Background:The risk of internal fixation failure remains relatively high in stable femoral neck fracture(FNF)(Garden I or II).Preoperative sagittal displacement of the femoral head has been proposed as a potential inf...Background:The risk of internal fixation failure remains relatively high in stable femoral neck fracture(FNF)(Garden I or II).Preoperative sagittal displacement of the femoral head has been proposed as a potential influencing factor.This study aimed to evaluate the impact of sagittal displacement on the outcomes of cannulated screw internal fixation(CSIF)in patients with stable FNF(Garden I or II)by reconstructing the axial sagittal oblique plane of the fracture using preoperative computed tomography(CT)imaging.Methods:This study included 167 patients with FNF who underwent CSIF.The sagittal tilt angle of the femoral head(STAFH)was evaluated using three-dimensional CT(3D-CT).The distribution of preoperative STAFH was analyzed,and its independent association with treatment failure was assessed.Treatment failure was defined as the need for revision surgery within 2 years postoperatively due to avascular necrosis,nonunion,or internal fixation failure.Results:Among the 167 patients,9(5.4%)exhibited anterior tilt(AT)of the femoral head,158(94.60%)presented with posterior tilt(PT).A total of 50 patients(29.9%)demonstrated excessive sagittal displacement(AT≥10°or PT≥20°).In the failure group,80.0%of patients had excessive sagittal displacement compared to 28.1%in the healed group.Excessive sagittal displacement was significantly associated with an increased risk of surgical failure(odds ratio:11.953,95%CI:3.656-39.083,p<0.05).Conclusions:In patients with Garden I or II FNF,greater preoperative sagittal displacement of the femoral head was correlated with a higher likelihood of CSIF failure.AT≥10°or PT≥20°were identified as independent predictors of CSIF failure in FNF patients.Nevertheless,these findings still require confirmation through prospective,multi-center clinical trials with large sample sizes.展开更多

Background:Telehealth has emerged as a powerful tool for managing chronic diseases and mental health conditions,offering increased access to care and improved patient outcomes.However,inequities in digital connectivit...Background:Telehealth has emerged as a powerful tool for managing chronic diseases and mental health conditions,offering increased access to care and improved patient outcomes.However,inequities in digital connectivity and technological resources have created significant disparities in access to these potentially life-changing services,disproportionately impacting marginalized and minoritized communities across the globe.Methods:Data on 473,716 telehealth encounters occurring between January 1,2022,and June 30,2023 were retrieved from the electronic health records(EHR)system used by University Hospitals.These encounters were classified into three groups:attended,canceled,and no-show.Relative risk was calculated based on age,sex,and race,and a multivariate linear regression was performed with age,sex,and race as inputs,to determine their effect on the encounter outcome.Results:Our analysis identified significant differences in relative risk between demographic groups.Patients 20-39 years of age had a high relative risk of cancellation and no-show,and Black patients demonstrated the highest relative risk for cancellation and no-show.The regression analysis illustrated a statistically significant link between no-shows and patients with a cellular plan with no other internet subscription(p<0.001),smartphone ownership(p<0.001),and not having a computer(p<0.05).Conclusions:This study highlights the clinical repercussions of the digital divide,as patients relying on a mobile phone and data plan to attend telehealth visits were more likely to no-show.Current disparities in digital connectivity for historically marginalized populations heightens the risk of creating a digital underclass.There is evidence this study may be applicable in multiple countries across the world.Further research on the causes of the observed no-shows is necessary to ensure equitable delivery of digital healthcare services.展开更多
Background:Precision medicine(PM)has taken center stage in healthcare since the completion of the genomic project.Developed countries have gradually integrated PM into mainstream patient management.However,Nigeria sti...Background:Precision medicine(PM)has taken center stage in healthcare since the completion of the genomic project.Developed countries have gradually integrated PM into mainstream patient management.However,Nigeria still grapples with wide acceptance,key translational research and implementation of PM.This study sought to explore the knowledge and attitude of PM among pharmacists as key stakeholders in the healthcare team.Methods:A cross‐sectional study was conducted in selected tertiary hospitals across the country.A 21‐item semi‐structured questionnaire was administered by hybrid online and physical methods and the results analyzed with Statistical Package for the Social Sciences Version 25.Descriptive statistics were used to summarize the data.A chi‐square test was employed to determine the association of knowledge of PM and the sociodemographic characteristics of the study population.Results:A total of 167 hospital pharmacists participated in the study.A high proportion of the participants are familiar with artificial intelligence(91.75%),Pharmacogenomics(84.5%),and precision medicine(61%).Overall,38.9%of the pharmacists had a good knowledge while 13.2%had a poor knowledge of PM and associated terms.The level of knowledge did not correlate significantly with gender(X^(2)=3.21,p=0.201),age(X^(2)=5,p=0.27),marital status(X^(2)=3.21,p=0.201),and professional level(X^(2)=6.85,p=0.144).The most important value of precision medicine to hospital pharmacists is the ability to minimize the impact of disease through preventive medicine(49%)while a large portion are pursuing and or actively planning to pursue additional education in precision medicine.Conclusions:There is a highly positive attitude toward the prospect of PM among hospital pharmacists in Nigeria.Education modules in this field are highly recommended as most do not have a holistic knowledge of terms used in PM.Also,more research aimed at translating PM knowledge into clinical practice is recommended.展开更多
