Intensivists are often plagued with the challenges of managing critically ill patients in the neurocritical intensive care unit(neuro ICU);one such challenge is the level of illness and the need for sedation,inhibitin...Intensivists are often plagued with the challenges of managing critically ill patients in the neurocritical intensive care unit(neuro ICU);one such challenge is the level of illness and the need for sedation,inhibiting the provider’s ability to adequately assess the patient.Most sedatives alter neurological and physical exam findings,only compounding potential barriers to providing the best care for each patient.It is important to emphasize that even in the altered mentation of these patients,physical and neurological exams reign supreme as diagnostic tools and should be used in conjunction with multimodal neuromonitoring methods,rather than labs or imaging alone.Additionally,selecting the appropriate analgesic(s)and sedative(s)based on these findings are highly important when determining the best course of individualized management.Thus,providers in the neuro ICU should be highly familiar with the appropriate analgesic and sedative options available in order to determine not only which may be best for each patient,but to also better understand how each drug may impact assessment findings.This comprehensive review aims to provide a structured overview of the pertinent sedatives commonly used in neuro ICUs,their risks and benefits,and how providers can best utilize each in practice to further improve patient outcomes.The novel contribution of this work provides comparative drug tables,dosing guidance for pediatric and very elderly(>85-years-old)populations,and an exploration into the future possibilities of utilizing artificial intelligence and the human gut microbiome to further enhance the prospects of precision medicine.展开更多

Objectives This study aimed to evaluate the effectiveness of the stepped self-care program on the self-care,self-efficacy,and quality of life of stroke survivors.Methods This quasi-experimental study allocated 110 str...Objectives This study aimed to evaluate the effectiveness of the stepped self-care program on the self-care,self-efficacy,and quality of life of stroke survivors.Methods This quasi-experimental study allocated 110 stroke survivors from two neurology wards into an intervention group(n=55)who received the stepped self-care program and a control group(n=55)who received usual care from June to December 2023.The Self-Care of Stroke Inventory,Stroke Self-Efficacy Questionnaire,and the short version of the Stroke Specific Quality of Life Scale were administered at baseline(T0),immediately post-intervention(T_(1)),and at 1-month(T_(2))and 3-month(T_(3))follow-ups.Data were analyzed using repeated measures analyses of variance,and generalized estimating equations.Results A total of 48 participants in the intervention group and 50 participants in the control group completed the study.No statistically significant differences were observed at T0 in any of the measured indicators(all P>0.05).The study showed significant group,time,and group×time interaction effects across the assessed outcomes(all P<0.05).Follow-up between-group comparisons at T_(1),T_(2),and T_(3) indicated that the intervention group had significantly higher scores in self-care maintenance,self-care monitoring,self-care management,self-efficacy,and quality of life than the control group(all P<0.001).Conclusions The stepped self-care program significantly improved self-care behaviors,self-efficacy,and quality of life among stroke survivors.These findings support the broader implementation of this approach in post-discharge home self-care.展开更多

Objective:To investigate the effect of dexmedetomidine(DEX)on the incidence of delirium and clinical prognosis in patients with neurosurgical ICU.Methods:This study was a retrospective cohort study that enrolled patie...Objective:To investigate the effect of dexmedetomidine(DEX)on the incidence of delirium and clinical prognosis in patients with neurosurgical ICU.Methods:This study was a retrospective cohort study that enrolled patients admitted to the Neurosurgery ICU of our hospital between January 2021 and December 2023 who required mechanical ventilation(duration>48 hours).Based on sedation protocols,patients were divided into a dexmedetomidine group(DEX group,n=60)and a midazolam routine sedation group(control group,n=60).The primary endpoint was the incidence of delirium during ICU stay,while secondary endpoints included mechanical ventilation duration,ICU length of stay,28-day mortality rate,and adverse events.Results:The incidence of delirium in the DEX group was significantly lower than that in the control group(31.7%vs.58.3%,p=0.004).Additionally,the duration of mechanical ventilation in the DEX group was significantly shorter than that in the control group[(7.2±2.5)days vs.(9.8±3.4)days,p<0.001],and the ICU stay was also significantly reduced[(13.5±4.2)days vs.(16.8±5.7)days,p=0.002].There were no statistically significant differences in the 28-day mortality rate(DEX group:13.3%vs.control group:16.7%,p=0.59)or the rate of unintended extubation(DEX group:3.3%vs.control group:5.0%,p=0.64)between the two groups.In terms of safety,the incidence of bradycardia was significantly higher in the DEX group than in the control group(15.0%vs.3.3%,p=0.03).Conclusion:In neurosurgical patients on mechanical ventilation,dexmedetomidine sedation can effectively reduce the incidence of delirium and shorten the duration of mechanical ventilation and ICU stay,but the potential risk of bradycardia should be monitored.展开更多

