Perceptual learning, a new method for treating amblyopia, improves the ability to process signals from the cerebral optic nerve system by specific visual stimulation and visual learning, as well as activation of the visual signal pathway utilizing brain nervous system plasticity. OBJECTIVE: This study investigated and evaluated the curative effects of perceptual learning, which can directionally increase brain plasticity, on the treatment of amblyopia in children. The relationship between curative effect and time was also analyzed. DESIGN: A self-control experiment. SETTING: Visual Science and Optometry Center, People's Hospital of Guangxi Zhuang Autonomous Region. PARTICIPANTS: A total of 125 amblyopic children (250 amblyopic eyes), 73 males, 52 females, averaging (6±2) years of age, received treatment at the Visual Science and Optometry Center, People's Hospital of Guangxi Zhuang Autonomous Region between September 2006 and February 2007 and were recruited for this study. All children presented with no structural disease of the eyeballs. Written informed consent for therapeutic regiments was obtained from each child's parent. The protocol received approval from the Hospital's Ethics Committee. METHODS: Visual function was tested with a perceptual learning system (Research Center for Human Health and Development of Sun Yat-sen University, National Engineering Technique Research Center for Medical Care Implement) for visual noise, position noise, contour discrimination, contrast sensitivity, grating stereogram, and random-dot fusion. These tests helped to evaluate the efficiency of visual information processing of these children, and to determine the degree of defects of the optic nerve cells and the connections of visual cortical neurons. According to results of visual function tests, individualized treatment was adopted for each amblyopia patient using perceptual learning system. One course of treatment lasted one month, and treatment was performed twice every day with two training procedures (each training procedure lasted for ten minutes). There was a ten-minute time interval between the two training procedures. The training treatment was performed in a quiet and dark environment. Visual acuity and recovery of visual function were tested every month. Original training procedure was continued or adjusted according to the results of visual function. MAIN OUTCOME MEASURES: Visual function change; relationship of curative effects and curative time. RESULTS: A total of 125 amblyopia children were included in the final analysis. The total efficiency of perceptual learning for treating amblyopia in children was 75.2%. Visual acuity began to greatly increase 3 months after treatment (P 〈 0.05). Visual acuity was best corrected from 0.60 ± 0.23 before treatment to 0.86 ± 0.26 after treatment (P 〈 0.05). The mean time to reach improved levels with curative effects was (2.82 ± 1.30) months, and to reach a basically cured level was (2.87 ±1.40) months. Percentage of improved visual acuity was the highest [98% (39/40)] in children that received 3 months of treatment and the lowest [55% (31/56)] in children that received 1 month of treatment (P 〈 0.05). The percentage of basically cured levels with curative effects increased with length of learning time and was the greatest in children that received 4 months of treatment [67% (31/46), P 〈 0.05]. CONCLUSION: Perceptual learning rapidly and remarkably improves visual function of amblyopia children; however, the curative effects are first apparent two and three months after intervention.展开更多

And analyze the relationship between compatibility of medicines, content and clinical curative effect. Methods: Water extracts were analyzed by high-performance liquid chromatography-diode array detection(HPLC-DAD), using an Agilent extend-C_(18)(4.6 mm×250 mm,5μm) column by using solvent A(0.2% phosphoric acid water) and solvent B(methanol) in the ratio of 52:48(v/v) at a flow rate of 1 ml/min. The column temperature was maintained at 25°C and the detection wavelength was set at 280 nm.Results: In quantitative analysis, the four selected markers showed good regression(R^2>0.999) within test ranges. The average recoveries were between 99.75~100.60% and their RSD values were between 0.97%~1.71%. The proportions of the four chemical compounds for different efficacy were 33.51~35.76: 6.78~6.87: 3.45~3.64: 1; 18.6~23.64: 3.63~4.27: 1.36~1.53: 1; 29.94~31.78: 5.14~5.23: 3.28~3.56: 1; 33.99~41.91:5.83~7.42: 2.83~3.39: 1.Conclusion: Clinical practice of thousands of years have proved that the curative effect of Traditional Chinese Medicine depends on the compatibility of different kinds of medicine material crude slices. According to this study we found that different compatibility of medicines leads to different proportions of the chemical compounds. We assume that there is a certain correlation between therapeutical effect of TCM and compatibility of medicines.展开更多

文摘BACKGROUND: Conventional methods (such as occlusion therapy, fine manipulation, complementary, and alternative medicine) take effects slowly, are time and labor consuming, and have uncertain curative effects in the treatment of amblyopia. CONCLUSION: Perceptual learning rapidly and remarkably improves visual function of amblyopia children; however, the curative effects are first apparent two and three months after intervention.
