Female Sexual Dysfunction (FSD) is a common public health issue that is defined as one or more problems of female sexual desire, arousal, orgasm and/or sexual pain/discomfort that leads to significant distress. Aim: This study aimed to assess the female sexual function among Libyan women. Methods and materials: This descriptive cross-sectional study was carried out at Tripoli University Hospital over six months duration between January and June 2024. It included 314 female participants who attended the hospital during that period. The data underwent analysis and prescription via a computerized program of SPSS version 24. Results: The most frequent age range was between 26 to 35 years accounting for 54.8% (172). The mean duration of marriage was 7.42 years ± 7.228 SD. The majority of participants were multipara accounting for 79.9% (251). Based on the classification of female sexual dysfunction severity, 41.7% (131) of participants had mild to moderate severity followed by 32.2% (101) had mild severity with an overall female sexual dysfunction rate of 81.2% (255). Based on female sexual function characteristics, 28.3% (89) had abnormal desire, 22% (69) had abnormal arousal, 55.4% (174) had abnormal lubrication, 24.5% (77) had undetected orgasm, 23.2% (73) had inappropriate satisfaction and 19.1% (60) had pain. Conclusion: The overall rate of female sexual dysfunction among Libyan women was (81.2%) which is considered high but with low severity, (41.7%) had mild to moderate dysfunction followed by (32.2%) with mild dysfunction. These findings had significant effects on sexual life and marital status which require a basic strategy approach to increase sexual awareness and marital counseling among couples and to avoid adverse sexual dysfunction consequences such as divorce, marital problems, insufficient sexual satisfaction, and psychosocial concern.展开更多
BACKGROUND End-stage renal disease(ESRD)is associated with a multitude of physical,psychological,and social health challenges,including a profound impact on sexual and reproductive health.Among males with ESRD,erectil...BACKGROUND End-stage renal disease(ESRD)is associated with a multitude of physical,psychological,and social health challenges,including a profound impact on sexual and reproductive health.Among males with ESRD,erectile dysfunction(ED)is highly prevalent due to factors such as underlying comorbidities,including diabetes and hypertension,and the physiological effects of long-term dialysis.Kidney transplantation(KTx)has been proposed as a potential intervention to mitigate the effects of ED by restoring renal function and improving hormonal balance.However,the evidence surrounding the effectiveness of KTx in improving sexual function,specifically erectile function(EF),remains inconclusive.This systematic review and meta-analysis aim to evaluate the effects of KTx on sexual dysfunction(SexDys),particularly ED,in male ESRD patients.AIM To evaluate the benefits and potential harms of KTx compared to other forms of renal replacement therapy in improving EF in adult males with ESRD,assessed using the international index of EF(IIEF),to survey the prevalence of SexDys in this population,and to assess the correlation between various factors and SexDys through regression analysis.METHODS A systematic search of PubMed,EMBASE,Cochrane Library,Scopus,Clinical-Trials.gov,and Google Scholar was conducted,following the PRISMA 2020 guidelines.Prospective and retrospective cohort studies,as well as cross-sectional studies assessing EF pre-and post-transplantation,were included.These studies used validated tools such as the IIEF to measure EF.Meta-analyses were performed using a random-effects model to estimate standardized mean differences(SMD)and hazard ratios(HR)with 95%confidence intervals(CI).Heterogeneity was assessed using the I²statistic,and publication bias was evaluated with a funnel plot and the Egger’s test.RESULTS A total of 2419 studies were identified,with 362 abstracts screened and 193 full-text articles reviewed.Ultimately,11 studies were included for qualitative analysis and 7 for quantitative synthesis.The random effects model for SMD yielded a combined estimate of 0.43(95%CI:-0.20-1.07),indicating a small but non-significant improvement in EF post-transplantation.The heterogeneity across studies was substantial(I²=90%),reflecting significant variability in outcomes.Subgroup analysis showed greater improvements in EF among living-donor transplant recipients compared to those receiving organs from deceased donors.Despite this trend,the overall result for changes in EF was not statistically significant(P=0.15).Additionally,the combined HR from the meta-analysis was 2.87(95%CI:1.76-4.69),suggesting that KTx significantly increases the likelihood of improved EF,though variability between studies persisted(I²=63%).CONCLUSION While KTx offers some promise for improving EF in male ESRD patients,the overall evidence remains inconclusive due to high heterogeneity between studies and a lack of statistical significance in the combined results.Despite this,individual studies suggest that KTx may lead to significant improvements in EF for certain subgroups,particularly living-donor recipients.Future research should focus on larger,well-designed cohort studies with standardized outcome measures to provide more definitive conclusions.Addressing SexDys as part of routine care for ESRD patients undergoing KTx is crucial to improving their overall quality of life.However,adjunct therapies such as phosphodiesterase type 5 inhibitors may be necessary for those who do not experience adequate improvements post-transplantation.展开更多
