Exogenous insulin-associated autoimmunity and the emergence of double diabetes in type 2 diabetes

BACKGROUND Exogenous insulin may trigger immune-mediated complications,particularly among East Asian populations.Double diabetes,characterized by overlapping features of type 1 diabetes(T1D)and type 2 diabetes(T2D),may arise from insulin-induced autoimmunity.This study aimed to explore the association between high-risk human leukocyte antigen(HLA)class Ⅱ genotypes and susceptibility to double diabetes in patients initially diagnosed with T2D.AIM To investigate clinical and immunogenic features of patients who develop double diabetes following exogenous insulin therapy.METHODS We retrospectively analyzed five cases from Peking Union Medical College Hospital and 18 cases identified from published literature.Patients were categorized into two groups:The T2D→T1D group,characterized by autoimmune progression,and the stable T2D(T2D→T2D).Clinical characteristics and HLA class Ⅱ genotypes were compared descriptively between the two groups.RESULTS A total of 23 patients were included in the analysis.Of these,10 progressed from theT2D→T1D with autoimmune features,while 13 remained in the stable T2D→T2D group.There was no statistically significant difference in age at diagnosis between the two groups(57.10±16.11 years vs 60.31±17.41 years).In the T2D→T1D group,70%of patients carried the HLA-DRB104:05 allele and 40%carried DRB109:01,both of which are commonly associated with a high risk of T1D.In contrast,the T2D→T2D group showed greater genetic heterogeneity,with a broader distribution of HLA-DRB1 alleles,including DRB103:02(n=4),DRB109:01(n=4),and several lower frequency alleles such as DRB1*04:05,*08:03,*03:01,*04:06,*14:01,*04:01,*12:02,*15:02 and*02:01.CONCLUSION These findings suggest that patients in the T2D→T1D group exhibit a stronger autoimmune genetic predisposition,characterized by an enrichment of high-risk HLA class II alleles.In contrast,individuals with stable T2D demonstrate greater HLA diversity and lack definitive autoimmune-associated markers. 展开更多

Objective To study the expression and switching of Thl/Th2 cytokines gene in human gliomas and its effects on occurring and developing of human gliomas. Methods Interleukin(IL)-2 and interferon-3, represent Thl type cytokines. IL-4, IL-6, IL-10, and IL-13 represent Th2 type cytokines. The gene expressions of Th1/Th2 cytokines in human glioma cells, glioma infiltrating lymphocytes, and glioma cell lines were detected by reverse transcription polymerase chain reaction (RT-PCR). The biological activity of cytokines in the supematant of glioma cell lines was assayed by enzyme-linked immunosorbent assay (ELISA) method. Results The total positive rates of Th1 and Th2 type cytokines gene in human glioma cells were 14.77% and 75%. The total positive rates of Th1 and Th2 type cytokines gene in glioma infiltrating lymphocytes were 22.73% and 68.17%. There was obviously predominant expression of Th2 type cytokines in human glioma tissues, glioma infiltrating lymphocytes, and glioma cell lines. There was no unbalanced expression of Th1/Th2 cytokines in normal brain tissues. Conclusion There is a predominant expression of Th2 type cytokines in human glioma cells. The switching of Th1/Th2 cytokines gene may play an important role in the occurring and developing of human gliomas. 展开更多

Objective:Lianhuaqingwen and Shuanghuanglian are drug treatment options for Corona Virus Disease 2019(COVID-19).In China,use of traditional Chinese medicine with Shuanghuanglian or Lianhuaqingwen(for them,forsythiaside is the active antiviral and antibacterial component)in combination with azithromycin is common for the treatment of pediatric pneumonia.It is important to understand the reason why the combination of these compounds is better than a single drug treatment.This study aimed to explore the pharmacokinetic interaction between forsythiaside and azithromycin.Methods:Twelve male Sprague-Dawley rats were randomly divided into an experimental group(Forsythia suspensa extract and azithromycin)and a control group(a single dose of Forsythia suspensa extract in 5%glucose solution).Plasma samples were collected at scheduled time points,and the high-performance liquid chromatography combined with ultraviolet method was used to determine the plasma forsythiaside concentration.Non-compartmental analysis and population pharmacokinetic methods were used to investigate the forsythiaside pharmacokinetic difference between the experimental and control group.Results:Compared with a single administration,the area under the curve and half-life of forsythiaside increased,and forsythiaside clearance decreased significantly after co-administration with azithromycin.The in vivo behavior of forsythiaside could be described by the one compartment model.The forsythiaside clearance decreased when combined with azithromycin.Visual evaluation and bootstrap results suggested that the final model was precise and stable.Conclusion:Co-administration of azithromycin can significantly decrease the forsythiaside clearance and increase drug exposure.A lower dose of azithromycin can obtain sufficient forsythiaside concentration to provide antiviral and antibacterial activity. 展开更多

