This paper was aimed to compare the clinical effectiveness and safety of adult male circumcision using the Shang RingTM (SR) with the no-flip technique compared with Dorsal Slit (DS) surgical method, A single-cent...This paper was aimed to compare the clinical effectiveness and safety of adult male circumcision using the Shang RingTM (SR) with the no-flip technique compared with Dorsal Slit (DS) surgical method, A single-centered, prospective study was conducted at the West China Hospital, where patients were circumcised using the no-flip SR (n = 408) or the DS (n = 94) procedure. The adverse events (AEs) and satisfaction were recorded for both groups, and ring-removal time and percentage of delayed removals were recorded for the SR group. Finally, complete follow-up data were collected for 76.1% of patients (SR: n = 306; DS: n = 76). The average ring-removal time for the SR group was 17.62 ± 6.30 days. The operation time (P 〈 0.001), pain scores during the procedure (P 〈 0.001) and at 24 h postoperatively (P 〈 0.001), bleeding (P = 0.001), infection (P = 0.034), and satisfaction with penile appearance (P 〈 0.001) in the SR group were superior to those in the DS group. After two postoperative weeks, the percentage of patients with edema in the SR group (P = 0.029) was higher but no differences were found at 4 weeks (P = 0, 185) between the two groups. In conclusions, the no-flip SR method was found to be superior to the DS method for its short operation time (〈5 min), involving less pain, bleeding, infection, and resulting in a satisfactory appearance. However, the time for recovery from edema took longer, and patients may wear device for 2-3 weeks after the procedure.展开更多

Emission lines of shallow oxygen donors and(VⅢ complex)1- as well as VN3+ and neutral Mg acceptors are observed, which indicate that self-compensation is occurred in Mg-doped AlGaN at highly doping levels. The fitting of the temperature-dependent Hall effect data shows that the acceptor activation energy values in Mgdoped AlxGa1-xN(x = 0.23, 0.35) are 172 meV and 242 meV, and the hole concentrations at room temperature are 1.2×1018 cm-3 and 3.3× 1017 cm-3, respectively. Therefore, it is believed that there exists the combined effect of the Coulomb potentials of the dopants and screening of the Coulomb potentials by a high hole concentration. Moreover, due to the high ionized acceptors’ concentration and compensation ratio, the impurity conduction becomes more prominent and the valence band mobility drops sharply at low temperature.展开更多

The destruction of concrete building materials in severely cold regions of the north is more severely affected by freeze-thaw cycles,and the relationship between the mechanical properties and pore structure of concret...The destruction of concrete building materials in severely cold regions of the north is more severely affected by freeze-thaw cycles,and the relationship between the mechanical properties and pore structure of concrete with fine aggregate from municipal solid waste(MSW)incineration bottom ash after freeze-thaw cycles is analyzed under the degree of freeze-thaw hazard variation.In this paper,the gray correlation method is used to calculate the correlation between the relative dynamic elastic modulus,compressive strength,and microscopic porosity parameters to speculate on the most important factors affecting their changes.The GM(1,1)model was established based on the compressive strength of the waste incineration ash aggregate concrete,the relative error between the simulated and actual values in the model was less than 5%,and the accuracy of the model was level 1,indicating that the GM(1,1)model can well reflect the change in the compressive strength of the MSW incineration bottom ash aggregate concrete during freeze-thaw cycles.Using the gray correlation method,the correlation between the relative dynamic elastic modulus,compressive strength,air content,specific surface area,pore spacing coefficient,and pore average chord length was calculated,and the pore spacing coefficient and pore average chord length were determined to be highly correlated with each other.This determination can help analyze and infer the deterioration mechanism of concrete subject to freeze-thaw cycles.These results can provide a theoretical basis for guiding the engineering practice of concrete with fine aggregates of household bottom ash in the northern cold region.展开更多
In recent years,advancements in single-cell and spatial transcriptomics,which are highly regarded developments in the current era,particularly the emerging integration of single-cell and spatiotemporal transcriptomics...In recent years,advancements in single-cell and spatial transcriptomics,which are highly regarded developments in the current era,particularly the emerging integration of single-cell and spatiotemporal transcriptomics,have enabled a detailed molecular comprehension of the complex regulation of cell fate.The insights obtained from these methodologies are anticipated to significantly contribute to the development of personalized medicine.Currently,single-cell technology is less frequently utilized for prostate cancer compared with other types of tumors.Start-ing from the perspective of RNA sequencing technology,this review outlined the signifcance of single-cell RNA sequencing(scRNA-seq)in prostate cancer research,encompassing preclinical medicine and clinical applications.We summarize the differences between mouse and human prostate cancer as revealed by scRNA-seq studies,as well as a combination of multi-omics methods involving scRNA-seq to highlight the key molecular targets for the diagnosis,treatment,and drug resistance characteristics of prostate cancer.These studies are expected to provide novel insights for the development of immunotherapy and other innovative treatment strategies for castration-resistant prostate cancer.Furthermore,we explore the potential clinical applications stemming from other single-cell technologies in this review,paving the way for future research in precision medicine.展开更多
BACKGROUND In randomized controlled trials(RCTs),the placebo arm has often been ignored as the attention tends to be focused on the treatment arm.We undertook a meta-analysis based on the data from the placebo arm in ...BACKGROUND In randomized controlled trials(RCTs),the placebo arm has often been ignored as the attention tends to be focused on the treatment arm.We undertook a meta-analysis based on the data from the placebo arm in RCTs of hepatocellular carci-noma(HCC),the response rates and survival status,and adverse events(AEs)were summarized and evaluated.AIM To systematically evaluate the response rates,survival status and AEs in the placebo arms of RCTs for HCC.METHODS A systematic search was performed on PubMed,Ovid MEDLINE,Embase and Cochrane Library to identify relevant trials evaluating the efficacy of drugs for the treatment of HCC,published until December 31,2023.Statistical analysis was performed using R statistical software(version 4.3.2).RESULTS A total of 18 RCTs,involving 2390 patients,met the criteria for inclusion in the meta-analysis.The pooled overall disease control rate and objective response rate in the placebo group were 38%[95%confidence interval(CI):33%-42%]and 1%(95%CI:1%-2%),respectively.Overall survival and progression-free survival in the placebo group were 7.9 months(95%CI:7.6-8.31 months)and 1.9 months(95%CI:1.6-2.1 months),respectively.The incidence of grade 3 or 4 AEs was 37%(95%CI:30%-43%).Additionally,the incidence of interruptions or dose reductions due to AEs was 20%(95%CI:13%-27%),while the incidence of treatment discontinuation due to AEs was 9%(95%CI:6%-12%).CONCLUSION Over one-third of advanced HCC patients exhibit therapy-free disease control,with placebo-arm AEs observed.These findings guide single-arm trials design and enhance patient acceptance of anticancer therapies.展开更多

