Median nerve is one of the peripheral nerves commonly affected in diabetic neuropathy. The median nerve size has been studied in non-Nigerian diabetic populations. In attempt to contribute to existing literature, a study in a Nigerian population is needed.AIM To evaluate the cross-sectional area(CSA) of the median nerve using B-mode ultrasonography(USS) and the presence of peripheral neuropathy(PN) in a cohort of adult diabetic Nigerians.METHODS Demographic and anthropometric data of 85 adult diabetes mellitus(DM) and 85 age-and sex-matched apparently healthy control(HC) subjects were taken. A complete physical examination was performed on all study subjects to determine the presence of PN and modified Michigan Neuropathy Screening Instrument(MNSI) was used to grade its severity. Venous blood was taken from the study subjects for fasting lipid profile(FLP), fasting blood glucose(FBG) and glycated haemoglobin(HbA1 c) while their MN CSA was evaluated at a point 5 cm proximal to(5 cmCATL) and at the carpal tunnel(CATL) by high-resolution Bmode USS. Data was analysed using SPSS version 22.RESULTS The mean MN CSA was significantly thicker in DM subjects compared to the HC at 5 cmCATL(P < 0.01) and at the CATL(P < 0.01) on both sides. The presence of diabetic peripheral neuropathy(DPN) further increased the MN CSA at the CATL(P < 0.05) but not at 5 cmCATL(P > 0.05). However, the severity of DPN had no additional effect on MN CSA 5 cm proximal to and at the CATL. There was no significant association between MN CSA and duration of DM and glycemic control.CONCLUSION Thickening of the MN CSA at 5 cmCATL and CATL is seen in DM. Presence of DPN is associated with worse thickening of the MN CSA at the CATL but not at5 cmCATL. Severity of DPN, duration of DM, and glycemic control had no additional effect on the MN CSA.展开更多

Results showed similar CSA values between the left and right sides, but the CSA 6 cm proximal to the tip of the wrist crease, proximal forearm, 4 cm proximal to tip of the medial epicondyle, and mid-humerus in males was greater than that of females. Moreover, CSA values at the wrist crease, pisiform bone, and hamate bone were greater in the middle-aged and old groups when compared to the young group, and correlated with body mass and height. Thus, reference values of median nerve CSA of the upper limbs can facilitate the analysis of abnormal nerve conditions.展开更多

The cross-sectional areas of the median nerve at the tunnel inlet and outlet can show swelling and compression of the nerve at the carpal. We hypothesized that the ratio of the cross-sectional areas of the median nerve at the carpal tunnel inlet to outlet accurately reflects the severity of carpal tunnel syndrome. To test this, high-resolution ultrasound with a linear array transducer at 5–17 MHz was used to assess 77 patients with carpal tunnel syndrome. The results showed that the cut-off point for the inlet-to-outlet ratio was 1.14. Significant differences in the inlet-to-outlet ratio were found among patients with mild, moderate, and severe carpal tunnel syndrome. The cut-off point in the ratio of cross-sectional areas of the median nerve was 1.29 between mild and more severe(moderate and severe) carpal tunnel syndrome patients with 64.7% sensitivity and 72.7% specificity. The cut-off point in the ratio of cross-sectional areas of the median nerve was 1.52 between the moderate and severe carpal tunnel syndrome patients with 80.0% sensitivity and 64.7% specificity. These results suggest that the inlet-to-outlet ratio reflected the severity of carpal tunnel syndrome.展开更多

Field investigation, analyses of deformation and construction of two restored balanced structural sections suggest 75-100 km (51%-64%) of N-S shortening in the Tanggula thrust system and 55-114 km (42%-47%) of N-S shortening in the Tuotuohe basin. The shortening ratios indicate that the Tanggula- Tuotuohe area has undergone intensive deformation and crustal shortening during the Early Tertiary, resulting not only in crustal thickening, but also in large scale volcanism and in rapid uplift of the Tanggula Mountains.展开更多