Land use change is a fundamental factor affecting ecosystem’s structures and functions.However,few studies have explored the ecological disturbance caused by land use change from a combined structural and functional ...Land use change is a fundamental factor affecting ecosystem’s structures and functions.However,few studies have explored the ecological disturbance caused by land use change from a combined structural and functional perspective.In this study,the ecological structural disturbance index(SDI)and functional disturbance index(FDI)were introduced to quantitatively evaluate ecological disturbance caused by land use change in a typical karst area—Huangping County,Guizhou Province,China during 2009–2019.Results show that although the area of ecological land increased during past 10 yr,there had been a fragmentation trend of ecological land.Agricultural occupation was more severe than construction encroachment on ecological land.The grids with negative structural disturbance were consistent with areas of obvious dynamic loss and gain of ecological land.Ecological fragmentation had a greater impact than habitat gain in grids with negative structural disturbance.The ecosystem service functions of supply,adjustment,support,and culture were obviously affected by land use change,and the total FDI reflects the trade-off among them.Negative FDI values were easily observed in the rocky desertification area or water and soil loss area.The combination analysis of the SDI and FDI indicated the characteristic of codirectional ecologically structural and functional disturbance in the majority of grids.The findings improve our understanding of multiple relationships among ecological disturbances and provide valuable information for guiding land use activities.展开更多

However, there is insufficient evidence regarding its validity in the new evidence-based-defined PE. This study was performed to evaluate the validity of PEDT and its association with IIEF-15 in different types of evidence-based-defined PE. From June 2015 to January 2016, a total of 260 men complaining of PE and defined as lifelong PE (LPE)/acquired PE (APE) according to the evidence-based definition from Andrology Clinic of the First Affiliated Hospital of Anhui Medical University, along with 104 male healthy controls without PE from a medical examination center, were enrolled in this study. All individuals completed questionnaires including demographics, medical and sexual history, as well as PEDT and IIEF-15. After statistical analysis, it was found that men with PE reported higher PEDT scores (14.28 ± 3.05) and lower IIEF-15 (41.26 ± 8.20) than men without PE (PEDT: 5.32 ± 3.42, IIEF-15:52.66 ± 6.86, P 〈 0.001 for both). It was suggested that a score of 〉9 indicated PE in both LPE and APE by sensitivity and specificity analyses (sensitivity: 0.875, 0.913; specificity: 0.865, 0.865, respectively). In addition, IIEF-15 were higher in men with LPE (42.64 ± 8.11) than APE (39.43 ± 7.84, P 〈 0.001). After adjusting for age, IIEF-15 was negatively related to PEDT in men with LPE (adjust r = -0.225, P 〈 0.001) and APE (adjust r = -0.378, P 〈 0.001). In this study, we concluded that PEDT was valid in the diagnosis of evidenced-based-defined PE. Furthermore, IIEF-15 was negatively related to PEDT in men with different types of PE.展开更多

Materials and Methods: This was an observational, single centre study conducted in India. A total of 50 patients who were admitted to department of cardiology from July, 2008 to February, 2009 with diagnosis of dilated cardiomyopathy and an ejection fraction of <40% were included. Results: Of the 50 patients, 34 (68%) were males. 27 (54%) patients were in NYHA class II and 23 (46%) patients were in NYHA class III. LA volume was found to be ≥40 ml in all patients. LV function and LA volume were found to be correlated (r = -0.789, p < 0.01). Similarly, there was a correlation between LV function and LA volume index (r = -0.826, p < 0.01). There was no correlation between LA volume index and duration of symptoms (r = 0.04). Conclusion: It can be concluded that there is a strong inverse correlation between LA volume and left ventricular function and also between LA volume index and left ventricular function. The patients with NYHA class III were having larger left atrial volume than those with NYHA class II. Moreover, the duration of symptoms has no correlation with left atrial volume index.展开更多

METHODS: Between October 2010 and May 2013, 606 patients who were operated due to gonarthrosis, coxarthrosis, aseptic loosening of the total knee and hip prosthesis were evaluated prospectively. Patients were operated by three surgeons in three medical centers. Patients who were between 30-90 years of age and who were underwent total knee arthroplasty, total hip arthroplasty, revision knee arthroplasty, or revision hip arthroplasty were included in the study. We excluded the patients who cannot tolerate our standard postoperative rehabilitation program. Additionally, patients who had systemic inflammatory diseases, diabetes mellitus, or endochrinopathies were excluded from the study. The remaining 513 patients comprised our study group. Preoperative functional joint scores, height, weightand BMI of all patients were recorded. We used the Knee Society Score(KSS) for knee and Harris Hip Score(HHS) for hip patients. Postoperative functional scores were measured at 1^(st), 6^(th) and 12^(th) months and recorded separately at outpatient visits. RESULTS: The mean age of the patients was 64.7(range: 30-90) years(207 males/306 females) and the mean follow-up duration