Background:Parenting exerts a profound influence on children’s mental health and behavioral development.Despite the high prevalence of children’s emotional and behavioral problems(CEBP)in China,evidence-based parent...Background:Parenting exerts a profound influence on children’s mental health and behavioral development.Despite the high prevalence of children’s emotional and behavioral problems(CEBP)in China,evidence-based parenting interventions remain scarcely investigated as preventive public health strategies.This pilot study evaluated a school-based intervention for preventing CEBP.Methods:We employed a quasi-experimental design with propensity score matching(PSM)to select 28 families(intervention:n=13;control:n=15)from two matched urban primary schools.Quantitative data from seven validated scales were analyzed using t-tests and ANCOVA.Qualitative insights were derived from 10 semi-structured interviews via thematic analysis.Results:Compared to the control group,the intervention group demonstrated significantly greater improvements in CEBP(p=0.020,Cohen’s d=0.92),parental adjustment(p=0.031,Cohen’s d=0.80),parenting confidence(p=0.003,Cohen’s d=1.04),and parentchild relationships(p=0.001,Cohen’s d=1.46).Non-significant effects were observed for parenting style,parental relationship,and parenting conflict(p>0.05).Qualitative analysis corroborated these findings and further identified contributing factors for non-significant outcomes,including challengeswithmeasurement adaptability and inconsistent co-parenting practices.Conclusions:This pilot study suggests that an authoritative parenting style may be effective and culturally adaptable in China.Positive parenting interventions appear to mitigate CEBP by reducing risk factors and enhancing protective factors.However,improving parental relationships and parenting conflict may require targeted strategies.Given the pilot nature of this PSM-matched study(n=28),the findings should be interpreted as exploratory and used primarily for intervention refinement.展开更多

Unhealthy eating habits and lack of proper nutrition knowledge cause non-communicable diseases and affect adolescents. It is in adolescence that eating habits are formed that persist till adulthood. Lifestyle interventions are needed to curb NCDs in adolescents. This paper reports the findings of a study that aimed to validate a lifestyle intervention program and its effect on food intake, physical activity, and nutrition knowledge. It was a clustered randomized control trial study conducted in four (4) junior secondary schools. There were 46 participants, 21 in the control and 25 in the intervention arm, who were blindly assigned to each arm by a statistician. Information and skills on nutrition were imparted using the Information, Motivation, and Behavioral Skills model. The program was implemented for eight (8) weeks hourly after school. A questionnaire was used to collect data pre- and post-intervention. Number, proportion, percentage, and independent t-test (mean and SD or median and IQR, p-value) were calculated using numerical and categorical data. The findings showed that the lifestyle intervention was valid, and there was a slight decrease in the intake of sweets among participants in both trial arms (p = 0.066). There was no significant difference in terms of food intake. Only a small number of participants still ate a few fruits, and there was no change in vegetable intake in both trial arms (p = 0.641). There was no change in the intake of fried foods in both trail arms (p = 0.402). Regarding nutrition knowledge, there was a slight significant difference of p = 0.079 between the trial arms. Though the effect of the lifestyle intervention program was not statistically significant, the results are promising, especially if the duration could be increased to a longer period and a larger sample size included.展开更多
Background:Early Hearing Detection and Intervention(EHDI)plays a critical role in improving language,cognitive,and socio-emotional outcomes for infants with hearing loss.In Nigeria,however,EHDI implementation remains ...Background:Early Hearing Detection and Intervention(EHDI)plays a critical role in improving language,cognitive,and socio-emotional outcomes for infants with hearing loss.In Nigeria,however,EHDI implementation remains limited by fragmented service delivery,uneven technological capacity,and sociocultural factors that delay timely diagnosis.This study explored the perspectives of paediatric audiologists and parents to provide a comprehensive understanding of the opportunities and challenges influencing early hearing care across diverse Nigerian settings.Methods:A mixed-methods design was employed across audiology facilities selected systematically from four Nigerian geopolitical zones.Twenty-five paediatric audiologists and twenty-three parents of children with congenital hearing loss participated.Quantitative data were collected using a structured questionnaire assessing awareness,diagnostic access,and intervention experiences.Qualitative data were obtained through semi-structured interviews and two focus group discussions.Thematic analysis followed Braun and Clarke's six-step framework,with dual coding,external auditing,and member validation to enhance credibility.Results:Quantitative findings demonstrated broad agreement on the diagnostic value of otoacoustic emissions(OAEs)and automated auditory brainstem responses(AABRs),the developmental benefits of early intervention,and the importance of active parental involvement.However,respondents identified persistent barriers including high costs of screening and therapy,poor public awareness of early hearing loss symptoms,and a critical shortage of trained personnel,and unequal distribution of diagnostic tools,particularly in rural and northern regions.Thematic analysis further underscored disparities in diagnostic capacity,sociocultural interpretations of deafness that delay clinical consultation,and economic constraints that hinder continuity of care.While families who accessed early intervention reported improved communication,social engagement,and learning readiness in their children,systemic gaps continue to limit widespread success.Conclusions:Despite growing technological capacity and awareness of EHDI benefits,significant structural,financial,and sociocultural challenges continue to impede timely diagnosis and intervention in Nigeria.Strengthening national policies,ensuring equitable distribution of diagnostic tools,expanding professional training,subsidising services,implementing culturally sensitive awareness campaigns and integration of Universal Newborn Hearing Screening into routine postnatal care are essential to improving outcomes for deaf infants.展开更多
