大语言模型赋能医疗卫生智库转型路径研究

[目的/意义]人工智能时代,大语言模型正在加速融入决策咨询领域,为医疗卫生智库提升咨政启民能力提供了新的技术支撑。本文聚焦医疗卫生智库引入大语言模型的转型路径与治理条件,旨在构建赋能框架,厘清其核心挑战与改进方向。[方法/过程]本文从大语言模型对智库建设的影响角度切入,结合医疗卫生智库的需求特点,构建大语言模型赋能医疗卫生智库决策咨询的三级功能架构,剖析医疗卫生智库的转型挑战及关键问题,并以上海“浦医智捷”医疗大模型为例,展开实践路径探索。[结果/结论]医疗卫生智库在引入大语言模型过程中面临4类关键挑战:数据质量与技术适配、专家主导与模型协同张力、不可解释性引发的信任困境,以及伦理风险与规制空白。案例分析表明,数据支撑、审查监管与模型调优的组合实践,能够支撑智库沿“使用效能—结构赋能—功能重塑”的路径实现转型。据此,本文进一步提出完善数据治理、优化“专家+AI”协作机制、加大数字智库研发投入、健全伦理监管规则,以及构建嵌入式安全体系等对策建议。 展开更多

选取2、30、60、90和120日龄的雄性哈萨克羊和新疆细毛羊各6只(无120日龄的哈萨克羊),测体重后屠宰,采下丘脑、垂体、心脏、肝脏、瘤胃、网胃、瓣胃、皱胃、十二指肠、背最长肌,用RT-PCR和荧光实时定量PCR法检测ghrelin基因表达的组织分... 选取2、30、60、90和120日龄的雄性哈萨克羊和新疆细毛羊各6只(无120日龄的哈萨克羊),测体重后屠宰,采下丘脑、垂体、心脏、肝脏、瘤胃、网胃、瓣胃、皱胃、十二指肠、背最长肌,用RT-PCR和荧光实时定量PCR法检测ghrelin基因表达的组织分布,及其在皱胃中的发育性变化。研究结果表明:(1)品种内各生长时期的体重差异显著(P<0.05)。雄性哈萨克羊和新疆细毛羊的体重在2日龄时无显著差异(P>0.05),30~90日龄间,前者的体重极显著高于后者(P<0.01);(2)所检测的各组织中都有ghrelinmRNA分布,但主要在皱胃中表达,其表达量远高于其他组织(P<0.05);(3)两品种绵羊皱胃ghrelin基因表达的发育性变化模式基本相似,都随着日龄的增加而呈上升趋势,其中雄性哈萨克羊的表达量在2~60日龄间持续上升,60日龄后趋于水平;雄性新疆细毛羊的表达量在2~90日龄间持续上升,90日龄后趋于水平。研究还发现雄性哈萨克羊皱胃ghrelin基因的表达量在2~90日龄间极显著高于新疆细毛羊(P<0.01)。 展开更多

Objectives:Irinotecan(CPT-11)-induced delayed diarrhea is a major dose-limiting toxicity that restricts its clinical application.Gegen Qinlian decoction(GQD),a traditional Chinese herbal formula,has shown potential in alleviating CPT-11-induced intestinal injury,but its underlying mechanisms remain incompletely understood.This study aimed to systematically elucidate the protective effects and mechanisms of GQD against CPT-11 enterotoxicity.Methods:We integrated ultra-performance liquid chromatography-quadrupole-Orbitrap-mass spectrometry(UPLC-QE-Orbitrap-MS)for chemical profiling of GQD,along with network pharmacology,molecular docking,and molecular dynamics simulation to identify key bioactive constituents and potential targets.Furthermore,a CPT-11-treated mouse model and Caco-2 cell monolayers were employed to evaluate the effects of GQD.Quantitative proteomics and untargeted metabolomics were applied to explore pathway alterations,and biochemical,histological,and functional analyses were conducted to assess inflammatory responses,oxidative stress,barrier integrity,and drug transporter activity.Results:A total of 65 compounds were identified in GQD,with 17 prioritized as putative bioactive constituents.Network analysis highlighted the IL-6/IL-22/STAT3 inflammatory axis and ABC transporter pathways as central mechanisms.In vivo,GQD administration ameliorated diarrhea,preserved colonic architecture and mucin production,reduced pro-inflammatory cytokines and oxidative stress,and restored expression of tight-junction proteins.Multi-omics integration showed that GQD counteracted CPT-11-induced dysregulation in complement/coagulation cascades and metabolic pathways,while enriching ABC transporter-related functions.In Caco-2 monolayers,GQD promoted SN-38 efflux,upregulated MDR1 and MRP2 expression,and decreased intracellular SN-38 accumulation.Consistent with this,GQD enhanced colonic clearance of SN-38 in mice.Conclusions:GQD protects against CPT-11-induced enterotoxicity primarily through two mechanisms:suppressing the pro-inflammatory IL-6/IL-22/STAT3 signaling pathway and enhancing the epithelial efflux of SN-38 via upregulation of ABC transporters.These findings provide a mechanistic basis for the potential use of GQD as an adjuvant therapy to mitigate intestinal toxicity during irinotecan chemotherapy. 展开更多