was 14.3(range: 12-26) mo. We found that arthroplasty patients had weight gain and had an increase in BMI at the postoperative 1^(st), 6^(th) and 12^(th) months. The mean BMI of the patients was 27.7 preoperatively, 27.8 at the postoperative 1^(st) month, 28.1 at the 6^(th) month and 28.6 at the 12^(th) month(P < 0.01). At the last visit, the mean postoperative HHS of the hip arthroplasty patients was 82.2 ± 7.12(preoperatively, 52.3; 1st month, 78.2; 6^(th) month, 81.1; 12^(th) month, 82.2), and the mean KSS of the knee arthroplasty patients was 79.3 ± 4.31(preoperatively, 35.8; 1^(st) month, 75.2; 6^(th) month, 79.1; 12^(th) month, 79.3). Worse functional results were noted in the patients who had a BMI increase, however, this correlation was statistically significant only at the postoperative 6^(th) month(P = 0.03). CONCLUSION: To prevent the negative functional effects of this weight gain during the postoperative period, arthroplasty patients should be advised for weight control and risky patients should consult with a dietician.展开更多

Methods: Lung function was detected on 59 cases of lung cancer and 63 normal controls. The relationship between lung function indexes and survival time was analyzed. Results: There was significant difference in ventilation function and diffusing capacity between lung cancer group and control group. Vital capacity (VC), Forced expiratory volume in one second (FEV1), Forced vital capacity (FVC), peak expiratory flow (PEF), peak expiratory flow% (PEF%), Maximal ventilatory volume (MVV) were positively correlated to survival time in patients with advanced lung cancer (r = 0.28522064, 0.28053851, 0.28289252, 0.26908133, 0.26335034, 0.28409036, P 0.05), residual volume/total lung capacity was negatively correlated to survival time (r = ?0.30760097, P 0.05). Conclusions: The lung function decrease in the patients with lung cancer. Previous published studies have shown an improvement of penile hemodynamic parameters after low-intensity extracorporeal shockwave therapy(Li-ESWT).However,the clinical significance of these findings remains unclear,a...Previous published studies have shown an improvement of penile hemodynamic parameters after low-intensity extracorporeal shockwave therapy(Li-ESWT).However,the clinical significance of these findings remains unclear,and definitive selection criteria for Li-ESWT based on preexisting comorbidities have yet to be established.This was an observational study of 113 patients with ED,evaluated between January 2019 and December 2021 in Andrology Unit at the Department of Urology and Renal Transplantation,University of Foggia(Foggia,Italy).Penile dynamic Doppler was performed to evaluate vascular parameters and 5-item version of the International Index of Erectile Dysfunction(IIEF-5)questionnaire was administered to assess the severity of ED.This was repeated 1 month after treatment.Patients with a peak systolic velocity(PSV)<30 cm s−1 were considered eligible for Li-ESWT.Our protocol consisted of 8 weekly sessions with 1500 strokes distributed in 5 different locations along the penis.After treatment,a significant mean(±standard deviation[s.d.])PSV increase of 5.0(±3.4)cm s−1 was recorded and 52/113(46.0%)patients reached a PSV>30 cm s−1 at posttherapeutic penile dynamic Doppler.A clinically significant IIEF-5 score improvement was observed in 7 patients,21 patients,and 2 patients with mild-to-moderate,moderate,and severe pretreatment ED,respectively.No different outcomes were assessed based on smoking habits,previous pelvic surgery,or use of oral phosphodiesterase-5 inhibitor(PDE5i).On the other side,only 1(6.7%)in 15 patients with diabetes mellitus showed an IIEF-5 score improvement after Li-ESWT.Shockwave treatment determined a significant increase in PSV and correlated IIEF-5 improvement in ED patients.This advantage seemed particularly evident for moderate ED and was not affected by smoking habits,previous pelvic surgery,and use of PDE5i.Conversely,diabetic patients did not benefit from the treatment.展开更多

METHODS: In this cross-sectional study, 100 athletes with NP and 50 healthy athletes participated and responded to the PFRI. Fifty athletes with NP completed the PFRI for at least 7 d later to establish test-retest reliability. RESULTS: The athletes with NP responded to all items, indicating excellent clinical utility. No floor and ceiling effects were found, indicating content validity and responsiveness. The PFRI revealed capability to discriminate between the athletes with NP and healthy athletes. The PFRI demonstrated strong correlation with the Numerical Rating Scale(Spearman's rho = 0.94), and the Persian Neck Disability Index(Pearson r = 0.995), supporting criterion and construct validity. Internal consistency reliability was high(Cronbach's α coefficient: 0.97). The test-retest reliability was excellent(ICCagreement = 0.96). The absolute reliability values of standard error of measurement and smallest detectable change were 3.2 and 8.84, respectively. An exploratory factor analysis yielded one factor explaining 78.03% of the total variance. CONCLUSION: The PFRI is a valid and reliable measure of functional status in athletes with NP.展开更多