BACKGROUND Approximately 30%of patients with head and neck cancer experience adverse effects caused by anxiety and depression.Considering the high prevalence,implementing customized interventions to ease adverse emoti...BACKGROUND Approximately 30%of patients with head and neck cancer experience adverse effects caused by anxiety and depression.Considering the high prevalence,implementing customized interventions to ease adverse emotional states is imperative.AIM To evaluate the efficacy of cognitive behavioral therapy(CBT)-based psychological interventions in improving the psychological well-being and quality of life(QoL)of patients with laryngeal carcinoma.METHODS This study enrolled 120 patients admitted from February 2022 to February 2024.The control group,comprising 50 participants,received standard supportive psychological care,while the research group,consisting 70 participants,underwent CBT-based interventions.Several clinical outcomes were systematically assessed that included postoperative recovery metrics(duration of tracheostomy and nasogastric tube dependence and length of hospitalization),psychological status(Self-Rating Anxiety Scale and Self-Rating Depression Scale),nutritional markers(serum albumin and hemoglobin levels),sleep quality(Self-Rating Scale of Sleep and Athens Insomnia Scale),and QoL(Functional Assessment of Cancer Therapy-Head and Neck).RESULTS The results demonstrated that the research group experienced superior outcomes,with significantly reduced durations of tracheostomy and nasogastric tube dependence,as well as shorter hospital stays,compared with the control group.Additionally,the research group exhibited markedly lower post-intervention Self-Rating Anxiety Scale,Self-Rating Depression Scale,Self-Rating Scale of Sleep,and Athens Insomnia Scale scores,along with minimal but higher change in serum albumin and hemoglobin levels compared with the control group.All five domains of Functional Assessment of Cancer Therapy-Head and Neck showed notable improvements in the research group,exceeding those observed in the control group.CONCLUSION CBT-based psychological support positively affects the mental well-being and QoL of patients with laryngeal carcinoma,highlighting its potential for broader clinical application.展开更多

Purpose:The aim of this mixed-studies systematic review was to ascertain the effectiveness of school-based interventions in increasing physical activity(PA) and/or reducing sedentary time(ST) in children aged 5-11 yea...Purpose:The aim of this mixed-studies systematic review was to ascertain the effectiveness of school-based interventions in increasing physical activity(PA) and/or reducing sedentary time(ST) in children aged 5-11 years,as well as to explore their effectiveness in relation to categories of the theory of expanded,extended,and enhanced opportunity(TEO).Methods:Adhering to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA) guidelines,5 databases were searched using predefined search terms.Following title and abstract screening of 1115 records,the removal of duplicates(n=584) and articles that did not meet the inclusion criteria agreed to a priori(n=419) resulted in 112 records that were full-text screened.Two independent reviewers subsequently used the mixed-methods appraisal tool to assess the methodological quality of 57 full-text studies that met the inclusion criteria after full-text screening.The interventions were summarised using the TIDierR checklist and TEO.The strength of evidence was determined using a 5-level rating system utilising a published decision tree.Results:Overall evidence ratings for interventions implemented within school settings were:no evidence of effects on moderate-to-vigorous physical activity(MVPA) and inconclusive evidence of effects on sedentary time.In relation to the TEO,expansion of PA appeared to be the most promising intervention type for MVPA,with moderate evidence of effect,whereas extension and enhancement of PA opportunity demonstrated no evidence of effect.A critical issue of possible compensatory behavior was identified by analysis of intervention effect in relation to PA measurement duration;when studies measured changes in PA during the actual intervention,there was moderate evidence of effect,whereas those that measured changes in PA during the school day presented inconclusive evidence of effect,and those that measured changes in PA over a whole day yielded no evidence of effect.Two meta-analyses of those studies using a whole-day accelerometer measure for MVPA or ST showed a significant but moderate effect for MVPA(effect size=0.51;95% confidence interval(CI):0.02-0.99) and a large but nonsignificant effect for ST(effect size=1.15;95%CI:-1.03 to 3.33);both meta-analyses demonstrated low precision,considerable inconsistency,and high heterogeneity.Conclusion:The findings have important implications for future intervention research in terms of intervention design,implementation,and evaluation.展开更多

This study investigated whether a one-year intervention, which aimed to increase physical activity (PA), could influence the motor performance of these children positively. Methods: A sub-sample of 525 migrant children (7.1 ± 0.7 years;48.6% male), participated in the study and included an intervention (IG) and control group (CG). The Dordel-Koch-Test was used to assess motor performance. An exploratory factor analysis was performed. Differences between IG and CG were examined using ANCOVA. Results: Boys in the IG showed a significant improvement in the conditional skills (F(1, 201) = 8.02, p ≤ 0.005) and girls showed a tendency towards better results. Additionally, girls showed a significant improvement in flexibility (F(1, 226) = 10.72, p ≤ 0.01) compared with the CG. Conclusion: The intervention affects parts of motor skills of migrant children significantly. Future interventions should aim at a target-group-specific promotion to get a holistic improvement in their motor abilities.展开更多

Participants (n = 1542;13.2 土 0.4 years, mean 土 SD) provided questionnaire data at baseline (shyness, activity level) and post-intervention(intervention acceptability, satisfaction with components). Between-group differences (boys vs. girls and shy/inactive vs. others) weretested with linear regression models, accounting for school clustering. Data from 16 individual interviews (shy/inactive) and 11 focus groupswith 48 participants (mean = 4;range 2—7) were thematically coded. Qualitative and quantitative data were merged in an integrative mixedmethods convergence matrix, which denoted convergence and dissonance across datasets.Results'. Effect sizes for quantitative results were small and may not represent substantial between-group differences. Boys (vs. girls) preferredclass-based sessions (0 = 0.2, 95% confidence interval (CI): 0.1—0.3);qualitative data suggested that this was because boys preferred competition,which was supported quantitatively (0 = 0.2, 95%CI: 0.1-0.3). Shy/inactive students did not enjoy the competition (0 = -0.3, 95%CI:—0.5 to —0.1). Boys enjoyed trying new activities more (0 = 0.1, 95%CI: 0.1 -0.2);qualitative data indicated a desire to try new activities acrossall subgroups but identified barriers to choosing unfamiliar activities with self-imposed choice restriction leading to boredom. Qualitative datahighlighted critique of mentorship;adolescents liked the idea, but older mentors did not meet expectations.Conclusion. We interpreted adolescent perspectives of intervention components and implementation to provide insights into future complexinterventions aimed at increasing young people's physical activity in school-based settings. The intervention