Hearing relies on the structural and functional integrity of cochlear hair cells,particularly their apical F-actin-filled stereocilia.Phospholipid scramblases are important for maintaining membrane asymmetry,but their roles in the stereocilia and auditory functions are not fully understood.Here,we identify Plscr5 as a downstream target of the transcription factor POU4F3 essential for hair cell function,whose mutation causes human DFNA15 deafness.Plscr5 knockout mice exhibit progressive hearing loss due to stereocilia degeneration and hair cell loss.Functional analyses reveal that PLSCR5 contributes to phosphatidylserine externalization in hair cell apical membranes,particularly in inner hair cells,and is important for outer hair cell and stereocilia maintenance.Our findings highlight PLSCR5 as an important downstream effector of POU4F3 and regulator of phosphatidylserine externalization and membrane dynamics required for auditory functions. 展开更多

Ni-based catalysts hold great potential in the light-driven dry reforming of methane(DRM)for syngas production due to their low cost and comparable catalytic performance to conventional noble-metal catalysts.However,the currently available Ni-based catalysts are confronted with low light-driven DRM efficiency and poor stability attributed to the coking.Herein,an atomically dispersed Ni-loaded CeO_(2)(Ni/CeO_(2))for light-drivenDRMis prepared by employing a polyol-mediated doping method to allow the high loading concentration of Ni on the CeO_(2),which overcomes the conventional atomically dispersed metal problem of low loading content.The atomically dispersed nature of the Ni can induce enormous CH4 activation sites for the reaction and photothermal effects for driving the reaction,while the CeO_(2) can facilitateCO_(2) activation.Therefore,the optimized atomically dispersed Ni-loaded CeO_(2) demonstrates an excellent light-drivenDRMperformance forH_(2)(626.5 mmol gcat^(-1) h^(-1))and CO(728.5 mmol gcat^(-1) h^(-1))production.More importantly,the optimized sample sustains its DRM performance after 100 h of continuous test,and such excellent stability of the presence of enormous Ni–O pairs can prevent the rapid conversion of CH_(x) intermediates into coke.This work demonstrates the meticulous design of non-noble metal catalysts for the lightdriven DRM with both high performance and stability. 展开更多

Noise-induced hearing loss is a worldwide public health issue that is characterized by temporary or permanent changes in hearing sensitivity.This condition is closely linked to inflammatory responses,and interventions targeting the inflammatory gene tumor necrosis factoralpha(TNFα)are known to mitigate cochlear noise damage.TNFα-induced proteins(TNFAIPs)are a family of translucent acidic proteins,and TNFAIP6 has a notable association with inflammatory responses.To date,there have been few reports on TNFAIP6 levels in the inner ear.To elucidate the precise mechanism,we generated transgenic mouse models with conditional knockout of Tnfaip6(Tnfaip6 cKO).Evaluation of hair cell morphology and function revealed no significant differences in hair cell numbers or ribbon synapses between Tnfaip6 cKO and wild-type mice.Moreover,there were no notable variations in hair cell numbers or hearing function in noisy environments.Our results indicate that Tnfaip6 does not have a substantial impact on the auditory system. 展开更多

Effects of Oral Vitamin D Supplementation on Vitamin D Levels and Glycemic Parameters in Patients with Type 2 Diabetes Mellitus:A Systematic Review and Network Meta-Analysis