文摘The anti-fungus mechanisms and curative effects of cinnamon oil and pogostemon oil complexes to wards intestinal Candida infections were investigat ed.We measured the minimal inhibitory concentra tion(MIC) values of the complexes against Candida using proportionally-diluted test-tube medium and examined the evolution of the morphology and structures of Candida albicans using scanning electronic microscopy(SEM) and transmission elec tronic microscopy(TEM).We found that the aver age MIC values of the complexes against the fung were 0.064 mg/mL(cinnamon oil),0.032 mg/mL(pogostemon oil) for Candida albicans,0.129 mg/mL(cinnamon oil),0.064 mg/mL(pogostemon oil for Candida tropicalis, and 0.129 mg/mL(cinnamon oil),0.064 mg/mL(pogostemon oil),for Candida krusei.SEM examination over a 24-48 h period showed that the morphology of Candida albicans cells changed significantly.Irregular hollows ap peared on the surfaces, inside organelles were destroyed and the cells burst after treatment.TEM examination over a 48-72 h period indicated that the cell walls were damaged, organelles were destroyed and most cytoplasms became empty bubbles.Sixty intestinal Candida-infected patients were treated with a capsule containing cinnamon and pogostemon oil.The curative ratio was 71.67%(43/60),and the improvement ratio was 28.33%(17/60),giving a to talratio of 100%.Thus,the cinnamonoil and pogostemon oil complexes had strong anti-funguseffectsag ainst Candida albicans,Candidatropicalis, and Candidakrusei. They impacted the morphology and sub-micro structures of the fungus within48-72h,and eventually denatured and killed the cells. The complexes have also shown considerable curative effects to intestinal Candida infections.

文摘Background Adult Fulminant myocarditis(FM)is the more serious form of acute myocarditis, and extracorporeal membrane oxygenation(ECMO)is an effective adjunction that helps to maintain circulation while providing adequate treatment. The blood supply of vital organs is guaranteed, which helps patients to survive the acute attack and improves the survival rate of FM.Methods In this study,50 clinical data of adult FM patients received in our hospital from July 2018 to July 2023 were collected for retrospective analysis.According to different treatment methods, they were divided into ECMO group(n=25)and non-ECMO group(n=25).After 14 days of treatment, the level of left ventricular ejection fraction(LVEF),left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD),creatine kinase isoenzymes(CK-MB),cardiac troponinⅠ(cTnⅠ)and brain natriuretic peptide(BNP),and the incidence of complications were compared between the two groups. Results Before treatment, there was no significant difference in cardiac function between the two groups(P>0.05).After 14 days of treatment,ECMO group showed greater decline than non-ECMO group in LVESD and LVEDD,and LVEF was higher than that in the non-ECMO group(P<0.05).The level of CK-MB,cTnⅠ and BNP in ECMO group were lower than those in non-ECMO(P<0.05).The overall incidence of complications was 8% in the ECMO group, which was lower than 32% in the non-ECMO group(P<0.05).Conclusions The early efficacy of ECMO treatment in adult patients with FM is considerable, which is conducive to improving the cardiac function of patients and preventing complications.