All-perovskite tandem solar cells have the potential to surpass the theoretical efficiency limit of single junction solar cells by reducing thermalization losses.However,the challenges encompass the oxidation of Sn^(2+)to Sn^(4+)and uncontrolled crystallization kinetics in Sn-Pb perovskites,leading to nonradiative recombination and compositional heterogeneity to decrease photovoltaic efficiency and operational stability.Herein,we introduced an ionic liquid additive,1-ethyl-3-methylimidazolium iodide (EMIMI) into Sn-Pb perovskite precursor to form low-dimensional Sn-rich/pure-Sn perovskites at grain boundaries,which mitigates oxidation of Sn^(2+)to Sn^(4+)and regulates the film-forming dynamics of Sn/Pb-based perovskite films.The optimized single-junction Sn-Pb perovskite devices incorporating EMIMI achieved a high efficiency of 22.87%.Furthermore,combined with wide-bandgap perovskite sub-cells in tandem device,we demonstrate 2-terminal all-perovskite tandem solar cells with a power conversion efficiency of 28.34%,achieving improved operational stability. 展开更多

Optical monitoring of object position and alignment with nanoscale precision is critical for ultra-precision measurement applications,such as micro/nano-fabrication,weak force sensing,and micro-scopic imaging.Traditional optical nanometry methods often rely on precision nanostructure fabrication,multi-beam interferometry,or complex post-processing algorithms,which can limit their practical use.In this study,we introduced a simplified and robust quantum measurement technique with an achievable resolution of 2.2 pm and an experimental demonstration of 1 nm resolution,distinguishing it from conventional interferometry,which depended on multiple reference beams.We designed a metasurface substrate with a mode-conversion function,in which an incident Gaussian beam is converted into higher-order transverse electromagnetic mode(TEM)modes.A theoretical analysis,including calculations of the Fisher information,demonstrated that the accuracy was maintained for nanoscale displacements.In conclusion,the study findings provide a new approach for precise alignment and metrology of nanofabrication and other advanced applications. 展开更多

BACKGROUND Older people are more likely to experience pelvic fractures than younger people.Multi-slice spiral computed tomography(CT)uses three-dimensional(3D)reconstruction technology to generate 3D images that can clearly demonstrate the 3D space of fractures and detect fractures at a higher rate.AIM To investigate the clinical value of multi-slice spiral CT 3D reconstruction in the diagnosis of unstable pelvic fractures in the elderly as well as the effect of less invasive stabilization.METHODS A total of 86 patients with unstable pelvic fractures treated between March 2016 and March 2019 underwent femoral supracondylar bone traction before surgery.Pelvic radiography and multi-row spiral CT were performed successively once the patient’s vital signs and hemodynamic indices were stable.Secondary processing of the original data was performed to obtain 3D reconstruction images and determine the vertical displacement of the pelvis.After basic or complete reduction,minimally invasive internal fixation using hollow lag screws was performed.The detection rates of fracture location and classification by X-ray and CT reconstruction were compared.Patients were divided into two groups according to the presence or absence of preoperative 3D reconstruction to compare postoperative reduction,wound healing time,fracture healing time,hospitalization time,visual analog scale(VAS)score,poor internal fixation,and functional recovery.RESULTS The diagnostic coincidence rates of X-rays for pubic symphysis,ilium wing,sacroiliac periarticular,and sacral fractures were lower than those of CT reconstruction.The coincidence rate of CT reconstruction in the clinical classification of pelvic fractures was 100%,whereas 11 cases were misdiagnosed by X-ray;the total coincidence rate was 87.21%.The total excellent and good rates of postoperative reduction were significantly higher in the study group than in the control group(P<0.05).The wound healing,fracture healing,and hospitalization times were significantly shorter in the study group than in the control group(P<0.05).The VAS scores decreased in both groups postoperatively and were lower in the study group than in the control group(P<0.05).The total incidence of poor postoperative internal fixation was significantly lower in the study group than in the control group(P<0.05).The overall rate of postoperative functional recovery was significantly higher in the study group than in the control group(P<0.05).CONCLUSION Multi-slice spiral CT has high guiding significance for the diagnosis,classification,and treatment of unstable pelvic fractures in the elderly.Preoperative 3D reconstruction can effectively shorten the operation time and promote fracture healing,while minimally invasive internal fixation can effectively reduce pain and promote functional recovery of fracture sites,making it worthy of clinical application. 展开更多