Given the unavailability of echocardiography in most developing countries, the diagnostic performance of current ECG indexes needs to be evaluated. Objective: To compare the diagnostic performance of 3 commonly used ECG indexes (Sokolow-Lyon, Cornell voltage and Cornell product) in black hypertensive patients. Methods: Electrocardiography and echocardiography estimated left ventricular mass of 155 consecutive hypertensive patients who participated from January 2012 to January 2013 to an echocardiographic cross-sectional study of left ventricular structure was analyzed to compare Cornell voltage and Cornell product indexes with Sokolow-Lyon voltage index as a reference. Reduced kidney function was defined as eGFR 2. ROC curves in relation to LVH diagnosis were used to estimate the sensitivities and specificities of each index. P Results: The sensitivity and specificity were 43% and 85%, 23% and 77% and 26% and 77% for Sokolow-Lyon, Cornell voltage and Cornell product indexes, respectively. However, Sokolow-Lyon index (AUC;95% CI: 0.64;0.50 - 0.78) showed better performance than Cornell voltage (0.42;0.25 - 0.59) and Cornell product (0.43;0.28 - 0.59). Sokolow-Lyon index cut-point ≥ 37 mm corresponded to the highest Youden index (39.4% of sensitivity and 92.3% of specificity). Conclusion: Although the overall performance of the 3 ECG indexes versus echocardiography was low, Sokolow-Lyon index performed better than the two other indexes in diagnosing LVH.展开更多

We will find the compact expression of the influence functions, which allow the quantification of the effect of an infinitesimal contamination of the probability of any pair of attributes of the bivariate random variable distributed according to the above-mentioned model. We prove that the only unbiased index is the chi square. In order to determine the indexes, which are less sensitive to contamination, we obtain the expressions of three synthetic measures of the influence function, which are the maximum contamination (gross sensitivity error), the mean square deviation and the variance. These results, even if don’t allow a definitive assessment of the overall optimum properties of the five indexes, as not all of them are unbiased, nevertheless they allow to appreciating the synthetic entity of the effect of the contaminations in the estimation of the parameter ρ of the bivariate Bernoulli distribution.展开更多

We identify the Vulnerability Intensity Function (VIF), and the Vulnerability Index Indicator (VII) of a computer operating network. Both of these functions, VIF and VII are entities of the stochastic process that we have identified, which characterizes the probabilistic behavior of the number of vulnerabilities of a computer operating network. The VIF identifies the rate at which the number of vulnerabilities changes with respect to time. The VII is an important index indicator that conveys the following information about the number of vulnerabilities of Desktop Operating Systems: the numbers are increasing, decreasing, or remaining the same at a particular time of interest. This decision type of index indicator is crucial in every strategic planning and decision-making. The proposed VIF and VII illustrate their importance by using real data for Microsoft Windows Operating Systems 10, 8, 7, and Apple MacOS. The results of the actual data attest to the importance of VIF and VII in the cybersecurity problem we are currently facing.展开更多

ME...OBJECTIVE: To observe the effects of Suhuang Zhike Capsule on pulmonary function, blood gas analysis index, serum PCT and CRP expression in patients with acute exacerbation of chronic obstructive pulmonary disease. METHODS: A total of 88 patients with acute exacerbation of chronic obstructive pulmonary disease admitted to the Department of Respiratory Medicine, Xuancheng People's Hospital of Anhui Province from December 2014 to December 2016 were randomly divided into the observation group and the control group, with 44 cases in each group. The control group was given routine clinical treatment, and the observation group was given Suhuang Zhike Capsule on the basis of the treatment method of the control group. After 7 days' treatment, the improvement of lung function indexes were observed and evaluated before and after treatment(forced expiratory volume in one second(FEV_1), forced expiratory volume occupancy in the 1^(st) second percentage of vital capacity(FEVl/FVC), peak expiatory flow(PEF)), blood gas analysis index(Arterial oxygen partial pressure(PaO_2) and arterial blood carbon dioxide partial pressure(PaCO_2), oxygenation index(OI)) and serum cytokine levels(procalcitonin(PCT) and C reactive protein(CRP). RESULTS: The total effective rate of the observation group was significantly higher than that of the control group(P < 0.05). The FEV_1, FEV_1/FVC and PEF in 2 groups were significantly increased after the treatment(P < 0.05), and the above lung function indexes in the observation group were significantly higher than the control group(P < 0.05). PaO_2 and OI were significantly increased after the treatment(P < 0.05), PaCO_2 was significantly decreased after the treatment(P < 0.05), and the improvement of above blood gas analysis indexes were significantly superior of the observation group than the control group(P < 0.05). The serum PCT and CRP levels in 2 groups were significantly decreased after the treatment(P < 0.05), and the improvement in the observation group were more significant than that in the control group(P < 0.05). CONCLUSION: Suhuang Zhike Capsule can inhibit serum inflammatory cytokine levels in patients with acute exacerbation of chronic obstructive pulmonary disease, improve blood gas analysis indicators, and improve lung functions.展开更多

These findings had significant effects on sexual life and marital status which require a basic strategy approach to increase sexual awareness and marital counseling among couples and to avoid adverse sexual dysfunction consequences such as divorce, marital problems, insufficient sexual satisfaction, and psychosocial concern.