BACKGROUND Pituitary stalk interruption syndrome(PSIS)is a rare disorder,often characterized by delayed growth and development,short stature,and hypogonadism as the main clinical manifestations.It is not clear whether PSIS can lead to liver cirrhosis.CASE SUMMARY This paper reported a case of liver cirrhosis of unknown origin.The patient was admitted to Beijing Ditan Hospital Affiliated to Capital Medical University in November 2023.The diagnosis of PSIS complicated with liver cirrhosis was established after a series of blood tests and pituitary magnetic resonance imaging examination.CONCLUSION We also reviewed the literature from both domestic and international sources to deepen the clinical understanding of PSIS in conjunction with liver cirrhosis among medical practitioners. 展开更多

L-Carnitine has been used to treat m... Cyclophosphamide(CP) is a widely used anti-cancer agent; however, it can also induce serious male infertility. There are currently no effective drugs to alleviate this side-effect. L-Carnitine has been used to treat male infertility, but whether it can be used to protect against CP-induced male infertility is still unclear. This study aims to explore the effect and mechanism of L-carnitine in male infertility induced by CP. CP was used to establish an animal model. After three weeks of treatment, rats were sacrificed and testis and serum were harvested for further evaluation. Testosterone and estrogen levels were measured by enzyme-linked immunosorbent assay(ELISA). Testicular injury was examined by hematoxylin and eosin(H & E) staining, and germ-cell apoptosis was evaluated by terminal deoxynucleotidyl transferase dU TP nick end labeling(TUNEL). The expression of LC3 and Beclin-1 was examined by immunohistochemistry, Western blot, and real-time polymerase chain reaction(PCR), respectively. Compared with the CP group, L-carnitine significantly increases sperm motility, viability, and testosterone level(P<0.05). Western blot and real-time PCR results showed that L-carnitine treatment can significantly up-regulate the LC3-II and Beclin-1 expression in the CP+L-carnitine group when compared with the control group(P<0.05). In addition, TUNEL-positive cells were also more numerous in the CP group; however, L-carnitine can effectively retard cell apoptosis in the CP+ L-carnitine group. In conclusion, L-carnitine contributes to the inhibition of cell apoptosis and the modulation of autophagy in protecting CP-induced testicular injury. These results suggest the applicability of L-carnitine in the treatment of male infertility. 展开更多

Objective To test the hypothesis that delayed X-irradiation can enhance the functional and structural recovery of the injured spinal cord in rats. Methods Seventy Sprague-Dawley rots were randomly divided into two groups, 35 rats in each. The control group sustained a one-minute clip compression ( force of clip was 30 g) injury of the spinal cord at the T2 level, without X-irradiation. The experimental group received X-irradiation 14 days after injury. Neurological function was assessed by the modified Tarlov method, including hind limbs movement, inclined plane, and pain withdrawal. These tests were performed in a blinded fashion at 3, 7, 14, 21, 28, 35, and 42 days after injury. At 43 days after injury, histological examination of the injured spinal cord was performed following decapitation of the rats. Results Sixty-two rats met the experimental requirements (spinal cord injury was similar), 32 rats in experimental group and 30 rats in control group. Statistically significant difference was observed between the two groups in hind limbs movement and inclined plane ( P 〈0. 01 ), but not in the pain withdrawal test. The edema and necrosis areas of injured spinal cords in experimental group were less than those in control group, and axons in experimental group were significantly more than those in control group (P 〈0.01). Conclusion Delayed X-irradiation following spinal cord injury may enhance functional recovery by improving and restoring structural integrity of the injured spinal cord in rats. 展开更多
