BACKGROUND Major depressive disorder(MDD)and obesity(OB)are bidirectionally comorbid conditions with common neurobiological underpinnings.However,the neurocognitive mechanisms of their comorbidity remain poorly unders...BACKGROUND Major depressive disorder(MDD)and obesity(OB)are bidirectionally comorbid conditions with common neurobiological underpinnings.However,the neurocognitive mechanisms of their comorbidity remain poorly understood.AIM To examine regional abnormalities in spontaneous brain activity among patients with MDD-OB comorbidity.METHODS This study adopted a regional homogeneity(ReHo)analysis of resting-state functional magnetic resonance imaging.The study included 149 hospital patients divided into four groups:Patients experiencing their first episode of drug-naive MDD with OB,patients with MDD without OB,and age-and sex-matched healthy individuals with and without OB.Whole-brain ReHo analysis was conducted using SPM12 software and RESTplus toolkits,with group comparisons via ANOVA and post-hoc tests.Correlations between ReHo values and behavioral measures were examined.RESULTS ANOVA revealed significant whole-brain ReHo differences among the four groups in four key regions:The left middle temporal gyrus(MTG.L),right cuneus,left precuneus,and left thalamus.Post-hoc analyses confirmed pairwise differences between all groups across these regions(P<0.05).OB was associated with ReHo alterations in the MTG.L,right cuneus,and left thalamus,whereas abnormalities in the precuneus suggested synergistic pathological mechanisms between MDD and OB.Statistically significant correlations were found between the drive and fun-seeking dimensions of the behavioral activation system,as well as behavioral inhibition and the corresponding ReHo values.CONCLUSION Our findings provide novel evidence for the neuroadaptive mechanisms underlying the MDD-OB comorbidity.Further validation could lead to personalized interventions targeting MTG.L hyperactivity and targeting healthy food cues.展开更多
Tomato(Solanum lycopersicum)is an extensively cultivated vegetable,and its growth and fruit quality can be significantly impaired by low temperatures.The widespread presence of N^(6)-methyladenosine(m^(6)A)modificatio...Tomato(Solanum lycopersicum)is an extensively cultivated vegetable,and its growth and fruit quality can be significantly impaired by low temperatures.The widespread presence of N^(6)-methyladenosine(m^(6)A)modification on RNA is involved in a diverse range of stress response processes.There is a significant knowledge gap regarding the precise roles of m^(6)A modification in tomato,particularly for cold stress response.Here,we assessed the m^(6)A modification landscape of S.lycopersicum'Micro-Tom'leaves in response to low-temperature stress.Furthermore,we investigated the potential relationship among m^(6)A modification,transcriptional regulation,alternative polyadenylation events,and protein translation via MeRIP-seq,RNA-seq,and protein mass spectrometry.After omic date analysis,11378 and 10735 significant m^(6)A peak associated genes were identified in the control and cold treatment tomato leaves,respectively.Additionally,we observed a UGUACAK(K=G/U)motif under both conditions.Differential m^(6)A site associated genes most likely play roles in protein translation regulatory pathway.Besides directly altering gene expression levels,m^(6)A also leads to differential poly(A)site usage under low-temperature.Finally,24 important candidate genes associated with cold stress were identified by system-level multi-omic analysis.Among them,m^(6)A modification levels were increased in SBPase(Sedoheptulose-1,7-bisphosphatase,Solyc05g052600.4)mRNA,causing distal poly(A)site usage,downregulation of mRNA expression level,and increased protein abundance.Through these,tomato leaves try to maintain normal photo synthetic carbon assimilation and nitro gen metabolism under low-temperature condition.The comprehensive investigation of the m^(6)A modification landscape and multi-omics analysis provide valuable insights into the epigenetic regulatory mechanisms in tomato cold stress response.展开更多
[Objectives]To explore the effects of motivational interviewing intervention based on the transtheoretical model(TTM)on psychological resilience and self-management in patients undergoing finger reimplantation after a...[Objectives]To explore the effects of motivational interviewing intervention based on the transtheoretical model(TTM)on psychological resilience and self-management in patients undergoing finger reimplantation after amputation.[Methods]The patients with finger replantation due to fractures admitted from October 2024 to June 2025 were divided into either the control group or the observation group according to the random number table method,with 40 cases in each group.The control group received conventional perioperative care,while the observation group underwent motivational interviewing based on TTM framework on the basis of the control group.The psychological resilience and self-management levels of the two patient groups were then compared following their respective care interventions.[Results]The psychological resilience and self-management scores of the patients were significantly higher than those of the control group after the intervention,with the difference being statistically significant(P<0.05).[Conclusions]Motivational interviewing based on the TTM can effectively improve the psychological resilience and self-management in patients undergoing severed finger reimplantation,while effectively reducing the occurrence of vascular crisis.展开更多
BACKGROUND Previous research yielded conflicting results regarding the association between prediabetes and colorectal cancer(CRC).AIM To systematically assess the incidence of CRC in individuals with prediabetes compa...BACKGROUND Previous research yielded conflicting results regarding the association between prediabetes and colorectal cancer(CRC).AIM To systematically assess the incidence of CRC in individuals with prediabetes compared with individuals with normoglycemia via a meta-analysis.METHODS Relevant cohort studies were acquired by searching MEDLINE,Web of Science,and EMBASE.A random-effects model was applied to combine the findings after accounting for heterogeneity.Several subgroup analyses were conducted to assess the impact of study characteristics on the results.RESULTS Eleven cohort studies involving 4996352 participants,including 383917(7.7%)with prediabetes at baseline,were analyzed in this meta-analysis.Over a mean follow-up period of 6.5 years,the combined findings revealed that individuals with prediabetes at baseline had a higher likelihood of developing CRC than those with normoglycemia[risk ratio(RR)=1.18,95%confidence interval=1.11 to 1.25,P<0.001]with low statistical heterogeneity(I2=27%).Subgroup analyses indicated that the association between prediabetes and an increased risk of CRC was mainly observed in studies defining prediabetes using impaired fasting glucose(RR=1.24)and slightly elevated hemoglobin A1c levels(RR=1.18)but not in those that defined prediabetes using impaired glucose tolerance(RR=1.06).Other study characteristics such as design,country,participant age and sex,the duration of follow-up,or adjustment for body mass index did not significantly impact the results(all P>0.05).CONCLUSION People with prediabetes might have a higher likelihood of developing CRC than individuals with normoglycemia.展开更多