文摘BACKGROUND End-stage renal disease(ESRD)is associated with a multitude of physical,psychological,and social health challenges,including a profound impact on sexual and reproductive health.Among males with ESRD,erectile dysfunction(ED)is highly prevalent due to factors such as underlying comorbidities,including diabetes and hypertension,and the physiological effects of long-term dialysis.Kidney transplantation(KTx)has been proposed as a potential intervention to mitigate the effects of ED by restoring renal function and improving hormonal balance.However,the evidence surrounding the effectiveness of KTx in improving sexual function,specifically erectile function(EF),remains inconclusive.This systematic review and meta-analysis aim to evaluate the effects of KTx on sexual dysfunction(SexDys),particularly ED,in male ESRD patients.AIM To evaluate the benefits and potential harms of KTx compared to other forms of renal replacement therapy in improving EF in adult males with ESRD,assessed using the international index of EF(IIEF),to survey the prevalence of SexDys in this population,and to assess the correlation between various factors and SexDys through regression analysis.METHODS A systematic search of PubMed,EMBASE,Cochrane Library,Scopus,Clinical-Trials.gov,and Google Scholar was conducted,following the PRISMA 2020 guidelines.Prospective and retrospective cohort studies,as well as cross-sectional studies assessing EF pre-and post-transplantation,were included.These studies used validated tools such as the IIEF to measure EF.Meta-analyses were performed using a random-effects model to estimate standardized mean differences(SMD)and hazard ratios(HR)with 95%confidence intervals(CI).Heterogeneity was assessed using the I²statistic,and publication bias was evaluated with a funnel plot and the Egger’s test.RESULTS A total of 2419 studies were identified,with 362 abstracts screened and 193 full-text articles reviewed.Ultimately,11 studies were included for qualitative analysis and 7 for quantitative synthesis.The random effects model for SMD yielded a combined estimate of 0.43(95%CI:-0.20-1.07),indicating a small but non-significant improvement in EF post-transplantation.The heterogeneity across studies was substantial(I²=90%),reflecting significant variability in outcomes.Subgroup analysis showed greater improvements in EF among living-donor transplant recipients compared to those receiving organs from deceased donors.Despite this trend,the overall result for changes in EF was not statistically significant(P=0.15).Additionally,the combined HR from the meta-analysis was 2.87(95%CI:1.76-4.69),suggesting that KTx significantly increases the likelihood of improved EF,though variability between studies persisted(I²=63%).CONCLUSION While KTx offers some promise for improving EF in male ESRD patients,the overall evidence remains inconclusive due to high heterogeneity between studies and a lack of statistical significance in the combined results.Despite this,individual studies suggest that KTx may lead to significant improvements in EF for certain subgroups,particularly living-donor recipients.Future research should focus on larger,well-designed cohort studies with standardized outcome measures to provide more definitive conclusions.Addressing SexDys as part of routine care for ESRD patients undergoing KTx is crucial to improving their overall quality of life.However,adjunct therapies such as phosphodiesterase type 5 inhibitors may be necessary for those who do not experience adequate improvements post-transplantation.

文摘Land use change is a fundamental factor affecting ecosystem’s structures and functions.However,few studies have explored the ecological disturbance caused by land use change from a combined structural and functional perspective.In this study,the ecological structural disturbance index(SDI)and functional disturbance index(FDI)were introduced to quantitatively evaluate ecological disturbance caused by land use change in a typical karst area—Huangping County,Guizhou Province,China during 2009–2019.Results show that although the area of ecological land increased during past 10 yr,there had been a fragmentation trend of ecological land.Agricultural occupation was more severe than construction encroachment on ecological land.The grids with negative structural disturbance were consistent with areas of obvious dynamic loss and gain of ecological land.Ecological fragmentation had a greater impact than habitat gain in grids with negative structural disturbance.The ecosystem service functions of supply,adjustment,support,and culture were obviously affected by land use change,and the total FDI reflects the trade-off among them.Negative FDI values were easily observed in the rocky desertification area or water and soil loss area.The combination analysis of the SDI and FDI indicated the characteristic of codirectional ecologically structural and functional disturbance in the majority of grids.The findings improve our understanding of multiple relationships among ecological disturbances and provide valuable information for guiding land use activities.

文摘Some studies have shown that left ventricular structure and function play an important role in the risk stratifi cation and prognosis of cardiovascular disease.The clinical application of left atrial function in cardiovascular disease has gradually attracted attention in the cardiovascular fi eld.There are many traditional methods to evaluate left atrial function.Left atrial function related indexes measured by echocardiography has been identifi ed as a powerful predictor of cardiovascular disease in recent years,but they have some limitations.The left atrial function index has been found to evaluate left atrial function more effectively than traditional parameters.Furthermore,it is a valuable predictor of the risk stratifi cation and prognosis in patients with clinical cardiovascular disease such as heart failure,atrial fi brillation,hypertension,and coronary heart disease.