文摘BACKGROUND Advanced gastric cancer is characterized by fast tumor growth and aggressive biological behavior.During neoadjuvant chemotherapy, patients are at risk of distant metastasis or local progression.Anemia is a frequent complication in these patients.AIM To analyze whether changes in hemoglobin and hematocrit can predict the survival and efficacy of neoadjuvant chemotherapy in patients with advanced gastric cancer.METHODS The clinical data of 185 patients with advanced gastric cancer admitted to the Third Affiliated Hospital of Chengdu Medical College,Pidu District People’s Hospital,Chengdu,China, between January 2016 and January 2021,were retrospectively analyzed. All patients underwent a tegafur+oxaliplatin+apatinib chemotherapy regimen.According to the efficacy of chemotherapy, they were divided into an effective group(complete or partial response,n=121)and an ineffective group(stable disease or disease progression,n=64).The factors related to chemotherapy efficacy in patients with advanced gastric cancer were analyzed by univariate and logistic multivariate analyses. The 3-year survival rates of the patients with different hemoglobin and hematocrit levels were compared.RESULTS Univariate analysis showed that the proportion of patients with a tumor diameter>5 cm,non-tubular adenocarcinoma, lymph node metastasis,hematocrit<33%,low mean red blood cell(RBC)protein content, low RBC distribution width,hemoglobin<107 g/L,and platelets>266×109/L in the ineffective group were significantly higher than those in the effective group(P<0.05).Logistic multivariate analysis showed that a tumor diameter>5 cm,lymph node metastasis,≤3 chemotherapy cycles,hematocrit<33%,and hemoglobin<107 g/L are risk factors for neoadjuvant chemotherapy failure in advanced gastric cancer(P<0.05).The 1-year,2-year,and 3-year survival rates in the effective group were 93.39%,83.47%,and 60.33%,respectively.These rates were significantly higher than those in the ineffective group(P<0.05).The 1-year,2-year,and 3-year survival rates of patients with hematocrit<33%were 74.67%,49.33%,and 29.33%,respectively,which were significantly lower than those of patients with hematocrit≥33%(P<0.05).The 1-year,2-year,and 3-year survival rates of patients with hemoglobin<107 g/L were 80.39%,58.82%,and 39.22%,respectively,which were significantly lower than those of patients with hemoglobin≥107 g/L(P<0.05).CONCLUSION Hematocrit<33%and hemoglobin<107 g/L are risk factors for chemotherapy failure in patients with advanced gastric cancer. They are associated with poorer prognosis and reduced 3-year survival rates.
文摘BACKGROUND Knee osteoarthritis(KOA)is a prevalent degenerative joint disorder characterized by complex neuroinflammatory mechanisms involving peripheral-central nervous system crosstalk.Current research gaps exist regarding the modulatory effects of biomechanical interventions such as postural correction training(PCT)on these pathways, particularly its impact on neurogenic inflammation and associated nerve dysfunction.AIM To examine the effect of PCT on chronic pain related to KOA,nerve function, and inflammatory factors and further assess the influencing factors.METHODS This study included 100 patients with chronic pain related to KOA admitted to our hospital from March 2022 to March 2024 who were selected as research subjects, and divided into a control group(conventional treatment,n=50)and observation group(combined treatment with PCT,n=50).Efficacy,pain[visual analog scale(VAS)],nerve function[the National Institute of Health Stroke Scale(NIHSS)]and inflammatory factors[interleukin(IL)-1β,IL-6,tumor necrosis factor-alpha(TNF-α),C-reactive protein(CRP)]were assessed and compared. Moreover, the factors influencing efficacy were assessed according to clinical efficacy.RESULTS The clinical effectiveness rate of 90.00%in the observation group was higher than that of 72.00%in the control group(P<0.05).VAS scores at 14 and 30 days of the intervention were lower than those before the intervention(P<0.05).Moreover,VAS scores in the observation group at 14 and 30 days after the intervention were lower than those in the control group(P<0.05).The NIHSS scores were lower after the intervention than those before the intervention for both groups(P<0.05).The improvement in NIHSS score in the observation group was higher than that in the control group(P<0.05).Inflammatory factors such as IL-1β,IL-6,TNF-α,and CRP in both groups among patients with osteoarthritis-related chronic pain were lower after the intervention than before the intervention(P<0.05).After the intervention, all inflammatory factors in the observation group were lower than those in the control group(P<0.05).The proportion of ineffective treatment combined with joint effusion,Kellgren-Lawrence(K-L)staging grade III-IV,fixed flexion contracture with varus and valgus deformity>5°,was higher in the control group than in the observation group(P<0.05),while the joint compartment involvement in the observation group was higher than that in the control group(P<0.05).The logistic regression results demonstrated that relevant joint effusion,K-L staging grade III-IV,fixed flexion contracture with varus and valgus deformity>5°,and intervention mode of PCT were higher in the control group than in the observation group(P<0.05)and were influencing factors on clinically ineffective treatment(P<0.05).CONCLUSION PCT can improve the treatment effect on chronic pain related to KOA,nerve function and inflammatory response.Joint effusion, joint stiffness, and KOA are factors for y ineffective treatment.Joint effusion, higher K-L stage, and larger flexion contracture were risk factors, while PCT was a protective factor for ineffective treatment.