Background:Continuous care for children with enterostomy and their families has been gaining popularity in China.Objective:To evaluate the feasibility of continuous care for children with enterostomy and their familie...Background:Continuous care for children with enterostomy and their families has been gaining popularity in China.Objective:To evaluate the feasibility of continuous care for children with enterostomy and their families in China.Methods:The PubMed,Web of Science,Embase,Cochrane Library,EBSCO,CNKI,CBM,VIP,and WanFang were searched for clinical trials until December 30,2025.Two reviewers independently searched articles,evaluated quality and extracted data.This review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA).Results:33 studies involving 2774 participants were included.The meta-analysis showed that continuous care strategy can significantly reduce the incidence of complications in children with enterostomy(OR=0.20,95%CI=0.16-0.26,p<0.001,I2=0%),effectively improve the family caregiver ability for enterostomy(MD=-10.34,95%CI=-13.82 to-6.85,p<0.001,I2=99%),shorten the time for family members to replace stoma bags(MD=-13.57,95%CI=-19.66 to-7.49,p<0.001,I2=100%),and alleviate negative emotions such as anxiety(SMD=-1.80,95%CI=-2.36 to-1.23,p<0.001,I2=92%)and depression(SMD=-1.54,95%CI=-2.04 to-1.04,p<0.001,I2=89%)in the families of the affected children.Conclusions:Continuous care can reduce complications of enterostomy in children,improve the family caregiver ability for enterostomy and alleviate negative emotions of family members such as anxiety and depression.展开更多
Background:The effective delivery of nursing care is crucial in hospital settings because it directly affects patient outcomes.However,nursing care can be missed because of various factors,including inadequate teamwor...Background:The effective delivery of nursing care is crucial in hospital settings because it directly affects patient outcomes.However,nursing care can be missed because of various factors,including inadequate teamwork among nursing staff.Understanding the interplay between missed nursing care and nursing teamwork is essential for enhancing care quality in inpatient settings.This study therefore explored the relationship between missed nursing care and nursing teamwork among registered nurses in hospital inpatient units.Methods:A descriptive,correlational,cross-sectional study was conducted,involving 375 registered nurses from four hospitals in three healthcare sectors in Jordan.Missed nursing care and nursing teamwork were measured using the Missed Nursing Care Survey and the Nursing Teamwork Survey.Data collection occurred between September and October 2024,with con-venience sampling used for participant recruitment.Descriptive and inferential statistics,including mean,standard deviation,percentage,frequency,and Pearson's r correlation coefficient,were used to analyze the data.Results:The overall average missed nursing care score was 2.35 out of 5,suggesting that nursing care is rarely missed.The most frequently missed care activities reported by registered nurses included attending interdisciplinary care conferences,providing mouth care,and ambulating patients three times daily or as ordered.Activities least often missed included medi-cation administration within 30 min of the scheduled time,assessing vital signs as ordered,and performing patient assessments each shift.The overall mean score for nursing teamwork was 3.5 out of 5(standard deviation=1.06).There was a moderate but significant negative correlation between missed nursing care and nursing teamwork(r=−0.310,p<0.001).Conclusions:The results underscore the urgent need for targeted interventions to enhance resource allocation and teamwork,ultimately reducing missed nursing care and improving patient outcomes.Addressing these areas will foster a more effective healthcare system and enable nursing professionals to consistently deliver high-quality care.展开更多
Objectives This study aimed to explore and clarify the concept of reflective supervision as a professional self-care strategy to create a positive Intensive Care Unit(ICU)practice environment.Methods Walker and Avant...Objectives This study aimed to explore and clarify the concept of reflective supervision as a professional self-care strategy to create a positive Intensive Care Unit(ICU)practice environment.Methods Walker and Avant’s eight-step concept analysis approach was utilized to identify and define the attributes,antecedents,and consequences of reflective supervision in the ICU.An extensive literature search was conducted across various databases,including Google Scholar,CINAHL,PubMed.Articles published from 2005 to 2025 were identified.We followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA)2020 statement to indicate the included articles and extract related data based on relevance.Results Forty articles were included in the analysis.The identified attributes included the supervisor-supervisee relationship,effective communication,teamwork,collaborations,reflection,competencies,feedback,continuous support,and autonomous choice.The identified antecedents included participation,supportive supervision,flexibility,open-door policy,training,and motivation.Consequences impacting the success of reflective supervision were identified as promotion of resiliency,autonomy,work-life balance,self-awareness,increased self-esteem,professional development,critical thinking,increased job satisfaction,and enhanced commitment.Conclusions Reflective supervision is a complex professional self-care strategy that enhances ICU practice,by promoting nurses’well-being,self-awareness,therapeutic skills,and professional development.展开更多