Background:Oral health issues persistently affect the overall health and well-being of rural populations.Village health volunteers(VHVs)play a crucial role in advancing oral health literacy in their community.This stu...Background:Oral health issues persistently affect the overall health and well-being of rural populations.Village health volunteers(VHVs)play a crucial role in advancing oral health literacy in their community.This study aimed to examine the factors related to nutritional literacy for oral health among VHVs.Methods:This was a mixed-methods study employing an explanatory sequential design.The quantitative data were gathered through questionnaires distributed to a cohort of 10,514 VHVs registered in Health Region 1.A stratified random sampling technique was used to ensure adequate representation of various subgroups within the VHV population,considering factors such as age,education level,and geographical distribution across the region.This approach allowed for a more representative sample that accurately reflects the diversity of the VHV population.Qualitative data were obtained through semi-structured interviews with a purposive sampling of 20 participants based on specific criteria.Quantitative data were analyzed using descriptive statistics and biserial correlation techniques,while qualitative data were analyzed using content analysis.Results:The study found that the sample group possessed a moderate level of knowledge of health literacy principles and nutrition for oral health.However,their self-assessed skills in nutritional literacy for oral health were rated as high.A statistically significant negative correlation was found between knowledge of nutrition for oral health and skills in nutritional literacy for oral health.VHVs equated health literacy with knowledge because their public health training had focused more on imparting knowledge rather than developing skills based on health literacy principles.Conclusion:There is a need to emphasize skill-based health literacy training and to use effective skill development techniques that are tailored to the specific roles and responsibilities of health volunteers.It is also recommended to continuously monitor and evaluate the outcomes of these efforts.展开更多

Background:To evaluate sex‐related differences in the risk factors associated with nonhealing or recurrence of hyper-thyroidism(NHRH)in patients with Graves'disease(GD)treated with radioactive iodine.Methods:In t...Background:To evaluate sex‐related differences in the risk factors associated with nonhealing or recurrence of hyper-thyroidism(NHRH)in patients with Graves'disease(GD)treated with radioactive iodine.Methods:In total,285 patients were enrolled.Data on radioactive iodine(RAI)dosage,ultrasound indexes of the thyroid,and other clinical factors were collected.Patients were divided into NHRH and non‐NHRH(hypothyroidism or euthyroidism)groups based on treatment outcomes.Univariate and multivariate weighted logistic regression analyses were used to identify factors associated with NHRH.Sex‐specific analyses of these risk factors were also conducted.Results:There were no significant differences between the two groups in terms of sex,thyroid shear wave elastography velocity values,or pretreatment serum free thyroxine(FT4)levels.Thyroid volume and age were independently associated with NHRH,with the odds of NHRH gradually decreasing as age increased.In subgroup analyses,both age and thyroid volume were independent risk factors for NHRH in female patients(p<0.05),while in male patients,only FT4 was independently associated with NHRH(p<0.05).Conclusions:In patients of different sexes,the influence of thyroid volume,age,and FT4 on treatment outcomes exhibits distinct patterns.展开更多

Background:Globally,the use of community pharmacies and pharmacists in the delivery of vaccination services has been hampered by several factors,laws,and regulations that do not support pharmacists to participate in t...Background:Globally,the use of community pharmacies and pharmacists in the delivery of vaccination services has been hampered by several factors,laws,and regulations that do not support pharmacists to participate in the delivery of vaccination services.With the advent of COVID-19 pandemic,many countries have included community pharmacists and pharmacies in vaccination services to improve coverage.This study described the delivery of vaccination services in community pharmacies using the COVID-19 experience and how their involvement impacted vaccination coverage in Nigeria.It also exposed how this experience can be used to support policy revisions to formally recognize pharmacists in immunization delivery.Methods:A descriptive cross-sectional study was conducted among 474 community pharmacists in two southwestern States in Nigeria,using a semi-structured questionnaire.It determines the number of community pharmacists who have been trained in the delivery of vaccination services,the types of vaccination services provided,and vaccines administered in their pharmacies.Data were analyzed with descriptive and inferential statistics and p-value at≤0.05.Results:Response rate was 86.7%.Less than half of the respondents(40.1%)had undergone vaccination training.Of the 129(31.4%)respondents that provide vaccination services,72(55.8%)administer vaccines in their pharmacies.Out of these 72 respondents;45(62.5%)were administering vaccines before their involvement in COVID-19 vaccine administration;57(79.2%)of the health personnel who administer vaccines were pharmacists;60(83.3%)of them administer vaccines on request;22(30.6%)administered COVID-19 vaccines only;and only 7(9.7%)of the respondents had administered over 500 doses of COVID-19 vaccines.Training in vaccination was associated with the vaccination services provided(p<0.05).Respondents suggested government support through legal framework and policy review,training and empowering pharmacists in vaccine administration,and recognition of community pharmacists as PHC providers.展开更多

