Objective:To characterize the group-29 allergens from Dermatophagoides(D.)pteronyssinus and investigate their ability to cross-react with other group-29 allergens from D.pteronyssinus as well as those from D.farinae and Tyrophagus putrescentiae.Methods:Der p 29,Der f 29,and Tyr p 29 cDNA sequences were amplified from total RNA isolated from D.pteronyssinus,D.farinae and Tyrophagus putrescentiae,respectively.Then they were cloned into the pET28a vector,expressed in Rosetta2(DE3)plysS,and purified using anion exchange chromatography.The IgE-binding rates of rDer p 29 were assessed by IgE Western blotting.The four epitopes of rDer p 29 were predicted,synthesized,and detected by IgE-ELISA.The cross-reactivity among the recombinant proteins rDer p 29,rDer f 29,and rTyr p 29 was investigated using dot blot and IgE-ELISA inhibition experiments.The allergens’physiochemical properties,amino acid sequences,and tertiary structures were also compared.Results:Der p 29 was successfully expressed in Rosetta2(DE3)plysS as a single,393-bp open reading frame.Western blotting showed that the purified rDer p 29 protein exhibited an IgE-binding rate of 100%when tested on patient sera.The following four Der p 29 epitopes were predicted and synthesized:37-45(EP1),57-69(EP2),75-80(EP3),and 104-117(EP4).IgE-ELISA tests on 20 D.pteronyssinus-positive sera yielded IgE-binding rates of 85%(rDer p 29),80%(EP1),55%(EP2),40%(EP3),and 55%(EP4),respectively.The dot blot experiments further confirmed cross-reactivity among the three group-29 proteins.When used as an inhibitor,rDer p 29 demonstrated an average cross-reactive inhibition rate of 49.7%against rDer f 29 and 54.4%against rTyr p 29.When rTyr p 29 was used as an inhibitor,it showed an average cross-reactive inhibition rate of 56.3%against rDer f 29.Conclusions:A recombinant protein,rDer p 29 with strong allergenicity was produced.Moreover,it was found that rDer p 29 cross-reacted with rDer f 29 and rTyr p 29,due to their highly homologous sequences and structures.These findings highlight the importance of considering inter-species epitope cross-reactivity when diagnosing and treating allergic diseases. 展开更多

Objective:To examined the immediate and 24 hours post-irradiation germicidal effects of UV-C lamp on.eggs and adults of house dust mites Dermatophagoides pteronyssinus(D.pteronyssinus)and Dermatophagoides farinae(D.farinae).Methods:This study investigated the immediate and24 hours post irradiation mortalities of adult mites exposed to UV-C at different exposure times(5mins,10 mins,15 mins,20 mins,30 mins and 60 mins)and distances(10 cm,25 cm,33 cm,45 cm and 55 cm).Fresh eggs of the 2 dust mites were also irradiated at 10,35 and 55 cm for 0.5,1,2,3,and 5 minutes,and observed daily post-irradiation for up to 7 days.Results:Highest immediate mortality of 100%occurred with direct irradiation al 10 cm distance from UV-C lamp and for 60mins,for both species of mites.The post 24 hours mean mortality rates were(58.4±17.4)%for D.pteronyssinus and(27.7±9.7)%for D.fitrinae when irradiated for 1 hour at 55 cm distance under UV-C lamp.When mites were irradiated in the presence of culture media,the highest mortality rates were lower compared to the direct irradiation;at 10 cm distance and 60 mins exposure,the mean mortality was(74.0±6.8)%for D.pteronyssinus and(70.3±6.7)%for D.farinae.Egg hatchability for both species of mites was also notably reduced by greater than 50%following irradiation.Conclusions:Ultraviolet C irradiation is lethal to an array of organisms by damaging their nucleic acids(DNA and RNA).This study demonstrates the increasing mite mortalities with increasing exposure times and decreasing distances. 展开更多