Objective Epidemiological studies have shown that vitamin D status affects glycemic control in individuals with type 2 diabetes mellitus(T2DM).However,findings from intervention studies remain inconsistent.Therefore,a network meta-analysis was conducted to evaluate the comparative efficacy of various vitamin D supplementation strategies on glucose indicators in adults with T2DM.Methods Eligible studies published before September 12,2024,were retrieved from PubMed,EMBASE,Cochrane Library,and Web of Science.A network meta-analysis of multiple dosage strategies—low(<1,000 IU/day,LDS),medium(1,000–2,000 IU/day,MDS),high(2,000–4,000 IU/day,HDS),and extremely high(≥4,000 IU/day,EHDS)—was performed.Results The network meta-analysis of 40 RCTs indicated that,compared with placebo,vitamin D_(3)supplementation increased 25-hydroxyvitamin D[25-(OH)-D]levels,with pooled mean difference(MD)showing a stepwise increase from LDS to EHDS.Ranking probabilities showed a corresponding rise in 25-(OH)-D levels from LDS(46.7%)to EHDS(91.2%).EHDS reduced fasting blood glucose(FBG)relative to no treatment.LDS significantly decreased hemoglobin A1c(HbA1c),and vitamin D_(2) significantly affected FBG levels.MDS led to a significant change in fasting insulin(FIN)compared to both placebo(MD:-4.76;95%CI-8.91 to-0.61)and no treatment(MD:-7.30;95%CI-14.44 to-0.17).Conclusion The findings suggest that vitamin D supplementation may be a viable approach for improving glycemic control in adults with T2DM,with lower doses potentially offering benefit.The analysis also showed a dose-dependent increase in 25-(OH)-D levels. 展开更多

Methods: 100 stroke patients with hemiplegia admitted to our hospital from January 2023 to December 2023 were randomly divided into two groups, the control group (50 cases) was given task-oriented training assisted by nurses, and the observation group (50 cases) was given lower limb rehabilitation robot with task-oriented training. Lower limb balance, lower limb muscle strength, motor function, ankle function, knee flexion range of motion and walking ability were observed. Results: After treatment, the scores of BBS, quadriceps femoris and hamstrings in the observation group were significantly higher than those in the control group (P Conclusion: In the clinical treatment of stroke patients, the combination of task-oriented training and lower limb rehabilitation robot can effectively improve the lower limb muscle strength, facilitate the recovery of balance function, and have a significant effect on the recovery of motor function, which can improve the walking ability of stroke patients and the range of motion of knee flexion, and achieve more ideal therapeutic effectiveness. 展开更多

Therefore, developing a scientific and effective rehabilitation training program is of great significance. This study first analyzes the theoretical basis of cardiopulmonary rehabilitation training, including the effects of aerobic exercise, interval training, and strength training on cardiopulmonary function. Based on this, a comprehensive rehabilitation training program is designed, which includes personalized training plans, comprehensive interventions, multidisciplinary collaboration, patient education, and regular follow-up visits. The cardiopulmonary rehabilitation training plan developed in this study has certain scientific practicability, which provides a theoretical basis for cardiopulmonary rehabilitation training, and also provides a reference for medical institutions, rehabilitation centers and communities, which is helpful for promotion and application to a wider range of patients with cardiopulmonary diseases. 展开更多

Contralateral needling at unblocked collaterals for hemiplegia following acute ischemic stroke 被引量:12

Hemiplegia caused by stroke indicates dysfunction of the network between the brain and limbs, namely collateral shock in the brain. Contralateral needling is the insertion of needles into acupoints on the relative healthy side of the body to treat diseases such as apoplexy. However, there is little well-designed and controlled clinical evidence for this practice. This study investigated whether contralateral needling could treat hemiplegia after acute ischemic stroke in 106 randomly selected patients with acute ischemic stroke. These patients were randomly assigned to three groups: 45 in the contralateral needling group, receiving acupuncture on the unaffected limbs; 45 in the conven- tional acupuncture group, receiving acupuncture on the hemiplegic limbs; and 16 in the control group, receiving routine treatments without acupuncture. Acupuncture at acupoints Chize (LU5) in the upper limb and Jianliao (TEl4) in the lower limb was performed for 45 minutes daily for 30 consecutive days. The therapeutic effective rate, Neurological Deficit Score, Modified Barthel Index and FugI-Meyer Assessment were evaluated. The therapeutic effective rate of contralateral nee- dling was higher than that of conventional acupuncture (46.67% vs. 31.11%, P 〈 0.05). The neuro- logical deficit score of contralateral needling was significantly decreased compared with conven- tional acupuncture (P 〈 0.01). The Modified Barthel Index and FugI-Meyer Assessment score of contralateral needling increased more significantly than those of conventional acupuncture (both P 〈 0.01). The present findings suggest that contralateral needling unblocks collaterals and might be more effective than conventional acupuncture in the treatment of hemiplegia following acute ischemic stroke. 展开更多

