Analysis on the Intervention Guidance of Critical Care Score for Children in Nursing

Objective: to analyze the effect and value of intervention and guidance of nursing scoring method for children with critical diseases in child care. Methods: the samples were taken from 100 critically ill children admitted to our hospital from January 2020 to June 2021. According to the different nursing intervention methods, the patients were divided into the control group (routine nursing) and the research group (critical illness nursing score method intervention), with a single group of 50 cases. The data differences such as the incidence of complications, severity of illness, blood gas index and nursing satisfaction were observed and compared. Results: the total incidence rate of heart failure, potential respiratory failure, compensatory shock and irreversible organ failure in the study group was 6.00% significantly lower than that in the control group by 20.00% (P < 0.05). After the intervention, APACHE Ⅱ score and SOFA score of the two groups were lower than before the intervention, and the study group was significantly lower than the control group (P < 0.05). After the intervention, the oxygen partial pressure and carbon dioxide partial pressure of the two groups were better than before the intervention, and the research group was significantly better than the control group (p < 0.05). The satisfaction rate of the research group's family members to the nursing service was 96.00% higher than that of the control group's 84.00% (p < 0.05). Compared with the control group, the study group had higher scores in basic nursing, ward management, nursing safety and nursing documents (p < 0.05). Conclusion: the pediatric critical illness nursing score method has good intervention and guidance value for pediatric nursing, which not only can reduce the incidence of complications, but also can significantly improve the severity of the illness, blood gas indicators, and enhance the satisfaction of family members to the nursing service and the quality of nursing. A total of 181 children in the int... Objective: to study the effect of the pediatric critical care case score for children in intensive care. Methods: the selected time for the study was from December 2019 to June 2021. A total of 181 children in the intensive care unit were used as the research objects. They were scored by the pediatric critical case score (PCIS), and then grouped according to the scoring results. , those with a score greater than 90 points were regarded as group A, those with a score between 70 and 89 points, and those with a score of less than 70 points in group B as group C. Then, the scores, combined functional failure, mechanical ventilation and other related indicators of the children in each group were counted. Results: there was no death in group A;The number of deaths in group B was 3, and the incidence was 4.76%;The number of deaths in group C was 6, and the incidence rate was 13.04%. The mortality in group A was the lowest (P < 0.05). According to the statistics of organ injury, in group A, there were 3 patients without organ failure, 63 patients with one organ failure, 6 patients with two organ failures, accounting for 8.33%;The number of people in group B with more than 3 functional failures was 3, and there were 3 functional failures in group B;In group C, there were more than 2 organ failures, 7 in 2, 25 in 3 and 14 in more than 3. Comparative data in group A, the situation of functional failure was the lightest and the number of people with multiple failures was the least. With the decrease of score, the situation of failure continued to worsen (P < 0.05). According to the statistics of children's mechanical ventilation, in group A, there were 5 (6.94%) in 1 ~ 2D, 3 (4.17%) in 3 ~ 4D and 1 (1.39%) in 5 ~ 6D;In group B, there were 16 (25.40%) in 1 ~ 2D, 13 (20.63%) in 3 ~ 4D, 8 (12.70%) in 5 ~ 6D and 5 (7.94%) in ≥ 7d;In group C, there were 25 (54.35%) in 1 ~ 2D, 17 (36.96%) in 3 ~ 4D, 10 (21.74%) in 5 ~ 6D and 7 (15.22%) in ≥ 7d.The patients with lower PCIS score, the higher the time and utilization rate of mechanical ventilation, P < 0.05, the difference was statistically significant. Conclusion: the pediatric critical case scoring system (PCIS) has good applicability, which can better help the judgment of critical diseases and improve the effectiveness of diagnosis and treatment. 展开更多

Risk factors and optimal predictive scoring system of mortality for children with acute paraquat poisoning 被引量:2

BACKGROUND There is no suitable scoring system that can be used to predict mortality in children with acute paraquat intoxication(APP).AIM To optimize a predictive scoring system for mortality in children with APP.METHODS A total of 113 children with APP from January 1,2010 to January 1,2020 were enrolled in this study.These patients were divided into survivors and nonsurvivors.We compared the clinical characteristics between the two groups and analyzed the independent prognostic risk factors.The survival rates of patients with different values of the pediatric critical illness score(PCIS)were assessed using kaplan-meier survival analysis.The best scoring system was established by using the area under the receiver operating characteristic curve analysis.RESULTS The overall mortality rate was 23.4%.All non-survivors died within 20 days;48.1%(13/27)died within 3 days,and 70.3%(19/27)died within 7 days.Compared to survivors,the non-survivors were older,had higher white blood cell count,alanine aminotransferase(ALT),aspartate aminotransferase,serum creatinine,blood urea nitrogen,glucose,and pediatric early warning score,and had lower platelet count,albumin,Serum sodium(Na+)and PCIS.ALT and PCIS were the independent prognostic risk factors for children with APP.The survival rate of children classified as extremely critical patients(100%)was lower than that of children classified as critical(60%)or noncritical(6.7%)patients.The specificity of ALT was high(96.51%),but the sensitivity was low(59.26%).The sensitivity and specificity of ALT combined with PCIS were high,92.59%and 87.21%,respectively.The difference in mortality was significantly higher for ALT combined with PCIS(area under the receiver operating characteristic:0.937;95%CI:0.875-0.974;P<0.05).CONCLUSION In our study,ALT and PCIS were independent prognostic risk factors for children with APP.ALT combined with PCIS is an optimal predictive mortality scoring system for children with APP. 展开更多

Internet of Vehicles(IoV)is an intelligent vehicular technology that allows vehicles to communicate with each other via internet.Communications and the Internet of Things(IoT)enable cutting-edge technologies including such self-driving cars.In the existing systems,there is a maximum communication delay while transmitting the messages.The proposed system uses hybrid Cooperative,Vehicular Communication Management Framework called CAMINO(CA).Further it uses,energy efficient fast message routing protocol with Common Vulnerability Scoring System(CVSS)methodology for improving the communication delay,throughput.It improves security while transmitting the messages through networks.In this research,we present a unique intelligent vehicular infrastructure communication management framework.This framework includes additional stability for both short and long-range mobile communications.It also includes built-in cooperative intelligent transport system(C-ITS)capabilities for experimental verification in real-world contexts.In addition,an energy efficient-fast message distribution routing protocol(EE-FMDRP)has been presented.This combines the benefits between both temporal and direction oriented routing methods.This has been suggested for distributing information from the origin ends to the predetermined objective in a quick,accurate,and effective manner in the event of an emergency.The critical value scale score(CVSS)employ ratings to measure the assault probability in Markov chains.Probabilities of chained transitions allow us to statistically evaluate the integrity of a group of IoVassets.Thus the proposed method helps to enhance the vehicular systems.The CAMINO with energy efficient fast protocol using CVSS(CA-EEFP-CVSS)method outperforms in terms of shortest transmission latency achieves 2.6 sec,highest throughput 11.6%,and lowest energy usage 17%and PDR 95.78%. 展开更多