Background:An urban medical group in Dapeng New District was established in 2017 with the objective of enhancing outcomes for common diseases and reinforcing primary care by integrating high‐level hospitals with prim...Background:An urban medical group in Dapeng New District was established in 2017 with the objective of enhancing outcomes for common diseases and reinforcing primary care by integrating high‐level hospitals with primary health services.This study aimed to evaluate the performance of the urban medical group using the triangular value chain framework.Methods:The evaluation was conducted using the Donabedian model,focusing on three key dimensions:safety and quality,accessibility,and affordability.Longitudinal data were collected from 2016 to 2022 through government annual reports,the medical insurance bureau,and hospital information systems.Preprogram and postprogram outcome measurements were compared to assess differences and trends,providing a clear picture of the program's effectiveness.Results:Accessibility improved significantly,with the number of hospital beds per 1000 residents increasing from 2.62 in 2017 to 3.76 in 2022.The availability of general practitioners(GPs)also rose markedly,from 0 per 10,000 residents in 2017 to 6.27 in 2022.Regarding safety and quality,the proportion of complex medical procedures conducted within the New District expanded substantially,from 7.35%in 2017 to 38.11%in 2021.Additionally,there was an enhancement in the standardized management rate of chronic diseases.Affordability assessments showed that the proportion of medical income derived from the medical insurance fund increased by nearly 22.81 percentage points between 2012 and 2021.By 2021,75.02%of medical patients were covered by medical insurance,representing an increase of approximately 44 percentage points from 31.19%in 2012.Conclusions:The implementation of the urban medical group in Dapeng New District has led to substantial improvements in healthcare accessibility,safety and quality,and affordability.Future initiatives will focus on advancing the“Dapeng Mode”to generate exemplary healthcare outcomes and minimize disparities in basic health services and health status between urban and rural populations.The reform agenda includes piloting payment reforms and innovative payment models within the Dapeng group,complemented by a health assessment and performance incentive system aimed at encouraging healthcare institutions to prioritize health management.展开更多
Background:Stroke is the second leading cause of death and third leading cause of disability worldwide and is the leading cause of death and disability among adults in China,with its incidence rate continuing to rise....Background:Stroke is the second leading cause of death and third leading cause of disability worldwide and is the leading cause of death and disability among adults in China,with its incidence rate continuing to rise.In China,the average age of firsttime stroke patients is 66.4 years,and the intravenous thrombolysis rate using recombinant tissue plasminogen activator within 3 h of onset is only 16%.Given this fact,there is a pressing need for real‐time predictive tools,particularly for elderly individuals at home,that can provide early warnings for potential strokes.Methods:We collected continuous monitoring data from nonintrusive smart beds and multimodal temporal data from electronic medical records at the National Center for Neurological Disorders.The data included smart bed monitoring indicators,laboratory tests,nurse observations,and static data as potential predictors,with stroke as the outcome.We applied feature representation and feature selection techniques and then input the predictors into machine learning models.Additionally,deep learning models were used after preprocessing the irregular temporal data.Finally,we evaluated the performance of the stroke prediction models and assessed the importance of the features.We used continuously updated vital signs and clinical data during hospitalization to generate timely stroke risk alerts during the same period of admission.Results:A total of 37,041 samples were analyzed,of which 7020 patients were diagnosed with stroke.When only the smart bed features were used for prediction,the model achieved an area under the receiver operating characteristic curve(AUROC)of 0.59−0.63,with an accuracy ranging from 60%−65%.Among the four artificial intelligence algorithms,the random forest model demonstrated the best performance.After all the available features were incorporated,the AUROC increased to 0.94,and the accuracy improved to 92%.Conclusions:In this study,the occurrence of stroke was successfully identified by integrating multimodal temporal data from electronic medical records.Noncontact monitoring of respiration and heart rate offers a promising approach for daily stroke surveillance in home‐based populations,particularly for elderly individuals living alone.展开更多