Objective:Both allogeneic hematopoietic stem cell transplantation(allo-HSCT)and autologous HSCT(ASCT)are important therapies for extranodal natural killer/T-cell lymphoma(ENKTCL);however,no large-scale,multicenter stu...Objective:Both allogeneic hematopoietic stem cell transplantation(allo-HSCT)and autologous HSCT(ASCT)are important therapies for extranodal natural killer/T-cell lymphoma(ENKTCL);however,no large-scale,multicenter study has compared the efficacy and safety between allo-HSCT and ASCT in these patients.Our multicenter,real-world study aimed to evaluate the outcomes of allo-HSCT vs.ASCT as consolidation in ENKTCL patients who had achieved a complete response(CR)or partial response(PR).Methods:This was a multicenter,retrospective study with nine hospitals in China,and 114 patients with ENKTCL were enrolled.Sixty patients received ASCT and 54 received allo-HSCT.The primary outcome was progression-free survival(PFS).In the sensitivity analysis,propensity score matching(PSM)analyses were conducted to adjust for baseline prognostic factors.Landmark analysis were conducted to minimize immortal-time bias.Results:Patients in the allo-HSCT group presented with more adverse prognostic factors.Allo-HSCT group showed a significantly better PFS and a lower disease progression rate compared with ASCT group in patients with Ann Arbor stageⅢ/Ⅳdisease(PFS:100%vs.82.0%,P=0.023;disease progression rate:0 vs.25.4%,P=0.024),those with intermediate/high prognostic index of natural killer lymphoma(PINK)scores(PFS:100%vs.84.4%,P=0.034;disease progression rate:0 vs.22.1%,P=0.034),those with intermediate/high international prognostic index(IPI)scores(PFS:100%vs.82.0%,P=0.038;disease progression rate:0 vs.25.4%,P=0.038),or those receiving HSCT at PR(PFS:100%vs.50%,P=0.046;disease progression rate:0 vs.50%,P=0.046)at the 1.5-4.0 follow-up.In multivariate analysis,receiving ASCT was significantly associated with a poorer PFS[hazard ratio(HR)=2.23,P=0.038]and overall survival(OS)(HR=2.45,P=0.045).In the sensitivity analysis,patients receiving allo-HSCT showed a significantly better PFS(70.3%vs.39.1%,P=0.039),OS(73.9%vs.42.0%,P=0.044),and a lower disease progression rate(22.6%vs.57.0%,P=0.017)compared with those receiving ASCT after propensity score matching.Conclusions:ENKTCL patients with high-risk characteristics could benefit more from allo-HSCT as consolidation.展开更多
Background:Cardiovascular and cerebrovascular diseases are significant health threats that have been found to involve ferroptosis in related cells.This study aims to comprehensively examine the role and mechanism of n...Background:Cardiovascular and cerebrovascular diseases are significant health threats that have been found to involve ferroptosis in related cells.This study aims to comprehensively examine the role and mechanism of natural products,derived from traditional Chinese herbal medicines,in intervening in ferroptosis from the perspective of cardiovascular and cerebrovascular aspects.Methods:A literature survey was conducted using Scopus,Pubmed,Reaxys,and Google Scholar databases to review studies on natural products(such as baicalin,puerarin,and ginsenosides)derived from herbs like scutellaria baicalensis,pueraria lobata,ginseng,panax quinquefolium,and panax notoginseng.The focus was on their potential as drugs for the treatment of cardiovascular and cerebrovascular diseases,particularly their ability to inhibit ferroptosis.Results:The review revealed that various natural products exhibit disease protection and prevention properties,inhibiting ferroptosis through diverse pathways and targets in cardiovascular and cerebrovascular diseases.The involvement of these natural products in key pathways associated with ferroptosis,including amino acid metabolism,iron metabolism,and lipid peroxidation,was elucidated.Notably,tanshinone IIA,resveratrol,astragaioside IV,and vitexin were found to regulate ferroptosis in both cardiovascular and cerebrovascular cells.Additionally,the impact of these natural products on key targets of ferroptosis,such as SLC7A11,DMTI,TfRI,Nrf2,HO-1,GPX4,NCOA4,and LOX,was discussed.Conclusion:This study provides a comprehensive understanding of the role and mechanism of natural products in intervening in ferroptosis related to cardiovascular and cerebrovascular diseases.The findings pave the way for potential future treatments targeting ferroptosis in these diseases.展开更多

Objective:This study aims to investigate the patterns of symptom occurrence in patients experiencing acute exacerbations of chronic obstructive pulmonary disease(AECOPD).It will explore the composition of symptom clus...Objective:This study aims to investigate the patterns of symptom occurrence in patients experiencing acute exacerbations of chronic obstructive pulmonary disease(AECOPD).It will explore the composition of symptom clusters and analyze the correlation between these clusters and health-related quality of life(HRQoL).Methods:A total of 207 patients with AE-COPD were surveyed from a tertiary grade A hospital.Data collection was conducted using three validated instruments:the Basic Information Questionnaire(BIQ),Disease Symptom Survey Questionnaire(MSAS),and Quality of Life Questionnaire(CAT).Statistical software SPSS 22.0 was used to analyze the correlation between symptom clusters and quality of life.Results:Exploratory factor analysis showed that five major symptom clusters existed in the patients,including the psycho-emotional symptom cluster,the sleep-related symptom cluster,the other side effects symptom cluster,the energy deficiency symptom cluster and the cough-loss of appetite symptom cluster,and the severity of the symptom clusters showed a significant negative correlation with the quality of life of the patients(P<0.05).Conclusion:Strengthening the comprehensive management of symptom clusters in patients with AE-COPD can help to effectively reduce the symptom burden of patients,and then significantly improve their quality of life.展开更多

