腹腔镜联合经内镜逆行性胰胆管造影术治疗胆总管结石的疗效观察 被引量:14

目的:探讨腹腔镜联合经内镜逆行性胰胆管造影术治疗胆总管结石的临床疗效及安全性。方法:选择2016年1月到2019年1月我院收治的胆总管结石患者350例为研究对象,根据手术方法将其分为观察组(n=176)和对照组(n=174)。对照组给予腹腔镜胆囊... 目的:探讨腹腔镜联合经内镜逆行性胰胆管造影术治疗胆总管结石的临床疗效及安全性。方法:选择2016年1月到2019年1月我院收治的胆总管结石患者350例为研究对象,根据手术方法将其分为观察组(n=176)和对照组(n=174)。对照组给予腹腔镜胆囊切除、胆总管切开取石术,观察组给予腹腔镜联合经内镜逆行性胰胆管造影术治疗。比较两组患者的手术情况、治疗前后总胆汁酸(TBA)、总胆红素(TBil)、直接胆红素(DBil)、间接胆红素(IBil)、谷草转氨酶(AST)、谷丙转氨酶(ALT)、γ-谷氨酰转肽酶(γ-GT)水平的变化、胃肠功能恢复情况及并发症的发生情况。结果:观察组手术时间明显长于对照组(P<0.05),术中出血量、术后下床时间及住院时间均显著低于对照组(P<0.05)。两组治疗后血清TBA、TBil、DBil、IBil、AST、ALT及γ-GT水平均较治疗前显著改善,且观察组血清以上指标水平均显著低于对照组(P<0.05)。观察组肠鸣音恢复时间、排气时间及排便时间均明显短于对照组(P<0.05)。治疗期间,两组并发症总发生率为1.70%、13.22%,观察组显著低于对照组(P<0.05)。结论:腹腔镜联合经内镜逆行性胰胆管造影术治疗胆总管结石患者的临床效果显著优于腹腔镜胆囊切除、胆总管切开取石术,其可有效缩短患者住院时间,改善其肝功能,且安全性较高。 展开更多

A novel complex [Pb(qina)2(DMSO)]·H2O was synthesized, of which qinais the quinaldic acid radical and DMSO the dimethyl sulfoxide. Elemental analysis, IR spectra, and X-ray single-crystal diffraction were carried out to determine the composition and crystal structure. This complex belongs to monoclinic system, space group P121/n1, with a = 7.1553(17), b = 17.543(2), c = 17.306(2) A, β = 91.671(4)°, C22H20N206PbS, Mr = 647.65, Z = 4, V= 2171.5(6) A3, Dc= 1.981 g/cm^3, μ= 7.908 mm^-1, F(000) = 1248, -9≤h≤6, -23≤k≤19, -22≤1≤22, R= 0.0221 and wR= 0.0488. Weak coordinate bond is formed between oxygen atom in DMSO and Pb( Ⅱ ). There are also weak interactions, such as π-π interaction, hydrogen bonds and so on, among the complex molecules. 展开更多

A novel binucleus complex [Cd2(phen)4(adip)(NO3)2] (phen = phenanthroline, H2adip = adipic acid) has been synthesized by the reaction of phen and H2adip with cadmium(Ⅱ) salt. Elemental analysis, IR spectra and X-ray crystal structure analysis were carried out to determine the crystal structure of the title complex. The crystal belongs to triclinic, space group P1^- with a = 9.638(7), b = 10.315(7), c = 13.109(10) A, α = 88.305(11), β = 69.011(11), γ = 75.818(11)°, C27H20N5O5Cd, Mr = 606.88, Z = 2, V = 1177.2(14)A^3, Dc = 1.712 g/cm^3,μ= 0.979 mm^-1, -8≤h ≤11, -12≤k≤ 12, -10≤l≤ 15, F(000) = 610, Rint = 0.0314, R = 0.0739 and wR = 0.1922 (Ⅰ〉 2σ(Ⅰ). The cadmium atom is seven-coordinated in a distorted pentagonal bipyramidal configuration. Two new complexes [Pd(L-phe)2·2H2O] (1) and [Pd(4,4'-dcbpy)(en)·H2O] (2) (L-phe = L-phenylalanine, 4,4'-dcbpy = 2,2'-bipyridyl-4,4'-dicarboxylate, en = ethylenediamine) were synthesized and characterized. L-phenylalanine was in situ formed via the hydrolysis of N-(tert-butoxycarbonyl)-L-phenylalanine, and ethylenediamine came from the in situ reaction of piperazine (pip) under hydrothermal conditions. The single-crystal X-ray structure analyses reveal that the Pd(Ⅱ) ion constructs a square planer coordination geometry in complexes 1 and 2, respectively. Complex 1 extends into a 2D layered structure via H-bond interactions and complex 2 connects into a 3D network through hydrogen bonds and 7r-tr stacking interactions. The interactions of 1 and 2 with fish sperm DNA (FS-DNA) were examined via UV-Vis absorption spectra and fluorescence spectroscopy. The cleavage reaction for plasmid DNA has been assayed by agarose gel electrophoresis. The results indicate that two complexes interact with DNA and the insertion is the main binding mode. 展开更多