Optical camouflage is a magical capability of animals as first noticed in 1794 by Erasmus Darwin in Zoonomia,but current biomimetic camouflage strategies cannot be readily applied in complex environments involving multispectral and in particular multi-polarization detection.Here we develop a plasmonic approach toward broadband infrared polarimetric crypsis,where the polarized thermal emission near the pseudo-Brewster angle is the main signal source and no existing polarizing camouflage technique has been discovered in nature.Based on all-metallic subwavelength structures,an electrodynamic resistance-reduction mechanism is proposed to avoid the significant polarization-dependent infrared absorption/radiation.It is also found that the structured metal surface presents giant extrinsic anisotropy regarding the phase shift between orthogonal polarization states,which helps to realize ultrahigh-efficiency and tunable polarization conversion in an unprecedented manner.Finally,we note that the catenary optical theory may provide a useful means to explain and predict these unusual performances. 展开更多

Metalenses have gained significant attention and have been widely utilized in optical systems for focusing and imaging,owing to their lightweight,high-integration,and exceptional-flexibility capabilities.Traditional design methods neglect the coupling effect between adjacent meta-atoms,thus harming the practical performance of meta-devices.The existing physical/data-driven optimization algorithms can solve the above problems,but bring significant time costs or require a large number of data-sets.Here,we propose a physics-data-driven method employing an“intelligent optimizer”that enables us to adaptively modify the sizes of the meta-atom according to the sizes of its surrounding ones.The implementation of such a scheme effectively mitigates the undesired impact of local lattice coupling,and the proposed network model works well on thousands of data-sets with a validation loss of 3×10^(−3).Based on the“intelligent optimizer”,a 1-cm-diameter metalens is designed within 3 hours,and the experimental results show that the 1-mm-diameter metalens has a relative focusing efficiency of 93.4%(compared to the ideal focusing efficiency)and a Strehl ratio of 0.94.Compared to previous inverse design method,our method significantly boosts designing efficiency with five orders of magnitude reduction in time.More generally,it may set a new paradigm for devising large-aperture meta-devices. 展开更多