文摘Ultra-high-strength aluminumalloy profile is an ideal choice for aerospace structuralmaterials due to its excellent specific strength and corrosion resistance. However, issues such as uneven metal flow, stress concentration, and forming defects are prone to occur during their extrusion. This study focuses on an Al-Zn-Mg-Cu ultra-high-strength aluminum alloy profile with a double-U,multi-cavity thin-walled structure.Firstly, hot compression experiments were conducted at temperatures of 350○C,400○C,and 450○C,with strain rates of 0.01 and 1.0 s^(−1),to investigate the plastic deformation behavior of the material.Subsequently,a 3D coupled thermo-mechanical extrusion simulation model was established using Deform-3D to systematically analyze the influence of die structure and process parameters on metal flow velocity, effective stress/strain,and temperature distribution. The simulation revealed significant velocity differences, stress concentration, and uneven temperature distribution.Key parameters, including mesh density, extrusion ratio, die fillet, and bearing length, were optimized through full-factorial experiments. This optimization, combined with a stepped flow-guiding die design, effectively improved the metal flow pattern during extrusion.Trial production based on both the initial and optimized parameters were carried out.A comparative analysis demonstrates that the optimized scheme results in a final profile whose cross-section matches the target design closely, with complete filling of complex features and no obvious forming defects. This research provides a valuable reference for the extrusion process optimization and die design of complex-section profiles made from ultra-high-strength aluminum alloys.

文摘Objective This study aimed to determine the temporal trends in sleep duration among Chinese adults. Methods In this series of repeated nationally representative cross-sectional surveys(China Chronic Disease and Risk Factors Surveillance)conducted between 2010 and 2018,a total of 645,420 adult participants(97,741 in 2010;175,749 in 2013;187,777 in 2015;and 184,153 in 2018)were included in the trend analysis.Linear and logistic regression models were utilized to assess trends in sleep duration. Results In 2018,the estimated overall mean sleep duration among the Chinese adult population was7.58(SD,1.45)hours per day, with no significant trend from 2010.A significant increase in short sleep duration(≤6 hours)was observed in the total population, from 15.3%(95%CI:14.1%–16.5%)in 2010 to18.5%(95%CI:17.7%–19.3%)in 2018(P<0.001).Similarly,the trend in long sleep duration(>9 hours)was also significant, increasing in weighted prevalence from 7.2%(95%CI:6.3%–8.1%)in 2010 to 9.0%(95%CI:8.2%–9.9%)in 2018(P<0.001).Conclusion The prevalence of both short and long sleep durations significantly increased among Chinese adults from 2010 to 2018,highlighting the urgency of health initiatives to promote optimal sleep duration in China.
文摘BACKGROUND Neuropathy is a common complication of diabetes mellitus resulting from direct damage by hyperglycemia to the nerves and/or ischemia by microvascular injury to the endoneurial vessels which supply the nerves. Severity of DPN, duration of DM, and glycemic control had no additional effect on the MN CSA.

文摘High-resolution ultrasonography was used to analyze the nerve cross-sectional area (CSA) of the median nerve at 7 sites: the wrist crease, pisiform bone, hamate bone, 6 cm proximal to the tip of the wrist crease, proximal forearm (where the nerve enters the pronator teres muscle), 4 cm proximal to the tip of the medial epicondyle, and mid-humerus (mid-point between elbow crease and axilla) in 200 healthy volunteers from Guiyang, China. Thus, reference values of median nerve CSA of the upper limbs can facilitate the analysis of abnormal nerve conditions.