In order to improve the deep learning training efficiency of the large reflector antenna active adjustment technique,this paper synthesizes the characteristic that each actuator can only adjust the panel connected to ...In order to improve the deep learning training efficiency of the large reflector antenna active adjustment technique,this paper synthesizes the characteristic that each actuator can only adjust the panel connected to it,and proposes a divided-ring antenna active adjustment deep learning training modeling method.The method organizes panel node data according to actuator ring positions,using panel displacements as input features and actuator adjustments as output labels.Through systematic sorting,reorganization,and normalization,the ring-divided data are transformed into grid-structured tensors suitable for convolutional processing.Multi-layer convolutional neural networks are then constructed for surface adjustment prediction,optimized through a hybrid strategy combining simulated annealing and the Adam algorithm.Through the dataset divided-ring preprocessing,active adjustment neural network construction and model training for the case of an 8 m reflector antenna,the analytical results show that the proposed method can effectively shorten the training time,and the final model's prediction accuracy is greatly improved,which demonstrates the feasibility and effectiveness of the proposed method.展开更多
The East Asian Very Long Baseline Interferometry(VLBI) Network(EAVN) is a rapidly evolving international VLBI array that is currently promoted under joint efforts among China,Japan and Korea.EAVN aims at forming a joi...The East Asian Very Long Baseline Interferometry(VLBI) Network(EAVN) is a rapidly evolving international VLBI array that is currently promoted under joint efforts among China,Japan and Korea.EAVN aims at forming a joint VLBI Network by combining a large number of radio telescopes distributed over East Asian regions.After the combination of the Korean VLBI Network(KVN) and the VLBI Exploration of Radio Astrometry(VERA) into Ka VA,further expansion with the joint array in East Asia is actively promoted.Here we report the first imaging results(at 22 and 43 GHz) of bright radio sources obtained with Ka VA connected to Tianma 65-m and Nanshan 26-m Radio Telescopes in China.To test the EAVN imaging performance for different sources,we observed four active galactic nuclei(AGN) having different brightness and morphology.As a result,we confirmed that the Tianma 65-m Radio Telescope(TMRT) significantly enhances the overall array sensitivity,a factor of 4 improvement in baseline sensitivity and 2 in image dynamic range compared to the case of Ka VA only.The addition of the Nanshan 26-m Radio Telescope(NSRT) further doubled the east-west angular resolution.With the resulting high-dynamic-range,high-resolution images with EAVN(Ka VA+TMRT+NSRT),various fine-scale structures in our targets,such as the counter-jet in M87,a kink-like morphology of the 3 C 273 jet and the weak emission in other sources are successfully detected.This demonstrates the powerful capability of EAVN to study AGN jets and to achieve other science goals in general.Ongoing expansion of EAVN will further enhance the angular resolution,detection sensitivity and frequency coverage of the network.展开更多
BACKGROUND Low anterior resection syndrome(LARS)is a prevalent and debilitating complication following sphincter-preserving surgery for rectal cancer.Evidence-based interventions for the concurrent psychological burde...BACKGROUND Low anterior resection syndrome(LARS)is a prevalent and debilitating complication following sphincter-preserving surgery for rectal cancer.Evidence-based interventions for the concurrent psychological burden are limited.Electroacupuncture has been proposed as a potential adjunctive therapy,but its psychological benefits remain inadequately studied.AIM To investigate the therapeutic effect of electroacupuncture on emotional recovery and gastrointestinal function in patients with moderate to severe LARS,and to explore its potential advantages in psychologically vulnerable subgroups.METHODS We conducted a retrospective,controlled study involving 100 patients with moderate to severe LARS(LARS score≥21)treated at two tertiary hospitals in China between January 2022 and December 2024.Patients received either standard postoperative care alone(n=50)or in combination with a standardized 4-week electroacupuncture protocol(n=50).Psychological and functional outcomes were assessed using validated instruments including Hospital Anxiety and Depression Scale(HADS),Body Image Scale(BIS),General Self-Efficacy Scale,Perceived Social Support Scale(PSSS),LARS score,and European Organization for Research and Treatment of Cancer Quality of Life Questionnaire Core 30 at four time points.The primary endpoint was emotional remission,defined as a≥3-point reduction in HADS-Anxiety subscale(HADS-A).Analyses included repeated-measures comparisons,Kaplan-Meier survival curves,Cox regression models,and subgroup-interaction testing.RESULTS At baseline,demographic,surgical,and psychosocial characteristics were comparable among groups.By week 4,patients receiving electroacupuncture demonstrated significantly greater reductions in anxiety(HADS-A:4.8±2.6 vs 7.3±3.0;P<0.001),depression,and body-image disturbance(BIS:8.7±3.6 vs 11.9±4.2;P<0.001),alongside enhanced coping capacity(Brief Coping Orientation to Problems Experienced),perceived social support(PSSS),and bowel function(LARS score).Emotional remission-defined as a≥3-point HADS-A reduction-was achieved more rapidly in the electroacupuncture group,as confirmed by Kaplan-Meier analysis(log-rank P<0.001;odds ratio=4.7).Multivariate Cox regression identified higher baseline LARS and BIS scores as independent predictors of delayed emotional recovery.Subgroup analyses revealed significantly amplified treatment benefits in patients with high baseline anxiety(HADS-A≥8),elevated body-image disturbance(BIS≥12),or low perceived social support(PSSS<60),with consistent interaction effects(P for interaction<0.05 across subgroups).CONCLUSION Electroacupuncture may accelerate emotional recovery and improve functional and psychosocial outcomes in patients with LARS.Its integration into postoperative care may offer particular benefits for psychologically vulnerable subgroups.展开更多
Observing and timing a group of millisecond pulsars with high rotational stability enables the direct detection of gravitational waves(GWs).The GW signals can be identified from the spatial correlations encoded in the...Observing and timing a group of millisecond pulsars with high rotational stability enables the direct detection of gravitational waves(GWs).The GW signals can be identified from the spatial correlations encoded in the times-of-arrival of widely spaced pulsar-pairs.The Chinese Pulsar Timing Array(CPTA)is a collaboration aiming at the direct GW detection with observations carried out using Chinese radio telescopes.This short article serves as a“table of contents”for a forthcoming series of papers related to the CPTA Data Release 1(CPTA DR1)which uses observations from the Five-hundred-meter Aperture Spherical radio Telescope.Here,after summarizing the time span and accuracy of CPTA DR1,we report the key results of our statistical inference finding a correlated signal with amplitude logA_(c)=-14.4_(-2.8)^(+1.0)for spectral index in the range ofα∈[-1.8,1.5]assuming a GW background(GWB)induced quadrupolar correlation.The search for the Hellings–Downs(HD)correlation curve is also presented,where some evidence for the HD correlation has been found that a 4.6σstatistical significance is achieved using the discrete frequency method around the frequency of 14 n Hz.We expect that the future International Pulsar Timing Array data analysis and the next CPTA data release will be more sensitive to the n Hz GWB,which could verify the current results.展开更多
Background:Many healthcare workers were infected by coronavirus disease 2019(COVID-19)early in the epidemic posing a big challenge for epidemic control.Hence,this study aims to explore perceived infection routes,influ...Background:Many healthcare workers were infected by coronavirus disease 2019(COVID-19)early in the epidemic posing a big challenge for epidemic control.Hence,this study aims to explore perceived infection routes,influencing factors,psychosocial changes,and management procedures for COVID-19 infected healthcare workers.Methods:This is a cross-sectional,single hospital-based study.We recruited all 105 confirmed COVID-19 healthcare workers in the Zhongnan Hospital of Wuhan University from February 15 to 29,2020.All participants completed a validated questionnaire.Electronic consent was obtained from all participants.Perceived causes of infection,infection prevention,control knowledge and behaviour,psychological changes,symptoms and treatment were measured.Results:Finally,103 professional staff with COVID-19 finished the questionnaire and was included(response rate:98.1%).Of them,87 cases(84.5%)thought they were infected in working environment in hospital,one(1.0%)thought their infection was due to the laboratory environment,and 5(4.9%)thought they were infected in daily life or community environment.Swab of throat collection and physical examination were the procedures perceived as most likely causing their infection by nurses and doctors respectively.Forty-three(41.8%)thought their infection was related to protective equipment,utilization of common equipment(masks and gloves).The top three first symptoms displayed before diagnosis were fever(41.8%),lethargy(33.0%)and muscle aches(30.1%).After diagnosis,88.3%staff experienced psychological stress or emotional changes during their isolation period,only 11.7%had almost no emotional changes.Arbidol(Umifenovir;an anti-influza drug;69.2%)was the drug most commonly used to target infection in mild and moderate symptoms.Conclusion:The main perceived mode of transmission was not maintaining protection when working at a close distance and having intimate contact with infected cases.Positive psychological intervention is necessary.展开更多
BACKGROUND Chronic atrophic gastritis(AG)with intestinal metaplasia(IM)significantly increases the risk of gastric cancer.Some medicines have showed definite therapeutic effects in AG and IM regression.AIM To validate...BACKGROUND Chronic atrophic gastritis(AG)with intestinal metaplasia(IM)significantly increases the risk of gastric cancer.Some medicines have showed definite therapeutic effects in AG and IM regression.AIM To validate the efficacy of Lamb’s tripe extract and vitamin B12 capsule(LTEVB12)initial therapy and celecoxib rescue therapy for IM and AG.METHODS A total of 255 patients were included to receive LTEVB12 initial therapy(2 capsules each time,three times daily for 6 mo)in hospital in this study.The patients with failure of IM regression continued to receive celecoxib rescue therapy(200 mg,once daily for 6 mo).After each therapy finished,the patients underwent endoscopy and biopsy examination.The regression efficiency was assessed by the operative link on gastritis assessment(OLGA)and the operative link on the gastric intestinal metaplasia assessment(OLGIM)staging system.Logistic regression analysis was applied to identify factors associated with the curative effect.RESULTS For LTEVB12 initial therapy,the reversal rates of IM and AG were 52.95%and 48.24%,respectively.Analogously,for celecoxib rescue therapy,the effective rates for IM and AG were 56.25%and 51.56%,respectively.The IM regression rate of complete therapy was up to 85.03%.In different OLGA and OLGIM stages of IM patients,therapeutic efficiency showed a significant difference in each group(P<0.05).For both therapies,patients with high stages(III or IV)of both the OLGA and OLGIM evaluation systems showed a higher IM or AG regression rate than those with low stages(I or II).Among patients with high stages(OLGIM III and IV),the IM regression rate was above 70%for each therapy.Eating habits,fresh vegetable intake,and high-salt diet were identified as independent factors for the IM reversal effect of LTEVB12 therapy,especially high-salt diet(odds ratio=1.852,P<0.05).CONCLUSION Monotherapy could reverse IM and AG.LTEVB12 initial therapy and celecoxib rescue therapy significantly increase the regression effect.IM may not be the point of no return among gastric precancerous lesions.展开更多
To effectively improve the performance and expand the applications of polymers, molecular dynamics(MD) simulations with the COMPASS force field have been applied to predict the miscibility, glass transition temperatur...To effectively improve the performance and expand the applications of polymers, molecular dynamics(MD) simulations with the COMPASS force field have been applied to predict the miscibility, glass transition temperature(Tg), and mechanical properties of poly(vinyl chloride)/dioctyl phthalate(PVC/DOP) blends. The solubility parameter values obtained are in good agreement with the reference data and the little difference(|Δδ|< 2.0 MPa0.5) between two components indicates that PVC/DOP is a miscible system. Tg is predicted by the slope of the free volume and density versus temperature simulation data based on density and free volume theory which is agree well with the experimental data. In addition, the analyses of mechanical properties results indicate that the values of Young’s modulus(E), bulk modulus(K), and shear modulus(G) decrease with the addition of DOP, demonstrating that the rigidity of material is weakened and the ductility is improved. The mechanical properties can also be effectively improved by increasing the temperature, which may provide a more flexible mixture, with lower E, K, G but an increased ductility.展开更多
The novel severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)is the cause of a rapidly spreading illness,coronavirus disease 2019(COVID-19),affecting more than seventeen million people around the world.Diagnos...The novel severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)is the cause of a rapidly spreading illness,coronavirus disease 2019(COVID-19),affecting more than seventeen million people around the world.Diagnosis and treatment guidelines for clinicians caring for patients are needed.In the early stage,we have issued"A rapid advice guideline for the diagnosis and treatment of 2019 novel coronavirus(2019-nCoV)infected pneumonia(standard version)";now there are many direct evidences emerged and may change some of previous recommendations and it is ripe for develop an evidence-based guideline.We formed a working group of clinical experts and methodologists.The steering group members proposed 29 questions that are relevant to the management of COVID-19 covering the following areas:chemoprophylaxis,diagnosis,treatments,and discharge management.We searched the literature for direct evidence on the management of COVID-19,and assessed its certainty generated recommendations using the Grading of Recommendations,Assessment,Development and Evaluation(GRADE)approach.Recommendations were either strong or weak,or in the form of ungraded consensus-based statement.Finally,we issued 34 statements.Among them,6 were strong recommendations for,14 were weak recommendations for,3 were weak recommendations against and 11 were ungraded consensus-based statement.They covered topics of chemoprophylaxis(including agents and Traditional Chinese Medicine(TCM)agents),diagnosis(including clinical manifestations,reverse transcription-polymerase chain reaction(RT-PCR),respiratory tract specimens,IgM and IgG antibody tests,chest computed tomography,chest X-ray,and CT features of asymptomatic infections),treatments(including lopinavirritonavir,umifenovir,favipiravir,interferon,remdesivir,combination of antiviral drugs,hydroxychloroquine/chloroquine,interleukin-6 inhibitors,interleukin-1 inhibitors,glucocorticoid,qingfei paidu decoction,lianhua qingwen granules/capsules,convalescent plasma,lung transplantation,invasive or noninvasive ventilation,and extracorporeal membrane oxygenation(ECMO)),and discharge management(including discharge criteria and management plan in patients whose RT-PCR retesting shows SARS-CoV-2 positive after discharge).We also created two figures of these recommendations for the implementation purpose.We hope these recommendations can help support healthcare workers caring for COVID-19 patients.展开更多
BACKGROUND:Biliary complications are a serious problem in patients after liver transplantation and often require reoperation.This study was conducted to summarize the endoscopic diagnosis and management of biliary com...BACKGROUND:Biliary complications are a serious problem in patients after liver transplantation and often require reoperation.This study was conducted to summarize the endoscopic diagnosis and management of biliary complications after orthotopic liver transplantation(OLT).METHODS:From December 2000 to November 2003,twelve endoscopic retrograde cholangiopancreatographies(ERCPs)were performed in 7 patients after OLT at Digestive Endoscopic Center of Changhai Hospital in Shanghai,China.The therapeutic maneuvers included endoscopic sphincterotomy(EST),biliary stent placement,balloon and basket extraction,irrigation,and nasobiliary tube placement.A retrospective study was made to determine the types of biliary tract complications after OLT.The success of ERCP and therapeutic maneuvers was also evaluated.RESULTS:Biliary tract complications including biliary stricture,biliary leak,biliary sludge,and stump leak of the cyst duct were treated respectively by endoscopic sphincterotomy with sludge extraction,stricture dilation or endoscopic retrograde biliary drainage.Two of the 3 patients with proximal common bile duct stricture were successfully treated with ERCP and stent placement.Four patients with anastomotic stricture and/without bile leak were treated successfully by dilation and stent placement or endoscopic nosobiliary drainage.No severe ERCP-related complications occurred.CONCLUSIONS:ERCP is an effective and accurate approach for the diagnosis of biliary tract complications after OLT,and placement of a stent is a safe initial treatment for biliary complications after liver transplantation.展开更多