component mentorship was liked inprinciple, but implementation issues undesirably impacted satisfaction;competition was disliked by girls and shy/inactive students. Design: Quasi-experimen...Objective: Use a school-based food garden as an instrument to improve healthy dietary consumption, habits, and practices among children from the third to fifth grade in public schools in Chile. Design: Quasi-experimental, six-month intervention with an evaluation of dietary consumption, habits, and practices and use of the school-based food garden among the 3rd to 5th grade students. Setting: Two municipal schools in a low-middle income district in Santiago de Chile;one with a school garden intervention and a control school without a garden. Subjects: 155 third to fifth grade children, 63 in the intervention school and 92 in the control school (average age 10 years old, 58% boys). Results: In the intervened school, there was a significant improvement in the children’s dietary habits, such as peeling and slicing fruit (p < 0.05), making sandwiches (p < 0.05), and increased motivation to cook (p < 0.05). Fruit purchases at the school rose from 4% to 37% (p < 0.05) by the end of the intervention. Interest in growing vegetables or fruits at home increased from 48% to 70% (p < 0.05). The school children at the intervention school significantly increased their knowledge about 16 of 21 garden crops (p < 0.05), expanding the spectrum of the vegetables they can consume. There were no significant changes in the control school. Conclusions: The school-based food garden was effective in achieving change in dietary habits and practices among the third to fifth grade children. The project should be carried out for a longer period of time, so that the changes will be reflected in the students’ food consumption and nutritional status.展开更多
BACKGROUND Diabetes is one of the most common chronic diseases of childhood.School plays an essential role in the management of student diabetes,which reduces the risk of short-and long-term diabetes complications and...BACKGROUND Diabetes is one of the most common chronic diseases of childhood.School plays an essential role in the management of student diabetes,which reduces the risk of short-and long-term diabetes complications and ensures that students are wellpositioned for optimal academic performance and growth.AIM The aim of this review was to systematically identify and synthesize the literature concerning theory-based diabetes care interventions in K-12 schools in the United States.It critically assessed the specific role of theories and associated essential constructs in intervention design,implementation,outcome measurement,and evaluation.METHODS Relevant literature was identified by keyword searches of the Cochrane Library,PubMed,and Web of Science.RESULTS Four interventions met the eligibility criteria and were included in the review.Of those,two evaluated online diabetes education programs for school personnel and the other two assessed in-person interventions.Three studies adopted a onegroup pre-post study design,and the remaining one adopted a one-shot casestudy design.Three of the interventions adopted social cognitive theory,and the remaining one was guided by the diffusion of innovations theory.Three studies identified core constructs of a theory as predictors of behavioral change.Two used theory to select or develop intervention techniques.Two studies used theory to customize participant intervention techniques.Two studies discussed their findings in the context of theory.No study used theory to select potential intervention participants.CONCLUSION In conclusion,despite the value of theory in intervention design and evaluation,theory-based diabetes interventions at school remain scarce.Future research may seek ways to better integrate theory and empirical research.展开更多
Objective:To evaluate the value of rehabilitation nursing based on mind mapping model combined with psychological intervention for patients with nephrotic syndrome(NS).Methods:A total of 60 patients with NS who visite...Objective:To evaluate the value of rehabilitation nursing based on mind mapping model combined with psychological intervention for patients with nephrotic syndrome(NS).Methods:A total of 60 patients with NS who visited our hospital from January 2024 to December 2024 were selected as samples and randomly divided into groups.The observation group received rehabilitation nursing based on the mind mapping model combined with psychological intervention,while the control group received routine intervention.The differences in emotional scores,self-care ability scores,compliance,and complications were compared between the two groups.Results:The anxiety(SAS)and depression(SDS)scores of the observation group were lower than those of the control group,while the self-care ability scale(ESCA)score was higher than that of the control group(P<0.05).The compliance rate of the observation group was higher than that of the control group(P<0.05).The complication rate of NS in the observation group was lower than that in the control group(P<0.05).Conclusion:Rehabilitation nursing based on the mind mapping model combined with psychological intervention can enhance self-care ability,reduce negative emotions,and reduce complications in NS nursing,which is efficient and feasible.展开更多
BACKGROUND The development of hepatocellular carcinoma(HCC)is influenced by multiple factors.Interventional therapy offers an effective treatment option for patients with unresectable intermediate-to-advanced HCC.Inte...BACKGROUND The development of hepatocellular carcinoma(HCC)is influenced by multiple factors.Interventional therapy offers an effective treatment option for patients with unresectable intermediate-to-advanced HCC.Interventional therapy can induce electrocardiographic(ECG)abnormalities that may be associated with liver dysfunction,electrolyte disorders,and cardiac injury.AIM To explore the ECG alterations and determinants following interventional therapy in patients with HCC.METHODS Sixty patients undergoing interventional treatment for liver cancer were selected as study participants.According to the results of the dynamic ECG examination 1 day after surgery,the patients were divided into an abnormal group(n=21)and a nonabnormal group(n=39).With the help of dynamic ECG examination,the ECG parameters were compared and the baseline data of patients was recorded in the two groups.RESULTS The 24 hours QT interval variability,24 hours normal atrial polarization to ventricular polarization(R-R)interval(standard deviation),24 hours consecutive 5 minutes normal R-R interval,and 24 hours continuous 5 minutes normal R-R interval(standard deviation mean)were lower than patients in the nonabnormal group(P<0.05).The logistic analysis showed that age>60 years,liver function grade B,and postoperative body temperature 38°C were risk factors for abnormal dynamic electrocardiogram in patients with liver cancer intervention(P<0.05).CONCLUSION Interventional therapy for HCC can lead to ECG abnormalities,underscoring the clinical need for enhanced cardiac monitoring to mitigate myocardial complications.展开更多