Objective:Nurse-led virtual outpatient clinics are now a familiar component of healthcare delivery across many disciplines,including cancer care,or thopedics,rheumatology,and gastroenterology.However,establishing a nu...Objective:Nurse-led virtual outpatient clinics are now a familiar component of healthcare delivery across many disciplines,including cancer care,or thopedics,rheumatology,and gastroenterology.However,establishing a nurse-led vir tual clinic is challenging for nursing management,par ticularly regarding resources.We aimed to investigate nursing practices and processes and patient experiences in relation to vir tual outpatient clinics.Methods:This was a cross-sectional,descriptive study using mixed data collection methods.Patients(n=324)from 4 specialist clinics completed the Virtual Clinics Patient Satisfaction Questionnaire(VCSQ)survey.Five Nurse Specialists participated in a focus group interview.Results:Most participants(86.3%)reported being satisfied/very satisfied with the virtual clinics,particularly those that were nurse-led.Nurse specialists identified electronic health records(EHRs)and additional IT and administrative support as important for efficiency and effectiveness of the clinics.Conclusions:Nurse-led virtual clinics can be an effective and efficient way to provide care to patients.Nurse managers need to ensure supportive structures are in place,for example,dedicated administrators,IT support and infrastructure,education/training,and relevant policies/procedures.The success of nurse-led virtual services requires key infrastructure to support nursing staff and sustain this service.展开更多

Methods: The study was conducted between January 2024 and January 2025, involving 60 healthcare workers who were randomly assigned to groups. The control group (n = 30) received conventional occupational protective nursing care, while the observation group underwent multidimensional protective nursing interventions. Results: Following intervention, the observation group demonstrated significantly higher scores than the control group in knowledge of hydrofluoric acid occupational exposure, occupational exposure risk perception, and protective skill operation (p < 0.05). Emergency response capability scores and protective behavior implementation scores were also higher in the observation group (p < 0.05). Compared to the control group, the observation group exhibited higher occupational health satisfaction rates and lower occupational exposure incidence rates (p < 0.05). Conclusion: Multidimensional protective nursing interventions effectively enhance dental healthcare personnel’s awareness and protective capabilities regarding hydrofluoric acid occupational exposure, improve compliance with protective measures, and significantly reduce occupational exposure risks. This approach warrants promotion and application in occupational protective nursing interventions within dental departments.展开更多

文摘Objective:To investigate the effect of dexmedetomidine(DEX)on the incidence of delirium and clinical prognosis in patients with neurosurgical ICU.Methods:This study was a retrospective cohort study that enrolled patients admitted to the Neurosurgery ICU of our hospital between January 2021 and December 2023 who required mechanical ventilation(duration>48 hours).Based on sedation protocols,patients were divided into a dexmedetomidine group(DEX group,n=60)and a midazolam routine sedation group(control group,n=60).The primary endpoint was the incidence of delirium during ICU stay,while secondary endpoints included mechanical ventilation duration,ICU length of stay,28-day mortality rate,and adverse events.Results:The incidence of delirium in the DEX group was significantly lower than that in the control group(31.7%vs.58.3%,p=0.004).Additionally,the duration of mechanical ventilation in the DEX group was significantly shorter than that in the control group[(7.2±2.5)days vs.(9.8±3.4)days,p<0.001],and the ICU stay was also significantly reduced[(13.5±4.2)days vs.(16.8±5.7)days,p=0.002].There were no statistically significant differences in the 28-day mortality rate(DEX group:13.3%vs.control group:16.7%,p=0.59)or the rate of unintended extubation(DEX group:3.3%vs.control group:5.0%,p=0.64)between the two groups.In terms of safety,the incidence of bradycardia was significantly higher in the DEX group than in the control group(15.0%vs.3.3%,p=0.03).Conclusion:In neurosurgical patients on mechanical ventilation,dexmedetomidine sedation can effectively reduce the incidence of delirium and shorten the duration of mechanical ventilation and ICU stay,but the potential risk of bradycardia should be monitored.

