Development and Initial Use of a New Inflammatory Bowel Disease Clinical Database Integrating Both Eastern and Western Clinical Characteristics

Background:The increasing incidence of inflammatory bowel disease(IBD)presents significant medical and societal challenges.A well-designed IBD database is crucial for both epidemiological studies and clinical management.However,inconsistencies between regional databases hinder cross-institutional and international research,especially between Eastern and Western societies.Methods:We developed a new IBD database,the 301 IBD database,integrating the IBD clinical characteristics from the Penn IBD database(USA)and the latest IBD guidelines and consensus and clinical practices of the Chinese PLA General Hospital(PLAGH).We applied this database to analyze clinical data of IBD inpatients at PLAGH from 2008 to 2023.Results:The 301 IBD database contains 490 items in 6 sections including demographic characteristics,personal history,clinical phenotype,disease activity,laboratory tests and examinations,and treatment.Features of the 301 IBD database include inpatient focus,biochemical indicators and opportunistic infection focus,and more about ulcerative colitis(UC)-associated complications.Single-center analysis revealed an increasing hospitalization trend,from 2.35%in 2008 to 3.94%in 2023.We found that the clinical characteristics of our UC inpatients are predominantly male(62.5%),extensive lesions(55.1%),low usage of biologics(4.1%),and a high incidence of UC-CRC(3.0%).The clinical characteristics of CD inpatients included male predominance(68.39%),early onset age(35.43±14.75-year-old),and high rate of surgery(25.81%).Conclusion:The 301 IBD database,integrating Eastern and Western clinical data,provides a valuable tool for IBD clinical research.Future international,multicenter collaborations are expected to further enhance its utility. 展开更多

BACKGROUND: Stroke presents as a transient or chronic brain dysfunction and is associated with high morbidity and high mortality. The doctors and scientists would like to argue how to enhance the validity of the rehabilitation treatment and how to further improve the level of treatment on stroke. OBJECTIVE: The aim of this study was to quantitatively analyze the current worldwide progress in research on stroke rehabilitation treatment based on Web of Science database and CiinicalTrial.gov in the past 10 years. METHODS: We conducted a quantitative analysis of clinical trial articles regarding stroke rehabilitation published in English from 2003 to 2013 and indexed in the National Institutes of Health Clinical Trials registry and Web of Science databases. Data were downloaded on March 15, 2013. RESULTS: (1) From 2003 to 2013, 2 654 clinical trials investigating stroke were indexed in ClinicalTrials.gov. There were only 58 clinical trials registered in 2003, and there was a marked increase from 2005. A total of 605 clinical trials on the rehabilitation of stroke were conducted in the past 10 years. (2) The analysis showed that most of the trials in the field were registered by North American institutions. With respect to the Asian countries, China and Taiwan Region of China also published a reasonable proportion of the trials, but comparatively speaking, the number of trials is really rare. Most of the interventions were drugs, followed by the devices, and behavioral interventions were ranked third. (3) In the past 10 years, there were 4 052 studies on stroke indexed by Web of Science database. CONCLUSION: From perspective of research progress, we found that the number of clinical trials and papers on stroke rehabilitation has increased significantly in the past 10 years, between them a remarkable positive correlation exists. 展开更多

A novel lightweight computerized ECG interpretation approach based on clinical 12-lead data

Although 12-lead electrocardiograms(ECGs)provide a wide range of spatiotemporal characteristics,interpreting them for arrhythmia detection is difficult due to a lack of reliable large-scale clinical datasets.Herein,we proposed an innovative lightweight computerized ECG interpretation approach based on 12-lead data.Our model was trained,validated,and tested on 53845 standard 12-lead ECG records collected at Shanghai First People’s Hospital in affiliation with Shanghai Jiao Tong University.The experiments revealed that our approach had a classification accuracy of 94.41%in the classification task of seven types of rhythms,which was markedly superior to related single-lead and 12-lead ECG classification methods.Moreover,the average receiver operating characteristic area under the curve reached a value of 0.940,and the precision values for sinus tachycardia and sinus bradycardia were 0.945 and 0.91,respectively,with specificity values of 0.996 and 0.994.By employing our boosting method,we were able to improve the accuracy to 94.85%.To investigate the performance degradation of the proposed neural network in some classes,an ECG cardiologist was enlisted to review questionable ECGs;this process provides a promising direction for network performance improvement.Therefore,the proposed computerized ECG interpretation approach has practical significance because it could help professional physicians analyze patients’heart conditions based on real-time 12-lead ECG or grade their disease severity in advance. 展开更多