文摘Although ultrasound measurements have been used in previous studies on carpal tunnel syndrome to visualize injury to the median nerve, whether such ultrasound data can indicate the severity of carpal tunnel syndrome remains controversial. These results suggest that the inlet-to-outlet ratio reflected the severity of carpal tunnel syndrome.
文摘Objective The relationship between non-high-density lipoprotein(NHDL)cholesterol to high-density lipoprotein cholesterol(HDL-C)ratio(NHHR)and stoke remains unknown. This study aimed to evaluate the association between the adult NHHR and stroke occurrence in the United States of America(USA).Methods To clarify the relationship between the NHHR and stroke risk, this study used a multivariable logistic regression model and a restricted cubic spline(RCS)model to investigate the association between the NHHR and stroke, and data from the National Health and Nutrition Examination Survey(NHANES)from 2005 to 2018.Subgroup and sensitivity analyses were conducted to test the robustness of the results. Results This study included 29,928 adult participants,of which 1,165 participants had a history of stroke.Logistic regression analysis of variables demonstrated a positive association between NHHR and stroke(OR 1.24,95%CI:1.03-1.50,P=0.026).Compared with the lowest reference group of NHHR,participants in the second, third, and fourth quartile had a significantly increased risk of stroke after full adjustments(OR:1.35,95%CI:1.08-1.69)(OR:1.83,95%CI:1.42-2.36)(OR:2.04,95%CI:1.50-2.79).In the total population,a nonlinear dose-response relationship was observed between the NHHR and stroke risk(P non-linearity=0.002).This association remained significant in several subgroup analyses.Further investigation of the NHHR may enhance our understanding of stroke prevention and treatment. Conclusion Our findings suggest a positive correlation between the NHHR and an increased prevalence of stroke, potentially serving as a novel predictive factor for stroke.Timely intervention and management of the NHHR may effectively mitigate stroke occurrence.Prospective studies are required to validate this association and further explore the underlying biological mechanisms.

文摘The umbilical,a key component in offshore energy extraction, plays a vital role in ensuring the stable operation of the entire production system. The extensive variety of cross-sectional components creates highly complex layout combinations.Furthermore, due to constraints in component quantity and geometry within the cross-sectional layout, filler bodies must be incorporated to maintain cross-section performance.Conventional design approaches based on manual experience suffer from inefficiency, high variability, and difficulties in quantification. This paper presents a multi-level automatic filling optimization design method for umbilical cross-sectional layouts to address these limitations.Initially, the research establishes a multi-objective optimization model that considers compactness, balance, and wear resistance of the cross-section,employing an enhanced genetic algorithm to achieve a near-optimal layout.Subsequently, the study implements an image processing-based vacancy detection technique to accurately identify cross-sectional gaps.To manage the variability and diversity of these vacant regions, the research introduces a multi-level filling method that strategically selects and places filler bodies of varying dimensions, overcoming the constraints of uniform-size fillers.Additionally, the method incorporates a hierarchical strategy that subdivides the complex cross-section into multiple layers, enabling layer-by-layer optimization and filling. This approach reduces manufac-turing equipment requirements while ensuring practical production process feasibility. The methodology is validated through a specific umbilical case study. The results demonstrate improvements in compactness, balance, and wear resistance compared with the initial cross-section,offering novel insights and valuable references for filler design in umbilical cross-sections.

文摘OBJECTIVE:To investigate the difference in gut microbiota between population with damp-heat constitution(DHC)and balanced constitution(BC).METHODS:A multi-centered cross-sectional casecontrol study was conducted, which included 249 participants with damp-heat constitution or balanced constitution.Baseline information of participants was collected, and stool samples were collected for gut microbiota analysis.Principal coordinate analysis, linear discriminant analysis effect size analysis, receiver operating characteristic, random forest model, and phylogenetic investigation of communities by reconstruction of unobserved states methods were used to reveal the relationship between gut microbiota and the damp-heat constitution.RESULTS:Compared to those in the BC group, the richness and diversity of the microbiota, specifically those of several short-chain fatty acid producing genera such as Barnesiella,Coprobacter, and Butyricimonas, were significantly decreased in the DHC group.Regarding biological functions, flavonoid biosynthesis, propanoate metabolism, and nucleotide sugar metabolism were suppressed, while arachidonic acid metabolism and glutathione metabolism were enriched in the DHC group. Finally,a classifier based on the microbiota was constructed to discriminate between the DHC and BC populations.CONCLUSION:The gut microbiota of the DHC population exhibits significantly reduced diversity and is closely related to inflammation, metabolic disorders, and liver steatosis, which is consistent with clinical observations, thus serving as a potential diagnostic tool for traditional Chinese medicine constitution discrimination.

