Promoting men's health through sports clubs:A systematic rapid realist review

Background Health promotion(HP)interventions delivered through sports clubs have demonstrated promising outcomes among men,but less is known about which aspects of the interventions work,for whom,and under what circumstances.This rapid realist review aimed to understand the contexts,mechanisms,and outcomes of HP interventions for men delivered through sports clubs.Methods A systematic literature search was conducted in February 2023 for studies published after 2013 in MEDLINE,Embase,and SPORTDiscus databases.Included studies:(a)were delivered by or in collaboration with sports clubs,(b)targeted men aged 18+years,and(c)reported 1 or more HP outcomes.A grey literature search was also performed.Studies were included in a realist synthesis based on richness and rigor.Hereafter,context-mechanism-outcome(CMO)configurations were developed.Results We identified and screened 3358 studies,finally including 59 studies describing 22 interventions.Most HP interventions were delivered in high income countries,included Caucasian men aged 35-65 years with overweight/obesity,and used professional sports clubs(mostly football)for recruitment and facilities.Quantitative HP outcomes were reported across 19 interventions.Of these 19 interventions,the majority reported on weight(n=18),physical activity(n=12),mental health(n=10),and diet(n=9).We identified 13 CMOs related to how HP interventions affected men's recruitment,engagement,and health behavior maintenance.Conclusion Our findings show that using sports clubs for HP interventions is effective for engaging men.Recruitment was facilitated by leveraging sports clubs’identity,addressing masculinity-related barriers,improving accessibility,and building trust.Engagement was enhanced through shared identity experiences,safe spaces,inclusive competition,and self-efficacy.However,there's limited evidence on behavior maintenance post-intervention,though involving community stakeholders seemed vital.In general,considerations should be made to avoid perpetuating traditional masculine norms,which may exclude some men and reinforce unhealthy behaviors.These findings can guide intervention development,emphasizing the need to harness men's perspectives in the process. 展开更多

Purpose: The present study evaluated the effects of regular participation in small-sided team handball training on body composition, osteogenic response, physical performance, and cardiovascular risk factors, as well as well-being and motivation, in young untrained women.Methods: Twenty-eight untrained 20- to 30-year-old women were randomized to a handball training group(HG; n = 14, height 170 ± 5 cm, weight 73 ± 11 kg, VO_(2peak) 37.7 ± 4.1 mL/min/kg) that trained 1.7 ± 0.3 times per week over 12 weeks(70 min 4 v 4 handball sessions) or an inactive control group(CG; n = 14, 169 ± 5 cm, 71 ± 12 kg, 38.1 ± 3.7 mL/min/kg). Physiological and psychological and motivational training adaptations were assessed pre-and post-intervention by dual-energy X-ray Absorptiometry(DXA) scans, blood sampling, physical tests, and questionnaires.Results: The average heart rate(HR) over all training sessions was equal to 85% ± 6% HR_(max). Between-group intervention effects were observed in favor of HG for muscle mass(2.1%, p = 0.024), proximal femur bone mineral density(0.8%, p = 0.041), Yo-Yo IE1 intermittent endurance test level 1(IE1) performance(35%, p < 0.001), and incremental treadmill test performance(11.5%, p = 0.003), but not total fat mass(p = 0.176),mean arterial blood pressure(p = 0.328), resting HR(p = 0.219), or blood lipids(p = 0.298—0.854). In CG, no changes were observed in any of the measured physiological variables after the training period. Compared to CG, HG had an increase in intrinsic motivation(p < 0.001) and in the well-being subscale "energy"(p = 0.010).Conclusion: Participation in regular recreational team handball training organized as small-sided games has marked beneficial effects on physical performance, musculoskeletal fitness, well-being, and motivation in untrained young women. 展开更多

Purpose: The aim of the present study was to investigate whether street basketball organized as 3 v 3 on either a half court(HC) with 1 basket or a full court(FC) with 2 baskets could improve fitness and health profiles of untrained men after 3 months of supervised training.Methods: Thirty-five untrained men(aged 20—42 years) completed the pre-and post-intervention testing(FC: n = 13, HC: n = 12, CO(control):n = 10). The training attendance was 2.0 ± 0.4 and 1.9 ± 0.3 times per week in FC and HC, respectively. Mean heart rate(HR) was 83.8 ± 6.0 percent of maximal heart rate(%HR_(max)) and 84.5 ± 2.9 %HR_(max) in FC and HC, respectively.Results: The 3 months of street basketball training on an FC with 2 baskets increased maximal oxygen uptake(2.4 mL/min/kg(95% confidence interval(CI): 1.0—3.9)), time to exhaustion(47 s(95%CI: 26—67)), lean body mass(0.8 kg(95%CI: 0.1—1.5)), and bone mineral density(0.021 g/cm^2(95%CI: 0.011—0.031)), whereas mean arterial pressure(—5.6 mm Hg(95%CI: —7.5 to 3.7)), body fat percentage(—1.6%,(95%CI: —2.5 to —0.7)), heart rate(—18 bpm(95%CI: —24 to —12)), and blood lactate(median: —1.4 mmol/L(interquartile range: —1.5 to —0.6)) during submaximal running were lowered. The changes were less pronounced after the training period when playing on an HC with 1 basket, but increases in maximal oxygen uptake(1.6 mL/min/kg(95%CI: —0.1 to 3.3)), time to exhaustion(28 s(95%CI: 9—47)), lean body mass(1.3 kg(95%CI: 0.3—2.4)), and lower body fat percentage(—0.9%(95%CI: —1.9 to —0.1)) were observed in this group.Conclusion: Three months of 3 v 3 street basketball training improved fitness and led to broad-spectrum improvements in variables related to overall health profile, with the most marked effects observed when playing on an FC with 2 baskets. 展开更多