文摘Background:Continuous care for children with enterostomy and their families has been gaining popularity in China.Objective:To evaluate the feasibility of continuous care for children with enterostomy and their families in China.Methods:The PubMed,Web of Science,Embase,Cochrane Library,EBSCO,CNKI,CBM,VIP,and WanFang were searched for clinical trials until December 30,2025.Two reviewers independently searched articles,evaluated quality and extracted data.This review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA).Results:33 studies involving 2774 participants were included.The meta-analysis showed that continuous care strategy can significantly reduce the incidence of complications in children with enterostomy(OR=0.20,95%CI=0.16-0.26,p<0.001,I2=0%),effectively improve the family caregiver ability for enterostomy(MD=-10.34,95%CI=-13.82 to-6.85,p<0.001,I2=99%),shorten the time for family members to replace stoma bags(MD=-13.57,95%CI=-19.66 to-7.49,p<0.001,I2=100%),and alleviate negative emotions such as anxiety(SMD=-1.80,95%CI=-2.36 to-1.23,p<0.001,I2=92%)and depression(SMD=-1.54,95%CI=-2.04 to-1.04,p<0.001,I2=89%)in the families of the affected children.Conclusions:Continuous care can reduce complications of enterostomy in children,improve the family caregiver ability for enterostomy and alleviate negative emotions of family members such as anxiety and depression.
文摘Background:The effective delivery of nursing care is crucial in hospital settings because it directly affects patient outcomes.However,nursing care can be missed because of various factors,including inadequate teamwork among nursing staff.Understanding the interplay between missed nursing care and nursing teamwork is essential for enhancing care quality in inpatient settings.This study therefore explored the relationship between missed nursing care and nursing teamwork among registered nurses in hospital inpatient units.Methods:A descriptive,correlational,cross-sectional study was conducted,involving 375 registered nurses from four hospitals in three healthcare sectors in Jordan.Missed nursing care and nursing teamwork were measured using the Missed Nursing Care Survey and the Nursing Teamwork Survey.Data collection occurred between September and October 2024,with con-venience sampling used for participant recruitment.Descriptive and inferential statistics,including mean,standard deviation,percentage,frequency,and Pearson's r correlation coefficient,were used to analyze the data.Results:The overall average missed nursing care score was 2.35 out of 5,suggesting that nursing care is rarely missed.The most frequently missed care activities reported by registered nurses included attending interdisciplinary care conferences,providing mouth care,and ambulating patients three times daily or as ordered.Activities least often missed included medi-cation administration within 30 min of the scheduled time,assessing vital signs as ordered,and performing patient assessments each shift.The overall mean score for nursing teamwork was 3.5 out of 5(standard deviation=1.06).There was a moderate but significant negative correlation between missed nursing care and nursing teamwork(r=−0.310,p<0.001).Conclusions:The results underscore the urgent need for targeted interventions to enhance resource allocation and teamwork,ultimately reducing missed nursing care and improving patient outcomes.Addressing these areas will foster a more effective healthcare system and enable nursing professionals to consistently deliver high-quality care.
文摘Objectives This study aimed to explore and clarify the concept of reflective supervision as a professional self-care strategy to create a positive Intensive Care Unit(ICU)practice environment.Methods Walker and Avant’s eight-step concept analysis approach was utilized to identify and define the attributes,antecedents,and consequences of reflective supervision in the ICU.An extensive literature search was conducted across various databases,including Google Scholar,CINAHL,PubMed.Articles published from 2005 to 2025 were identified.We followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA)2020 statement to indicate the included articles and extract related data based on relevance.Results Forty articles were included in the analysis.The identified attributes included the supervisor-supervisee relationship,effective communication,teamwork,collaborations,reflection,competencies,feedback,continuous support,and autonomous choice.The identified antecedents included participation,supportive supervision,flexibility,open-door policy,training,and motivation.Consequences impacting the success of reflective supervision were identified as promotion of resiliency,autonomy,work-life balance,self-awareness,increased self-esteem,professional development,critical thinking,increased job satisfaction,and enhanced commitment.Conclusions Reflective supervision is a complex professional self-care strategy that enhances ICU practice,by promoting nurses’well-being,self-awareness,therapeutic skills,and professional development.
文摘With the accelerating aging process of China’s population,the demand for community elderly care services has shown diversified and personalized characteristics.However,problems such as insufficient total care service resources,uneven distribution,and prominent supply-demand contradictions have seriously affected service quality.Big data technology,with core advantages including data collection,analysis and mining,and accurate prediction,provides a new solution for the allocation of community elderly care service resources.This paper systematically studies the application value of big data technology in the allocation of community elderly care service resources from three aspects:resource allocation efficiency,service accuracy,and management intelligence.Combined with practical needs,it proposes optimal allocation strategies such as building a big data analysis platform and accurately grasping the elderly’s care needs,striving to provide operable path references for the construction of community elderly care service systems,promoting the early realization of the elderly care service goal of“adequate support and proper care for the elderly”,and boosting the high-quality development of China’s elderly care service industry.