文摘To investigate the effects of the maximum principal stress direction(θ)and cross-section shape on the failure characteristics of sandstone,true-triaxial compression experiments were conducted using cubic samples with rectangular, circular, and D-shaped holes.Asθincreases from 0°to 60°in the rectangular hole, the left failure location shifts from the left corner to the left sidewall, the left corner, and then the floor, while the right failure location shifts from the right corner to the right sidewall, right roof corner, and then the roof.Furthermore, the initial failure vertical stress first decreases and then increases. In comparison, the failure severity in the rectangular hole decreases for variousθvalues as 30°>45°>60°>0°.With increasingθ,the fractal dimension(D)of rock slices first increases and then decreases. For the rectangular and D-shaped holes,whenθ=0°,30°,and 90°,D for the rectangular hole is less than that of the D-shaped hole.Whenθ=45°and 60°,D for the rectangular hole is greater than that of the D-shaped hole.Theoretical analysis indicates that the stress concentration at the rectangular and D-shaped corners is greater than the other areas. The failure location rotates with the rotation ofθ,and the failure occurs on the side with a high concentration of compressive stress, while the side with the tensile and compressive stresses remains relatively stable. Therefore, the fundamental reason for the rotation of failure location is the rotation of stress concentration, and the external influencing factor is the rotation ofθ.

文摘Hepatitis E virus(HEV)infection is a common public health problem in developing countries. However, the current prevalence of HEV and the relationship of HEV genotype between swine and human within high-density pig-farming areas in central China are still inadequately understood.Here,cross-sectional serological and genotypic surveys of HEV among the 1232 general population,273 workers occupationally exposed to swine, and 276 pigs in a high-density pig-breeding area, were undertaken by ELISA and nested RT-PCR methods.Anti-HEV IgG was detected in 26.22%of general population and 48.35%of occupational workers. The prevalence of swine serum HEV-Ag was 6.52%.The prevalence of anti-HEV IgG was significantly higher among the workers occupationally exposed to swine than among the general population. An increased HEV seropositivity risk among the general population was associated with either being a peasant or male and was very strongly associated with the increase of age.Among the occupationally exposed group, the prevalence of anti-HEV IgG antibodies increased with age and working years.Among the 30 HEV-IgM-positive people, the infection rates of clerks in the public, peasants, pork retailers, and pig farmers were higher than those of others.A phylogenetic analysis revealed that all the isolates belonged to subgenotype 4d,and four people and four pigs shared 97.04%-100%sequence homology. This study revealed a high HEV seroprevalence among the general population and workers occupationally exposed to swine in the Anlu City, and supports the notion that swine are a source of human HEV infection.