文摘BACKGROUND Major depressive disorder(MDD)and obesity(OB)are bidirectionally comorbid conditions with common neurobiological underpinnings.However,the neurocognitive mechanisms of their comorbidity remain poorly understood.AIM To examine regional abnormalities in spontaneous brain activity among patients with MDD-OB comorbidity.METHODS This study adopted a regional homogeneity(ReHo)analysis of resting-state functional magnetic resonance imaging.The study included 149 hospital patients divided into four groups:Patients experiencing their first episode of drug-naive MDD with OB,patients with MDD without OB,and age-and sex-matched healthy individuals with and without OB.Whole-brain ReHo analysis was conducted using SPM12 software and RESTplus toolkits,with group comparisons via ANOVA and post-hoc tests.Correlations between ReHo values and behavioral measures were examined.RESULTS ANOVA revealed significant whole-brain ReHo differences among the four groups in four key regions:The left middle temporal gyrus(MTG.L),right cuneus,left precuneus,and left thalamus.Post-hoc analyses confirmed pairwise differences between all groups across these regions(P<0.05).OB was associated with ReHo alterations in the MTG.L,right cuneus,and left thalamus,whereas abnormalities in the precuneus suggested synergistic pathological mechanisms between MDD and OB.Statistically significant correlations were found between the drive and fun-seeking dimensions of the behavioral activation system,as well as behavioral inhibition and the corresponding ReHo values.CONCLUSION Our findings provide novel evidence for the neuroadaptive mechanisms underlying the MDD-OB comorbidity.Further validation could lead to personalized interventions targeting MTG.L hyperactivity and targeting healthy food cues.