With the development of Unmanned Aerial Vehicle(UAV) system autonomy, network communication technology and group intelligence theory, mission execution in the form of a UAV swarm will be an important realization of future applications. Traditional single-UAV mission reliability modeling methods have been unable to meet the requirements of UAV swarm mission reliability modeling. Therefore, the UAV swarm mission reliability modeling and evaluation method is proposed. First, aimed at the interdependence among the multiple layers, a multi-layer network model of a UAV swarm is established. At the same time, based on the system having the following characteristics—using a mission chain to complete the mission and applying the connectivity of the mission network—the mission network model of a UAV swarm is established. Second, vulnerability and connectivity are selected as two indicators to reflect the reliability of the mission, and aimed at random attack and deliberate attack, vulnerability and connectivity evaluation methods are proposed. Finally, the validity and accuracy of the constructed model are verified through simulations,and the model and selected indicators can meet the reliability requirements of the UAV swarm mission. In this way, this study provides quantitative reference for UAV-swarm-related decisionmaking work and supports the development of UAV-swarm-related work. 展开更多

OBJECTIVE: To discuss the mechanism of Yiqiyang- yin recipe in rats with adriamycin (ADR)-induced ne- phropathyo METHODS: We randomly divided 30 Sprague-Daw- ley rats into 5 groups: control, model, glucocorti- coid, Chinese herb, and Chinese herb plus glucocor- ticoid groups. The 24-h urine volume was collected on days 7, 14, 21, and 28 after ADR injection, and all rats were killed on day 28. We measured the 24-h levels of urinary protein, serum cholesterol, and se- rum triglycerides, and renal function of all rats by using routine biochemical methods. Pathological changes in the rat kidneys were observed under light and electron microscopes. Heparanase (HPA) mRNA expression levels were measured using re-al-time fluorescence-quantitative polymerase chain reaction. Urine levels of HPA in all groups were measured using enzyme-linked immunosor- bent assay. The expression of nephrin was detected by immunohistochemical staining and quantitative- ly analyzed using Motic image analysis 3.2 software. RESULTS: The levels of urinary protein and serum triglycerides and cholesterol in rats with ADR-in- duced nephropathy increased on day 14. The se- rum albumin levels simultaneously decreased. All the changes reached the peak on day 28. Examina- tion under the light microscope showed inflamma- tory cells and slight fibroplasia in the renal intersti- tium in the model group, but fewer inflammatory cells were observed in the intervention groups than those in the model group. Examination under the electron microscope showed extensive fusion of foot processes in ADR rats. HPA mRNA expres- sion was higher in the model group than that in the control group. The HPA mRNA levels in the in- tervention groups, especially in the Chinese herb group, and Chinese herb plus glucocorticoid group were significantly lower than the level in the model group. The HPA expression levels correlated signifi- cantly with the proteinuria level. No significant dif- ference was found in the HPA level in urine be- tween the intervention groups and the model group, whereas the model group had a higher uri- nary HPA level than the control group. Nephrin mRNA expression levels in the model group were higher than those in the control group. Nephrin mRNA expression levels were significantly lower in the intervention groups than that in the model group, especially the Chinese herb plus glucocorti- cold group. Compared with the control group, themodel group showed increased nephrin expression in the kidney. Nephrin levels in the other groups, es- pecially in the Chinese herb plus glucocorticoid group, were significantly lower than that in the model group. The nephrin levels in the kidney were negatively correlated with the proteinuria level. CONCLUSION: Yiqiyangyin recipe could attenuate foot process injury particularly in combination with a steroid reduce the development of proteinuria possibly by inhibition of HPA in the kidney, and reg- ulate the expression of nephrin in rats with ADR-in- duced nephropathy. Our study showed that treat- ment with Yiqiyangyin recipe plus glucocorticoid was better than a singular intervention, and we ex- plored the pharmacological mechanism of this combination by biochemical and molecular biologi- cal analysis to provide a basis for clinical applica- tion. 展开更多