Objective:To investigate the short and long tenn efficacy of a commercial air ionizer in killing Dermatophagoides pteronyssinus(D.pteronyssinus)and Dermalophagoides farinae(D.farinae)mites.Methods:The effect of a commercial ionizer on D.pteronyssinus and D.farinae was evaluated in the laboratory,using a specially designed test.Mortality was assessed after 6,16and 24 hours for direct exposure and after 24,36,48,60 and 72 hours for exposure in simulated mattress.New batches of mites were used for each exposure time.Results:LT_(50)for direct exposure of ionizer was 10 hours for D.pteronyssinus and 18 hours for D.farinae.The LT_(50)for exposure in simulated mattress was 132 hours or 5.5 days for D.pteronyssinus and 72 hours or 3days for D.farinae.LT_(95)for direct exposure of ionizer was 36 hours for D.pteronyssinus and D.farinae.Meanwhile,the LT_(95)for exposure in simulated mattress was 956 hours or 39.8 days for D.pteronyssinus and 403 hours or 16.8 days for D.farinae.Conclusions:This study demonstrates the increasing mite mortalities with increasing exposure time of a commercial ionizer and suggests that negative ions produced by an ionizer kill dust mites and can be used to reduce natural mile populations on exposed surfaces such as floors,clothes,curtains,etc.However,there is reduced efficacy on mites inside stuffed materials as in mattresses and furniture. 展开更多

Objective: To investigate the effects of dermatophagoides farinae drops combined with pidotimod on inflammatory response and immune response in children with allergic rhinitis combined with asthma. Methods: A total of 118 children with allergic rhinitis combined with asthma who were treated in our hospital between December 2014 and May 2017 were selected as the research subjects and divided into the control group (n=59) and the dermatophagoides farinae drops group (n=59) by random number table method. Control group received both conventional therapy and pidotimod therapy, and dermatophagoides farinae drops group received both conventional therapy and dermatophagoides farinae drops combined with pidotimod therapy. The differences in inflammatory factor, cellular immunity index and humoral immunity index levels were compared between the two groups before treatment (T0), after 3 months of treatment (T1), after 6 months of treatment (T2) and after 12 months of treatment (T3). Results: At T0, inflammatory factor, cellular immunity index and humoral immunity index levels were not significantly different between the two groups. At T1, T2 and T3, serum inflammatory factor IL-2 contents of dermatophagoides farinae drops group were higher than those of control group at the corresponding time points whereas IL-4, IL-5, IL-9 and IL-18 contents were lower than those of control group at the corresponding time points;peripheral blood cellular immunity indexes CD3+, CD4+, CD8+ and CD4+/CD8+levels were higher than those of control group at the corresponding time points;serum humoral immunity indexes IgA, IgM and IgG contents were higher than those of control group at the corresponding time points whereas IgE contents were lower than those of control group at the corresponding time points. Conclusion: Dermatophagoides farinae drops combined with pidotimod therapy can effectively reduce the inflammatory response degree and optimize the cellular and humoral immune function of the children with allergic rhinitis combined with asthma. 展开更多

Objective:To study the ventilation function after sublingual dermatophagoides farinae drops combined with loratadine treatment of children with asthma and allergic rhinitis and the influence on serum biochemical indexes.Methods: A total of 40 children with asthma and allergic rhinitis treated in our hospital between September 2013 and March 2015 were collected and divided into the control group (n=22) who accepted loratadine therapy alone and the observation group (n=18) who accepted sublingual dermatophagoides farinae drops combined with loratadine therapy after the treatment was reviewed. Before treatment and after 6 months and 1 year of treatment, spirometer was used to test ventilation function indexes;ELISA method was used to determine the contents of inflammatory mediators;RIA method was used to determine the contents of airway remodeling indicators.Results:Before treatment, differences in ventilation function index levels as well as inflammatory mediator and airway remodeling index contents were not statistically significant between two groups of children. After 6 months and 1 year of treatment, FEV1, FVC and PEF levels of observation group were higher than those of control group;serum IL-2 content was higher than that of control group while IL-5, IL-17 and IL-33 contents were lower than those of control group;serum PDGF-BB, TGF-β1 and NF-κB contents were lower than those of control group.Conclusion:Sublingual dermatophagoides farinae drops combined with loratadine therapy can optimize the ventilation function, reduce the systemic inflammatory response and inhibit airway remodeling in children with asthma and allergic rhinitis. 展开更多