Chronic kidney disease(CKD)is a progressive disease with high morbidity and mortality.Disturbed gut microbiota and toxin accumulation are the main pathologic features of CKD.Current treatments are limited to those tha...Chronic kidney disease(CKD)is a progressive disease with high morbidity and mortality.Disturbed gut microbiota and toxin accumulation are the main pathologic features of CKD.Current treatments are limited to those that alleviate renal impairment in CKD patients,but few interventions are available that specifically target the regulatory mechanisms of gut microbiota.In this context,researchers urgently need new approaches that can significantly improve survival time and quality of life in these patients.In this review,we outline the involvement of the gut-kidney axis in kidney injury through disturbances of gut microbiota and dysregulation of endogenous metabolites,as well as the prominent contribution of gut microbiota in the discovery of CKD that can be used to prevent,diagnose,and treat CKD.Next,we describe several major metabolites associated with the host-gut microbiota that arise from the synthesis of microbial nutrient transformations and subsequent interactions with the kidney.Then,we summarize the role and potential targets of a compromised gut barrier in CKD.Finally,we discuss research advances in the prevention and treatments of CKD through probiotics modulation of gut microbiota structure to reduce enterogenous toxins and their maintenance of gut barrier function.A growing body of research suggests that intervention through probiotics may be a new and promising therapeutic strategy for CKD.展开更多
BACKGROUND Excessive noise in healthcare environments—commonly described as"unwanted sound"—has been linked to a range of negative impacts on both patients and staff.In clinical settings,elevated noise lev...BACKGROUND Excessive noise in healthcare environments—commonly described as"unwanted sound"—has been linked to a range of negative impacts on both patients and staff.In clinical settings,elevated noise levels have been associated with sleep disruption,heightened cardiovascular stress,and an increased risk of delirium in patients.Among healthcare workers,noise can impair focus and cognitive performance,potentially compromising care quality.AIM To evaluate the effectiveness of educational and behavioural interventions in reducing noise levels within intensive care units(ICUs),recognizing their potential impact on patient outcomes and healthcare effectiveness.METHODS A prospective interventional study in two Singaporean teaching hospitals compared peak and average sound levels between control and intervention groups.An educational and behavioural intervention comprising talks,posters,and self-audits by nurse champions was initiated in two ICUs in one hospital on November 18,2023.Sound measurements were collected at 4 Locations within each ICU before and after intervention.Baseline measurements were taken from October 22,2023 to October 29,2023,and post-intervention measurements from December 21,2023 to December 22,2023.The hospitals served as the primary exposure variable,controlled for ICU type(medical vs surgical)and hour of the day.RESULTS Our analysis generated 48 pairs of peak and average sound level readings for each unit(control n=48 readings;intervention n=48 readings).The effect of the intervention was associated with a significant 4.8 dB decrease in average sound level(P=0.009)and a nonsignificant 4.3 dB decrease in peak sound level(P=0.104),adjusted for hour of day and type of ICU.CONCLUSION Educational and behavioural interventions successfully reduced average sound levels,emphasizing their positive impact on noise control.These findings contribute valuable insights for optimizing noise reduction efforts in critical care settings.Future studies may explore additional systemic and environmental interventions to enhance noise management strategies.展开更多
The ongoing advancement of aerospace technology globally offers technical support for human exploration of outer space.Nevertheless,astronauts encounter microgravity environments during their missions in outer space,w...The ongoing advancement of aerospace technology globally offers technical support for human exploration of outer space.Nevertheless,astronauts encounter microgravity environments during their missions in outer space,which significantly affects the functionality of various physiological systems,including the skeletal,muscular and immune systems.Among them,microgravity-induced bone loss is particularly severe.Bone loss markedly elevates the risk of osteoporosis and fractures,presenting a substantial obstacle to astronauts’ability to perform tasks in the space environment and maintain their physical health.Consequently,implementing scientifically grounded preventive and therapeutic measures is essential for mitigating microgravity-induced bone loss.Currently,numerous intervention strategies have been demonstrated to effectively address microgravity-induced bone loss,such as pharmacological treatment,nutritional supplementation,and exercise intervention.However,the efficacy of these interventions varies,and some may result in adverse effects.Therefore,this narrative review analyzes and summarizes the effects of various interventions on bone loss caused by microgravity,aiming to provide a scientific theoretical basis for determining the optimal intervention strategy.展开更多

文摘Background:Parenting exerts a profound influence on children’s mental health and behavioral development.Despite the high prevalence of children’s emotional and behavioral problems(CEBP)in China,evidence-based parenting interventions remain scarcely investigated as preventive public health strategies.This pilot study evaluated a school-based intervention for preventing CEBP.Methods:We employed a quasi-experimental design with propensity score matching(PSM)to select 28 families(intervention:n=13;control:n=15)from two matched urban primary schools.Quantitative data from seven validated scales were analyzed using t-tests and ANCOVA.Qualitative insights were derived from 10 semi-structured interviews via thematic analysis.Results:Compared to the control group,the intervention group demonstrated significantly greater improvements in CEBP(p=0.020,Cohen’s d=0.92),parental adjustment(p=0.031,Cohen’s d=0.80),parenting confidence(p=0.003,Cohen’s d=1.04),and parentchild relationships(p=0.001,Cohen’s d=1.46).Non-significant effects were observed for parenting style,parental relationship,and parenting conflict(p>0.05).Qualitative analysis corroborated these findings and further identified contributing factors for non-significant outcomes,including challengeswithmeasurement adaptability and inconsistent co-parenting practices.Conclusions:This pilot study suggests that an authoritative parenting style may be effective and culturally adaptable in China.Positive parenting interventions appear to mitigate CEBP by reducing risk factors and enhancing protective factors.However,improving parental relationships and parenting conflict may require targeted strategies.Given the pilot nature of this PSM-matched study(n=28),the findings should be interpreted as exploratory and used primarily for intervention refinement.