文摘Tomato(Solanum lycopersicum)is an extensively cultivated vegetable,and its growth and fruit quality can be significantly impaired by low temperatures.The widespread presence of N^(6)-methyladenosine(m^(6)A)modification on RNA is involved in a diverse range of stress response processes.There is a significant knowledge gap regarding the precise roles of m^(6)A modification in tomato,particularly for cold stress response.Here,we assessed the m^(6)A modification landscape of S.lycopersicum'Micro-Tom'leaves in response to low-temperature stress.Furthermore,we investigated the potential relationship among m^(6)A modification,transcriptional regulation,alternative polyadenylation events,and protein translation via MeRIP-seq,RNA-seq,and protein mass spectrometry.After omic date analysis,11378 and 10735 significant m^(6)A peak associated genes were identified in the control and cold treatment tomato leaves,respectively.Additionally,we observed a UGUACAK(K=G/U)motif under both conditions.Differential m^(6)A site associated genes most likely play roles in protein translation regulatory pathway.Besides directly altering gene expression levels,m^(6)A also leads to differential poly(A)site usage under low-temperature.Finally,24 important candidate genes associated with cold stress were identified by system-level multi-omic analysis.Among them,m^(6)A modification levels were increased in SBPase(Sedoheptulose-1,7-bisphosphatase,Solyc05g052600.4)mRNA,causing distal poly(A)site usage,downregulation of mRNA expression level,and increased protein abundance.Through these,tomato leaves try to maintain normal photo synthetic carbon assimilation and nitro gen metabolism under low-temperature condition.The comprehensive investigation of the m^(6)A modification landscape and multi-omics analysis provide valuable insights into the epigenetic regulatory mechanisms in tomato cold stress response.

文摘[Objectives]To explore the effects of motivational interviewing intervention based on the transtheoretical model(TTM)on psychological resilience and self-management in patients undergoing finger reimplantation after amputation.[Methods]The patients with finger replantation due to fractures admitted from October 2024 to June 2025 were divided into either the control group or the observation group according to the random number table method,with 40 cases in each group.The control group received conventional perioperative care,while the observation group underwent motivational interviewing based on TTM framework on the basis of the control group.The psychological resilience and self-management levels of the two patient groups were then compared following their respective care interventions.[Results]The psychological resilience and self-management scores of the patients were significantly higher than those of the control group after the intervention,with the difference being statistically significant(P<0.05).[Conclusions]Motivational interviewing based on the TTM can effectively improve the psychological resilience and self-management in patients undergoing severed finger reimplantation,while effectively reducing the occurrence of vascular crisis.