Objective:To investigate current status of diagnosis and treatment of bladder cancer in China.Methods:A database was generated by Chinese Bladder Cancer Consortium(CBCC).From January 2007 to December 2012,14,260 cases...Objective:To investigate current status of diagnosis and treatment of bladder cancer in China.Methods:A database was generated by Chinese Bladder Cancer Consortium(CBCC).From January 2007 to December 2012,14,260 cases from 44 CBCC centers were included.Data of diagnosis,treatment and pathology were collected.Results:The average age was 63.5 year-old and most patients were male(84.3%).The most common histologic types were urothelial carcinoma(91.4%),adenocarcinoma(1.8%),and squamous carcinoma(1.9%).According to 1973 and 2004 WHO grading system,42.0%,41.0%,and 17.0% of patients were grade 1,2,and 3,and 16.0%,48.7%,and 35.3% of patients were papillary urothelial neoplasms of low malignant potential,low,and high grade,respectively.Non-muscle invasive bladder cancer(NMIBC)and muscle invasive bladder cancer(MIBC)were 25.2% and 74.1%,respectively(0.8% not clear).Carcinoma in situ was only 2.4%.Most patients were diagnosed by white-light cystoscopy with biopsy(74.3%).Fluorescence and narrow band imaging cystoscopy had additional detection rate of 1.0% and 4.0%,respectively.Diagnostic transurethral resection(TUR)provided detection rate of 16.9%.Most NMIBCs were treated with TUR(89.2%).After initial TUR,2.6%accepted second TUR,and 45.7%,69.9%,and 58.7% accepted immediate,induced,and maintenance chemotherapy instillation,respectively.Most MIBCs were treated with radical cystectomy(RC,59.7%).Laparoscopic RCs were 35.1%,while open RC 63.4%.Extended and standard pelvic lymph node dissection were 7% and 66%,respectively.Three most common urinary diversions were orthotopic neobladder(44%),ileal conduit(31%),and ureterocutaneostomy(23%).Only 2.3% of patients accepted neo-adjuvant chemotherapy and only 18%of T3 and T4 patients accepted adjuvant chemotherapy.Conclusion:Disease characteristics are similar to international reports,while differences of diagnosis and treatment exist.This study can provide evidences for revisions of the guideline on bladder cancer in China.展开更多
Klippel-Trénaunay syndrome (KTS)is arare angio-osteo-hypertrophic syndrome characterized by vascular malformations,soft tissue and/or bone hypertrophy, and varicose veins.For the purpose of describing the imaging...Klippel-Trénaunay syndrome (KTS)is arare angio-osteo-hypertrophic syndrome characterized by vascular malformations,soft tissue and/or bone hypertrophy, and varicose veins.For the purpose of describing the imaging findings and elucidating the role of medical imaging in the diagnosis and assessment of patient with KTS,we have reviewed the imaging data of 14 KTS patients.The imaging features on different imaging modalities were analyzed.Unilateral lower limb involvement was evident in 71% of cases (n=10)and bilateral but asymmetric lower limb involvement in the remaining 29% of cases (n=4).The most commonly depicted imaging features were varicosities in 93% (n=13),muscle hypertrophy in 79%(n=11)and venous anomalies in 64%(n=9).Other less common imaging findings included lymphedema in 29%(n=4),arterial malformations 29%(n=4),soft tissue hemangiomas 21%(n=3),pelvic and thigh phleboliths 21%(n=3), venous aneurysrns 21%(n=3),bone abnormalities 14%(n=2)and lymphadenopathy 14%(n=2).A severe unilateral lower limb deformity resulting in contractures and muscle atrophy of the whole limb was depicted in 1 case.The pathognomonic marginal vein of Servelle was identified in 2 cases.AV shunt was highly suspected in 4 cases and was confirmed by DSA in 1 case,making Klippel-Trénaunay-Weber syndrome a more apt diagnosis.Associated ipsilateral duplicated renal artery:was found in 1 case.We have concluded that medical imaging is the cornerstone in the.diagnosis and assessment of severity and complications,follow-up and differentiation of KTS from other similar conditions.Different imaging modalities play complementary roles in the evaluation of KTS patients.展开更多

