Sickle cell crises are their most common acute complication, characterized by episodes of intense pain and systemic manifestations that impair quality of life and impose a high healthcare burden. We present the case of a 19-year-old male diagnosed with SCA since the age of two, who developed a sickle cell crisis precipitated by right basal pneumonia. The patient exhibited sudden-onset, cyclic lumbar pain with progressive dyspnea. Initial management included multimodal pain control, volume optimization, and targeted antimicrobial therapy to achieve clinical stabilization. This case underscores the importance of a comprehensive approach to managing sickle cell crises, addressing both symptomatic relief and the prevention and treatment of complications. It also highlights the need for public health strategies promoting early diagnosis, access to disease-modifying therapies such as hydroxyurea, and interdisciplinary follow-up to mitigate the socioeconomic and clinical impact of SCA.展开更多
The recent Russian-Ukrainian crisis has had a substantial impact on the global natural gas supply landscape and brought major challenges to liquified natural gas(LNG)supply chain safety in many countries.However,acade...The recent Russian-Ukrainian crisis has had a substantial impact on the global natural gas supply landscape and brought major challenges to liquified natural gas(LNG)supply chain safety in many countries.However,academic research in this field remains scarce.This study aims to analyze the global LNG marine supply network changes during the recent Russian-Ukrainian crisis.Big data analytics are applied to the global vessel trajectory information recorded by automatic identification system(AIS).The weighted degree index indicates that global LNG marine supply is still primarily led by a few exporting countries,and the Middle East Port of Ras Laffan now occupies the central position.In 2022,the betweenness centrality and other indicators of the LNG shipping network showed a high middle and low end,indicating that the network has a certain degree of resilience.After the Russian-Ukrainian crisis happened,LNG exports from Qatar,the United States,and Russia increased.Resource flows have reshaped the competitive standing of countries.The European LNG market witnessed a significant intensification in the export competition between Russia and the United States.Despite the ongoing crisis,Russia's LNG export competitiveness remained resilient and displayed no indications of weakening.The results show new evidence for the changes in global natural resources marine supply influenced by the geopolitical events among countries.This has important implications for policymaking of LNG supply safety to the countries that rely heavily on imports.展开更多
Objective:To analyze the preventive and management effects of the 4R crisis management theory on incontinence-associated dermatitis(IAD)in ICU patients with fecal incontinence.Methods:A hundred patients with fecal inc...Objective:To analyze the preventive and management effects of the 4R crisis management theory on incontinence-associated dermatitis(IAD)in ICU patients with fecal incontinence.Methods:A hundred patients with fecal incontinence who were admitted to the ICU for treatment between January 2024 and December 2024 were selected and randomly divided into two groups using a random number table.The observation group received 4R crisis management theory,while the control group received routine management.The IAD risk score,incidence of IAD,severity of IAD,and formation and healing time of IAD were compared between the two groups.Results:After management,the IAD risk score of the observation group was lower than that of the control group(P<0.05).The incidence of IAD in the observation group was lower than that in the control group,the proportion of mild IAD was higher than that in the control group,the formation time of IAD was later than that in the control group,and the healing time was shorter than that in the control group(P<0.05).Conclusion:Adopting the 4R crisis management theory for ICU patients with fecal incontinence can reduce the risk of IAD occurrence,effectively prevent IAD,reduce its severity,delay its formation time,and promote good healing.展开更多

BACKGROUND Although chronic-phase chronic myeloid leukemia(CP-CML)is treatable and nearly curable in about 50%of patients,accelerated-phase chronic myeloid leukemia(AP-CML)shows concerning drug resistance,while blast ...BACKGROUND Although chronic-phase chronic myeloid leukemia(CP-CML)is treatable and nearly curable in about 50%of patients,accelerated-phase chronic myeloid leukemia(AP-CML)shows concerning drug resistance,while blast crisis chronic myeloid leukemia(BC-CML)is highly lethal.Advances in whole exome sequencing(WES)reveal pan-cancer mutations in BC-CML,supporting mutation-guided therapies beyond Breakpoint cluster region-Abelson.Artificial intelligence(AI)and machine learning(ML)enable genomic stratification and drug repurposing,addressing overlooked actionable mutations.AIM To stratify BC-CML into molecular subtypes using WES,ML,and AI for precision drug repurposing.METHODS Included 123 CML patients(111 CP-CML,5 AP-CML,7 BC-CML).WES identified pan-cancer mutations.Variants annotated via Ensembl Variant Effect Predictor and Catalogue of Somatic Mutations in Cancer(COSMIC).ML(principal component analysis,K-means)stratified BC-CML.COSMIC signatures and PanDrugs prioritized drugs.Analysis of variance/Kruskal-Wallis validated differences(P<0.05).RESULTS In this exploratory,hypothesis-generating study of BC-CML patients(n=7),we detected over 2500 somatic mutations.ML identified three BC-CML clusters:(1)Cluster 1[breast cancer susceptibility gene 2(BRCA2),TP53];(2)Cluster 2[isocitrate dehydrogenase(IDH)1/2,ten-eleven translocation 2];and(3)Cluster 3[Janus kinase(JAK)2,colony-stimulating factor 3 receptor],with distinct COSMIC signatures.Therapies:(1)Polyadenosinediphosphate-ribose polymerase inhibitors(olaparib);(2)IDH inhibitors(ivosidenib);and(3)JAK inhibitors(ruxolitinib).Mutational burden,signatures,and targets varied significantly across clusters,supporting precision stratification.CONCLUSION This WES-AI-ML framework provides mutation-guided therapies for BC-CML,enabling real-time stratification and Food and Drug Administration-approved drug repurposing.While this exploratory study is limited by its small sample size(n=7),it establishes a methodological framework for precision oncology stratification that warrants validation in larger,multi-center cohorts.展开更多