Several clinical studies have reported that hearing loss is correlated with autism in children.However,little is known about the underlying mechanism between hearing loss and autism.p21-activated kinases(PAKs)are a family of serine/threonine kinases that can be activated by multiple signaling molecules,particularly the Rho family of small GTPases.Previous studies have shown that Pak1 mutations are associated with autism.In the present study,we take advantage of Pak1 knockout(Pak1a/a)mice to investigate the role of PAK1 in hearing function.We find that PAK1 is highly expressed in the postnatal mouse cochlea and that PAK1 deficiency leads to hair cell(HC)apoptosis and severe hearing loss.Further investigation indicates that PAK1 deficiency downregulates the phosphorylation of cofilin and ezrin-radixin-moesin and the expression of b II-spectrin,which further decreases the HC synapse density in the basal turn of cochlea and disorganized the HC stereocilia in all three turns of cochlea in Pak1à/àmice.Overall,our work demonstrates that the autism-related gene Pak1 plays a crucial role in hearing function.As the first candidate gene linking autism and hearing loss,Pak1 may serve as a potential target for the clinical diagnosis of autism-related hearing loss. 展开更多

Objective: To observe the clinical efficacy of surrounding electroacupuncture needling for breast mass after acute mastitis.Methods: Thirty patients with breast mass after acute mastitis who conformed to the inclusion criteria were given surrounding electroacupuncture needling. Acupoint selection: 4-8 needles were needled around the mass and at Zusanli(足三里ST 36),Sanyinjiao(三阴交SP 6),Fengl6 ng(丰隆 ST 40) and Rugen(乳根 ST 18). Electroacupuncture was performed at ST 36, SP 6 and ST 40. The above treatment was once a day. Treatment for 7 times was considered as 1 course of treatment, and 3 weeks were free from treatment between courses. Color ultrasound in breast was used to examine the breast mass before the next course of treatment, treatment can be terminated if the mass disappeared, otherwise, treatment should be continued. Three courses at most were needed.Results: After 1-3 courses of treatment, 16 patients were cured, accounting for 53.3%(16/30); markedly effective: 8 cases, accounting for 26.7%(8/30); effective: 4 cases, accounting for 13.3%(4/30); ineffective:2 cases, accounting for 6.7%(2/30).Conclusion: Surrounding electroacupuncture needling for breast mass after acute mastitis was effective clinically. 展开更多