文摘BACKGROUND Previous research yielded conflicting results regarding the association between prediabetes and colorectal cancer(CRC).AIM To systematically assess the incidence of CRC in individuals with prediabetes compared with individuals with normoglycemia via a meta-analysis.METHODS Relevant cohort studies were acquired by searching MEDLINE,Web of Science,and EMBASE.A random-effects model was applied to combine the findings after accounting for heterogeneity.Several subgroup analyses were conducted to assess the impact of study characteristics on the results.RESULTS Eleven cohort studies involving 4996352 participants,including 383917(7.7%)with prediabetes at baseline,were analyzed in this meta-analysis.Over a mean follow-up period of 6.5 years,the combined findings revealed that individuals with prediabetes at baseline had a higher likelihood of developing CRC than those with normoglycemia[risk ratio(RR)=1.18,95%confidence interval=1.11 to 1.25,P<0.001]with low statistical heterogeneity(I2=27%).Subgroup analyses indicated that the association between prediabetes and an increased risk of CRC was mainly observed in studies defining prediabetes using impaired fasting glucose(RR=1.24)and slightly elevated hemoglobin A1c levels(RR=1.18)but not in those that defined prediabetes using impaired glucose tolerance(RR=1.06).Other study characteristics such as design,country,participant age and sex,the duration of follow-up,or adjustment for body mass index did not significantly impact the results(all P>0.05).CONCLUSION People with prediabetes might have a higher likelihood of developing CRC than individuals with normoglycemia.

文摘Objective:Both allogeneic hematopoietic stem cell transplantation(allo-HSCT)and autologous HSCT(ASCT)are important therapies for extranodal natural killer/T-cell lymphoma(ENKTCL);however,no large-scale,multicenter study has compared the efficacy and safety between allo-HSCT and ASCT in these patients.Our multicenter,real-world study aimed to evaluate the outcomes of allo-HSCT vs.ASCT as consolidation in ENKTCL patients who had achieved a complete response(CR)or partial response(PR).Methods:This was a multicenter,retrospective study with nine hospitals in China,and 114 patients with ENKTCL were enrolled.Sixty patients received ASCT and 54 received allo-HSCT.The primary outcome was progression-free survival(PFS).In the sensitivity analysis,propensity score matching(PSM)analyses were conducted to adjust for baseline prognostic factors.Landmark analysis were conducted to minimize immortal-time bias.Results:Patients in the allo-HSCT group presented with more adverse prognostic factors.Allo-HSCT group showed a significantly better PFS and a lower disease progression rate compared with ASCT group in patients with Ann Arbor stageⅢ/Ⅳdisease(PFS:100%vs.82.0%,P=0.023;disease progression rate:0 vs.25.4%,P=0.024),those with intermediate/high prognostic index of natural killer lymphoma(PINK)scores(PFS:100%vs.84.4%,P=0.034;disease progression rate:0 vs.22.1%,P=0.034),those with intermediate/high international prognostic index(IPI)scores(PFS:100%vs.82.0%,P=0.038;disease progression rate:0 vs.25.4%,P=0.038),or those receiving HSCT at PR(PFS:100%vs.50%,P=0.046;disease progression rate:0 vs.50%,P=0.046)at the 1.5-4.0 follow-up.In multivariate analysis,receiving ASCT was significantly associated with a poorer PFS[hazard ratio(HR)=2.23,P=0.038]and overall survival(OS)(HR=2.45,P=0.045).In the sensitivity analysis,patients receiving allo-HSCT showed a significantly better PFS(70.3%vs.39.1%,P=0.039),OS(73.9%vs.42.0%,P=0.044),and a lower disease progression rate(22.6%vs.57.0%,P=0.017)compared with those receiving ASCT after propensity score matching.Conclusions:ENKTCL patients with high-risk characteristics could benefit more from allo-HSCT as consolidation.

文摘Background:Cardiovascular and cerebrovascular diseases are significant health threats that have been found to involve ferroptosis in related cells.This study aims to comprehensively examine the role and mechanism of natural products,derived from traditional Chinese herbal medicines,in intervening in ferroptosis from the perspective of cardiovascular and cerebrovascular aspects.Methods:A literature survey was conducted using Scopus,Pubmed,Reaxys,and Google Scholar databases to review studies on natural products(such as baicalin,puerarin,and ginsenosides)derived from herbs like scutellaria baicalensis,pueraria lobata,ginseng,panax quinquefolium,and panax notoginseng.The focus was on their potential as drugs for the treatment of cardiovascular and cerebrovascular diseases,particularly their ability to inhibit ferroptosis.Results:The review revealed that various natural products exhibit disease protection and prevention properties,inhibiting ferroptosis through diverse pathways and targets in cardiovascular and cerebrovascular diseases.The involvement of these natural products in key pathways associated with ferroptosis,including amino acid metabolism,iron metabolism,and lipid peroxidation,was elucidated.Notably,tanshinone IIA,resveratrol,astragaioside IV,and vitexin were found to regulate ferroptosis in both cardiovascular and cerebrovascular cells.Additionally,the impact of these natural products on key targets of ferroptosis,such as SLC7A11,DMTI,TfRI,Nrf2,HO-1,GPX4,NCOA4,and LOX,was discussed.Conclusion:This study provides a comprehensive understanding of the role and mechanism of natural products in intervening in ferroptosis related to cardiovascular and cerebrovascular diseases.The findings pave the way for potential future treatments targeting ferroptosis in these diseases.