Background The dust mites, which are mostly represented by Dermatophagoides spp. (Acari: Pyroglyphidae), are the major sources of indoor allergens. Identification and characterization of these mite allergen molecules are an important step in the development of new effective diagnostic procedures and possible therapeutic strategies for allergic disorders associated with dust mites. Methods Total RNA was extracted from Dermatophagoides farinae. The gene coding for Der f 3 was amplified by RT-PCR with the primers designed based on previous sequence published in GenBank. The target gene was cloned intermediately into pMD19-T plasmid and finally into plasmid pET28a (+), expressed in E. coil BL21 at the aid of the inducer isopropyI-D-thiogalactopyranoside (IPTG). The physicochemical properties, spatial structure of the allergen were analyzed with bioinformatics software. Results The cDNA coding for group 3 allergen of Dermatophagoides farinae from China was cloned and expressed successfully. Sequencing analysis showed that there were nineteen mismatched nucleotides in five Der f3 cDNA clones in comparison with the reference (GenBank Accession No. AY283291), which resulted in deduced amino acid sequence incompatibility in eleven residues. Bioinformatics analysis revealed that the Der f 3 pro-protein was an extracellular hydrophobic protein, consisting of 259 amino acids with a 16 amino acid signal peptide. The protein was deduced to have three chymotrypsin active sites (53-68 AA, 108-122 AA and 205-217 AA), one N-glycosylation site, one cAMP- and cGMP-dependent protein kinase phosphorylation site, four protein kinase C phosphorylation sites, two casein kinase II phosphorylation sites, and five N-myristoylation sites. Conclusions Der f3 is an extracellular hydrophobic protein which possesses multiple activation and phosphorylation sites. Polymorphism may exist in the Der f3 gene but this needs to be further confirmed in the future. 展开更多

目的:评价粉尘螨滴剂舌下免疫治疗(SLIT)联合吸入性糖皮质激素(ICS)对粉尘螨过敏的哮喘患儿在临床症状、气道炎症及肺功能方面的改善作用,以明确SLIT能否作为有效的辅助治疗改善哮喘患儿的肺功能与整体控制水平。方法:选取2017年3月至2... 目的:评价粉尘螨滴剂舌下免疫治疗(SLIT)联合吸入性糖皮质激素(ICS)对粉尘螨过敏的哮喘患儿在临床症状、气道炎症及肺功能方面的改善作用,以明确SLIT能否作为有效的辅助治疗改善哮喘患儿的肺功能与整体控制水平。方法:选取2017年3月至2019年3月于成都市妇女儿童中心医院门诊就诊的133名对尘螨过敏的哮喘患儿,按随机数字表法分为观察组[接受尘螨花粉滴剂联合吸入性糖皮质激素(ICS)治疗]和对照组(仅接受ICS治疗)。统计分析两组患儿治疗前、治疗一年后的鼻炎症状总评分(TRSS)、日间哮喘症状总评分(DASS)、夜间哮喘症状总评分(NASS)、生活质量视觉模拟量表(Qol-VAS)、药物总评分(TMS)、呼出气一氧化氮(FeNO)、用力肺活量(FVC)、一秒用力呼气容积(FEV_(1))、呼气峰流速(PEF)、用力呼气量50%和75%(FEF_(50)和FEF_(75))、最大呼气中期流速(MMEF)。结果:观察组治疗1年后TRSS、DASS、NASS、Qol-VAS、TMS、FeNO、FVC、FEV1、PEF、FEF_(50)、FEF_(75)和MMEF的改善程度均高于对照组(均P<0.05)。结论:作为对尘螨过敏的哮喘患儿的辅助治疗,尘螨滴剂舌下免疫治疗(SLIT)能显著改善哮喘控制水平。 展开更多