文摘Non-communicable diseases (NCDs) are on the rise worldwide and in developing countries like Botswana. Unhealthy eating habits and lack of proper nutrition knowledge cause non-communicable diseases and affect adolescents. It is in adolescence that eating habits are formed that persist till adulthood. Lifestyle interventions are needed to curb NCDs in adolescents. This paper reports the findings of a study that aimed to validate a lifestyle intervention program and its effect on food intake, physical activity, and nutrition knowledge. It was a clustered randomized control trial study conducted in four (4) junior secondary schools. There were 46 participants, 21 in the control and 25 in the intervention arm, who were blindly assigned to each arm by a statistician. Information and skills on nutrition were imparted using the Information, Motivation, and Behavioral Skills model. The program was implemented for eight (8) weeks hourly after school. A questionnaire was used to collect data pre- and post-intervention. Number, proportion, percentage, and independent t-test (mean and SD or median and IQR, p-value) were calculated using numerical and categorical data. The findings showed that the lifestyle intervention was valid, and there was a slight decrease in the intake of sweets among participants in both trial arms (p = 0.066). There was no significant difference in terms of food intake. Only a small number of participants still ate a few fruits, and there was no change in vegetable intake in both trial arms (p = 0.641). There was no change in the intake of fried foods in both trail arms (p = 0.402). Regarding nutrition knowledge, there was a slight significant difference of p = 0.079 between the trial arms. Though the effect of the lifestyle intervention program was not statistically significant, the results are promising, especially if the duration could be increased to a longer period and a larger sample size included.
文摘Background:Early Hearing Detection and Intervention(EHDI)plays a critical role in improving language,cognitive,and socio-emotional outcomes for infants with hearing loss.In Nigeria,however,EHDI implementation remains limited by fragmented service delivery,uneven technological capacity,and sociocultural factors that delay timely diagnosis.This study explored the perspectives of paediatric audiologists and parents to provide a comprehensive understanding of the opportunities and challenges influencing early hearing care across diverse Nigerian settings.Methods:A mixed-methods design was employed across audiology facilities selected systematically from four Nigerian geopolitical zones.Twenty-five paediatric audiologists and twenty-three parents of children with congenital hearing loss participated.Quantitative data were collected using a structured questionnaire assessing awareness,diagnostic access,and intervention experiences.Qualitative data were obtained through semi-structured interviews and two focus group discussions.Thematic analysis followed Braun and Clarke's six-step framework,with dual coding,external auditing,and member validation to enhance credibility.Results:Quantitative findings demonstrated broad agreement on the diagnostic value of otoacoustic emissions(OAEs)and automated auditory brainstem responses(AABRs),the developmental benefits of early intervention,and the importance of active parental involvement.However,respondents identified persistent barriers including high costs of screening and therapy,poor public awareness of early hearing loss symptoms,and a critical shortage of trained personnel,and unequal distribution of diagnostic tools,particularly in rural and northern regions.Thematic analysis further underscored disparities in diagnostic capacity,sociocultural interpretations of deafness that delay clinical consultation,and economic constraints that hinder continuity of care.While families who accessed early intervention reported improved communication,social engagement,and learning readiness in their children,systemic gaps continue to limit widespread success.Conclusions:Despite growing technological capacity and awareness of EHDI benefits,significant structural,financial,and sociocultural challenges continue to impede timely diagnosis and intervention in Nigeria.Strengthening national policies,ensuring equitable distribution of diagnostic tools,expanding professional training,subsidising services,implementing culturally sensitive awareness campaigns and integration of Universal Newborn Hearing Screening into routine postnatal care are essential to improving outcomes for deaf infants.
文摘BACKGROUND Approximately 30%of patients with head and neck cancer experience adverse effects caused by anxiety and depression.Considering the high prevalence,implementing customized interventions to ease adverse emotional states is imperative.AIM To evaluate the efficacy of cognitive behavioral therapy(CBT)-based psychological interventions in improving the psychological well-being and quality of life(QoL)of patients with laryngeal carcinoma.METHODS This study enrolled 120 patients admitted from February 2022 to February 2024.The control group,comprising 50 participants,received standard supportive psychological care,while the research group,consisting 70 participants,underwent CBT-based interventions.Several clinical outcomes were systematically assessed that included postoperative recovery metrics(duration of tracheostomy and nasogastric tube dependence and length of hospitalization),psychological status(Self-Rating Anxiety Scale and Self-Rating Depression Scale),nutritional markers(serum albumin and hemoglobin levels),sleep quality(Self-Rating Scale of Sleep and Athens Insomnia Scale),and QoL(Functional Assessment of Cancer Therapy-Head and Neck).RESULTS The results demonstrated that the research group experienced superior outcomes,with significantly reduced durations of tracheostomy and nasogastric tube dependence,as well as shorter hospital stays,compared with the control group.Additionally,the research group exhibited markedly lower post-intervention Self-Rating Anxiety Scale,Self-Rating Depression Scale,Self-Rating Scale of Sleep,and Athens Insomnia Scale scores,along with minimal but higher change in serum albumin and hemoglobin levels compared with the control group.All five domains of Functional Assessment of Cancer Therapy-Head and Neck showed notable improvements in the research group,exceeding those observed in the control group.CONCLUSION CBT-based psychological support positively affects the mental well-being and QoL of patients with laryngeal carcinoma,highlighting its potential for broader clinical application.