An individual's mental health influences their capacity to think effectively,feel emotionally stable,and perform daily activities.As mental health concerns become more prevalent worldwide,new awareness and diagnos...An individual's mental health influences their capacity to think effectively,feel emotionally stable,and perform daily activities.As mental health concerns become more prevalent worldwide,new awareness and diagnostic and treatment tactics are needed.Digital tools and technology are helping solve these problems by providing scalable,tailored solutions for large populations.This detailed review examines mental health‐promoting internet tools.Smartphone applications,web‐based therapy systems,wearable tech,artificial intelligence‐powered resources,and virtual reality(VR)technologies were evaluated for efficacy and side effects.PubMed,PsycINFO,Scopus,IEEE Xplore,and Google Scholar were carefully searched.Search terms included“digital mental health tools,”“online therapy,”and“AI in mental health.”Randomized controlled trials,cohort studies,cross‐sectional studies,systematic reviews,and meta‐analyses of digital technology and mental health were included from among the literature published after 2010.Cognitive behavioral therapy methods,mood monitoring,and mindfulness exercises are among the numerous features of smartphone applications that have been demonstrated to mitigate symptoms of anxiety,depression,and tension.Online therapy platforms let marginalized individuals obtain therapy remotely.Wearable technology may detect heart rate,blood pressure,and sleep length,which may reveal mental health difficulties.Chatbots employ machine learning algorithms and natural language processing to deliver customized support and show promise for quick intervention.Exposure therapy for anxiety and trauma is increasingly using virtual reality environments.Although digital mental health therapies face challenges in relation to data privacy,limited long‐term efficacy,and technological inequality,digital technologies are modernizing mental healthcare.By offering inexpensive and effective alternatives to traditional therapies,digital technologies may help healthcare systems meet the growing demand for mental health services and overall well‐being.展开更多

文摘Background:The risk of internal fixation failure remains relatively high in stable femoral neck fracture(FNF)(Garden I or II).Preoperative sagittal displacement of the femoral head has been proposed as a potential influencing factor.This study aimed to evaluate the impact of sagittal displacement on the outcomes of cannulated screw internal fixation(CSIF)in patients with stable FNF(Garden I or II)by reconstructing the axial sagittal oblique plane of the fracture using preoperative computed tomography(CT)imaging.Methods:This study included 167 patients with FNF who underwent CSIF.The sagittal tilt angle of the femoral head(STAFH)was evaluated using three-dimensional CT(3D-CT).The distribution of preoperative STAFH was analyzed,and its independent association with treatment failure was assessed.Treatment failure was defined as the need for revision surgery within 2 years postoperatively due to avascular necrosis,nonunion,or internal fixation failure.Results:Among the 167 patients,9(5.4%)exhibited anterior tilt(AT)of the femoral head,158(94.60%)presented with posterior tilt(PT).A total of 50 patients(29.9%)demonstrated excessive sagittal displacement(AT≥10°or PT≥20°).In the failure group,80.0%of patients had excessive sagittal displacement compared to 28.1%in the healed group.Excessive sagittal displacement was significantly associated with an increased risk of surgical failure(odds ratio:11.953,95%CI:3.656-39.083,p<0.05).Conclusions:In patients with Garden I or II FNF,greater preoperative sagittal displacement of the femoral head was correlated with a higher likelihood of CSIF failure.AT≥10°or PT≥20°were identified as independent predictors of CSIF failure in FNF patients.Nevertheless,these findings still require confirmation through prospective,multi-center clinical trials with large sample sizes.

文摘Background:Telehealth has emerged as a powerful tool for managing chronic diseases and mental health conditions,offering increased access to care and improved patient outcomes.However,inequities in digital connectivity and technological resources have created significant disparities in access to these potentially life-changing services,disproportionately impacting marginalized and minoritized communities across the globe.Methods:Data on 473,716 telehealth encounters occurring between January 1,2022,and June 30,2023 were retrieved from the electronic health records(EHR)system used by University Hospitals.These encounters were classified into three groups:attended,canceled,and no-show.Relative risk was calculated based on age,sex,and race,and a multivariate linear regression was performed with age,sex,and race as inputs,to determine their effect on the encounter outcome.Results:Our analysis identified significant differences in relative risk between demographic groups.Patients 20-39 years of age had a high relative risk of cancellation and no-show,and Black patients demonstrated the highest relative risk for cancellation and no-show.The regression analysis illustrated a statistically significant link between no-shows and patients with a cellular plan with no other internet subscription(p<0.001),smartphone ownership(p<0.001),and not having a computer(p<0.05).Conclusions:This study highlights the clinical repercussions of the digital divide,as patients relying on a mobile phone and data plan to attend telehealth visits were more likely to no-show.Current disparities in digital connectivity for historically marginalized populations heightens the risk of creating a digital underclass.There is evidence this study may be applicable in multiple countries across the world.Further research on the causes of the observed no-shows is necessary to ensure equitable delivery of digital healthcare services.