Allergenicity assay with immunological sera indicated that the extracts from the transgenic tobacco gave obvious positive IgE binding reaction with specific serum pool. 目的比较屋尘螨(安脱达)和屋尘螨/粉尘螨(阿罗格)变应原集群免疫治疗递增期的安全性及疗效。方法回顾性分析2022年6月至2024年4月,在华中科技大学同济医学院附属同济医院过敏反应科接受安脱达(33例)和阿罗格(30例)集群免疫治疗的变应性... 目的比较屋尘螨(安脱达)和屋尘螨/粉尘螨(阿罗格)变应原集群免疫治疗递增期的安全性及疗效。方法回顾性分析2022年6月至2024年4月,在华中科技大学同济医学院附属同济医院过敏反应科接受安脱达(33例)和阿罗格(30例)集群免疫治疗的变应性鼻炎患者剂量递增期不良反应发生情况,并比较两组完成集群免疫治疗的患者递增期治疗前后鼻炎症状及生活质量评分。结果安脱达和阿罗格集群免疫治疗组递增期,速发型大局部反应发生率分别为0和0.35%,速发型全身反应发生率分别为0.24%和0.35%,迟发型大局部反应发生率分别为0和0.70%,迟发型全身反应发生率为2.41%和5.57%。两组速发型不良反应发生率差异无统计学意义(P>0.05),但阿罗格组迟发型全身反应发生率较安脱达组高(P=0.029)。阿罗格组出现全身反应者的屋尘螨特异性IgE/总IgE比值较未出现全身反应者高(P=0.044)。两组患者治疗后鼻炎症状评分和生活质量评分均较治疗前有改善(P<0.05),但两组间比较差异无统计学意义(P>0.05)结论屋尘螨和屋尘螨/粉尘螨变应原制剂集群免疫治疗递增期可快速起效,速发型不良反应发生率低,屋尘螨特异性IgE/总IgE比值高者需警惕全身反应发生。 展开更多

目的旨在建立尘螨过敏原鼻激发试验(nasal allergen challenge,NAC)的标准化操作流程、结果判读及临床应用规范,并系统评估尘螨NAC的操作稳定性及安全性。方法2023年1月~2024年12月纳入49例尘螨血清特异性IgE(sIgE)阳性患者及50名健康... 目的旨在建立尘螨过敏原鼻激发试验(nasal allergen challenge,NAC)的标准化操作流程、结果判读及临床应用规范,并系统评估尘螨NAC的操作稳定性及安全性。方法2023年1月~2024年12月纳入49例尘螨血清特异性IgE(sIgE)阳性患者及50名健康对照者,并获得受试者的人口统计学和临床数据。采用标准化尘螨过敏原提取物试剂配制得浓度梯度递增(1000、3000、10000、30000 BU/ml)的尘螨激发液,对受试者进行双侧同步鼻喷雾激发,期间使用鼻部症状总评分、视觉模拟量表、鼻声反射、主动经前鼻测压法和四相鼻阻力测试进行评估,记录评估数据及判定结果。结果与基线水平对比,NAC后患者主观评分和客观评价指标均显著变化(P≤0.001),其中40例(81.6%)患者对尘螨的NAC呈现阳性反应。所有健康受试者均为NAC阴性。结论尘螨过敏原皮肤点刺液可作为可靠的NAC激发液来源,建立尘螨NAC的标准化流程及临床应用规范,包括试验的适应证、禁忌证、激发液配制、操作流程、操作标准、阳性判定标准、检查前后注意事项等,为尘螨NAC的临床应用提供指导。 展开更多