Scanning electron microscope examination of the stone showed a compact crystal image externally and sparse spherical crystal structure in kernel. Composition of the stone was calcium fluorophosphate on X-ray diffractometer. The follow-up time was 15 months with no recurrence of cyst or stone. To our knowledge, this case is the first to describe laparoscopic removal of a stone within a seminal vesicle cyst, and the first to describe calcium fluorophosphate as the composition of seminal vesicle stones. Non-muscle invasive bladder cancer(NMIBC)is a major type of bladder cancer with a high incidence worldwide,resulting in a great disease burden.Treatment and surveillance are the most important part of NIMBC management...Non-muscle invasive bladder cancer(NMIBC)is a major type of bladder cancer with a high incidence worldwide,resulting in a great disease burden.Treatment and surveillance are the most important part of NIMBC management.In 2018,we issued“Treatment and surveillance for non-muscle-invasive bladder cancer in China:an evidencebased clinical practice guideline”.Since then,various studies on the treatment and surveillance of NMIBC have been published.There is a need to incorporate these materials and also to take into account the relatively limited medical resources in primary medical institutions in China.Developing a version of guideline which takes these two issues into account to promote the management of NMIBC is therefore indicated.We formed a working group of clinical experts and methodologists.Through questionnaire investigation of clinicians including primary medical institutions,24 clinically concerned issues,involving transurethral resection of bladder tumor(TURBT),intravesical chemotherapy and intravesical immunotherapy of NMIBC,and follow-up and surveillance of the NMIBC patients,were determined for this guideline.Researches and recommendations on the management of NMIBC in databases,guideline development professional societies and monographs were referred to,and the European Association of Urology was used to assess the certainty of generated recommendations.Finally,we issued 29 statements,among which 22 were strong recommendations,and 7 were weak recommendations.These recommendations cover the topics of TURBT,postoperative chemotherapy after TURBT,Bacillus Calmette–Guérin(BCG)immunotherapy after TURBT,combination treatment of BCG and chemotherapy after TURBT,treatment of carcinoma in situ,radical cystectomy,treatment of NMIBC recurrence,and follow-up and surveillance.We hope these recommendations can help promote the treatment and surveillance of NMIBC in China,especially for the primary medical institutions.展开更多
Objective:To investigate oncological outcomes in patients with bladder cancer who underwent minimally invasive radical cystectomy(MIRC)or open radical cystectomy(ORC).Methods:We identified patients with bladder cancer...Objective:To investigate oncological outcomes in patients with bladder cancer who underwent minimally invasive radical cystectomy(MIRC)or open radical cystectomy(ORC).Methods:We identified patients with bladder cancer who underwent radical cystectomy(RC)in 13 centers of the Chinese Bladder Cancer Consortium(CBCC).Perioperative outcomes were compared between MIRC and ORC.The influence of surgical approaches on overall survival(OS)and cancer-specific survival(CSS)in the entire study group and subgroups classified according to pathologic stage or lymph node(LN)status was assessed with the log-rank test.Multivariable Cox proportional hazard models were used to evaluate the association among OS,CSS and risk factors of interest.Results:Of 2098 patients who underwent RC,1243 patients underwent MIRC(1087 laparoscopic RC and 156 robotic-assisted RC,respectively),while 855 patients underwent ORC.No significant differences were noted in positive surgical margin rate and 90-day postoperative mortality rate.MIRC was associated with less estimated blood loss,more LN yield,higher rate of neobladder diversion,longer operative time,and longer length of hospital stay.There was no significant difference in OS and CSS according to surgical approaches(pZ0.653,and 0.816,respectively).Subgroup analysis revealed that OS and CSS were not significantly different regardless of the status of extravesical involvement or LN involvement.Multivariable Cox regression analyses showed that the surgical approach was not a significant predictor of OS and CSS.Conclusions:Our study showed that MIRC was comparable to conventional ORC in terms of OS and CSS.展开更多

An economically viable and ecologically sustainable approach to nutrient pollution control could involve the integration of retention ponds, wetlands and greenways into water management systems. Plants not only play an invaluable role in the assimilation and removal of nutrients, but they also support fauna richness and can be aesthetically pleasing. Pandanus amaryllifolius, a tropical terrestrial plant, was found to establish well in hydrophytic conditions and was highly effective in remediating high nutrient levels in an aquatic environment showing 100% removal of NO^-N up to 200 mg/L in 14 days. Phosphate uptake by the plant was less efficient with 64% of the PO4-P removed at the maximum concentration of 100 mg/L at the end of 6 weeks. With its high NO^-N and PO43--P removal efficiency, P. amaryllifolius depleted the nutrient-rich media and markedly contained the natural colonization of algae. The impediment of algal growth led to improvements in the water quality with significant decreases in turbidity, pH and electrical conductivity. In addition, the plants did not show stress symptoms when grown in high nutrient levels as shown by the changes in their biomass, total soluble proteins and chlorophyll accumulation as well as photochemical efficiency. Thus, P. amaryUifolius is a potential candidate for the mitigation of nutrient pollution in phytoremediation systems in the tropics as the plant requires low maintenance, is tolerant to the natural variability of weather conditions and fluctuating hydro-periods, and exhibit good nutrient removal capabilities.展开更多