文摘Objective:Nurse-led virtual outpatient clinics are now a familiar component of healthcare delivery across many disciplines,including cancer care,or thopedics,rheumatology,and gastroenterology.However,establishing a nurse-led vir tual clinic is challenging for nursing management,par ticularly regarding resources.We aimed to investigate nursing practices and processes and patient experiences in relation to vir tual outpatient clinics.Methods:This was a cross-sectional,descriptive study using mixed data collection methods.Patients(n=324)from 4 specialist clinics completed the Virtual Clinics Patient Satisfaction Questionnaire(VCSQ)survey.Five Nurse Specialists participated in a focus group interview.Results:Most participants(86.3%)reported being satisfied/very satisfied with the virtual clinics,particularly those that were nurse-led.Nurse specialists identified electronic health records(EHRs)and additional IT and administrative support as important for efficiency and effectiveness of the clinics.Conclusions:Nurse-led virtual clinics can be an effective and efficient way to provide care to patients.Nurse managers need to ensure supportive structures are in place,for example,dedicated administrators,IT support and infrastructure,education/training,and relevant policies/procedures.The success of nurse-led virtual services requires key infrastructure to support nursing staff and sustain this service.

文摘Objective: To investigate the efficacy of multidimensional protective nursing interventions in reducing occupational exposure risks to hydrofluoric acid in dental departments. Methods: The study was conducted between January 2024 and January 2025, involving 60 healthcare workers who were randomly assigned to groups. The control group (n = 30) received conventional occupational protective nursing care, while the observation group underwent multidimensional protective nursing interventions. Results: Following intervention, the observation group demonstrated significantly higher scores than the control group in knowledge of hydrofluoric acid occupational exposure, occupational exposure risk perception, and protective skill operation (p < 0.05). Emergency response capability scores and protective behavior implementation scores were also higher in the observation group (p < 0.05). Compared to the control group, the observation group exhibited higher occupational health satisfaction rates and lower occupational exposure incidence rates (p < 0.05). Conclusion: Multidimensional protective nursing interventions effectively enhance dental healthcare personnel’s awareness and protective capabilities regarding hydrofluoric acid occupational exposure, improve compliance with protective measures, and significantly reduce occupational exposure risks. This approach warrants promotion and application in occupational protective nursing interventions within dental departments.

文摘For India to achieve elimination by 2030,the challenges posed by Plasmodium(P.)vivax cannot be overlooked owing to its burden and unique biology.In 2023,in India,about 224000 malaria cases were reported,and a significant proportion(40%)were P.vivax cases.In P.vivax infection,the persistence of dormant liver stage of parasite,i.e.,hypnozoites,leading to relapses weeks or months later poses challenge in its elimination.