6种植物精油对粉尘螨体外杀灭活性的研究

目的:分析并测定从6种中药材植物中提取的精油对粉尘螨的触杀活性,为新型植物源杀螨剂的开发提供思路和理论依据。方法:每种植物精油分别配制6个浓度梯度的精油试液,采用直接接触法测定6种植物精油不同浓度下对粉尘螨的触杀活性。结果:... 目的:分析并测定从6种中药材植物中提取的精油对粉尘螨的触杀活性,为新型植物源杀螨剂的开发提供思路和理论依据。方法:每种植物精油分别配制6个浓度梯度的精油试液,采用直接接触法测定6种植物精油不同浓度下对粉尘螨的触杀活性。结果:在精油浓度为25 mg/mL时,山鸡椒、姜黄、地构叶的24 h校正死亡率分别为100.00%、100.00%、97.32%;在精油浓度为15 mg/mL时,山鸡椒、姜黄、地构叶的24 h校正死亡率分别可以达到100.00%、75.38%、82.11%;在精油浓度为10 mg/mL时,甜杏仁、苦楝、葡萄籽的24 h校正死亡率分别为3.10%、3.74%、5.17%;山鸡椒、姜黄、地构叶的LC 50分别为2.811、8.571、2.591 mg/mL;按照触杀活性进行系统聚类分析,可将6种植物精油分为3类:第一类为山鸡椒,第二类为地构叶和姜黄,第三类为苦楝、甜杏仁和葡萄籽。结论:山鸡椒、姜黄、地构叶精油具有良好的杀螨效果,可为新型植物源杀螨剂的研究开发提供理论依据。 展开更多

目的观察氯雷他定联合粉尘螨滴剂舌下含服治疗变应性鼻炎(AR)的临床效果。方法选取2022年6月—2023年6月龙岩人民医院收治的AR患者100例,按随机数字表法分为观察组和对照组,各50例。对照组给予粉尘螨滴剂舌下含服治疗,观察组在对照组基... 目的观察氯雷他定联合粉尘螨滴剂舌下含服治疗变应性鼻炎(AR)的临床效果。方法选取2022年6月—2023年6月龙岩人民医院收治的AR患者100例,按随机数字表法分为观察组和对照组,各50例。对照组给予粉尘螨滴剂舌下含服治疗,观察组在对照组基础上给予氯雷他定治疗,2组均治疗6个月。比较2组治疗前后鼻部症状[鼻部症状评分表(TNSS)评分]、视觉模拟评分法(VAS)评分、鼻腔功能[鼻阻力(NR)、鼻黏膜纤毛传输时间(MTT)、鼻黏膜纤毛清除速率(MCV)]、生活质量[鼻结膜炎生活质量问卷(RQLQ)评分]及不良反应。结果治疗6个月后,2组鼻痒、鼻塞、喷嚏、流涕评分低于治疗前,且观察组低于对照组(P<0.01);2组VAS评分低于治疗前,且观察组低于对照组(P<0.01);2组NR小于治疗前,MTT短于治疗前,MCV高于治疗前,且观察组变化幅度大于对照组(P<0.01);2组RQLQ各项评分低于治疗前,且观察组低于对照组(P<0.01)。观察组不良反应总发生率为12.00%,与对照组的20.00%比较差异无统计学意义(χ^(2)=1.191,P=0.275)。结论氯雷他定联合粉尘螨滴剂舌下含服治疗AR临床效果较好,可有效缓解鼻部相关症状,改善鼻腔功能,提高生活质量。 展开更多