文摘BACKGROUND Currently, colonoscopy still needs continuous optimization and exploration of novel alternative approaches to enhance the experience of patients during co-lonoscopy.AIM To analyze the efficacy of water infusion combined with defoamers in colono-scopy.METHODS This study included 97 patients undergoing colonoscopy from January 2024 to June 2024.The participants were categorized into two groups, namely, the control group(n=47),who underwent conventional colonoscopy, and the experimental group(n=50),who received colonoscopy using water injection combined with defoamers.A comparative analysis was then conducted on the disease detection rate(colonic polyps, colonorrhagia, colonic ulcers, colonic mucosal lesions, and others),colonoscopy duration, abdominal pain[visual analog scale(VAS)],Boston bowel preparation scale(BBPS),self-rating anxiety scale(SAS),bowel preparation comfort, complications(intestinal perforation, bleeding, nausea and vomiting, abdominal pain, and abdominal distension),and patient satisfaction.RESULTS The experimental group demonstrated a significantly higher total disease detection rate,BBPS scores, and patient satisfaction compared with the control group.Further, the research group exhibited shorter colonoscopy duration, lower VAS and SAS scores and total complication rate, and better patient comfort and satisfaction.CONCLUSION These results indicate that the combination of water injection and defoamers exhibited an overall better therapeutic effect than conventional colonoscopy, mainly reflected in higher disease detection rate, faster examination ef-ficiency,lower abdominal pain, anxiety, and complication incidences, and significantly better bowel preparation, comfort, and patient satisfaction.
文摘BACKGROUND Although substantial evidence supports the advantages of cold snare polypectomy(CSP)in terms of polypectomy efficacy and reduced postoperative adverse events, few studies have examined the cost differences between CSP and traditional endoscopic mucosal resection(EMR)for the treatment of intestinal polyps.AIM To compare the efficacy-cost of EMR and CSP in the treatment of intestinal polyps.METHODS A total of 100 patients with intestinal polyps were included in the retrospective data of our hospital from April 2022 to May 2023.According to the treatment methods, they were divided into EMR(n=46)group and CSP(n=54)group.The baseline data of the two groups were balanced by 1:1 propensity score matching(PSM),and the cost-effectiveness analysis was performed on the two groups after matching. The recurrence rate of the two groups of patients was followed up for 1 year, and they were divided into recurrence group and non-recurrence group according to whether they recurred.Multivariate logistic regression analysis was used to screen out the influencing factors affecting the recurrence of intestinal polyps after endoscopic resection.RESULTS Significant disparities were observed in the number of polyps and smoking background between the two groups before PSM(P<0.05).Following PSM,the number of polyps and smoking history were well balanced between the EMR and CSP groups. The direct cost incurred by the CSP group was markedly higher than that incurred by the EMR group.Concurrently, the cost-effectiveness ratio in the CSP group was substantially reduced when juxtaposed with that in the EMR group(P<0.05).Upon completion of the 1-year follow-up,the rate of recurrence after endoscopic intestinal polypectomy was 38.00%.Multivariate methods revealed that age≥60 years, male sex, number of polyps≥3,and pathological type of adenoma were risk factors for recurrence after endoscopic intestinal polypectomy(all P<0.05).CONCLUSION CSP was more cost-effective for the treatment of intestinal polyps. An age≥60 years, male sex, having a number of polyps≥3,and pathological type of adenoma are independent influencing factors for recurrence.