文摘Microglia are the resident macrophages of the central nervous system.They act as the first line of defense against pathogens and play essential roles in neuroinflammation and tissue repair after brain insult or in neurodegenerative and demyelinating diseases(Borst et al.,2021).Together with infiltrating monocyte-derived macrophages,microglia also play a critical role for brain tumor development,since immunosuppressive interactions between tumor cells and tumor-associated microglia and macrophages(TAM)are linked to malignant progression.This mechanism is of particular relevance in glioblastoma(GB),the deadliest form of brain cancer with a median overall survival of less than 15 months(Khan et al.,2023).Therefore,targeting microglia and macrophage activation is a promising strategy for therapeutic interference in brain disease.

文摘Purpose:The aim of this mixed-studies systematic review was to ascertain the effectiveness of school-based interventions in increasing physical activity(PA) and/or reducing sedentary time(ST) in children aged 5-11 years,as well as to explore their effectiveness in relation to categories of the theory of expanded,extended,and enhanced opportunity(TEO).Methods:Adhering to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA) guidelines,5 databases were searched using predefined search terms.Following title and abstract screening of 1115 records,the removal of duplicates(n=584) and articles that did not meet the inclusion criteria agreed to a priori(n=419) resulted in 112 records that were full-text screened.Two independent reviewers subsequently used the mixed-methods appraisal tool to assess the methodological quality of 57 full-text studies that met the inclusion criteria after full-text screening.The interventions were summarised using the TIDierR checklist and TEO.The strength of evidence was determined using a 5-level rating system utilising a published decision tree.Results:Overall evidence ratings for interventions implemented within school settings were:no evidence of effects on moderate-to-vigorous physical activity(MVPA) and inconclusive evidence of effects on sedentary time.In relation to the TEO,expansion of PA appeared to be the most promising intervention type for MVPA,with moderate evidence of effect,whereas extension and enhancement of PA opportunity demonstrated no evidence of effect.A critical issue of possible compensatory behavior was identified by analysis of intervention effect in relation to PA measurement duration;when studies measured changes in PA during the actual intervention,there was moderate evidence of effect,whereas those that measured changes in PA during the school day presented inconclusive evidence of effect,and those that measured changes in PA over a whole day yielded no evidence of effect.Two meta-analyses of those studies using a whole-day accelerometer measure for MVPA or ST showed a significant but moderate effect for MVPA(effect size=0.51;95% confidence interval(CI):0.02-0.99) and a large but nonsignificant effect for ST(effect size=1.15;95%CI:-1.03 to 3.33);both meta-analyses demonstrated low precision,considerable inconsistency,and high heterogeneity.Conclusion:The findings have important implications for future intervention research in terms of intervention design,implementation,and evaluation.

文摘Purpose: Motor abilities of migrant children show worse performance compared with children without migration background. This study investigated whether a one-year intervention, which aimed to increase physical activity (PA), could influence the motor performance of these children positively. Methods: A sub-sample of 525 migrant children (7.1 ± 0.7 years;48.6% male), participated in the study and included an intervention (IG) and control group (CG). The Dordel-Koch-Test was used to assess motor performance. An exploratory factor analysis was performed. Differences between IG and CG were examined using ANCOVA. Results: Boys in the IG showed a significant improvement in the conditional skills (F(1, 201) = 8.02, p ≤ 0.005) and girls showed a tendency towards better results. Additionally, girls showed a significant improvement in flexibility (F(1, 226) = 10.72, p ≤ 0.01) compared with the CG. Conclusion: The intervention affects parts of motor skills of migrant children significantly. Future interventions should aim at a target-group-specific promotion to get a holistic improvement in their motor abilities.

To examine adolescent experiences and perspectives of the GoActive intervention (ISRCTN31583496) using mixed methods processevaluation to determine satisfaction with intervention components and interpret a*dolescents experiences of the intervention process in order toprovide insights for future intervention design.Methods'. Participants (n = 1542;13.2 土 0.4 years, mean 土 SD) provided questionnaire data at baseline (shyness, activity level) and post-intervention(intervention acceptability, satisfaction with components). Between-group differences (boys vs. girls and shy/inactive vs. others) weretested with linear regression models, accounting for school clustering. Data from 16 individual interviews (shy/inactive) and 11 focus groupswith 48 participants (mean = 4;range 2—7) were thematically coded. Qualitative and quantitative data were merged in an integrative mixedmethods convergence matrix, which denoted convergence and dissonance across datasets.Results'. Effect sizes for quantitative results were small and may not represent substantial between-group differences. Boys (vs. girls) preferredclass-based sessions (0 = 0.2, 95% confidence interval (CI): 0.1—0.3);qualitative data suggested that this was because boys preferred competition,which was supported quantitatively (0 = 0.2, 95%CI: 0.1-0.3). Shy/inactive students did not enjoy the competition (0 = -0.3, 95%CI:—0.5 to —0.1). Boys enjoyed trying new activities more (0 = 0.1, 95%CI: 0.1 -0.2);qualitative data indicated a desire to try new activities acrossall subgroups but identified barriers to choosing unfamiliar activities with self-imposed choice restriction leading to boredom. Qualitative datahighlighted critique of mentorship;adolescents liked the idea, but older mentors did not meet expectations.Conclusion. We interpreted adolescent perspectives of intervention components and implementation to provide insights into future complexinterventions aimed at increasing young people's physical activity in school-based settings. The intervention component mentorship was liked inprinciple, but implementation issues undesirably impacted satisfaction;competition was disliked by girls and shy/inactive students. The resultshighlight the importance of considering gender differences in preference of competition and extensive mentorship training.