转录组测序分析粉尘螨抗辣根素的基因表达与机制

目的通过转录组测序分析辣根素作用后粉尘螨差异表达基因(DEG)的变化及作用机制。方法将90个熏蒸瓶随机均分为实验组和对照组。每瓶取80只粉尘螨背部朝下置于熏蒸瓶的胶板上,实验组熏蒸瓶中加入0.25 ml/L的辣根素稀释液25µl,对照... 目的通过转录组测序分析辣根素作用后粉尘螨差异表达基因(DEG)的变化及作用机制。方法将90个熏蒸瓶随机均分为实验组和对照组。每瓶取80只粉尘螨背部朝下置于熏蒸瓶的胶板上,实验组熏蒸瓶中加入0.25 ml/L的辣根素稀释液25µl,对照组加入石蜡。熏蒸24 h后,每组收集2000只存活螨。实验组和对照组各重复6次,3次用于测序,3次用于实时荧光定量PCR(qPCR)验证。提取粉尘螨RNA并逆转录为cDNA,构建文库并进行转录组测序。测序数据进行质控组装后与非冗余蛋白(NR)、基因本体功能注释(GO)、京都基因与基因组百科全书(KEGG)等数据库进行比对。使用edgeR软件进行差异表达分析,对DEG进行GO与KEGG功能注释与富集。选取粉尘螨在辣根素作用后的DEG,进行qPCR验证,使用R软件对qPCR数据进行方差分析。结果转录组测序获得56625条转录本,总长度为72418610 nt。共获得的39510条单基因簇,比对到NR数据库中的数量最多,为25450个,其次分别为直系同源蛋白分组比对(eggNOG)数据库(17643个)、蛋白质(Swiss-prot)数据库(13338个)、GO数据库(13234个)、KEGG数据库(9445个)。辣根素作用粉尘螨后,共检测到2719个DEG,其中上调基因1185个,下调基因1534个。GO功能注释结果显示,DEG主要与细胞过程、结合与催化活性等功能相关,GO富集分析结果显示,上调基因主要为ATP结合盒转运蛋白G亚家族(ABCG)、细胞色素P450(CYP450)、热休克蛋白、乙酰胆碱受体等;下调基因主要为钙调蛋白、RNA聚合酶等。KEGG注释显示,DEG主要参与运输与分解代谢、信号转导、翻译等过程。KEGG富集分析显示,DEG富集于30条通路,主要富集在核糖体、不同环境中微生物代谢以及次生代谢物的生物合成等通路。qPCR结果显示,实验组UDP-葡萄糖醛酸转移酶(UGT)、多重耐药相关蛋白1(MRP1)基因的相对转录水平分别为1.18±0.22、6.22±0.42,较对照组(0.76±0.30、2.52±1.75)显著上调(F=11.22、16.83,均P<0.05);线粒体核糖体相关GTP酶(MTG)的相对转录水平为0.05±0.04,较对照组(1.23±0.62)显著下调(F=13.95,P<0.05);均与转录组测序结果一致。结论辣根素作用后粉尘螨体内多种基因存在表达差异,UGT、CYP450、MRP1、ABCG基因在对辣根素产生抗性或解毒代谢中起重要作用。 展开更多

目的研究粉尘螨体内微生物群落的组成、结构和功能,并分析其功能基因特征。方法采集3份粉尘螨样品(每份150只),提取3份粉尘螨总DNA,构建双末端测序文库。桥式PCR扩增后进行宏基因组测序。将获得的基因序列与非冗余蛋白(NR)、直系同源蛋... 目的研究粉尘螨体内微生物群落的组成、结构和功能,并分析其功能基因特征。方法采集3份粉尘螨样品(每份150只),提取3份粉尘螨总DNA,构建双末端测序文库。桥式PCR扩增后进行宏基因组测序。将获得的基因序列与非冗余蛋白(NR)、直系同源蛋白分组比对(EggNOG)、京都基因与基因组百科全书(KEGG)、碳水化合物活性酶(CAZY)、全面抗生素抗性(CARD)等数据库进行BLAST比对。采用α多样性指数实际观测物种数指数、Ace指数、Chao1指数、香农多样性指数、辛普森多样性指数、覆盖度指数分析粉尘螨体内微生物的基因多样性。结果3份粉尘螨样品宏基因组测序获得1.18×10^(7)~1.29×10^(7)个碱基。经拼接后,得到的重叠群数目为34527个,N50为25005个,重叠群的平均长度为11509个。对粉尘螨体内微生物预测基因进行物种注释,选取水平上相对丰度前20位的物种进行分析,共注释到粉尘螨体内微生物73门430属689种,其中细菌最为丰富,占80.9%,其次为病毒(5.5%),再次为真菌(1.2%)。在细菌群落中,假单胞菌门(97.8%)和厚壁菌门(0.8%)是优势菌门,巴尔通体属(59.0%)和未分类的α变形菌属(11.0%)为优势菌属。在真菌群落中,微孢子门(66.6%)和子囊菌门(19.6%)占主导地位,Pancytospora(41.4%)和Saccharomyces(12.2%)为优势菌属。在病毒群落中,Cressdnaviricota(96.2%)和Uroviricota(2.6%)为优势菌门,环状病毒属(96.2%)为优势菌属。α多样性结果显示,3份粉尘螨样品重复性好。EggNOG数据库蛋白序列注释结果显示,细菌和真菌分别注释到23和22种功能类别,其中翻译/核糖体结构和生物发生类别的基因数在细菌(15.3%)和真菌(27.4%)中均占比最高。KEGG数据库分析基因功能结果显示,细菌中辅因子和维生素的代谢相关基因占主导地位(10.5%),在真菌中,折叠、分类和降解相关的基因更为突出(12.0%)。CAZY数据库对碳水化合物代谢酶家族的注释结果表明,在细菌中,高达98.3%的基因能够成功匹配到65个不同的碳水化合物代谢酶家族,其中以糖基转移酶家族占主导地位(57.6%)。真菌中仅有0.4%的基因能够注释到12个碳水化合物代谢酶家族,糖基转移酶家族也是主要酶家族(87.7%)。CARD数据库的抗性基因注释结果显示,在细菌中,99.1%的基因能够注释到17种不同的抗生素抗性基因,其中多重耐药性基因占比最高(39.5%)。在真菌中,0.3%的基因能够注释到5种抗生素抗性基因,以四环素抗性基因和多重耐药性基因为主,分别占41.6%和39.7%。结论粉尘螨体内微生物群落存在复杂性和多样性,细菌和真菌在基因功能和代谢途径存在差异。 展开更多