文摘Background:Precision medicine(PM)has taken center stage in healthcare since the completion of the genomic project.Developed countries have gradually integrated PM into mainstream patient management.However,Nigeria still grapples with wide acceptance,key translational research and implementation of PM.This study sought to explore the knowledge and attitude of PM among pharmacists as key stakeholders in the healthcare team.Methods:A cross‐sectional study was conducted in selected tertiary hospitals across the country.A 21‐item semi‐structured questionnaire was administered by hybrid online and physical methods and the results analyzed with Statistical Package for the Social Sciences Version 25.Descriptive statistics were used to summarize the data.A chi‐square test was employed to determine the association of knowledge of PM and the sociodemographic characteristics of the study population.Results:A total of 167 hospital pharmacists participated in the study.A high proportion of the participants are familiar with artificial intelligence(91.75%),Pharmacogenomics(84.5%),and precision medicine(61%).Overall,38.9%of the pharmacists had a good knowledge while 13.2%had a poor knowledge of PM and associated terms.The level of knowledge did not correlate significantly with gender(X^(2)=3.21,p=0.201),age(X^(2)=5,p=0.27),marital status(X^(2)=3.21,p=0.201),and professional level(X^(2)=6.85,p=0.144).The most important value of precision medicine to hospital pharmacists is the ability to minimize the impact of disease through preventive medicine(49%)while a large portion are pursuing and or actively planning to pursue additional education in precision medicine.Conclusions:There is a highly positive attitude toward the prospect of PM among hospital pharmacists in Nigeria.Education modules in this field are highly recommended as most do not have a holistic knowledge of terms used in PM.Also,more research aimed at translating PM knowledge into clinical practice is recommended.
文摘Background:Oral health issues persistently affect the overall health and well-being of rural populations.Village health volunteers(VHVs)play a crucial role in advancing oral health literacy in their community.This study aimed to examine the factors related to nutritional literacy for oral health among VHVs.Methods:This was a mixed-methods study employing an explanatory sequential design.The quantitative data were gathered through questionnaires distributed to a cohort of 10,514 VHVs registered in Health Region 1.A stratified random sampling technique was used to ensure adequate representation of various subgroups within the VHV population,considering factors such as age,education level,and geographical distribution across the region.This approach allowed for a more representative sample that accurately reflects the diversity of the VHV population.Qualitative data were obtained through semi-structured interviews with a purposive sampling of 20 participants based on specific criteria.Quantitative data were analyzed using descriptive statistics and biserial correlation techniques,while qualitative data were analyzed using content analysis.Results:The study found that the sample group possessed a moderate level of knowledge of health literacy principles and nutrition for oral health.However,their self-assessed skills in nutritional literacy for oral health were rated as high.A statistically significant negative correlation was found between knowledge of nutrition for oral health and skills in nutritional literacy for oral health.VHVs equated health literacy with knowledge because their public health training had focused more on imparting knowledge rather than developing skills based on health literacy principles.Conclusion:There is a need to emphasize skill-based health literacy training and to use effective skill development techniques that are tailored to the specific roles and responsibilities of health volunteers.It is also recommended to continuously monitor and evaluate the outcomes of these efforts.

文摘Objective:The scarcity of healthcare resources and inadequate access to medical services in rural and remote areas are pervasive challenges many countries face,particularly in the developing world.Telemedicine,with its capacity to overcome geographical barriers and provide patients with real‐time medical services,has shown considerable potential in addressing these issues,attracting wide-spread attention.Compact medical communities and family doctor systems play important roles in improving healthcare accessibility.However,despite the critical nature of patients'perceptions of healthcare accessibility,research in this domain is sparse.This study aimed to explore the impact of telemedicine on rural residents'perceived healthcare accessibility in China,analyze the mechanisms underpinning this relationship,and elucidate the roles of compact medical communities and the family doctor system.Methods:Survey data from 3311 rural residents were analyzed using a probit model,instrumental variables,and subgroup regression analyses to ascertain causal effects,perform heterogeneity analysis,examine mechanisms,and ascertain the robustness of the findings.Results:Telemedicine significantly enhanced rural residents'perceived healthcare accessibility,with particularly notable benefits for those in sparsely populated areas,regions with high‐speed internet access,within the purview of compact healthcare consortiums,and those with access to family doctor services.Furthermore,telemedicine improved rural residents'perceived healthcare accessibility by encouraging the use of primary care services.Conclusion:Telemedicine in China has played a significant role in improving the perceived healthcare accessibility among rural residents and aiding in the reduction of disparities in accessibility across different demographic groups.This is consistent with the broader objective of achieving universal health coverage.However,the efficacy of telemedicine in enhancing healthcare accessibility is contingent upon certain preconditions.Policymakers must confront local infrastructure challenges,particularly regarding internet connectivity,when expanding telemedicine services to ensure their effective operation.The synergistic interaction observed between telemedicine,the family doctor system,and compact medical communities highlights the importance of integrating telemedicine into existing healthcare systems.Such integration could enhance collaboration with current healthcare frameworks,ensuring the provision of safe,accessible,and affordable healthcare services,and promoting the health and well‐being of local populations.