文摘[Objectives]To discuss the curative effect of the external application of self-prepared Chinese prescription decoction combined with gabapentin and cotton moxibustion on postherpetic neuralgia(PHN)and analyze its mechanism.[Methods]96 patients with PHN were divided into the control group(group A)and observation group(group B)(n=48).In group A,the patients were given gabapentin orally and treated by cotton moxibustion. In group B,the patients were treated by the external application of self-prepared Chinese prescription decoction on the basis of the above basic treatment. The total effective rate, apparent time and recurrence rate of the two groups were compared.[Results]The cure rate(72.91%)and total effective rate(95.83%)of patients in group B were higher than those in group A(54.16%,83.33%).The average apparent time[(5.79±1.40)d]and recurrence rate(5.97%)of patients in group B were significantly lower than those in group A[(7.03±4.37)d,10.45%],and the difference between the two groups was statistically significant(P<0.05).[Conclusions]The external application of self-prepared Chinese prescription decoction combined with gabapentin and cotton moxibustion therapy had a significant curative effect and low recurrence rate.

文摘This article delves into the research conducted by Liu DW et al on the integration of mindfulness-based stress reduction training(MSRT)and flavored Jinshui Liujun decoction(FJLD)with conventional chemotherapy(CC)in the treatment of non-small cell lung cancer. The study investigates the impact of this combined approach on immune function and emotional well-being of non-small cell lung cancer patients.Patients were divided into a control group(CG)receiving CC alone and a treatment group(TG)receiving MSRT+FJLD alongside CC.The primary outcomes were progression-free survival and overall survival, with secondary outcomes including Karnofsky performance status scores, clinical efficacy, immune function markers, and emotional state assessments. The findings underscore significant improvements in median progression-free survival(20.50 months in TG vs 13.10 months in CG,P<0.05),Karnofsky performance status scores(60 in TG vs 50 in CG,P<0.05),and immune cell ratios, along with reductions in negative emotions(self-rating anxiety scale and self-rating depression scale scores significantly lower in TG,P<0.05)among patients receiving MSRT+FJLD in conjunction with CC.This article highlights the potential of combining traditional Chinese medicine and modern psychological interventions to enhance the efficacy of conventional cancer treatments and improve patient outcomes.
文摘BACKGROUND Aging is an inevitable aspect of human life, characterized by the gradual decline in the function of individual cells and structural components, including bones, muscles, and ligaments.AIM To evaluate the clinical effects of radiofrequency technology in treating facial skin wrinkles and laxity.METHODS This study included 60 female patients, aged 36-58 years(mean age 47.71±1.56 years),who received focused radiofrequency technology treatment for facial wrinkles and laxity in the Department of Medical Cosmetology at our hospital between January 2021 and June 2022.Each patient underwent three treatment sessions, one every two months.Facial photographs were taken before treatment and one week after the final session.A single physician assessed wrinkle severity using a standardized wrinkle severity scale, and patients completed a satisfaction questionnaire one week after the last treatment.RESULTS After three consecutive radiofrequency treatments, performed every two months, patients exhibited significantly reduced wrinkles and skin laxity compared to baseline. One week after the third treatment, the mean facial wrinkle severity score had significantly decreased from 3.00±0.79 to 2.71±0.47(t=2.58,P<0.05).Additionally,88.24%of patients reported noticeable improvements in facial wrinkles and skin laxity. No serious adverse reactions occurred during or follow-ing treatment.CONCLUSION Radiofrequency technology demonstrates significant clinical efficacy in improving facial skin wrinkles and laxity.
文摘BACKGROUND Common bile duct stones pose a high risk of recurrence or disease progression if not promptly treated. However, there is still no optimal treatment approach.AIM To investigate the clinical efficacy of modified pancreatic duct stent drainage in endoscopic retrograde cholangiopancreatography(ERCP)for treating common bile duct stones.METHODS This retrospective study included 175 patients with common bile duct stones treated at Taizhou Fourth People’s Hospital between January 1,2021,and November 30,2023.The patients were divided into three groups-the modified pancreatic duct stent drainage group(59 cases),the nasobiliary drainage group(58 cases),and the standard biliary drainage group(58 cases).Preoperative general clinical data, laboratory indicators, and the visual analog scale(VAS)at two time points(24 hours before and after surgery)were compared, along with postoperative complications across the three groups.RESULTS Serum levels of aspartate aminotransferase, alanine aminotransferase, alkaline phosphatase,gamma-glutamyltransferase,total bilirubin, direct bilirubin,Creactive protein, and amylase were significantly lower in the modified pancreatic duct stent drainage group and the standard biliary drainage group than those in the nasobiliary drainage group(P<0.05).However,no statistically significant differences were observed in white blood cells, hemoglobin,or neutrophil levels among the three groups(P>0.05).The standard biliary drainage group had significantly lower VAS scores[(4.36±1.18)points]than those for the modified pancreatic duct stent drainage group[(4.92±1.68)points](P=0.033),and the nasobiliary drainage group[(5.54±1.24)points](P=0.017).There were no statistically significant differences in complication rates across the three groups(P>0.05).CONCLUSION Compared to standard biliary drainage and nasobiliary drainage, the modified pancreatic duct stent used during ERCP for patients with bile duct stones significantly reduced hepatocyte injury, improved liver function parameters, alleviated inflammation and pain, enhanced patient comfort, and demonstrated superior safety.