目的:探究粉尘螨滴剂联合布地奈德治疗哮喘患儿的临床效果,分析其对患儿气道炎症、户尘螨特异性免疫球蛋白G4(sIgG4)及粉尘螨sIgG4水平的影响。方法:选取2021年12月—2023年12月上饶市妇幼保健院收治的80例哮喘患儿作为研究对象,根据随... 目的:探究粉尘螨滴剂联合布地奈德治疗哮喘患儿的临床效果,分析其对患儿气道炎症、户尘螨特异性免疫球蛋白G4(sIgG4)及粉尘螨sIgG4水平的影响。方法:选取2021年12月—2023年12月上饶市妇幼保健院收治的80例哮喘患儿作为研究对象,根据随机数字表法分为对照组和试验组,各40例。对照组接受布地奈德吸入治疗,试验组在对照组基础上给予粉尘螨滴剂治疗,两组均治疗6个月。比较两组临床症状积分、肺功能指标[呼气峰流速实测值占预计值百分比(PEF%pred)、FEV1%pred、FVC%pred、FEV1/FVC]、气道炎症指标[IL-4、IFN-γ、呼出气一氧化氮(FeNO)]及户尘螨sIgG4、粉尘螨sIgG4水平。结果:治疗后,试验组日间症状积分、夜间症状积分均低于对照组,FEV1%pred、FVC%pred、FEV1/FVC和PEF%pred水平均高于对照组(P<0.05)。治疗后,试验组血清IFN-γ、户尘螨sIgG4、粉尘螨sIgG4水平均高于对照组,血清IL-4、FeNO水平均低于对照组(P<0.05)。结论:粉尘螨滴剂联合布地奈德有利于改善哮喘患儿的临床症状,提高肺功能,减轻气道炎症反应,使户尘螨sIgG4、粉尘螨sIgG4表达水平升高。 展开更多