文摘Background:To evaluate sex‐related differences in the risk factors associated with nonhealing or recurrence of hyper-thyroidism(NHRH)in patients with Graves'disease(GD)treated with radioactive iodine.Methods:In total,285 patients were enrolled.Data on radioactive iodine(RAI)dosage,ultrasound indexes of the thyroid,and other clinical factors were collected.Patients were divided into NHRH and non‐NHRH(hypothyroidism or euthyroidism)groups based on treatment outcomes.Univariate and multivariate weighted logistic regression analyses were used to identify factors associated with NHRH.Sex‐specific analyses of these risk factors were also conducted.Results:There were no significant differences between the two groups in terms of sex,thyroid shear wave elastography velocity values,or pretreatment serum free thyroxine(FT4)levels.Thyroid volume and age were independently associated with NHRH,with the odds of NHRH gradually decreasing as age increased.In subgroup analyses,both age and thyroid volume were independent risk factors for NHRH in female patients(p<0.05),while in male patients,only FT4 was independently associated with NHRH(p<0.05).Conclusions:In patients of different sexes,the influence of thyroid volume,age,and FT4 on treatment outcomes exhibits distinct patterns.

文摘Background:Globally,the use of community pharmacies and pharmacists in the delivery of vaccination services has been hampered by several factors,laws,and regulations that do not support pharmacists to participate in the delivery of vaccination services.With the advent of COVID-19 pandemic,many countries have included community pharmacists and pharmacies in vaccination services to improve coverage.This study described the delivery of vaccination services in community pharmacies using the COVID-19 experience and how their involvement impacted vaccination coverage in Nigeria.It also exposed how this experience can be used to support policy revisions to formally recognize pharmacists in immunization delivery.Methods:A descriptive cross-sectional study was conducted among 474 community pharmacists in two southwestern States in Nigeria,using a semi-structured questionnaire.It determines the number of community pharmacists who have been trained in the delivery of vaccination services,the types of vaccination services provided,and vaccines administered in their pharmacies.Data were analyzed with descriptive and inferential statistics and p-value at≤0.05.Results:Response rate was 86.7%.Less than half of the respondents(40.1%)had undergone vaccination training.Of the 129(31.4%)respondents that provide vaccination services,72(55.8%)administer vaccines in their pharmacies.Out of these 72 respondents;45(62.5%)were administering vaccines before their involvement in COVID-19 vaccine administration;57(79.2%)of the health personnel who administer vaccines were pharmacists;60(83.3%)of them administer vaccines on request;22(30.6%)administered COVID-19 vaccines only;and only 7(9.7%)of the respondents had administered over 500 doses of COVID-19 vaccines.Training in vaccination was associated with the vaccination services provided(p<0.05).Respondents suggested government support through legal framework and policy review,training and empowering pharmacists in vaccine administration,and recognition of community pharmacists as PHC providers.

文摘Background:Developmental dysplasia of the hip(DDH)is a prevalent pediatric condition with a multifactorial etiology.Its incidence varies geographically,with notably higher rates observed on the Xizang plateau.This study was performed to evaluate the lipidomics signatures associated with DDH by analyzing plasma samples.Methods:Fifty infants were recruited,including 25 diagnosed with DDH and 25 age-matched healthy controls.In addition to plasma samples,comprehensive laboratory test results and medical records were collected for each participant.An untargeted lipidomics profiling approach was employed to identify distinguishing metabolic signatures.Results:Lipidomics profiles differed significantly between patients with DDH and healthy controls.Several differential metabolites were identified,including triacylglycerol(TAG)(17:0/18:1/20:1),TAG(17:0/17:0/17:0),phosphatidylethanolamine(PE)(10:0/26:4),TAG(17:0/18:0/18:0),TAG(16:0/17:0/22:1),TAG(16:0/18:0/22:0),TAG(17:0/19:0/19:0),TAG(13:0/20:0/20:0),TAG(18:0/18:0/22:0),and TAG(16:0/20:0/20:0).The primary lipid species showing differences were TAGs and PE.Conclusions:Distinct shifts in lipidomics profiles were observed in infants with DDH.To the best of our knowledge,this study is the first to explore lipidomics signatures in patients with DDH.The combined assessment of TAG(17:0/18:1/20:1)and TAG(17:0/17:0/17:0)may serve as a potential diagnostic tool for DDH.