文摘The premature ejaculation diagnostic tool (PEDT) is a brief diagnostic measure to assess premature ejaculation (PE). However, there is insufficient evidence regarding its validity in the new evidence-based-defined PE. This study was performed to evaluate the validity of PEDT and its association with IIEF-15 in different types of evidence-based-defined PE. From June 2015 to January 2016, a total of 260 men complaining of PE and defined as lifelong PE (LPE)/acquired PE (APE) according to the evidence-based definition from Andrology Clinic of the First Affiliated Hospital of Anhui Medical University, along with 104 male healthy controls without PE from a medical examination center, were enrolled in this study. All individuals completed questionnaires including demographics, medical and sexual history, as well as PEDT and IIEF-15. After statistical analysis, it was found that men with PE reported higher PEDT scores (14.28 ± 3.05) and lower IIEF-15 (41.26 ± 8.20) than men without PE (PEDT: 5.32 ± 3.42, IIEF-15:52.66 ± 6.86, P 〈 0.001 for both). It was suggested that a score of 〉9 indicated PE in both LPE and APE by sensitivity and specificity analyses (sensitivity: 0.875, 0.913; specificity: 0.865, 0.865, respectively). In addition, IIEF-15 were higher in men with LPE (42.64 ± 8.11) than APE (39.43 ± 7.84, P 〈 0.001). After adjusting for age, IIEF-15 was negatively related to PEDT in men with LPE (adjust r = -0.225, P 〈 0.001) and APE (adjust r = -0.378, P 〈 0.001). In this study, we concluded that PEDT was valid in the diagnosis of evidenced-based-defined PE. Furthermore, IIEF-15 was negatively related to PEDT in men with different types of PE.
文摘Background: The aim of this study was to determine the relation of left atrial (LA) volume and LA volume index with left ventricular function and to determine the association of duration of symptoms and left atrial volume index in patients with dilated cardiomyopathy. Materials and Methods: This was an observational, single centre study conducted in India. A total of 50 patients who were admitted to department of cardiology from July, 2008 to February, 2009 with diagnosis of dilated cardiomyopathy and an ejection fraction of <40% were included. Results: Of the 50 patients, 34 (68%) were males. 27 (54%) patients were in NYHA class II and 23 (46%) patients were in NYHA class III. LA volume was found to be ≥40 ml in all patients. LV function and LA volume were found to be correlated (r = -0.789, p < 0.01). Similarly, there was a correlation between LV function and LA volume index (r = -0.826, p < 0.01). There was no correlation between LA volume index and duration of symptoms (r = 0.04). Conclusion: It can be concluded that there is a strong inverse correlation between LA volume and left ventricular function and also between LA volume index and left ventricular function. The patients with NYHA class III were having larger left atrial volume than those with NYHA class II. Moreover, the duration of symptoms has no correlation with left atrial volume index.
文摘AIM: To evaluate the body mass index(BMI) change in arthroplasty patients and its impact on the patients' functional results. METHODS: Between October 2010 and May 2013, 606 patients who were operated due to gonarthrosis, coxarthrosis, aseptic loosening of the total knee and hip prosthesis were evaluated prospectively. Patients were operated by three surgeons in three medical centers. Patients who were between 30-90 years of age and who were underwent total knee arthroplasty, total hip arthroplasty, revision knee arthroplasty, or revision hip arthroplasty were included in the study. We excluded the patients who cannot tolerate our standard postoperative rehabilitation program. Additionally, patients who had systemic inflammatory diseases, diabetes mellitus, or endochrinopathies were excluded from the study. The remaining 513 patients comprised our study group. Preoperative functional joint scores, height, weightand BMI of all patients were recorded. We used the Knee Society Score(KSS) for knee and Harris Hip Score(HHS) for hip patients. Postoperative functional scores were measured at 1^(st), 6^(th) and 12^(th) months and recorded separately at outpatient visits. RESULTS: The mean age of the patients was 64.7(range: 30-90) years(207 males/306 females) and the mean follow-up duration was 14.3(range: 12-26) mo. We found that arthroplasty patients had weight gain and had an increase in BMI at the postoperative 1^(st), 6^(th) and 12^(th) months. The mean BMI of the patients was 27.7 preoperatively, 27.8 at the postoperative 1^(st) month, 28.1 at the 6^(th) month and 28.6 at the 12^(th) month(P < 0.01). At the last visit, the mean postoperative HHS of the hip arthroplasty patients was 82.2 ± 7.12(preoperatively, 52.3; 1st month, 78.2; 6^(th) month, 81.1; 12^(th) month, 82.2), and the mean KSS of the knee arthroplasty patients was 79.3 ± 4.31(preoperatively, 35.8; 1^(st) month, 75.2; 6^(th) month, 79.1; 12^(th) month, 79.3). Worse functional results were noted in the patients who had a BMI increase, however, this correlation was statistically significant only at the postoperative 6^(th) month(P = 0.03). CONCLUSION: To prevent the negative functional effects of this weight gain during the postoperative period, arthroplasty patients should be advised for weight control and risky patients should consult with a dietician.