In this paper, we reviewed the long-term survival outcomes, safety, and quality-of-life of androgen-deprivation therapy (ADT) alone versus combined with radiation therapy (RT) or chemotherapy for locally advanced ...In this paper, we reviewed the long-term survival outcomes, safety, and quality-of-life of androgen-deprivation therapy (ADT) alone versus combined with radiation therapy (RT) or chemotherapy for locally advanced and metastatic prostate cancer (PCa). A literature search was performed using OvidSP. Randomized controlled trials (RCTs) that met the following criteria were included: including locally advanced or metastatic PCa, comparing ADT alone versus combined with any treatment method and reporting quantitative data of disease control or survival outcomes. Finally, eight RCTs met the inclusion criteria. Among these, three compared ADT versus ADT plus RT (n = 2344) and one compared ADT versus ADT plus docetaxel-estramustine (n = 413) in locally advanced PCa; two compared ADT versus ADT plus docetaxel (n = 1175) and two compared ADT versus ADT plus estramustine (n = 114) in metastatic PCa. For locally advanced PCa, the addition of RT to long-term ADT can improve the outcomes of survival and tumor control with fully acceptable adverse effects. Specially, the pooled odds ratio (OR) of overall survival (OS) was 1.43 (95% confidence interval 1.20-1.71) when compared ADT plus RT with ADT alone (P 〈 0.0001). For metastatic hormonally sensitive PCa, the concurrent use of docetaxel plus ADT was effective and safe (pooled OR of OS: 1.29 [1.01-1.65]: P = 0.04). In all, long-term ADT plus RT and long-term ADT plus docetaxel should be considered as proper treatment option in locally advanced and metastatic hormonally sensitive PCa, respectively. The major limitation for the paper was that only eight RCTs were available.展开更多
Marine aquaculture in semi-enclosed bays can significantly influence nutrient cycling in coastal ecosystems.However,the impact of marine aquaculture on the dynamics of dissimilatory nitrate reduction processes(DNRPs)a...Marine aquaculture in semi-enclosed bays can significantly influence nutrient cycling in coastal ecosystems.However,the impact of marine aquaculture on the dynamics of dissimilatory nitrate reduction processes(DNRPs)and the fate of reactive nitrogen remain poorly understood.In this study,the rates of DNRPs and the abundances of related functional genes were investigated in aquaculture and non-aquaculture areas.The results showed that marine aquaculture significantly increased the denitrification(DNF)and dissimilatory nitrate reduction to ammonium(DNRA)rates and decreased the rate of anaerobic ammonium oxidation(ANA),as compared with non-aquaculture sites.DNF was the dominant pathway contributing to the total nitrate reduction,and its contribution to the total nitrate reduction significantly increased from 66.72%at non-aquaculture sites to 78.50%at aquaculture sites.Marine aquaculture can significantly affect the physicochemical characteristics of sediment and the abundances of related functional genes,leading to variations in the nitrate reduction rates.Although nitrate removal rates increased in the marine aquaculture area,ammonification rates and the nitrogen retention index in the aquaculture areas were 2.19 and 1.24 times,respectively,higher than those at non-aquaculture sites.Net reactive nitrogen retention exceeded nitrogen removal in the aquaculture area,and the retained reactive nitrogen could diffuse with the tidal current to the entire bay,thereby aggravating N pollution in the entire study area.These results show that marine aquaculture is the dominant source of nitrogen pollution in semi-enclosed bays.This study can provide insights into nitrogen pollution control in semi-enclosed bays with well-developed marine aquaculture.展开更多
文摘This paper was aimed to compare the clinical effectiveness and safety of adult male circumcision using the Shang RingTM (SR) with the no-flip technique compared with Dorsal Slit (DS) surgical method, A single-centered, prospective study was conducted at the West China Hospital, where patients were circumcised using the no-flip SR (n = 408) or the DS (n = 94) procedure. The adverse events (AEs) and satisfaction were recorded for both groups, and ring-removal time and percentage of delayed removals were recorded for the SR group. Finally, complete follow-up data were collected for 76.1% of patients (SR: n = 306; DS: n = 76). The average ring-removal time for the SR group was 17.62 ± 6.30 days. The operation time (P 〈 0.001), pain scores during the procedure (P 〈 0.001) and at 24 h postoperatively (P 〈 0.001), bleeding (P = 0.001), infection (P = 0.034), and satisfaction with penile appearance (P 〈 0.001) in the SR group were superior to those in the DS group. After two postoperative weeks, the percentage of patients with edema in the SR group (P = 0.029) was higher but no differences were found at 4 weeks (P = 0, 185) between the two groups. In conclusions, the no-flip SR method was found to be superior to the DS method for its short operation time (〈5 min), involving less pain, bleeding, infection, and resulting in a satisfactory appearance. However, the time for recovery from edema took longer, and patients may wear device for 2-3 weeks after the procedure.