文摘Objective: Use a school-based food garden as an instrument to improve healthy dietary consumption, habits, and practices among children from the third to fifth grade in public schools in Chile. Design: Quasi-experimental, six-month intervention with an evaluation of dietary consumption, habits, and practices and use of the school-based food garden among the 3rd to 5th grade students. Setting: Two municipal schools in a low-middle income district in Santiago de Chile;one with a school garden intervention and a control school without a garden. Subjects: 155 third to fifth grade children, 63 in the intervention school and 92 in the control school (average age 10 years old, 58% boys). Results: In the intervened school, there was a significant improvement in the children’s dietary habits, such as peeling and slicing fruit (p < 0.05), making sandwiches (p < 0.05), and increased motivation to cook (p < 0.05). Fruit purchases at the school rose from 4% to 37% (p < 0.05) by the end of the intervention. Interest in growing vegetables or fruits at home increased from 48% to 70% (p < 0.05). The school children at the intervention school significantly increased their knowledge about 16 of 21 garden crops (p < 0.05), expanding the spectrum of the vegetables they can consume. There were no significant changes in the control school. Conclusions: The school-based food garden was effective in achieving change in dietary habits and practices among the third to fifth grade children. The project should be carried out for a longer period of time, so that the changes will be reflected in the students’ food consumption and nutritional status.
文摘BACKGROUND Diabetes is one of the most common chronic diseases of childhood.School plays an essential role in the management of student diabetes,which reduces the risk of short-and long-term diabetes complications and ensures that students are wellpositioned for optimal academic performance and growth.AIM The aim of this review was to systematically identify and synthesize the literature concerning theory-based diabetes care interventions in K-12 schools in the United States.It critically assessed the specific role of theories and associated essential constructs in intervention design,implementation,outcome measurement,and evaluation.METHODS Relevant literature was identified by keyword searches of the Cochrane Library,PubMed,and Web of Science.RESULTS Four interventions met the eligibility criteria and were included in the review.Of those,two evaluated online diabetes education programs for school personnel and the other two assessed in-person interventions.Three studies adopted a onegroup pre-post study design,and the remaining one adopted a one-shot casestudy design.Three of the interventions adopted social cognitive theory,and the remaining one was guided by the diffusion of innovations theory.Three studies identified core constructs of a theory as predictors of behavioral change.Two used theory to select or develop intervention techniques.Two studies used theory to customize participant intervention techniques.Two studies discussed their findings in the context of theory.No study used theory to select potential intervention participants.CONCLUSION In conclusion,despite the value of theory in intervention design and evaluation,theory-based diabetes interventions at school remain scarce.Future research may seek ways to better integrate theory and empirical research.
文摘Objective:To evaluate the value of rehabilitation nursing based on mind mapping model combined with psychological intervention for patients with nephrotic syndrome(NS).Methods:A total of 60 patients with NS who visited our hospital from January 2024 to December 2024 were selected as samples and randomly divided into groups.The observation group received rehabilitation nursing based on the mind mapping model combined with psychological intervention,while the control group received routine intervention.The differences in emotional scores,self-care ability scores,compliance,and complications were compared between the two groups.Results:The anxiety(SAS)and depression(SDS)scores of the observation group were lower than those of the control group,while the self-care ability scale(ESCA)score was higher than that of the control group(P<0.05).The compliance rate of the observation group was higher than that of the control group(P<0.05).The complication rate of NS in the observation group was lower than that in the control group(P<0.05).Conclusion:Rehabilitation nursing based on the mind mapping model combined with psychological intervention can enhance self-care ability,reduce negative emotions,and reduce complications in NS nursing,which is efficient and feasible.
文摘BACKGROUND The development of hepatocellular carcinoma(HCC)is influenced by multiple factors.Interventional therapy offers an effective treatment option for patients with unresectable intermediate-to-advanced HCC.Interventional therapy can induce electrocardiographic(ECG)abnormalities that may be associated with liver dysfunction,electrolyte disorders,and cardiac injury.AIM To explore the ECG alterations and determinants following interventional therapy in patients with HCC.METHODS Sixty patients undergoing interventional treatment for liver cancer were selected as study participants.According to the results of the dynamic ECG examination 1 day after surgery,the patients were divided into an abnormal group(n=21)and a nonabnormal group(n=39).With the help of dynamic ECG examination,the ECG parameters were compared and the baseline data of patients was recorded in the two groups.RESULTS The 24 hours QT interval variability,24 hours normal atrial polarization to ventricular polarization(R-R)interval(standard deviation),24 hours consecutive 5 minutes normal R-R interval,and 24 hours continuous 5 minutes normal R-R interval(standard deviation mean)were lower than patients in the nonabnormal group(P<0.05).The logistic analysis showed that age>60 years,liver function grade B,and postoperative body temperature 38°C were risk factors for abnormal dynamic electrocardiogram in patients with liver cancer intervention(P<0.05).CONCLUSION Interventional therapy for HCC can lead to ECG abnormalities,underscoring the clinical need for enhanced cardiac monitoring to mitigate myocardial complications.
文摘BACKGROUND Simultaneous acute ischemic stroke(AIS)and myocardial infarction(cardio-cerebral ischemic attack)have rarely been reported in the literature.Currently,no clear evidence-based guidelines or clinical trials exist to determine the optimal therapeutic strategy for these patients.CASE SUMMARY We present the case of a 27-year-old Chinese man who simultaneously experie-nced acute concomitant cerebrocardiac infarction(CCI)and painless ST-elevation myocardial infarction.The patient was successfully treated with elective percu-taneous coronary intervention(PCI)after receiving urgent systemic thrombolysis at the standard dose for AIS.CONCLUSION Urgent thrombolysis followed by elective PCI was an appropriate strategy for the management of simultaneous CCI.