目的探讨分泌型糖蛋白Slit2在卵清蛋白(OVA)、金黄色葡萄球菌肠毒素B(SEB)致敏小鼠特应性皮炎模型(AD模型+sponge模型)的皮肤炎性浸润、血管新生中的作用。方法1 g/L卵清蛋白(OVA)3周期贴片法重复刺激Balb/c小鼠背部皮肤构建小鼠AD模型... 目的探讨分泌型糖蛋白Slit2在卵清蛋白(OVA)、金黄色葡萄球菌肠毒素B(SEB)致敏小鼠特应性皮炎模型(AD模型+sponge模型)的皮肤炎性浸润、血管新生中的作用。方法1 g/L卵清蛋白(OVA)3周期贴片法重复刺激Balb/c小鼠背部皮肤构建小鼠AD模型,期间分别用0.5/5μg SEB处理该模型小鼠,35 d后评估皮炎程度,ELISA检测IgE水平。评估模型情况后,选皮炎程度最重的OVA+5μg SEB组给予重组Slit2蛋白处理。HE染色计数各组真皮细胞内炎症细胞数、表皮厚度,Western blot检测各组Slit2蛋白表达量,ELISA检测各组皮肤IL-13、INF-γ表达量;构建小鼠Sponge海绵模型观察各组血管新生情况,免疫组化检测各组海绵内Slit2^(+)细胞数,以及CD4^(+)T、CD8^(+)T细胞的浸润情况。结果小鼠AD模型中OVA+5μg SEB组皮炎程度最重,炎性细胞浸润最多,表皮炎性增生最厚,IgE、IL-13、IFN-γ水平明显升高,Slit2蛋白表达量最高;给予重组Slit2蛋白处理后皮炎程度、炎细胞浸润、表皮厚度均减少,IL-13、IFN-γ水平降低,但IgE水平无统计学差异。小鼠Sponge模型中,OVA+5μg SEB组血管新生、Slit2^(+)细胞数最多,CD4^(+)T、CD8^(+)T细胞浸润最多;给予Slit2蛋白处理后血管新生、Slit2^(+)细胞数明显减少,CD4^(+)T、CD8^(+)T细胞浸润减少。结论在AD病程中,Slit2蛋白有一定的抗炎作用,可减少淋巴细胞浸润,且Slit2同时具有抗血管生成和促血管生成的作用。 展开更多

Objective This study aimed to develop a nomogram to predict the overall survival(OS)of patients with acinar-predominant adenocarcinoma(APA).Methods Data from patients with APA obtained from the Surveillance,Epidemiology,and End Results(SEER)database between 2008 and 2016 were used.Significant prognostic factors were incorporated to construct a nomogram for predicting the 1-,3-,and 5-year OS in these patients.The discrimination and calibration abilities of the nomogram were assessed using a C-index and calibration curves,respectively.Results A total of 2242 patients with APA were randomly divided into a training cohort(n=1576)and validation cohort(n=666).The independent prognostic factors for OS incorporated into the nomogram included marital status,age,gender,differentiation grade,T stage,N stage,and M stage.The nomogram showed good prediction capability,as indicated by the C-index[0.713,95%confidence interval(CI):0.705–0.721 in the training cohort,and 0.662,95%CI:0.649–0.775 in the validation cohort].The calibration curves demonstrated that the 1-,3-,and 5-year OS probabilities were consistent between the observed and predicted outcome frequencies.Patients were divided into the high-risk and low-risk groups with the former showing significantly worse survival than the latter(P<0.001).Conclusion Using the SEER database,a nomogram was established to predict the 1-,3-,and 5-year OS of patients with APA and was superior to the tumor size,lymph node,and metastasis staging system in terms of evaluating long-term prognosis. 展开更多

Dispersion control is crucial in optical systems,and chromatic aberration is an important factor affecting imaging quality in imaging systems.Due to the inherent property of materials,dispersion engineering is complex and needs to trade off other aberration in traditional ways.Although metasurface offers an effective method to overcome these limits and results in well-engineered dispersion,off-axis dispersion control is still a challenging topic.In this paper,we design a single-layer metalens which is capable of focusing at three wavelengths(473 nm,532 nm,and 632 nm)with different incident angles(0°,-17°and 17°)into the same point.We also demonstrate that this metalens can provide an alternative for the bulky color synthetic prism in a 3-chips digital micromirror device(DMD)laser projection system.Through this approach,various off-axis dispersion controlling optical devices could be realized. 展开更多

Modeling influencing factors of battle damage is one of essential works in implementing military industrial logistics simulation to explore battle damage laws knowledge.However,one of key challenges in designing the simulation system could be how to reasonably determine simulation model input and build a bridge to link battle damage model and battle damage laws knowledge.In this paper,we propose a novel knowledge-oriented modeling method for influencing factors of battle damage in military industrial logistics,integrating conceptual analysis,conceptual modeling,quantitative modeling and simulation implementation.We conceptualize influencing factors of battle damage by using the principle of hierarchical decomposition,thus classifying the related battle damage knowledge logically.Then,we construct the conceptual model of influencing factors of battle damage by using Entity-Relations hip approach,thus describing their interactions reasonably.Subsequently,we extract the important influencing factors by using social network analysis,thus evaluating their importance quantitatively and further clarifying the elements of simulation.Finally,we develop an agent-based military industry logistics simulation system by taking the modeling results on influencing factors of battle damage as simulation model input,and obtain simulation model output,i.e.,new battle damage laws knowledge,thus verifying feasibility and effectiveness of the proposed method.The results show that this method can be used to support human decision-making and action. 展开更多