文摘Background: To those patients with advanced lung cancer, the ultimate objective is to improve the quality of life, and lung function is an important factor affecting quality of life. We detect lung function of patients with lung cancer and study the correlation between lung function and the patients’ survival time, to provide reference for evaluation of disease progression and prognosis. Methods: Lung function was detected on 59 cases of lung cancer and 63 normal controls. The relationship between lung function indexes and survival time was analyzed. Results: There was significant difference in ventilation function and diffusing capacity between lung cancer group and control group. Vital capacity (VC), Forced expiratory volume in one second (FEV1), Forced vital capacity (FVC), peak expiratory flow (PEF), peak expiratory flow% (PEF%), Maximal ventilatory volume (MVV) were positively correlated to survival time in patients with advanced lung cancer (r = 0.28522064, 0.28053851, 0.28289252, 0.26908133, 0.26335034, 0.28409036, P 0.05), residual volume/total lung capacity was negatively correlated to survival time (r = ?0.30760097, P 0.05). Conclusions: The lung function decrease in the patients with lung cancer. Vital capacity (VC), Forced expiratory volume in one second (FEV1), Forced vital capacity (FVC), peak expiratory flow (PEF), peak expiratory flow% (PEF%), Maximal ventilatory volume (MVV), and residual volume/total lung capacity are correlated to survival time in patients with advanced lung cancer. The lung function indexes are important marker of prognosis of patients with lung cancer.
文摘Previous published studies have shown an improvement of penile hemodynamic parameters after low-intensity extracorporeal shockwave therapy(Li-ESWT).However,the clinical significance of these findings remains unclear,and definitive selection criteria for Li-ESWT based on preexisting comorbidities have yet to be established.This was an observational study of 113 patients with ED,evaluated between January 2019 and December 2021 in Andrology Unit at the Department of Urology and Renal Transplantation,University of Foggia(Foggia,Italy).Penile dynamic Doppler was performed to evaluate vascular parameters and 5-item version of the International Index of Erectile Dysfunction(IIEF-5)questionnaire was administered to assess the severity of ED.This was repeated 1 month after treatment.Patients with a peak systolic velocity(PSV)<30 cm s−1 were considered eligible for Li-ESWT.Our protocol consisted of 8 weekly sessions with 1500 strokes distributed in 5 different locations along the penis.After treatment,a significant mean(±standard deviation[s.d.])PSV increase of 5.0(±3.4)cm s−1 was recorded and 52/113(46.0%)patients reached a PSV>30 cm s−1 at posttherapeutic penile dynamic Doppler.A clinically significant IIEF-5 score improvement was observed in 7 patients,21 patients,and 2 patients with mild-to-moderate,moderate,and severe pretreatment ED,respectively.No different outcomes were assessed based on smoking habits,previous pelvic surgery,or use of oral phosphodiesterase-5 inhibitor(PDE5i).On the other side,only 1(6.7%)in 15 patients with diabetes mellitus showed an IIEF-5 score improvement after Li-ESWT.Shockwave treatment determined a significant increase in PSV and correlated IIEF-5 improvement in ED patients.This advantage seemed particularly evident for moderate ED and was not affected by smoking habits,previous pelvic surgery,and use of PDE5i.Conversely,diabetic patients did not benefit from the treatment.
文摘AIM: To validate the culturally-adapted Persian Functional Rating Index(PFRI) for assessing neck pain(NP) in athletes. METHODS: In this cross-sectional study, 100 athletes with NP and 50 healthy athletes participated and responded to the PFRI. Fifty athletes with NP completed the PFRI for at least 7 d later to establish test-retest reliability. RESULTS: The athletes with NP responded to all items, indicating excellent clinical utility. No floor and ceiling effects were found, indicating content validity and responsiveness. The PFRI revealed capability to discriminate between the athletes with NP and healthy athletes. The PFRI demonstrated strong correlation with the Numerical Rating Scale(Spearman's rho = 0.94), and the Persian Neck Disability Index(Pearson r = 0.995), supporting criterion and construct validity. Internal consistency reliability was high(Cronbach's α coefficient: 0.97). The test-retest reliability was excellent(ICCagreement = 0.96). The absolute reliability values of standard error of measurement and smallest detectable change were 3.2 and 8.84, respectively. An exploratory factor analysis yielded one factor explaining 78.03% of the total variance. CONCLUSION: The PFRI is a valid and reliable measure of functional status in athletes with NP.