文摘The Mg acceptor activation mechanism and hole transport characteristics in AlGaN alloy with Mg doping concentration(~ 1020 cm-3) grown by metal–organic chemical vapor deposition(MOCVD) are systematically studied through optical and electrical properties. Emission lines of shallow oxygen donors and(VⅢ complex)1- as well as VN3+ and neutral Mg acceptors are observed, which indicate that self-compensation is occurred in Mg-doped AlGaN at highly doping levels. The fitting of the temperature-dependent Hall effect data shows that the acceptor activation energy values in Mgdoped AlxGa1-xN(x = 0.23, 0.35) are 172 meV and 242 meV, and the hole concentrations at room temperature are 1.2×1018 cm-3 and 3.3× 1017 cm-3, respectively. Therefore, it is believed that there exists the combined effect of the Coulomb potentials of the dopants and screening of the Coulomb potentials by a high hole concentration. Moreover, due to the high ionized acceptors’ concentration and compensation ratio, the impurity conduction becomes more prominent and the valence band mobility drops sharply at low temperature.

The resulting copolyesters were characterized by WAXD, DSC and PLM.All the copolyesters formed a nematic phase.

文摘The destruction of concrete building materials in severely cold regions of the north is more severely affected by freeze-thaw cycles,and the relationship between the mechanical properties and pore structure of concrete with fine aggregate from municipal solid waste(MSW)incineration bottom ash after freeze-thaw cycles is analyzed under the degree of freeze-thaw hazard variation.In this paper,the gray correlation method is used to calculate the correlation between the relative dynamic elastic modulus,compressive strength,and microscopic porosity parameters to speculate on the most important factors affecting their changes.The GM(1,1)model was established based on the compressive strength of the waste incineration ash aggregate concrete,the relative error between the simulated and actual values in the model was less than 5%,and the accuracy of the model was level 1,indicating that the GM(1,1)model can well reflect the change in the compressive strength of the MSW incineration bottom ash aggregate concrete during freeze-thaw cycles.Using the gray correlation method,the correlation between the relative dynamic elastic modulus,compressive strength,air content,specific surface area,pore spacing coefficient,and pore average chord length was calculated,and the pore spacing coefficient and pore average chord length were determined to be highly correlated with each other.This determination can help analyze and infer the deterioration mechanism of concrete subject to freeze-thaw cycles.These results can provide a theoretical basis for guiding the engineering practice of concrete with fine aggregates of household bottom ash in the northern cold region.

文摘In recent years,advancements in single-cell and spatial transcriptomics,which are highly regarded developments in the current era,particularly the emerging integration of single-cell and spatiotemporal transcriptomics,have enabled a detailed molecular comprehension of the complex regulation of cell fate.The insights obtained from these methodologies are anticipated to significantly contribute to the development of personalized medicine.Currently,single-cell technology is less frequently utilized for prostate cancer compared with other types of tumors.Start-ing from the perspective of RNA sequencing technology,this review outlined the signifcance of single-cell RNA sequencing(scRNA-seq)in prostate cancer research,encompassing preclinical medicine and clinical applications.We summarize the differences between mouse and human prostate cancer as revealed by scRNA-seq studies,as well as a combination of multi-omics methods involving scRNA-seq to highlight the key molecular targets for the diagnosis,treatment,and drug resistance characteristics of prostate cancer.These studies are expected to provide novel insights for the development of immunotherapy and other innovative treatment strategies for castration-resistant prostate cancer.Furthermore,we explore the potential clinical applications stemming from other single-cell technologies in this review,paving the way for future research in precision medicine.
文摘BACKGROUND In randomized controlled trials(RCTs),the placebo arm has often been ignored as the attention tends to be focused on the treatment arm.We undertook a meta-analysis based on the data from the placebo arm in RCTs of hepatocellular carci-noma(HCC),the response rates and survival status,and adverse events(AEs)were summarized and evaluated.AIM To systematically evaluate the response rates,survival status and AEs in the placebo arms of RCTs for HCC.METHODS A systematic search was performed on PubMed,Ovid MEDLINE,Embase and Cochrane Library to identify relevant trials evaluating the efficacy of drugs for the treatment of HCC,published until December 31,2023.Statistical analysis was performed using R statistical software(version 4.3.2).RESULTS A total of 18 RCTs,involving 2390 patients,met the criteria for inclusion in the meta-analysis.The pooled overall disease control rate and objective response rate in the placebo group were 38%[95%confidence interval(CI):33%-42%]and 1%(95%CI:1%-2%),respectively.Overall survival and progression-free survival in the placebo group were 7.9 months(95%CI:7.6-8.31 months)and 1.9 months(95%CI:1.6-2.1 months),respectively.The incidence of grade 3 or 4 AEs was 37%(95%CI:30%-43%).Additionally,the incidence of interruptions or dose reductions due to AEs was 20%(95%CI:13%-27%),while the incidence of treatment discontinuation due to AEs was 9%(95%CI:6%-12%).CONCLUSION Over one-third of advanced HCC patients exhibit therapy-free disease control,with placebo-arm AEs observed.These findings guide single-arm trials design and enhance patient acceptance of anticancer therapies.