文摘This study proposes a virtual healthcare assistant framework designed to provide support in multiple languages for efficient and accurate healthcare assistance.The system employs a transformer model to process sophisticated,multilingual user inputs and gain improved contextual understanding compared to conventional models,including long short-term memory(LSTM)models.In contrast to LSTMs,which sequence processes information and may experience challenges with long-range dependencies,transformers utilize self-attention to learn relationships among every aspect of the input in parallel.This enables them to execute more accurately in various languages and contexts,making them well-suited for applications such as translation,summarization,and conversational Comparative evaluations revealed the superiority of the transformer model(accuracy rate:85%)compared with that of the LSTM model(accuracy rate:65%).The experiments revealed several advantages of the transformer architecture over the LSTM model,such as more effective self-attention,the ability for models to work in parallel with each other,and contextual understanding for better multilingual compatibility.Additionally,our prediction model exhibited effectiveness for disease diagnosis,with accuracy of 85%or greater in identifying the relationship between symptoms and diseases among different demographics.The system provides translation support from English to other languages,with conversion to French(Bilingual Evaluation Understudy score:0.7),followed by English to Hindi(0.6).The lowest Bilingual Evaluation Understudy score was found for English to Telugu(0.39).This virtual assistant can also perform symptom analysis and disease prediction,with output given in the preferred language of the user.
文摘Background:An urban medical group in Dapeng New District was established in 2017 with the objective of enhancing outcomes for common diseases and reinforcing primary care by integrating high‐level hospitals with primary health services.This study aimed to evaluate the performance of the urban medical group using the triangular value chain framework.Methods:The evaluation was conducted using the Donabedian model,focusing on three key dimensions:safety and quality,accessibility,and affordability.Longitudinal data were collected from 2016 to 2022 through government annual reports,the medical insurance bureau,and hospital information systems.Preprogram and postprogram outcome measurements were compared to assess differences and trends,providing a clear picture of the program's effectiveness.Results:Accessibility improved significantly,with the number of hospital beds per 1000 residents increasing from 2.62 in 2017 to 3.76 in 2022.The availability of general practitioners(GPs)also rose markedly,from 0 per 10,000 residents in 2017 to 6.27 in 2022.Regarding safety and quality,the proportion of complex medical procedures conducted within the New District expanded substantially,from 7.35%in 2017 to 38.11%in 2021.Additionally,there was an enhancement in the standardized management rate of chronic diseases.Affordability assessments showed that the proportion of medical income derived from the medical insurance fund increased by nearly 22.81 percentage points between 2012 and 2021.By 2021,75.02%of medical patients were covered by medical insurance,representing an increase of approximately 44 percentage points from 31.19%in 2012.Conclusions:The implementation of the urban medical group in Dapeng New District has led to substantial improvements in healthcare accessibility,safety and quality,and affordability.Future initiatives will focus on advancing the“Dapeng Mode”to generate exemplary healthcare outcomes and minimize disparities in basic health services and health status between urban and rural populations.The reform agenda includes piloting payment reforms and innovative payment models within the Dapeng group,complemented by a health assessment and performance incentive system aimed at encouraging healthcare institutions to prioritize health management.

文摘Background:Stroke is the second leading cause of death and third leading cause of disability worldwide and is the leading cause of death and disability among adults in China,with its incidence rate continuing to rise.In China,the average age of firsttime stroke patients is 66.4 years,and the intravenous thrombolysis rate using recombinant tissue plasminogen activator within 3 h of onset is only 16%.Given this fact,there is a pressing need for real‐time predictive tools,particularly for elderly individuals at home,that can provide early warnings for potential strokes.Methods:We collected continuous monitoring data from nonintrusive smart beds and multimodal temporal data from electronic medical records at the National Center for Neurological Disorders.The data included smart bed monitoring indicators,laboratory tests,nurse observations,and static data as potential predictors,with stroke as the outcome.We applied feature representation and feature selection techniques and then input the predictors into machine learning models.Additionally,deep learning models were used after preprocessing the irregular temporal data.Finally,we evaluated the performance of the stroke prediction models and assessed the importance of the features.We used continuously updated vital signs and clinical data during hospitalization to generate timely stroke risk alerts during the same period of admission.Results:A total of 37,041 samples were analyzed,of which 7020 patients were diagnosed with stroke.When only the smart bed features were used for prediction,the model achieved an area under the receiver operating characteristic curve(AUROC)of 0.59−0.63,with an accuracy ranging from 60%−65%.Among the four artificial intelligence algorithms,the random forest model demonstrated the best performance.After all the available features were incorporated,the AUROC increased to 0.94,and the accuracy improved to 92%.Conclusions:In this study,the occurrence of stroke was successfully identified by integrating multimodal temporal data from electronic medical records.Noncontact monitoring of respiration and heart rate offers a promising approach for daily stroke surveillance in home‐based populations,particularly for elderly individuals living alone.