Idiopathic chronic ulcerative enteritis (ICUE) is a distinct entity without a defined etiology and is rarely seen in the clinic. Patients with ICUE mainly present with insidious abdominal symptoms such as chronic abdominal pain and intermittent gastrointestinal hemorrhage and symptoms of malnourishment in the early stages of the disease. ICUE is always difficult to diagnose. However, as the disease progresses, patients have a variety of acute abdominal complications such as hemorrhage, perforation, or ileus. Surgical intervention is always needed, and the condition can recur and require repeat laparotomy. When diffuse ulceration of the small bowel is present in the absence of recognizable causes, it is classified as nonspecific or idiopathic. The histological examination always demonstrates an acute, chronic inflammatory infiltration without giant cells, granulomas, or villous atrophy. The etiology of ICUE has not been identified, and its pathogenesis is poorly understood; therefore, radical surgical resection is considered the best available treatment. Here, we report a case of ICUE characterized by nonspecific, multiple, small intestinal ulcers resulting in perforation and recurrent bleeding. A series of cellulose 3,5-dimethylphenylcarbamates (CDMPCs) with different degrees of substitution (DS) and degrees of polymerization (DP) were homogeneously synthesized in 1-allyl-3-methylimidazolium chloride (AmimC1). Then, the CDMPCs were coated on silica gel and used as chiral stationary phases (CSPs), and their chiral recognition abilities for seven racemates were evaluated by high performance liquid chromatography. The results showed that DS and DP of CDMPCs had a great influence on chiral recognition abilities of the CSPs. The CSPs with the DS ~ 1 gives a low chiral recognition to most racemates. On the contrast, the CSPs with the DS _〉 2 exhibited high chiral separation abilities. For example, six racemates could be separated on the CSP with CDMPC of DS ~ 2 (CSP-2). Especially, for the enantioseparation of 1-(2-naphthyl) ethanol and Tr6ger's base, CSP-2 gave the highest separation ability in all of CSPs. On the other hand, when the DP of cellulose was in a range from 39 to 220, the chiral separation abilities of CDMPCs increased as the DP increased. This work demonstrates that the structure of cellulose esters such as DS and DP has important effect on their chiral separation ability, and therefore provides a practical method to design and prepare desirable CSPs for different racemates. 展开更多

BACKGROUND: This study aimed to investigate the epidemiological and clinical characteristics of patients with heat-related illness, and guide the prevention, diagnosis and treatment of heat-related illness.METHODS: From June 2013 to August 2013, seventy patients with heat-related illness were treated at Jinshan Hospital of Fudan University, and their epidemiological characteristics, laboratory results, treatment and prognosis were retrospectively analyzed.RESULTS: In the 70 patients, 18 patients suffered from heat stroke and 52 patients from nonheat stroke. When the environmnent temperature was above 35 °C, the body temperature of the patients began to increase markedly. The patients with heat stroke were significantly older than those with non-heat stroke(P<0.05). The body temperature, heart rate, blood glucose, blood lactate dehydrogenase and blood creatine kinase in the patients with heat stroke were higher than those in the patients with non-heat stroke(P<0.05). Blood lactate dehydrogenase and blood creatine kinase were positively correlated with body temperature(r=0.801).CONCLUSION: When the environmental temperature goes above 35 °C, heat-related illness should be prevented, especially in the elderly. The body temperature, heart rate, blood glucose, blood lactate dehydrogenase and blood creatine kinase in patients with heat stroke are higher than those in patients with non-heat stroke. Blood lactate dehydrogenase and blood creatine kinase are positively correlated with body temperature, but their relationship with heat-related illness awaits further study. 展开更多

An electrochemical method based on a directly electrochemically reduced graphene oxide (ERGO) film coated on a glassy carbon electrode (GCE) was developed for the rapid and convenient determination of rutin in plasma. ERGO was modified on the surface of GCE by one-step electro-deposition method. Electrochemical behavior of rutin on ERGO/GCE indicated that rutin underwent a surface-controlled quasi-reversible process and the electrochemical parameters such as charge transfer coefficient (α), electron transfer number (n) and electrode reaction standard rate constant (ks) were 0.53, 2 and 3.4 s -1, respectively. The electrochemical sensor for rutin in plasma provided a wide linear response range of 4.70 × 10 ^-7 1.25 × 10^-5 M with the detection limit (s/n=3) of 1.84 × 10^-8 M. The assay was success- fully used to the pharmacokinetic study of rutin. The pharmacokinetic parameters such as elimination rate half-life (t1/2), area under curve (AUC), and plasma clearance (CL) were calculated to be 3.345 ± 0.647 rain, 5750 ±656.0 μg min/mL, and 5.891± 0.458 mL/min/kg, respectively. The proposed method utilized a small sample volume of 10 μL and had no complicated sample pretreatment (without deproteinization), which was simple, eco-friendly, and time- and cost-efficient for rutin pharmacokinetic studies. 展开更多