文摘Polycystic ovary syndrome(PCOS)is a prevalent endocrine disorder that poses a significant threat to women’s health.As a classical traditional Chinese medicine(TCM)formulation,Guizhi-Fuling Wan(GFW)has a good application prospect in the complementary treatment of PCOS.This study aimed to systematically summarize the traditional efficacy and pharmacological composition of the Chinese medicines contained in GFW and evaluate the clinical efficacy and safety of their active ingredients in the complementary treatment of PCOS.A growing number of studies have demonstrated that GFW is effective at complementally treating PCOS through various mechanisms,including inhibiting inflammatory responses,modulating the intraovarian extracellular matrix,regulating apoptosis and autophagy in granulosa cells,and correcting oxidative stress imbalances.In addition,the GFW has been shown to be effective in treating the complications of PCOS.However,there are several problems,and future work should focus on elucidating the efficacy,safe dosage,and safety of different dosage forms of GFW,increasing the toxicological value of traditional Chinese medicine,and confirming the safety of the combination of GFW with Western medicine for the treatment of PCOS.

文摘Objective:To study the expression of tumor necrosis factor receptor-associated protein 1(TRAP1)in gastric cancer tissue and its correlation with malignant biology.Methods:Gastric cancer tissue and adjacent normal tissue were collected,and mRNA content and protein content of TRAP1 were detected;gastric cancer cell lines SGC7901,BGC823,AGS and MGC803 were cultured,and mRNA contents and protein contents of TRAP1,CyclinB1,CyclinD1,CyclinE,MMP-2 and VEGF were detected.Results:mRNA and protein expression levels of TRAP1 in gastric cancer tissue were significantly higher than those in adjacent normal tissue,and mRNA and protein expression levels of TRAP1 in gastric cancer tissue with muscularis and serosa infiltration,lymph node metastasis,distant organ metastasis and TNM Ⅲ/Ⅳ stage were significantly higher than those in gastric cancer tissue with mucosa and submucosa infiltration,non-lymph node metastasis,non-distant organ metastasis and TNM Ⅰ/Ⅱ stage.mRNA and protein expression levels of TRAP1,CyclinB1,CyclinD1,CyclinE,MMP-2 and VEGF in MGC803 were the highest,and mRNA and protein expression levels of TRAP1,CyclinB1,CyclinD1,CyclinE,MMP-2 and VEGF in SGC7901 were the lowest.mRNA and protein expression levels of TRAP1 in gastric cancer cell lines were positively correlated with mRNA and protein expression of CyclinB1,CyclinD1.CyclinE,MMP-2 and VEGF.Conclusions:The expression of TRAP1 significantly increases in gastric cancer tissue;TRAP1 may regulate the malignant biology of cells by increasing the expression of CyclinB1,CyclinD1,CyclinE,MMP-2 and VEGF,thereby resulting in the occurrence and development of gastric cancer.
文摘Objective:To investigate current status of diagnosis and treatment of bladder cancer in China.Methods:A database was generated by Chinese Bladder Cancer Consortium(CBCC).From January 2007 to December 2012,14,260 cases from 44 CBCC centers were included.Data of diagnosis,treatment and pathology were collected.Results:The average age was 63.5 year-old and most patients were male(84.3%).The most common histologic types were urothelial carcinoma(91.4%),adenocarcinoma(1.8%),and squamous carcinoma(1.9%).According to 1973 and 2004 WHO grading system,42.0%,41.0%,and 17.0% of patients were grade 1,2,and 3,and 16.0%,48.7%,and 35.3% of patients were papillary urothelial neoplasms of low malignant potential,low,and high grade,respectively.Non-muscle invasive bladder cancer(NMIBC)and muscle invasive bladder cancer(MIBC)were 25.2% and 74.1%,respectively(0.8% not clear).Carcinoma in situ was only 2.4%.Most patients were diagnosed by white-light cystoscopy with biopsy(74.3%).Fluorescence and narrow band imaging cystoscopy had additional detection rate of 1.0% and 4.0%,respectively.Diagnostic transurethral resection(TUR)provided detection rate of 16.9%.Most NMIBCs were treated with TUR(89.2%).After initial TUR,2.6%accepted second TUR,and 45.7%,69.9%,and 58.7% accepted immediate,induced,and maintenance chemotherapy instillation,respectively.Most MIBCs were treated with radical cystectomy(RC,59.7%).Laparoscopic RCs were 35.1%,while open RC 63.4%.Extended and standard pelvic lymph node dissection were 7% and 66%,respectively.Three most common urinary diversions were orthotopic neobladder(44%),ileal conduit(31%),and ureterocutaneostomy(23%).Only 2.3% of patients accepted neo-adjuvant chemotherapy and only 18%of T3 and T4 patients accepted adjuvant chemotherapy.Conclusion:Disease characteristics are similar to international reports,while differences of diagnosis and treatment exist.This study can provide evidences for revisions of the guideline on bladder cancer in China.
文摘Klippel-Trénaunay syndrome (KTS)is arare angio-osteo-hypertrophic syndrome characterized by vascular malformations,soft tissue and/or bone hypertrophy, and varicose veins.For the purpose of describing the imaging findings and elucidating the role of medical imaging in the diagnosis and assessment of patient with KTS,we have reviewed the imaging data of 14 KTS patients.The imaging features on different imaging modalities were analyzed.Unilateral lower limb involvement was evident in 71% of cases (n=10)and bilateral but asymmetric lower limb involvement in the remaining 29% of cases (n=4).The most commonly depicted imaging features were varicosities in 93% (n=13),muscle hypertrophy in 79%(n=11)and venous anomalies in 64%(n=9).Other less common imaging findings included lymphedema in 29%(n=4),arterial malformations 29%(n=4),soft tissue hemangiomas 21%(n=3),pelvic and thigh phleboliths 21%(n=3), venous aneurysrns 21%(n=3),bone abnormalities 14%(n=2)and lymphadenopathy 14%(n=2).A severe unilateral lower limb deformity resulting in contractures and muscle atrophy of the whole limb was depicted in 1 case.The pathognomonic marginal vein of Servelle was identified in 2 cases.AV shunt was highly suspected in 4 cases and was confirmed by DSA in 1 case,making Klippel-Trénaunay-Weber syndrome a more apt diagnosis.Associated ipsilateral duplicated renal artery:was found in 1 case.We have concluded that medical imaging is the cornerstone in the.diagnosis and assessment of severity and complications,follow-up and differentiation of KTS from other similar conditions.Different imaging modalities play complementary roles in the evaluation of KTS patients.