文摘Objective:To investigate the incidence of delirium in elderly patients with Stanford-type B aortic dissection and analyze its risk factors.Methods:A convenience sample of 767 elderly patients with Stanford-type B aortic dissection admitted to the ICU from January 2020 to December 2023 was selected.Data were collected using a delirium-related questionnaire and the Confusion Assessment Method for the Intensive Care Unit(CAM-ICU).Results:The incidence of delirium in elderly Stanford B aortic dissection patients was 23.73%.Logistic regression analysis showed that gender,length of stay in the ICU,and duration of sedative drug use were independent risk factors for delirium in elderly patients(P<0.05).The model likelihood ratio test x^(2)=28.462,P<0.001;Hosmer-Lemeshow goodness-of-fit test x^(2)=0.715,P=0.878.Conclusion:The incidence of delirium in elderly patients with Stanford-type B aortic dissection is relatively low.Medical staff should conduct adequate and effective preoperative assessment according to the condition of elderly Stanford-type B aortic dissection patients,and use analgesic and sedative drugs reasonably to create a good treatment environment for patients,thereby minimizing the incidence of delirium in elderly patients with Stanford-type B aortic dissection as much as possible.
文摘Finger reimplantation is a pivotal technique in microsurgery,playing an irreplaceable role in restoring the functional integrity of fingers,maintaining the aesthetic appearance of the hand,ensuring the patient’s ability to perform delicate hand operations,and enhancing their social participation and quality of life.This paper reports on the perioperative nursing approach employed in a successful case of replantation involving a single hand with a multi-planar,20-segment amputation of four fingers.By establishing a specialized nursing team,conducting a comprehensive assessment of the patient’s condition,formulating a personalized nursing plan,and implementing precise nursing care,we achieved successful outcomes.After nearly four weeks of meticulous diagnosis,treatment,and nursing,17 segments of the replanted fingers survived,preserving the functional length of the amputated fingers and maximizing the restoration of hand function.This nursing method provides valuable insights and approaches for effectively managing vascular crises following digital replantation,improving the replantation survival rate,and enhancing patient satisfaction.
文摘Objective:This study aims to investigate the patterns of symptom occurrence in patients experiencing acute exacerbations of chronic obstructive pulmonary disease(AECOPD).It will explore the composition of symptom clusters and analyze the correlation between these clusters and health-related quality of life(HRQoL).Methods:A total of 207 patients with AE-COPD were surveyed from a tertiary grade A hospital.Data collection was conducted using three validated instruments:the Basic Information Questionnaire(BIQ),Disease Symptom Survey Questionnaire(MSAS),and Quality of Life Questionnaire(CAT).Statistical software SPSS 22.0 was used to analyze the correlation between symptom clusters and quality of life.Results:Exploratory factor analysis showed that five major symptom clusters existed in the patients,including the psycho-emotional symptom cluster,the sleep-related symptom cluster,the other side effects symptom cluster,the energy deficiency symptom cluster and the cough-loss of appetite symptom cluster,and the severity of the symptom clusters showed a significant negative correlation with the quality of life of the patients(P<0.05).Conclusion:Strengthening the comprehensive management of symptom clusters in patients with AE-COPD can help to effectively reduce the symptom burden of patients,and then significantly improve their quality of life.

文摘In order to improve the deep learning training efficiency of the large reflector antenna active adjustment technique,this paper synthesizes the characteristic that each actuator can only adjust the panel connected to it,and proposes a divided-ring antenna active adjustment deep learning training modeling method.The method organizes panel node data according to actuator ring positions,using panel displacements as input features and actuator adjustments as output labels.Through systematic sorting,reorganization,and normalization,the ring-divided data are transformed into grid-structured tensors suitable for convolutional processing.Multi-layer convolutional neural networks are then constructed for surface adjustment prediction,optimized through a hybrid strategy combining simulated annealing and the Adam algorithm.Through the dataset divided-ring preprocessing,active adjustment neural network construction and model training for the case of an 8 m reflector antenna,the analytical results show that the proposed method can effectively shorten the training time,and the final model's prediction accuracy is greatly improved,which demonstrates the feasibility and effectiveness of the proposed method.

文摘The East Asian Very Long Baseline Interferometry(VLBI) Network(EAVN) is a rapidly evolving international VLBI array that is currently promoted under joint efforts among China,Japan and Korea.EAVN aims at forming a joint VLBI Network by combining a large number of radio telescopes distributed over East Asian regions.After the combination of the Korean VLBI Network(KVN) and the VLBI Exploration of Radio Astrometry(VERA) into Ka VA,further expansion with the joint array in East Asia is actively promoted.Here we report the first imaging results(at 22 and 43 GHz) of bright radio sources obtained with Ka VA connected to Tianma 65-m and Nanshan 26-m Radio Telescopes in China.To test the EAVN imaging performance for different sources,we observed four active galactic nuclei(AGN) having different brightness and morphology.As a result,we confirmed that the Tianma 65-m Radio Telescope(TMRT) significantly enhances the overall array sensitivity,a factor of 4 improvement in baseline sensitivity and 2 in image dynamic range compared to the case of Ka VA only.The addition of the Nanshan 26-m Radio Telescope(NSRT) further doubled the east-west angular resolution.With the resulting high-dynamic-range,high-resolution images with EAVN(Ka VA+TMRT+NSRT),various fine-scale structures in our targets,such as the counter-jet in M87,a kink-like morphology of the 3 C 273 jet and the weak emission in other sources are successfully detected.This demonstrates the powerful capability of EAVN to study AGN jets and to achieve other science goals in general.Ongoing expansion of EAVN will further enhance the angular resolution,detection sensitivity and frequency coverage of the network.
文摘BACKGROUND Low anterior resection syndrome(LARS)is a prevalent and debilitating complication following sphincter-preserving surgery for rectal cancer.Evidence-based interventions for the concurrent psychological burden are limited.Electroacupuncture has been proposed as a potential adjunctive therapy,but its psychological benefits remain inadequately studied.AIM To investigate the therapeutic effect of electroacupuncture on emotional recovery and gastrointestinal function in patients with moderate to severe LARS,and to explore its potential advantages in psychologically vulnerable subgroups.METHODS We conducted a retrospective,controlled study involving 100 patients with moderate to severe LARS(LARS score≥21)treated at two tertiary hospitals in China between January 2022 and December 2024.Patients received either standard postoperative care alone(n=50)or in combination with a standardized 4-week electroacupuncture protocol(n=50).Psychological and functional outcomes were assessed using validated instruments including Hospital Anxiety and Depression Scale(HADS),Body Image Scale(BIS),General Self-Efficacy Scale,Perceived Social Support Scale(PSSS),LARS score,and European Organization for Research and Treatment of Cancer Quality of Life Questionnaire Core 30 at four time points.The primary endpoint was emotional remission,defined as a≥3-point reduction in HADS-Anxiety subscale(HADS-A).Analyses included repeated-measures comparisons,Kaplan-Meier survival curves,Cox regression models,and subgroup-interaction testing.RESULTS At baseline,demographic,surgical,and psychosocial characteristics were comparable among groups.By week 4,patients receiving electroacupuncture demonstrated significantly greater reductions in anxiety(HADS-A:4.8±2.6 vs 7.3±3.0;P<0.001),depression,and body-image disturbance(BIS:8.7±3.6 vs 11.9±4.2;P<0.001),alongside enhanced coping capacity(Brief Coping Orientation to Problems Experienced),perceived social support(PSSS),and bowel function(LARS score).Emotional remission-defined as a≥3-point HADS-A reduction-was achieved more rapidly in the electroacupuncture group,as confirmed by Kaplan-Meier analysis(log-rank P<0.001;odds ratio=4.7).Multivariate Cox regression identified higher baseline LARS and BIS scores as independent predictors of delayed emotional recovery.Subgroup analyses revealed significantly amplified treatment benefits in patients with high baseline anxiety(HADS-A≥8),elevated body-image disturbance(BIS≥12),or low perceived social support(PSSS<60),with consistent interaction effects(P for interaction<0.05 across subgroups).CONCLUSION Electroacupuncture may accelerate emotional recovery and improve functional and psychosocial outcomes in patients with LARS.Its integration into postoperative care may offer particular benefits for psychologically vulnerable subgroups.