We propose a semantic recommendation mechanism based on sentiment analysis for microblog. Firstly, the keywords and sensibility words in this mechanism are extracted by natural language processing including segmentation, lexical analysis and strategy selection. Then, we query the background knowledge base based on linked open data (LOD) with the basic information of users. The experiment result shows that the accuracy of recommendation is within the range of 70% -89% with sentiment analysis and semantic query. Compared with traditional recommendation method, this method can satisfy users' requirement greatly. 展开更多

Sensors based on personalized healthcare systems have been widely used in the medical field.However,energy limitations have greatly hindered the further development of medical sensors.For the traditional Medium Access Control(MAC)protocol,the duration of low-power listening is fixed because it ignores that the available energy of sensors is different in some situations,which leads to a high delay and low energy utilization.In this paper,a Maximum Listening Length MAC(MLL-MAC)protocol is proposed to fully utilize the energy in the sensor-based systems.The MLL-MAC protocol is an improvement of the Receiver-Initiated(RI)MAC protocol.The main advance is that the sensor node performs the following additional operations:(1)The sender sends a beacon when it wakes up and sends data,thus establishing a communication link with the receiver in the listening state;(2)The receiver keeps listening as long as possible to reduce the delay when it wakes up and listens to the channel,which is different from the previous strategy in which the node turns into a sleep state immediately without receiving data.Furthermore,the sensor node can dynamically determine whether to send beacons and prolong listening duration according to its available energy level.The MLL-MAC protocol is evaluated through theoretical analysis and experimental results.The results show that,compared with the RI-MAC protocol,the MLL-MAC protocol can reduce the average end-to-end delay by 41.4%and improve the energy utilization by 15.1%. 展开更多

目的观察分泌型糖蛋白Slit2在咪喹莫特诱导的小鼠银屑病样皮炎模型中的表达及作用。方法采用免疫荧光观察咪喹莫特诱导的小鼠银屑病样皮损中Slit2的定位,PCR及Western blot检测皮损中Slit2的表达。30只小鼠随机分为5组:正常组、IMQ模型... 目的观察分泌型糖蛋白Slit2在咪喹莫特诱导的小鼠银屑病样皮炎模型中的表达及作用。方法采用免疫荧光观察咪喹莫特诱导的小鼠银屑病样皮损中Slit2的定位,PCR及Western blot检测皮损中Slit2的表达。30只小鼠随机分为5组:正常组、IMQ模型组、Slit2 siRNA组、Slit2蛋白低剂量组(100 ng/ml)、Slit2蛋白高剂量组(300 ng/ml),每组6只小鼠。每天对小鼠皮肤进行PASI评分,HE染色测量表皮厚度,计数各组真皮内炎症细胞数及血管密度,RT-PCR检测各组皮损处IL-23、IL-17A、IL-22 mRNA表达量,免疫组化检测各组皮损处Ki67表达及CD3 T细胞数量。结果咪喹莫特诱导的小鼠银屑病样皮炎模型皮损处Slit2的表达升高。相较于模型组,Slit2 siRNA处理后PASI评分增加,表皮变厚、表皮细胞增殖程度增加,真皮炎性细胞浸润增多及血管密度升高,IL-23、IL-17A、IL-22 mRNA水平升高,CD3 T细胞数量明显增加。给予重组Slit2蛋白处理后,PASI评分降低,表皮变薄,表皮细胞增殖程度降低,真皮炎性细胞浸润及血管密度降低,IL-23、IL-17A、IL-22 mRNA水平降低,CD3 T细胞浸润明显减少。结论Slit2在咪喹莫特诱导的小鼠银屑病样皮损中表达升高,使用Slit2 siRNA抑制皮损处Slit2表达可加重皮炎程度,在皮损处使用重组Slit2蛋白处理可减轻皮炎程度,Slit2对咪喹莫特诱导的小鼠银屑病样皮炎具一定的抗炎作用。 展开更多