文摘Background: Reduced kidney function in blacks is associated with an increased frequency of left ventricular hypertrophy. Given the unavailability of echocardiography in most developing countries, the diagnostic performance of current ECG indexes needs to be evaluated. Objective: To compare the diagnostic performance of 3 commonly used ECG indexes (Sokolow-Lyon, Cornell voltage and Cornell product) in black hypertensive patients. Methods: Electrocardiography and echocardiography estimated left ventricular mass of 155 consecutive hypertensive patients who participated from January 2012 to January 2013 to an echocardiographic cross-sectional study of left ventricular structure was analyzed to compare Cornell voltage and Cornell product indexes with Sokolow-Lyon voltage index as a reference. Reduced kidney function was defined as eGFR 2. ROC curves in relation to LVH diagnosis were used to estimate the sensitivities and specificities of each index. P Results: The sensitivity and specificity were 43% and 85%, 23% and 77% and 26% and 77% for Sokolow-Lyon, Cornell voltage and Cornell product indexes, respectively. However, Sokolow-Lyon index (AUC;95% CI: 0.64;0.50 - 0.78) showed better performance than Cornell voltage (0.42;0.25 - 0.59) and Cornell product (0.43;0.28 - 0.59). Sokolow-Lyon index cut-point ≥ 37 mm corresponded to the highest Youden index (39.4% of sensitivity and 92.3% of specificity). Conclusion: Although the overall performance of the 3 ECG indexes versus echocardiography was low, Sokolow-Lyon index performed better than the two other indexes in diagnosing LVH.
文摘In this paper we examine 5 indexes (the two Yule’s indexes, the chi square, the odds ratio and an elementary index) of a two-by-two table, which estimate the correlation coefficient ρ in a bivariate Bernoulli distribution. We will find the compact expression of the influence functions, which allow the quantification of the effect of an infinitesimal contamination of the probability of any pair of attributes of the bivariate random variable distributed according to the above-mentioned model. We prove that the only unbiased index is the chi square. In order to determine the indexes, which are less sensitive to contamination, we obtain the expressions of three synthetic measures of the influence function, which are the maximum contamination (gross sensitivity error), the mean square deviation and the variance. These results, even if don’t allow a definitive assessment of the overall optimum properties of the five indexes, as not all of them are unbiased, nevertheless they allow to appreciating the synthetic entity of the effect of the contaminations in the estimation of the parameter ρ of the bivariate Bernoulli distribution.
文摘The objective of the present study is to define two important aspects of the computer operating system concerning the number of its vulnerabilities behavior. We identify the Vulnerability Intensity Function (VIF), and the Vulnerability Index Indicator (VII) of a computer operating network. Both of these functions, VIF and VII are entities of the stochastic process that we have identified, which characterizes the probabilistic behavior of the number of vulnerabilities of a computer operating network. The VIF identifies the rate at which the number of vulnerabilities changes with respect to time. The VII is an important index indicator that conveys the following information about the number of vulnerabilities of Desktop Operating Systems: the numbers are increasing, decreasing, or remaining the same at a particular time of interest. This decision type of index indicator is crucial in every strategic planning and decision-making. The proposed VIF and VII illustrate their importance by using real data for Microsoft Windows Operating Systems 10, 8, 7, and Apple MacOS. The results of the actual data attest to the importance of VIF and VII in the cybersecurity problem we are currently facing.
文摘OBJECTIVE: To observe the effects of Suhuang Zhike Capsule on pulmonary function, blood gas analysis index, serum PCT and CRP expression in patients with acute exacerbation of chronic obstructive pulmonary disease. METHODS: A total of 88 patients with acute exacerbation of chronic obstructive pulmonary disease admitted to the Department of Respiratory Medicine, Xuancheng People's Hospital of Anhui Province from December 2014 to December 2016 were randomly divided into the observation group and the control group, with 44 cases in each group. The control group was given routine clinical treatment, and the observation group was given Suhuang Zhike Capsule on the basis of the treatment method of the control group. After 7 days' treatment, the improvement of lung function indexes were observed and evaluated before and after treatment(forced expiratory volume in one second(FEV_1), forced expiratory volume occupancy in the 1^(st) second percentage of vital capacity(FEVl/FVC), peak expiatory flow(PEF)), blood gas analysis index(Arterial oxygen partial pressure(PaO_2) and arterial blood carbon dioxide partial pressure(PaCO_2), oxygenation index(OI)) and serum cytokine levels(procalcitonin(PCT) and C reactive protein(CRP). RESULTS: The total effective rate of the observation group was significantly higher than that of the control group(P < 0.05). The FEV_1, FEV_1/FVC and PEF in 2 groups were significantly increased after the treatment(P < 0.05), and the above lung function indexes in the observation group were significantly higher than the control group(P < 0.05). PaO_2 and OI were significantly increased after the treatment(P < 0.05), PaCO_2 was significantly decreased after the treatment(P < 0.05), and the improvement of above blood gas analysis indexes were significantly superior of the observation group than the control group(P < 0.05). The serum PCT and CRP levels in 2 groups were significantly decreased after the treatment(P < 0.05), and the improvement in the observation group were more significant than that in the control group(P < 0.05). CONCLUSION: Suhuang Zhike Capsule can inhibit serum inflammatory cytokine levels in patients with acute exacerbation of chronic obstructive pulmonary disease, improve blood gas analysis indicators, and improve lung functions.