This paper explains how t...The mortgage loan has evolved from a local lending instrument into a major global security and its role is unparallel to other financial instruments in the process of financial globalization. This paper explains how technology and financial innovation transformed the mortgage loan from a local security into a premier global security traded worldwide. It examines the fundamental flaws of this process and why it does not work in regards to mortgage lending and the re-securitization products that were created through financial innovation. The findings show that regulation was unable to keep pace with financial innovation, which created an environment where actors in the financial service sector were able to behave geographically irresponsibly by using information asymmetries to their advantage by par- ticipating in moral hazard activities and engaging in other immoral and unethical business practices that were centered around localized geography, which ultimately contributed to the global financial crisis. It also examines the roll of financial innovation in regard to the Lehman Brothers Mini-Bond in Hung and its role as a driving force behind China's newly emerging shadow banking sector. The commencement of China–Pakistan Economic Corridor has led to the appreciation of Pakistan’s economic outlook from 5.4%to 5.8%by the World Bank.The upgraded outlook is a welcome sign but it is still trivial,essent...The commencement of China–Pakistan Economic Corridor has led to the appreciation of Pakistan’s economic outlook from 5.4%to 5.8%by the World Bank.The upgraded outlook is a welcome sign but it is still trivial,essentially attributable to the electric power crisis,which approximately trims 2%of Pakistan’s economic growth annually.Almost 60%of the CPEC(China–Pakistan Economic Corridor)funds are directed at Pakistan’s energy sector,hence,demanding careful attention of both researchers and policy analysts alike.The study is based upon a meta-analytic review of literature concerning CPEC and Pakistan’s energy sector.The results of the study demonstrate that CPEC is an easing agent for Pakistan’s energy crisis(82.30%).The results also highlight points of concern,including inadequate planning(47%),dilapidated electricity distribution system causing losses(64.7%),and an unsustainable energy mix(64.7%).The study further validates the findings via Spearman’s Rho-Correlation.The rρvalue for the possible“resolution of Pakistan’s energy crisis”is 0.5426 achieving a significance level of 98%and a corresponding p-value of 0.0252.The significant negative rρvalue attained is−0.4894 which establishes the fact that lack of planning can hinder the energy crisis resolution.展开更多

文摘The recent Russian-Ukrainian crisis has had a substantial impact on the global natural gas supply landscape and brought major challenges to liquified natural gas(LNG)supply chain safety in many countries.However,academic research in this field remains scarce.This study aims to analyze the global LNG marine supply network changes during the recent Russian-Ukrainian crisis.Big data analytics are applied to the global vessel trajectory information recorded by automatic identification system(AIS).The weighted degree index indicates that global LNG marine supply is still primarily led by a few exporting countries,and the Middle East Port of Ras Laffan now occupies the central position.In 2022,the betweenness centrality and other indicators of the LNG shipping network showed a high middle and low end,indicating that the network has a certain degree of resilience.After the Russian-Ukrainian crisis happened,LNG exports from Qatar,the United States,and Russia increased.Resource flows have reshaped the competitive standing of countries.The European LNG market witnessed a significant intensification in the export competition between Russia and the United States.Despite the ongoing crisis,Russia's LNG export competitiveness remained resilient and displayed no indications of weakening.The results show new evidence for the changes in global natural resources marine supply influenced by the geopolitical events among countries.This has important implications for policymaking of LNG supply safety to the countries that rely heavily on imports.

文摘Objective:To analyze the preventive and management effects of the 4R crisis management theory on incontinence-associated dermatitis(IAD)in ICU patients with fecal incontinence.Methods:A hundred patients with fecal incontinence who were admitted to the ICU for treatment between January 2024 and December 2024 were selected and randomly divided into two groups using a random number table.The observation group received 4R crisis management theory,while the control group received routine management.The IAD risk score,incidence of IAD,severity of IAD,and formation and healing time of IAD were compared between the two groups.Results:After management,the IAD risk score of the observation group was lower than that of the control group(P<0.05).The incidence of IAD in the observation group was lower than that in the control group,the proportion of mild IAD was higher than that in the control group,the formation time of IAD was later than that in the control group,and the healing time was shorter than that in the control group(P<0.05).Conclusion:Adopting the 4R crisis management theory for ICU patients with fecal incontinence can reduce the risk of IAD occurrence,effectively prevent IAD,reduce its severity,delay its formation time,and promote good healing.

