphene sheet (SLGS) with multiple attached nanoparticles is developed as nanoscale mass sensor in thermal environments. Graphene sensors are assumed to be in simplysupported configuration. Based on the nonlocal plate the- ory which incorporates size effects into the classical theory, closed-form expressions lot the frequencies and relative fre- quency shills of SLGS-based mass sensor are derived using the Galerkin method. The suggested model is justified by a good agreement between the results given by the present model and available data in literature. The effects of tem- perature difference, nonlocal parameter, the location of the nanoparticle and the number of nanoparticles on the relative frequency shift of the mass sensor are also elucidated. The obtained results show that the sensitivity of the SLGS- based mass sensor increases with increasing temperature difference. 展开更多

The role of multicomponent rare earth oxides in phase stability, thermophysical properties and sintering for ZrO2-based thermal barrier coatings (TBCs) materials is investigated. 8YSZ codoped with 3 mol% Gd2O3 and 3 mol% Yb2O3 (GYb-YSZ) powders are synthesized by solid state reaction for 24 h at various temperatures. As temperature increases, stabilizers are dissolved into zirconia matrix gradually. Synthesized at 1 500 °C, GYb-YSZ is basically composed of cubic phase. GYb-YSZ exhibits excellent phase stability and sinters lower than 8YSZ by nearly three times. The thermal conductivity of GYb-YSZ is much lower than that of 8YSZ, and the thermal expansion coefficient of GYb-YSZ is comparable to that of 8YSZ. The influence of Gd2O3 and Yb2O3 co-doping on phase stability, thermal conductivity and sintering of 8YSZ is discussed. 展开更多

Under isothermal quasi-static stretching the phase transition of a superelastic NiTi tube involves the formation (during loading) and vanishing (in unloading) of a high strain (martensite) domain. The two events are accompanied by a rapid stress drop/rise due to the formation/vanishing of do- main fronts. From a thermodynamic point of view, both are instability phenomena that occur once the system reaches its critical state. This paper investigates the stability of a shrink- ing cylindrical domain in a tube configuration during unload- ing. The energetics and thermodynamic driving force of the cylindrical domain are quantified by using an elastic inclu- sion model. It is demonstrated that the two domain fronts ex- hibit strong interaction when they come close to each other, which brings a peak in the total energy and a sign change in the thermodynamic driving force. It is proved that such domain front interaction plays an important role in control- ling the stability of the domain and in the occurrence of stress jumps during domain vanishing. It is also shown that the pro- cess is governed by two nondimensional length scales (the normalized tube length and normalized wall-thickness) and that the length scale dependence of the critical domain length and stress jump for the domain vanishing can be quantified by the elastic inclusion model. 展开更多

Thermo-mechanical Properties of In-situ Formed Al_2O_3-SiC-SiAION Composites

Al2 O3 - SiC - SiAION composite specimens have been prepared using fused alumina, ultra fine α-Al2O3, Si and Al powders as starting materials and liquid phenol formaldehyde resin as binder and firing at 1 500 ℃ for 3 h in carbon embedded condition. Thermo-mechanical properties of Al2 O3 - SiC - SiAlON composites have been studied. The results show : ( 1 ) Al2 O3 - SiC - SiAION composites can be prepared and sintered at 1 500 ℃ under carbon embedded condition. When Si/Al ratio is 8/0 and 7/1, the prepared composite is Al2O3 - SiC - O' -SiAlON; when Si/Al ratio is 5/3 and 3/5, the pre- pared composite is Al2 O3 - SiC - β-SiAION. ( 2 ) The composites possess high temperature strength properties. Hot modulus of rupture at 1 400 ℃ is 10 - 30 MPa. They are in elastic range up to 1 000 ℃, after which plastic deformation is observed; even at 1 400 ℃, amount of deformation is relatively low. (3) The com- posites possess good thermal shock resistance, their resid- ual strength ratio (AT = 1 100 ℃ ) is 65% - 80%. (4) The marked improvement in thermo-mechanical properties of Al2O3 - SiC - SiAlON composites may be at- tributed to the in-situ formation of SiC, SiAlON and AIN, the fibrous SiC, columnar SiAlON and tabular-like AlN fill in the corundum skeleton structure creating strengthening and toughening effects. 展开更多

In this paper, based on the theory of locked-in stress developed by academician Chen Zongji, the locked-in stress problem in underground rock is simulated by the thermal expansion of hard rubber particles. The pore inclusion in rock is assumed to be uniformly distributed spherical cavities. Using the thermal stress theory, the stress of rock with a spherical pore inclusion is equivalent to the thermal stress generated by the spherical hard rubber inclusion. The elastic theory formula of the temperature increment and the equivalent pore pressure of the spherical hard rubber inclusion is derived.