Purpose: The purpose of the present controlled cross-sectional study was to investigate proximal femur and whole-body bone mineral density(BMD), as well as bone turnover profile, in lifelong trained elderly male football players and young elite football players compared with untrained age-matched men.Methods: One hundred and forty healthy, non-smoking men participated in the study, including lifelong trained football players(FTE, n = 35)aged 65—80 years, elite football players(FTY, n = 35) aged 18—30 years, as well as untrained age-matched elderly(UE, n = 35) and young(UY,n = 35) men. All participants underwent a regional dual-energy X-ray Absorptiometry(DXA) scan of the proximal femur and a whole-body DXA scan to determine BMD. From a resting blood sample, the bone turnover markers(BTMs) osteocalcin, carboxy-terminal type-1 collagen crosslinks(CTX-1), procollagen type-1 amino-terminal propeptide(P1NP), and sclerostin were measured.Results: FTE had 7.3%—12.9% higher(p < 0.05) BMD of the femoral neck, wards, shaft, and total proximal femur in both legs compared to UE,and 9.3%—9.7% higher(p < 0.05) BMD in femoral trochanter in both legs compared to UY. FTY had 24.3%—37.4% higher(p < 0.001) BMD in all femoral regions and total proximal femur in both legs compared to UY. The whole-body DXA scan confirmed these results, with FTE showing similar whole-body BMD and 7.9% higher(p < 0.05) leg BMD compared to UY, and with FTY having 9.6% higher(p < 0.001) wholebody BMD and 18.2% higher(p < 0.001) leg BMD compared to UY. The plasma concentration of osteocalcin, CTX-1, and P1NP were 29%,53%, and 52% higher(p < 0.01), respectively, in FTY compared to UY.Conclusion: BMD of the proximal femur and whole-body BMD are markedly higher in lifelong trained male football players aged 65—80 years and young elite football players aged 18—30 years compared to age-matched untrained men. Elderly football players even show higher BMD in femoral trochanter and leg BMD than untrained young despite an age difference of 47 years. 展开更多

Purpose: The present study investigated the effects of 16 weeks of small-volume, small-sided soccer training soccer group (SG, n = 13) and oscillating whole-body vibration training vibration group (VG, n = 17) on body composition, aerobic fitness, and muscle PCr kinetics in healthy inactive premenopausal women in comparison with an inactive control group (CO, n = 14). Methods: Training for SG and VG consisted of twice-weekly 15-min sessions with average heart rates (HRs) of ~ 155 and 90 bpm respectively. Pre- and post-measurements of body composition (DXA), phosphocreatine (PCr) on- and off-kinetics, and HR measurements during stand- ardised submaximal exercise were performed. Results: After 16 weeks of training in SG, fat percentage was lowered (p = 0.03) by 1.7%±2.4% from 37.5% ± 6.9% to 35.8% ± 6.2% and the PCr decrease in the quadriceps during knee-extension ramp exercise was attenuated (4% ± 8%, p = 0.04), with no changes in VG or CO (time- group effect: p=0.03 and p = 0.03). Submaximal exercise HR was also reduced in SG after 16 weeks of training (6%± 5% of HRmax, p = 0.01). Conclusion: Short duration soccer training for 16 weeks appears to be sufficient to induce favourable changes in body composition and indicators of aerobic fitness and muscle oxidative capacity in untrained premenopausal women. 展开更多

Purpose: Androgen deprivation therapy (ADT) is a cornerstone in prostate cancer (PCa) management that prolongs PCa-free and overall survival, but effects of ADT on human cardiac function have not been investigated. We used echocardiography to examine cardiac structure and function in patients with prostate cancer receiving ADT and to determine whether an exercise intervention can elicit cardiac adaptations in these subjects. Methods: Forty-three patients with prostate cancer receiving ADT were randomized to 12 weeks football training (ST group;n = 20) or usual care (control [CO] group;n = 23). Cardiac function was assessed at baseline and after 12 weeks by comprehensive echocardiography. Peak oxygen consumption, blood pressure and peripheral microvascular function was also measured. Results: At baseline, no considerable echocardiographic abnormalities were observed. In the ST group, increases in left ventricular diastolic function variables including E/A ratio (P = 0.03), E’ (P = 0.016), E’TDIcolor (P = 0.040) and in left atrial diameter (P = 0.001) were observed after 12 weeks. In addition, diastolic blood pressure (P = 0.027) and resting heart rate (P 0.001) were reduced after ST. In the CO group, no significant changes were observed in the examined variables after 12 weeks. Despite within group changes in the ST group, no significant differences were observed after 12 weeks between groups in echocardiographic variables, peak oxygen consumption, blood pressure and peripheral microvascular function. Conclusion: In men with prostate cancer receiving ADT, echocardiography showed no abnormalities in cardiac structure and function. Twelve weeks of ST failed to elicit significant cardiovascular adaptations and ADT may blunt cardiovascular adaptations to short-term exercise training. 展开更多