文摘The Tanggula (唐古拉) thrust system and the Tuotuohe (沱沱河) foreland basin, which represent major Cenozoic tectonic units of the central Tibetan plateau, have been recently studied. Field investigation, analyses of deformation and construction of two restored balanced structural sections suggest 75-100 km (51%-64%) of N-S shortening in the Tanggula thrust system and 55-114 km (42%-47%) of N-S shortening in the Tuotuohe basin. The shortening ratios indicate that the Tanggula- Tuotuohe area has undergone intensive deformation and crustal shortening during the Early Tertiary, resulting not only in crustal thickening, but also in large scale volcanism and in rapid uplift of the Tanggula Mountains.
文摘Electroslag casting with parallel fixed-movable dual electrodes is a new method for achieving better quality of castings in complex mold cavities. In this work,a mathematical model,y=kx^2+(k+1)x,was established to describe the ideal correlation between the current ratio(y)and the cross-sectional area ratio(x)of the dual electrodes, where k is the filling ratio.Investigation was conducted on the electroslag casting process with dual electrodes of various cross-sectional areas, but at a constant k value. The experimental results indicated that the ideal correlation was obtained at the stable casting stage, and the fitting results were consistent with the experimental results at certain k values. The experimental findings show that better castings can be obtained when the current ratio is greater than 1.536 and the cross-sectional area ratio is greater than 0.5.
文摘BACKGROUND Acromioclavicular joint(ACJ)space narrowing has been considered to be an important diagnostic image parameter of ACJ osteoarthritis(ACJO).However, the morphology of the ACJ space is irregular because of osteophyte formation, subchondral irregularity, capsular distention, sclerosis, and erosion. Therefore,we created the ACJ cross-sectional area(ACJCSA)as a new diagnostic image parameter to assess the irregular morphologic changes of the ACJ.AIM To hypothesize that the ACJCSA is a new diagnostic image parameter for ACJO.METHODS ACJ samples were obtained from 35 patients with ACJO and 30 healthy individuals who underwent shoulder magnetic resonance(S-MR)imaging that revealed no evidence of ACJO.Oblique coronal,T2-weighted,fat-suppressed SMR images were acquired at the ACJ level from the two groups. We measured the ACJCSA and the ACJ space width(ACJSW)at the ACJ on the S-MR images using our imaging analysis program. The ACJCSA was measured as the cross-sectional area of the ACJ.The ACJSW was measured as the narrowest point between the acromion and the clavicle.RESULTS The average ACJCSA was 39.88±10.60 mm;in the normal group and 18.80±5.13 mm;in the ACJO group. The mean ACJSW was 3.51±0.58 mm in the normal group and 2.02±0.48 mm in the ACJO group.ACJO individuals had significantly lower ACJCSA and ACJSW than the healthy individuals.Receiver operating characteristic curve analyses demonstrated that the most suitable ACJCSA cutoff score was 26.14 mm^(2),with 91.4%sensitivity and 90.0%specificity.CONCLUSION The optimal ACJSW cutoff score was 2.37 mm,with 88.6%sensitivity and 96.7%specificity.Even though both the ACJCSA and ACJSW were significantly associated with ACJO,the ACJCSA was a more sensitive diagnostic image parameter.

文摘A train body's cross-sectional shape has a significant impact on aerodynamic drag and operational safety in high-speed trains(HSTs).This study extracts five design variables from a real-world HST body:height, width, side arc radius, arc radius at the connection between the side and the roof, and arc radius at the connection between the side and the train's bottom. The cross-validated Kriging surrogate model and the genetic algorithm are used to perform two types of aerodynamic optimization, with the cross-sectional area as a constraint.Cross-sectional shapes are optimized in both windless and windy conditions.Numerical results indicate that in a windless environment, the aerodynamic drag coefficient of the whole train is reduced by 2.4%;in a windy condition, the aerodynamic drag coefficient of the entire vehicle is reduced by 2.4%,and the aerodynamic lateral force of the leading car is reduced by 37.8%.These suggest that a flat and wide shape helps to reduce not only overall aerodynamic drag in a windless environment but also aerodynamic load in a windy environment, which can be accomplished by reducing the area of the side wall and top region, lowering the train body's height, increasing its width, and lowering the radius of the side and top arcs.

文摘The coconut structure exhibits inherent impact resistance, with the macroscopically ordered distribution of variable crosssection fibers in its husk playing a crucial role in stress wave propagation and scaling.Inspired by the natural structure and fibers, this study proposes a stress wave propagation model for a variable cross-section bar considering viscous effects.A theoretical model for stress wave propagation in a fusiform-shaped bar with variable cross-section is established, elucidating the stress wave scaling effect observed in coconut fibers.Additionally,a quasi-one-dimensional method for analyzing and measuring stress wave propagation is introduced, and an experimental setup is assembled.Experimental validation of the stress wave scaling effect confirms the theory’s accuracy for stress wave scaling in variable cross-section bars. This research provides theoretical guidance and measurement methods for the design of space landers, automobile anti-collision beams, stress wave collectors, and scalers,as well as for impact testing of macro and micro materials and the design of sustainable plant-based materials for impact protection.