目的:探究芍麻止痉颗粒联合粉尘螨滴剂治疗抽动障碍(TD)合并过敏性鼻炎的临床价值。方法:选取瑞金市人民医院2024年1—8月收治的60例TD合并过敏性鼻炎患儿为研究对象,以随机数字表法分为对照组、观察组,每组30例。两组均予盐酸西替利嗪... 目的:探究芍麻止痉颗粒联合粉尘螨滴剂治疗抽动障碍(TD)合并过敏性鼻炎的临床价值。方法:选取瑞金市人民医院2024年1—8月收治的60例TD合并过敏性鼻炎患儿为研究对象,以随机数字表法分为对照组、观察组,每组30例。两组均予盐酸西替利嗪滴口服溶液治疗。对照组予以芍麻止痉颗粒治疗,观察组予以芍麻止痉颗粒联合粉尘螨滴剂治疗。记录两组抽动症状、过敏性鼻炎症状及TD、过敏性鼻炎复发情况。结果:治疗30 d后,观察组耶鲁综合抽动严重程度量表(YGTSS)中抽动频率、类型评分及总分较对照组低(P<0.05);治疗45、56 d后,观察组YGTSS中抽动类型、频率、强度、复杂程度、干扰程度评分及总分较对照组低(P<0.05)。治疗10、15 d后,观察组鼻症状总分表(TNSS)评分与对照组比较,差异无统计学意义(P>0.05);治疗30、45、56 d后,观察组TNSS评分较对照组低(P<0.05)。观察组TD、过敏性鼻炎复发率均较对照组低(P<0.05)。结论:在TD合并过敏性鼻炎患儿中应用芍麻止痉颗粒联合粉尘螨滴剂可有效控制患儿抽动症状及过敏症状,降低TD、过敏性鼻炎复发风险。 展开更多

探讨舌下含服粉尘螨滴剂治疗不同年龄段变应性鼻炎患者的临床疗效。选取安顺市人民医院2022年1月—2024年10月收治的78例AR患者作为研究对象,所有患者均采用舌下含服粉尘螨滴剂进行治疗。根据年龄段将患者分为青少年组(12~18岁)和成年人... 探讨舌下含服粉尘螨滴剂治疗不同年龄段变应性鼻炎患者的临床疗效。选取安顺市人民医院2022年1月—2024年10月收治的78例AR患者作为研究对象,所有患者均采用舌下含服粉尘螨滴剂进行治疗。根据年龄段将患者分为青少年组(12~18岁)和成年人组(≥18岁),每组39例,比较两组患者的治疗效果。结果显示,治疗后,两组症状评分、视觉模拟评分及药物评分均显著降低,且青少年组各项评分均显著低于成年人组(P<0.05);青少年组不良反应发生率显著高于成年人组(P<0.05);青少年组生活质量各维度评分均显著优于成年人组(P<0.05)。研究发现,舌下含服粉尘螨滴剂对各年龄段变应性鼻炎患者均有效,青少年组在症状控制和生活质量改善方面效果更显著,但需注意其较高的不良反应风险,临床应根据年龄差异制定个体化治疗方案。 展开更多

目的: 回顾性调查粉尘螨(Derm atophagoidesfarinae)变应原诊断和免疫治疗哮喘及过敏性鼻炎的安全性。方法:问卷评估1974~1995 年用粉尘螨变应原(SMU-Df)诊断和免疫治疗变态反应的安全性。结果:统计分析846 342 例次。全身性不良反应... 目的: 回顾性调查粉尘螨(Derm atophagoidesfarinae)变应原诊断和免疫治疗哮喘及过敏性鼻炎的安全性。方法:问卷评估1974~1995 年用粉尘螨变应原(SMU-Df)诊断和免疫治疗变态反应的安全性。结果:统计分析846 342 例次。全身性不良反应共发生142 例次, 总发生率为1.68(CL= 1.4~2.0); 依次为全身性荨麻疹0.82、哮喘大发作0.77、过敏性休克0.07(CL= 1.4/百万~12.0/百万)、血管神经性水肿0.02。出现即时全身性反应在皮试或注射后30 m in 内有32例次,1 h 和2 h 各6 例次; 后期反应3 h~48 h 有88 例次, 以哮喘大发作的机率最高。发生的时期在皮试时共有18 例次,免疫递增期96例次, 维持期14 例次。共有6 例次过敏性休克立刻处理, 全部缓解。41 例次全身性反应的原因, 多半是在哮喘发作期或由于差错注射浓度过大及剂量过大。结论:22 年应用SMU-Df皮试和免疫皮下注射治疗。 展开更多
