Therapeutic endoscopic retrograde cholan-giopancreatography using balloon enteroscopy has been shown to be feasible and effective in patients with such strictures. However, when a benign anas-tomotic stricture is severe, a dilation catheter cannot pass through the stricture despite successful insertion of the guidewire. We report on the usefulness of the Soehendra Stent Retriever over a guidewire for dilating a severe bilioenteric or pancreatoenteric anastomotic stricture under short double-balloon enteroscopy, in two patients with surgically altered anatomies.展开更多
An 85-year-old man with HCV infection and diabetes mellitus was diagnosed as having hepatocellular carcinoma (HCC, 13 cm in diameter) based on high serum alpha-fetoprotein (AFP), AFP-L3, and des-y-carboxy prothrom...An 85-year-old man with HCV infection and diabetes mellitus was diagnosed as having hepatocellular carcinoma (HCC, 13 cm in diameter) based on high serum alpha-fetoprotein (AFP), AFP-L3, and des-y-carboxy prothrombin levels as well as typical enhancement pattern on contrast-enhanced CT. The patient did not receive any interventional treatments because of advanced age and the advanced stage of HCC. He chose to take vitamin K, which was reported to suppress the growth of HCC in vitro. Three months after starting vitamin K, all three tumor markers were normalized and HCC was markedly regressed, showing no enhancement in the early arterial phase on CT. Here we present the report describing the regression of HCC during the administration of vitamin K.展开更多
Carbonate-electrolyte-based lithium–sulfur(Li–S)batteries with solid-phase conversion offer promising safety and scalability,but their reversible capacities are limited.In addition,large-format pouch cells are pavin...Carbonate-electrolyte-based lithium–sulfur(Li–S)batteries with solid-phase conversion offer promising safety and scalability,but their reversible capacities are limited.In addition,large-format pouch cells are paving the way for large-scale production.This study demonstrates the in situ formation of a solid-electrolyte interphase(SEI)as a protective layer using vinylene carbonate(VC),highlighting its industrial adaptability.A high reversible capacity is achieved by the lithiated poly-VC SEI formed inside the cathode particles as a nanoscale ionic conduction path,along with the traditional surface protective layer.Furthermore,the severe dissolution of poly-VC is mitigated by LiF derived from fluorine ethylene carbonate as a co-solvent,enabling high rate performance and a long cycle life.A large 8 Ah pouch cell is successfully developed,which shows a high energy density of 400 Wh kg^(−1) based on the cell weight.This work demonstrates the high performance of large-scale Li–S batteries with the in situ formation of a protective layer as a scalable technique for future applications.展开更多

She underwent left upper lobectomy with mediastinal lymph node dissection and concomitant r...We report a patient with a large pulmonary pleomorphic carcinoma (PPC) in the left upper lobe greater than 10 cm in diameter. She underwent left upper lobectomy with mediastinal lymph node dissection and concomitant resection of the left phrenic nerve, vagus nerve and pericardium. She received adjuvant chemotherapy, but had tumor recurrence. We then performed left completion pneumonectomy, but could not remove the tumor completely because of rapid tumor invasion to the left atrium. Urgent surgery should be considered for recurrent resectable PPC.展开更多
