Modern assessment does not reflect disadvantages of older responders, for whom more time for reply is needed. Specialists with wide clinical experience may choose more than one correct answer in alternative questions. Reduced ability to remember in older people restricts examination without additional sources of information. We offer an individualised system for testing doctors. It provides personalised choice of examination questions using multiple choice questions with weight characteristics and absence of distractors, interactive cooperation in case of negative answers and the final decision of an expert in relation to the person tested. A special algorithm is proposed for typical questions that combines the advantages of known approaches to testing. The questioning system is complex for the creators of tests, but is more convenient and objective than existing ones for medical doctors.展开更多

Indeed, it remains uncertain whether the current Japanese medical system can be sustained. In this paper, we analyzed the medical expenditures for educational hospitalization of patients with type 2 diabetes. A dataset of 6173 patients from 36 hospitals was used in the analysis. The sample period was from July 2008 to March 2012. We analyzed the medical expenditures in two ways. First, we analyzed the length of hospital stay (LOS). Next, we analyzed the daily expenditure per patient. The Box-Cox transformation model was used in the first analysis and the ordinary least squares method in the second. Comorbidities and complications prolonged LOS and increased daily expenditures. The LOS was significantly different among hospitals. On the other hand, the differences in daily expenditures among hospitals were relatively small, such that LOS was the main determinant of medical expenditures. Previous studies suggested that LOS could be shortened without degradation of medical quality. Moreover, LOS could be controlled by introducing proper critical paths and improving and standardizing educational programs. Hence, it would be possible to control the medical expenditures for this disease.展开更多

A net model of formation of mathematical knowledge as a deductive theory is suggested here. Within this model the formation of deductive theory is represented as the development of a certain informational space, the elements of which are structured in the form of the orientated semantic net. This net is properly metrized and characterized by a certain system of coverings. It allows injecting net optimization parameters, regulating qualitative aspects of knowledge system under consideration. To regulate the creative processes of the formation and realization of mathematical know- edge, stochastic model of formation deductive theory is suggested here in the form of branching Markovian process, which is realized in the corresponding informational space as a semantic net. According to this stochastic model we can get correct foundation of criterion of optimization creative processes that leads to “great main points” strategy (GMP-strategy) in the process of realization of the effective control in the research work in the sphere of mathematics and its applications.展开更多

The medical cost of diabetes was estimated at 1215 billion yen for fiscal year 2011. We analyzed the length of stay (LOS) in a hospital for type 2 diabetes mellitus patients who participated in educational programs to determine factors affecting LOS. Data on 991 patients obtained from 28 Red Cross hospitals in 2008 were used. For the analysis, we used the Box-Cox transformation model and Hausman test. The results revealed that patients aged 75 years and above and those with comorbidities and complications were prone to longer LOS. The analysis also revealed significant differences in LOS across the hospitals, even after controlling for patient characteristics. Finally, we applied the least squares method to determine the effects of hospital and regional factors and revealed that patients’ LOS was affected by the region’s average LOS. 文摘In the rapidly evolving landscape of digital transformation and industrial integration, higher education faces the challenge of cultivating applied talents equipped with interdisciplinary knowledge, engineering skills, and innovative thinking.Traditional teaching models often fail to bridge the gap between theoretical knowledge and practical application, resulting in passive learning and limited problem-solving capabilities. This paper proposes a three-dimensional integrated teaching model centered on“Information Technology-Domain Knowledge-Outcome Production”(the“2+2+2”credit framework)to address these challenges.Drawing on constructivist theories,Bloom’s Taxonomy, and the CDIO model, the framework uses real projects to drive learning, facilitating the seamless integration of theoretical teaching and practical innovation. The model emphasizes tiered teaching objectives and interdisciplinary pathways, supported by dynamic assessment systems that track students’growth in knowledge, skills, and abilities.Applied in smart health and financial technology domains, this approach enhances students’comprehensive capabilities, aligning educational outcomes with industry demands. This study offers replicable strategies for educational reform in new engineering disciplines, aiming to transform students into proactive innovators and versatile talents.
文摘In a world growing ever more cruel and violent as nihilism becomes generalized,a world in which the disintegration of the human assumes dimensions that threaten its very cohesion, the demand for the humanization of the Western individual asserts itself as an urgent historical task.Among the institutions implicated in this process, education assumes a central role,as it is within it that wonder and critical thinking are gradually eliminated, together with the demand for shared deliberation, coherent meaning, and the tragic dimension of human existence, while an education of cruelty comes to permeate it from end to end.In order to prevent the full entrenchment of evil in the social world, from which only destruction can be expected,a decisive role could be played by caring thinking that would be coordinated with corresponding action and would come to deeply characterize the educational institution.Such a form of thinking was proposed in the second half of the twentieth century and took flesh and bones within the framework of the Philosophy for Children movement. Its contemporary application within educational practice could reactivate phronesis(practical wisdom)and contribute to enabling the Western human being to re-create themselves as a democratic human being-one who desires the creation of a world of freedom, equality, and justice, and who acts accordingly to bring it about:the creation of a compassionate and prudent world, the preservation of which would be the responsibility of all.
文摘BACKGROUND Orthopaedic surgical education has traditionally depended on the apprenticeship model of“see one,do one, teach one”.However, reduced operative exposure, stricter work-hour regulations, medicolegal constraints, and patient safety concerns have constrained its practicality.Simulation-based training has become a reliable, safe, and cost-efficient alternative.Dry lab techniques, especially virtual and augmented reality, make up 78%of current dry lab research, whereas wet labs still set the standard for anatomical realism.AIM To evaluate the effectiveness, limitations, and future directions of wet and dry lab simulation in orthopaedic training.METHODS A scoping review was carried out across four databases-PubMed,Cochrane Library,Web of Science, and EBSCOhost-up to 2025.Medical Subject Headings included:"Orthopaedic Education","Wet Lab","Dry Lab","Simulation Training","Virtual Reality",and"Surgical Procedure".Eligible studies focused on orthopaedic or spinal surgical education, employed wet or dry lab techniques, and assessed training effectiveness.Exclusion criteria consisted of non-English publications, abstracts only,non-orthopaedic research, and studies unrelated to simulation. Two reviewers independently screened titles, abstracts, and full texts, resolving discrepancies with a third reviewer.RESULTS From 1851 records,101 studies met inclusion:78 on dry labs,7 on wet labs,4 on both.Virtual reality(VR)simulations were most common, with AI increasingly used for feedback and assessment.Cadaveric training remains the gold standard for accuracy and tactile feedback, while dry labs-especially VR-offer scalability, lower cost(40%-60%savings in five studies),and accessibility for novices.Senior residents prefer wet labs for complex tasks;juniors favour dry labs for basics.Challenges include limited transferability data, lack of standard outcome metrics, and ethical concerns about cadaver use and AI assessment.CONCLUSION Wet and dry labs each have unique strengths in orthopaedic training.A hybrid approach combining both, supported by standardised assessments and outcome studies,is most effective.Future efforts should aim for uniform reporting, integrating new technologies, and policy support for hybrid curricula to enhance skills and patient care.