文摘Two-inch Ga_2O_3 films with(ˉ201)-orientation are grown on c-sapphire at 850–1050°C by hydride vapor phase epitaxy. High-resolution x-ray diffraction shows that pure β-Ga_2O_3 with a smooth surface has a higher crystal quality, and the Raman spectra reveal a very small residual strain in β-Ga_2O_3 grown by hydride vapor phase epitaxy compared with bulk single crystal. The optical transmittance is higher than 80% in the visible and near-UV regions, and the optical bandgap energy is calculated to be 4.9 e V.
文摘Stones in the seminal vesicles are extremely rare. We present a 62-year-old patient with a stone within a seminal vesicle cyst, who was cured by laparoscopic treatment. The operative time was 80 rnin, and the estimated blood loss was 90 mL. Scanning electron microscope examination of the stone showed a compact crystal image externally and sparse spherical crystal structure in kernel. Composition of the stone was calcium fluorophosphate on X-ray diffractometer. The follow-up time was 15 months with no recurrence of cyst or stone. To our knowledge, this case is the first to describe laparoscopic removal of a stone within a seminal vesicle cyst, and the first to describe calcium fluorophosphate as the composition of seminal vesicle stones. 文摘Non-muscle invasive bladder cancer(NMIBC)is a major type of bladder cancer with a high incidence worldwide,resulting in a great disease burden.Treatment and surveillance are the most important part of NIMBC management.In 2018,we issued“Treatment and surveillance for non-muscle-invasive bladder cancer in China:an evidencebased clinical practice guideline”.Since then,various studies on the treatment and surveillance of NMIBC have been published.There is a need to incorporate these materials and also to take into account the relatively limited medical resources in primary medical institutions in China.Developing a version of guideline which takes these two issues into account to promote the management of NMIBC is therefore indicated.We formed a working group of clinical experts and methodologists.Through questionnaire investigation of clinicians including primary medical institutions,24 clinically concerned issues,involving transurethral resection of bladder tumor(TURBT),intravesical chemotherapy and intravesical immunotherapy of NMIBC,and follow-up and surveillance of the NMIBC patients,were determined for this guideline.Researches and recommendations on the management of NMIBC in databases,guideline development professional societies and monographs were referred to,and the European Association of Urology was used to assess the certainty of generated recommendations.Finally,we issued 29 statements,among which 22 were strong recommendations,and 7 were weak recommendations.These recommendations cover the topics of TURBT,postoperative chemotherapy after TURBT,Bacillus Calmette–Guérin(BCG)immunotherapy after TURBT,combination treatment of BCG and chemotherapy after TURBT,treatment of carcinoma in situ,radical cystectomy,treatment of NMIBC recurrence,and follow-up and surveillance.We hope these recommendations can help promote the treatment and surveillance of NMIBC in China,especially for the primary medical institutions.

文摘Objective:To investigate oncological outcomes in patients with bladder cancer who underwent minimally invasive radical cystectomy(MIRC)or open radical cystectomy(ORC).Methods:We identified patients with bladder cancer who underwent radical cystectomy(RC)in 13 centers of the Chinese Bladder Cancer Consortium(CBCC).Perioperative outcomes were compared between MIRC and ORC.The influence of surgical approaches on overall survival(OS)and cancer-specific survival(CSS)in the entire study group and subgroups classified according to pathologic stage or lymph node(LN)status was assessed with the log-rank test.Multivariable Cox proportional hazard models were used to evaluate the association among OS,CSS and risk factors of interest.Results:Of 2098 patients who underwent RC,1243 patients underwent MIRC(1087 laparoscopic RC and 156 robotic-assisted RC,respectively),while 855 patients underwent ORC.No significant differences were noted in positive surgical margin rate and 90-day postoperative mortality rate.MIRC was associated with less estimated blood loss,more LN yield,higher rate of neobladder diversion,longer operative time,and longer length of hospital stay.There was no significant difference in OS and CSS according to surgical approaches(pZ0.653,and 0.816,respectively).Subgroup analysis revealed that OS and CSS were not significantly different regardless of the status of extravesical involvement or LN involvement.Multivariable Cox regression analyses showed that the surgical approach was not a significant predictor of OS and CSS.Conclusions:Our study showed that MIRC was comparable to conventional ORC in terms of OS and CSS.