文摘BACKGROUND Although chronic-phase chronic myeloid leukemia(CP-CML)is treatable and nearly curable in about 50%of patients,accelerated-phase chronic myeloid leukemia(AP-CML)shows concerning drug resistance,while blast crisis chronic myeloid leukemia(BC-CML)is highly lethal.Advances in whole exome sequencing(WES)reveal pan-cancer mutations in BC-CML,supporting mutation-guided therapies beyond Breakpoint cluster region-Abelson.Artificial intelligence(AI)and machine learning(ML)enable genomic stratification and drug repurposing,addressing overlooked actionable mutations.AIM To stratify BC-CML into molecular subtypes using WES,ML,and AI for precision drug repurposing.METHODS Included 123 CML patients(111 CP-CML,5 AP-CML,7 BC-CML).WES identified pan-cancer mutations.Variants annotated via Ensembl Variant Effect Predictor and Catalogue of Somatic Mutations in Cancer(COSMIC).ML(principal component analysis,K-means)stratified BC-CML.COSMIC signatures and PanDrugs prioritized drugs.Analysis of variance/Kruskal-Wallis validated differences(P<0.05).RESULTS In this exploratory,hypothesis-generating study of BC-CML patients(n=7),we detected over 2500 somatic mutations.ML identified three BC-CML clusters:(1)Cluster 1[breast cancer susceptibility gene 2(BRCA2),TP53];(2)Cluster 2[isocitrate dehydrogenase(IDH)1/2,ten-eleven translocation 2];and(3)Cluster 3[Janus kinase(JAK)2,colony-stimulating factor 3 receptor],with distinct COSMIC signatures.Therapies:(1)Polyadenosinediphosphate-ribose polymerase inhibitors(olaparib);(2)IDH inhibitors(ivosidenib);and(3)JAK inhibitors(ruxolitinib).Mutational burden,signatures,and targets varied significantly across clusters,supporting precision stratification.CONCLUSION This WES-AI-ML framework provides mutation-guided therapies for BC-CML,enabling real-time stratification and Food and Drug Administration-approved drug repurposing.While this exploratory study is limited by its small sample size(n=7),it establishes a methodological framework for precision oncology stratification that warrants validation in larger,multi-center cohorts.
文摘Countries have to pre-evaluate threats and risks that may incur their national security and national interests, take the necessary preventive measures timely, and pursue decisive policies within the limits of the possibilities of their national power, by keeping a close watch on ongoing and conceivable crises and new developments. Like statesmen, also the international security organizations have to foresee the coming or anticipated threats and opportunities, take the necessary precautions and when necessary even to react by using force. Starting from time of peace, all these require an effective crisis management system that would warn and manage the threats that would create such crises. There are, however, crisis management centers and situation rooms in the focal points of the physical substructure of the crisis management system. In this article, by looking at the attributions of an effective crisis management system, structure of the crisis management centers and situation rooms will be questioned at international and national levels.

文摘The mortgage loan has evolved from a local lending instrument into a major global security and its role is unparallel to other financial instruments in the process of financial globalization. This paper explains how technology and financial innovation transformed the mortgage loan from a local security into a premier global security traded worldwide. It examines the fundamental flaws of this process and why it does not work in regards to mortgage lending and the re-securitization products that were created through financial innovation. The findings show that regulation was unable to keep pace with financial innovation, which created an environment where actors in the financial service sector were able to behave geographically irresponsibly by using information asymmetries to their advantage by par- ticipating in moral hazard activities and engaging in other immoral and unethical business practices that were centered around localized geography, which ultimately contributed to the global financial crisis. It also examines the roll of financial innovation in regard to the Lehman Brothers Mini-Bond in Hung and its role as a driving force behind China's newly emerging shadow banking sector. It concludes with a policy recommendation and its implication for China's continued economic development.

文摘The commencement of China–Pakistan Economic Corridor has led to the appreciation of Pakistan’s economic outlook from 5.4%to 5.8%by the World Bank.The upgraded outlook is a welcome sign but it is still trivial,essentially attributable to the electric power crisis,which approximately trims 2%of Pakistan’s economic growth annually.Almost 60%of the CPEC(China–Pakistan Economic Corridor)funds are directed at Pakistan’s energy sector,hence,demanding careful attention of both researchers and policy analysts alike.The study is based upon a meta-analytic review of literature concerning CPEC and Pakistan’s energy sector.The results of the study demonstrate that CPEC is an easing agent for Pakistan’s energy crisis(82.30%).The results also highlight points of concern,including inadequate planning(47%),dilapidated electricity distribution system causing losses(64.7%),and an unsustainable energy mix(64.7%).The study further validates the findings via Spearman’s Rho-Correlation.The rρvalue for the possible“resolution of Pakistan’s energy crisis”is 0.5426 achieving a significance level of 98%and a corresponding p-value of 0.0252.The significant negative rρvalue attained is−0.4894 which establishes the fact that lack of planning can hinder the energy crisis resolution.