文摘Liver transplantation,as an effective therapy for patients with liver cancer,plays an important role in improving the quality of life of patients.However,the com-plexity and trauma of liver transplantation can easily lead to the occurrence of malnutrition in patients,and then increase the risk of postoperative complica-tions,which has aroused widespread clinical attention.Reasonable nutritional support can not only maintain the stability of the body’s internal environment,reduce the occurrence of complications,but also promote the recovery of liver and other organ functions.In recent years,with the in-depth understanding of nut-ritional metabolism after liver transplantation,the application of enteral nutrition and parenteral nutrition in nutritional support after liver transplantation has been increasingly extensive and achieved remarkable results.This paper discusses the effect of early postoperative nutritional intervention on patients with liver cancer and liver transplantation,and combined with its mechanism of action,can better understand the effectiveness of intervention,and provide reference for the deve-lopment of scientific and reasonable nutritional support programs in clinical pra-ctice.
文摘This study aims to explore the characteristics of novice teachers’inappropriate behaviors in classroom teaching and their intervention strategies.With the continuous improvement of education quality,novice teachers face increasing challenges in teaching practice.Their inappropriate behaviors not only affect the classroom atmosphere but may also negatively impact students’learning outcomes.Therefore,researching the characteristics of novice teachers’inappropriate behaviors and their intervention strategies holds significant scientific and social value.This study employs a combination of quantitative and qualitative methods to analyze the behavioral patterns of novice teachers in classroom teaching and proposes corresponding intervention strategies.The results indicate that novice teachers’inappropriate behaviors mainly manifest as poor classroom management,monotonous teaching methods,and insufficient interaction with students.Based on these findings,the study proposes a series of effective intervention strategies,including enhancing teacher training,optimizing teaching design,and promoting positive interactions between teachers and students.The conclusions of the study not only provide practical guidance for educational practice but also point out directions for future research,emphasizing the crucial role of teacher professional development in improving teaching quality.

文摘Chronic kidney disease(CKD)is a progressive disease with high morbidity and mortality.Disturbed gut microbiota and toxin accumulation are the main pathologic features of CKD.Current treatments are limited to those that alleviate renal impairment in CKD patients,but few interventions are available that specifically target the regulatory mechanisms of gut microbiota.In this context,researchers urgently need new approaches that can significantly improve survival time and quality of life in these patients.In this review,we outline the involvement of the gut-kidney axis in kidney injury through disturbances of gut microbiota and dysregulation of endogenous metabolites,as well as the prominent contribution of gut microbiota in the discovery of CKD that can be used to prevent,diagnose,and treat CKD.Next,we describe several major metabolites associated with the host-gut microbiota that arise from the synthesis of microbial nutrient transformations and subsequent interactions with the kidney.Then,we summarize the role and potential targets of a compromised gut barrier in CKD.Finally,we discuss research advances in the prevention and treatments of CKD through probiotics modulation of gut microbiota structure to reduce enterogenous toxins and their maintenance of gut barrier function.A growing body of research suggests that intervention through probiotics may be a new and promising therapeutic strategy for CKD.
文摘BACKGROUND Excessive noise in healthcare environments—commonly described as"unwanted sound"—has been linked to a range of negative impacts on both patients and staff.In clinical settings,elevated noise levels have been associated with sleep disruption,heightened cardiovascular stress,and an increased risk of delirium in patients.Among healthcare workers,noise can impair focus and cognitive performance,potentially compromising care quality.AIM To evaluate the effectiveness of educational and behavioural interventions in reducing noise levels within intensive care units(ICUs),recognizing their potential impact on patient outcomes and healthcare effectiveness.METHODS A prospective interventional study in two Singaporean teaching hospitals compared peak and average sound levels between control and intervention groups.An educational and behavioural intervention comprising talks,posters,and self-audits by nurse champions was initiated in two ICUs in one hospital on November 18,2023.Sound measurements were collected at 4 Locations within each ICU before and after intervention.Baseline measurements were taken from October 22,2023 to October 29,2023,and post-intervention measurements from December 21,2023 to December 22,2023.The hospitals served as the primary exposure variable,controlled for ICU type(medical vs surgical)and hour of the day.RESULTS Our analysis generated 48 pairs of peak and average sound level readings for each unit(control n=48 readings;intervention n=48 readings).The effect of the intervention was associated with a significant 4.8 dB decrease in average sound level(P=0.009)and a nonsignificant 4.3 dB decrease in peak sound level(P=0.104),adjusted for hour of day and type of ICU.CONCLUSION Educational and behavioural interventions successfully reduced average sound levels,emphasizing their positive impact on noise control.These findings contribute valuable insights for optimizing noise reduction efforts in critical care settings.Future studies may explore additional systemic and environmental interventions to enhance noise management strategies.

文摘The ongoing advancement of aerospace technology globally offers technical support for human exploration of outer space.Nevertheless,astronauts encounter microgravity environments during their missions in outer space,which significantly affects the functionality of various physiological systems,including the skeletal,muscular and immune systems.Among them,microgravity-induced bone loss is particularly severe.Bone loss markedly elevates the risk of osteoporosis and fractures,presenting a substantial obstacle to astronauts’ability to perform tasks in the space environment and maintain their physical health.Consequently,implementing scientifically grounded preventive and therapeutic measures is essential for mitigating microgravity-induced bone loss.Currently,numerous intervention strategies have been demonstrated to effectively address microgravity-induced bone loss,such as pharmacological treatment,nutritional supplementation,and exercise intervention.However,the efficacy of these interventions varies,and some may result in adverse effects.Therefore,this narrative review analyzes and summarizes the effects of various interventions on bone loss caused by microgravity,aiming to provide a scientific theoretical basis for determining the optimal intervention strategy.