文摘Effective control of eutrophication is generally established through the reduction of nutrient loading into waterways and water bodies. An economically viable and ecologically sustainable approach to nutrient pollution control could involve the integration of retention ponds, wetlands and greenways into water management systems. Plants not only play an invaluable role in the assimilation and removal of nutrients, but they also support fauna richness and can be aesthetically pleasing. Pandanus amaryllifolius, a tropical terrestrial plant, was found to establish well in hydrophytic conditions and was highly effective in remediating high nutrient levels in an aquatic environment showing 100% removal of NO^-N up to 200 mg/L in 14 days. Phosphate uptake by the plant was less efficient with 64% of the PO4-P removed at the maximum concentration of 100 mg/L at the end of 6 weeks. With its high NO^-N and PO43--P removal efficiency, P. amaryllifolius depleted the nutrient-rich media and markedly contained the natural colonization of algae. The impediment of algal growth led to improvements in the water quality with significant decreases in turbidity, pH and electrical conductivity. In addition, the plants did not show stress symptoms when grown in high nutrient levels as shown by the changes in their biomass, total soluble proteins and chlorophyll accumulation as well as photochemical efficiency. Thus, P. amaryUifolius is a potential candidate for the mitigation of nutrient pollution in phytoremediation systems in the tropics as the plant requires low maintenance, is tolerant to the natural variability of weather conditions and fluctuating hydro-periods, and exhibit good nutrient removal capabilities.

文摘Square microdisks with round corners are fabricated using a standard GaN-based blue LED on Si substrates.Whispering gallery-like inodes in the square microdisks are investigated by finite-difference time-domain simulation.The simulation results reveal that the round corners in square microdisks can substantially suppress the number of light propagation paths and further reduce the number of optical modes.A con focal microphotoluminescence is performed to analyze the optical properties of the square microdisks at room temperature.The single-mode dominant resonant emission is obtained in the square microdisk with corner radius of 1.5μm.

Randomized controlled trials (RCTs) that met the following criteria were included: including locally advanced or metastatic PCa, comparing ADT alone versus combined with any treatment method and reporting quantitative data of disease control or survival outcomes. Finally, eight RCTs met the inclusion criteria. Among these, three compared ADT versus ADT plus RT (n = 2344) and one compared ADT versus ADT plus docetaxel-estramustine (n = 413) in locally advanced PCa; two compared ADT versus ADT plus docetaxel (n = 1175) and two compared ADT versus ADT plus estramustine (n = 114) in metastatic PCa. For locally advanced PCa, the addition of RT to long-term ADT can improve the outcomes of survival and tumor control with fully acceptable adverse effects. Specially, the pooled odds ratio (OR) of overall survival (OS) was 1.43 (95% confidence interval 1.20-1.71) when compared ADT plus RT with ADT alone (P 〈 0.0001). For metastatic hormonally sensitive PCa, the concurrent use of docetaxel plus ADT was effective and safe (pooled OR of OS: 1.29 [1.01-1.65]: P = 0.04). In all, long-term ADT plus RT and long-term ADT plus docetaxel should be considered as proper treatment option in locally advanced and metastatic hormonally sensitive PCa, respectively. The major limitation for the paper was that only eight RCTs were available.

文摘Marine aquaculture in semi-enclosed bays can significantly influence nutrient cycling in coastal ecosystems.However,the impact of marine aquaculture on the dynamics of dissimilatory nitrate reduction processes(DNRPs)and the fate of reactive nitrogen remain poorly understood.In this study,the rates of DNRPs and the abundances of related functional genes were investigated in aquaculture and non-aquaculture areas.The results showed that marine aquaculture significantly increased the denitrification(DNF)and dissimilatory nitrate reduction to ammonium(DNRA)rates and decreased the rate of anaerobic ammonium oxidation(ANA),as compared with non-aquaculture sites.DNF was the dominant pathway contributing to the total nitrate reduction,and its contribution to the total nitrate reduction significantly increased from 66.72%at non-aquaculture sites to 78.50%at aquaculture sites.Marine aquaculture can significantly affect the physicochemical characteristics of sediment and the abundances of related functional genes,leading to variations in the nitrate reduction rates.Although nitrate removal rates increased in the marine aquaculture area,ammonification rates and the nitrogen retention index in the aquaculture areas were 2.19 and 1.24 times,respectively,higher than those at non-aquaculture sites.Net reactive nitrogen retention exceeded nitrogen removal in the aquaculture area,and the retained reactive nitrogen could diffuse with the tidal current to the entire bay,thereby aggravating N pollution in the entire study area.These results show that marine aquaculture is the dominant source of nitrogen pollution in semi-enclosed bays.This study can provide insights into nitrogen pollution control in semi-enclosed bays with well-developed marine aquaculture.