文摘Background:The extensor carpi radial muscle may become imbalanced compared to other muscles in the upper arm and can disrupt the overall muscle balance in the upper limb.This muscular imbalance may then cause inflammation or irritation in the tendon.Regular physical activity is rarely used as the only treatment for lowering limb edema.Heavy slow resistance(HSR)training is effective compared to regular exercises.HSR affects functional abilities and physical refinement differently from conventional workouts.This study evaluated the effects of HSR and conventional exercise(CE)on subjects aged 45–65 years with unilateral elbow pain on,in Chennai city.Methods:Patient received conservative care in the Physiotherapy Department from January 2022 to April 2022.This study compares the pre-and posttypes.Tennis Elbow Evaluation(Patient-Rated Tennis Elbow Evaluation,PRTEE)questionnaires,a patient-specific functional scale,and grip strength tests were used in the study.Results from the study's treatment HSR group and treatment CE group are contrasted.Results:After 12 weeks of therapy,investigators reassessed grip strength,PRTEE scores,and functional scale responses.In the posttest results of the Patient-Rated Tennis Elbow Evaluation survey,there is a statistically significant difference between Group HSR and Group CE(p<0.001).The posttest outcomes from the patient-specific activity scoring scheme questionnaire revealed a statistically significant disparity between Groups HSR and CE(p<0.001).The mean values for both groups exhibited a noteworthy change,with Team HSR demonstrating higher values in contrast to the control treatment.Conclusion:HSR exercise demonstrates superior effectiveness over CEs for lateral epicondylitis(LE)management,emphasizing its significance in treatment.
文摘1|Goalie Phenomenon The goalie phenomenon describes a group of health issues that may become apparent in children who prefer playing as goalkeepers.This preference might not only be about the excitement of the role but also as a coping mechanism for underlying health issues that make continuous running and high physical exertion challenging.The term highlights how certain positions in sports can sometimes obscure or reveal hidden health conditions,particularly among young athletes[1–3].To our knowledge,this is the first paper to formally introduce the term“Goalie phenomenon”in the literature exploring exerciseinduced asthma and sports role preferences in children.
文摘1|Introduction The Federal Republic of Somalia,often perceived as linguistically homogeneous,is home to a rich tapestry of dialects and minority languages that reflect its diverse cultural heritage.While Somali is the official medium of communication,it is divided into two major dialects:Maxaa Tiri(spoken by approximately 60%of the population)and Maay(spoken by approximately 20%of the population)[1].Minority languages such as Bravanese(also known as Chimwiini or Chimbalazi),Mushunguli,Benadiri Somali,and Kibajuni are spoken by smaller communities,particularly in the southern and coastal regions[1].
文摘Digital health is transforming healthcare by integrating advanced technologies to make healthcare more accessible,efficient,and personalized.From electronic health records,telemedicine,wearable devices,and artificial intelligence to the recent smart hospitals,digital health is improving patient care and outcomes while reducing healthcare costs.However,the integration of digital health faces several challenges,including data privacy,cybersecurity risks,and inequitable access to technology.This article provides an overview of the current state of digital health,key challenges in implementation,and potential solutions to maximize the benefits of digital health and ensure efficient,equitable,and patient‐centered healthcare in the future.
文摘An individual's mental health influences their capacity to think effectively,feel emotionally stable,and perform daily activities.As mental health concerns become more prevalent worldwide,new awareness and diagnostic and treatment tactics are needed.Digital tools and technology are helping solve these problems by providing scalable,tailored solutions for large populations.This detailed review examines mental health‐promoting internet tools.Smartphone applications,web‐based therapy systems,wearable tech,artificial intelligence‐powered resources,and virtual reality(VR)technologies were evaluated for efficacy and side effects.PubMed,PsycINFO,Scopus,IEEE Xplore,and Google Scholar were carefully searched.Search terms included“digital mental health tools,”“online therapy,”and“AI in mental health.”Randomized controlled trials,cohort studies,cross‐sectional studies,systematic reviews,and meta‐analyses of digital technology and mental health were included from among the literature published after 2010.Cognitive behavioral therapy methods,mood monitoring,and mindfulness exercises are among the numerous features of smartphone applications that have been demonstrated to mitigate symptoms of anxiety,depression,and tension.Online therapy platforms let marginalized individuals obtain therapy remotely.Wearable technology may detect heart rate,blood pressure,and sleep length,which may reveal mental health difficulties.Chatbots employ machine learning algorithms and natural language processing to deliver customized support and show promise for quick intervention.Exposure therapy for anxiety and trauma is increasingly using virtual reality environments.Although digital mental health therapies face challenges in relation to data privacy,limited long‐term efficacy,and technological inequality,digital technologies are modernizing mental healthcare.By offering inexpensive and effective alternatives to traditional therapies,digital technologies may help healthcare systems meet the growing demand for mental health services and overall well‐being.