文摘OBJECTIVE:To evaluate the clinical outcome of Traditional Chinese Medicine(TCM)syndrome differentiation combined with hysteroscopic treatment for uterine incision defect after cesarean section[previous cesarean scar defect(PCSD)]after cesarean section.METHODS:This is a single-center retrospective study.A total of 120 PCSD patients were enrolled from February 2022 to February 2023 and divided into two groups according to different treatment methods, the TCM group(n=60)and the control group(n=60).The control group was treated with hysteroscopy, and the TCM group combined TCM syndrome differentiation with hysteroscopy.Clinical outcome included menstrual scores, menstrual days,TCM symptom scores and intrauterine pregnancy recurrence rate was analyzed in two groups.RESULTS:The total response rate of the TCM group was significantly higher than that of the control group(P<0.05);after treatment, the menstrual scores, menstrual days and TCM symptoms of the two groups were decreased, and the menstrual scores, menstrual days and TCM symptoms of the TCM group were all lower than that of the control group(P<0.05);the recurrence rate of the TCM group was significantly lower than that of the control group(P<0.05).Follow-up results showed higher healing of incisional scar diverticulum in the TCM group than in the control group(P<0.05).The duration of menstruation before and after treatment, and the TCM group was better than the control group(P<0.05).CONCLUSION:TCM syndrome differentiation combined with hysteroscopy presented favorable outcome on the prolonged menstrual period of PCSD,which could significantly improve the recovery of menstruation, relief the symptoms of TCM,reduce the recurrence rate and accelerate wound healing.
文摘Infrarde thermography is a kind of functional imaging technology to detect the surface temperature of human body.To a certain extent,it provides a scientific basis for the datamation, visualization and objectification of traditional Chinese medicine(TCM).It has the characteristics of noninvasive, accuracy and predictability. In this paper, infrarde thermography was applied, and summed up its advantages and disadvantages,so as to play a more important role in the field of TCM infrarde thermography.
文摘OBJECTIVE:To evaluate effects of acupoints moxibustion therapy on postsurgical gastroparesis syndrome(PGS) after resection of esophageal cardia cancer.METHODS:60 patients with PGS after resection of esophageal cardia cancer, admitted to the second affiliated hospital to Nanyang medical college from August 2012 to August 2015,were selected and studied randomly. They were divided into the treatment group and the control group by single blind controlled method,30 cases in each group. The gastric paralysis main symptom index(GCSI,by symptom ratings quantitative score table) score, drainage volume of gastric juice and value of motilin were recorded respectively at 1,7,14 and 21 days after treatment in two groups.Apart from conventional therapies, acupoints moxibustion was given on Shenque,Guanyuan,Qihai,Zhongwan acupoints, twice a day,30 minutes once,7 days as a course of treatment and for 3 courses in the treatment group. In the control group, the conventional therapies were given,7 days as a course of treatment and for 3 courses.RESULTS:The GCSI score, drainage volume of gastric juice, value of motilin in the treatment group were better than that of the control group at 7,14,21 days respectively, and there were significant differences between two groups(P<0.05).The treatment group had a more sigi curative effect than that in the control group at 7,14 days respectively(P<0.05),but there was no significant difference at 21 day between the two groups(x^2=5.68,P<0.13);nevertheless,there were more recovery cases in the treatment group than those in the control group at 21 day(x^2=4.59,P<0.032).The total effective rate was 100.00% in treatment group, and 83.00% in the control group(P<0.05).CONCLUSIONS:Acupoints moxibustion has the advantageof addressing both the symptoms and root causes of PGS after resection of esophageal cardia cancer.