文摘Observing and timing a group of millisecond pulsars with high rotational stability enables the direct detection of gravitational waves(GWs).The GW signals can be identified from the spatial correlations encoded in the times-of-arrival of widely spaced pulsar-pairs.The Chinese Pulsar Timing Array(CPTA)is a collaboration aiming at the direct GW detection with observations carried out using Chinese radio telescopes.This short article serves as a“table of contents”for a forthcoming series of papers related to the CPTA Data Release 1(CPTA DR1)which uses observations from the Five-hundred-meter Aperture Spherical radio Telescope.Here,after summarizing the time span and accuracy of CPTA DR1,we report the key results of our statistical inference finding a correlated signal with amplitude logA_(c)=-14.4_(-2.8)^(+1.0)for spectral index in the range ofα∈[-1.8,1.5]assuming a GW background(GWB)induced quadrupolar correlation.The search for the Hellings–Downs(HD)correlation curve is also presented,where some evidence for the HD correlation has been found that a 4.6σstatistical significance is achieved using the discrete frequency method around the frequency of 14 n Hz.We expect that the future International Pulsar Timing Array data analysis and the next CPTA data release will be more sensitive to the n Hz GWB,which could verify the current results.

文摘Background:Many healthcare workers were infected by coronavirus disease 2019(COVID-19)early in the epidemic posing a big challenge for epidemic control.Hence,this study aims to explore perceived infection routes,influencing factors,psychosocial changes,and management procedures for COVID-19 infected healthcare workers.Methods:This is a cross-sectional,single hospital-based study.We recruited all 105 confirmed COVID-19 healthcare workers in the Zhongnan Hospital of Wuhan University from February 15 to 29,2020.All participants completed a validated questionnaire.Electronic consent was obtained from all participants.Perceived causes of infection,infection prevention,control knowledge and behaviour,psychological changes,symptoms and treatment were measured.Results:Finally,103 professional staff with COVID-19 finished the questionnaire and was included(response rate:98.1%).Of them,87 cases(84.5%)thought they were infected in working environment in hospital,one(1.0%)thought their infection was due to the laboratory environment,and 5(4.9%)thought they were infected in daily life or community environment.Swab of throat collection and physical examination were the procedures perceived as most likely causing their infection by nurses and doctors respectively.Forty-three(41.8%)thought their infection was related to protective equipment,utilization of common equipment(masks and gloves).The top three first symptoms displayed before diagnosis were fever(41.8%),lethargy(33.0%)and muscle aches(30.1%).After diagnosis,88.3%staff experienced psychological stress or emotional changes during their isolation period,only 11.7%had almost no emotional changes.Arbidol(Umifenovir;an anti-influza drug;69.2%)was the drug most commonly used to target infection in mild and moderate symptoms.Conclusion:The main perceived mode of transmission was not maintaining protection when working at a close distance and having intimate contact with infected cases.Positive psychological intervention is necessary.

文摘BACKGROUND Chronic atrophic gastritis(AG)with intestinal metaplasia(IM)significantly increases the risk of gastric cancer.Some medicines have showed definite therapeutic effects in AG and IM regression.AIM To validate the efficacy of Lamb’s tripe extract and vitamin B12 capsule(LTEVB12)initial therapy and celecoxib rescue therapy for IM and AG.METHODS A total of 255 patients were included to receive LTEVB12 initial therapy(2 capsules each time,three times daily for 6 mo)in hospital in this study.The patients with failure of IM regression continued to receive celecoxib rescue therapy(200 mg,once daily for 6 mo).After each therapy finished,the patients underwent endoscopy and biopsy examination.The regression efficiency was assessed by the operative link on gastritis assessment(OLGA)and the operative link on the gastric intestinal metaplasia assessment(OLGIM)staging system.Logistic regression analysis was applied to identify factors associated with the curative effect.RESULTS For LTEVB12 initial therapy,the reversal rates of IM and AG were 52.95%and 48.24%,respectively.Analogously,for celecoxib rescue therapy,the effective rates for IM and AG were 56.25%and 51.56%,respectively.The IM regression rate of complete therapy was up to 85.03%.In different OLGA and OLGIM stages of IM patients,therapeutic efficiency showed a significant difference in each group(P<0.05).For both therapies,patients with high stages(III or IV)of both the OLGA and OLGIM evaluation systems showed a higher IM or AG regression rate than those with low stages(I or II).Among patients with high stages(OLGIM III and IV),the IM regression rate was above 70%for each therapy.Eating habits,fresh vegetable intake,and high-salt diet were identified as independent factors for the IM reversal effect of LTEVB12 therapy,especially high-salt diet(odds ratio=1.852,P<0.05).CONCLUSION Monotherapy could reverse IM and AG.LTEVB12 initial therapy and celecoxib rescue therapy significantly increase the regression effect.IM may not be the point of no return among gastric precancerous lesions.

文摘To effectively improve the performance and expand the applications of polymers, molecular dynamics(MD) simulations with the COMPASS force field have been applied to predict the miscibility, glass transition temperature(Tg), and mechanical properties of poly(vinyl chloride)/dioctyl phthalate(PVC/DOP) blends. The solubility parameter values obtained are in good agreement with the reference data and the little difference(|Δδ|< 2.0 MPa0.5) between two components indicates that PVC/DOP is a miscible system. Tg is predicted by the slope of the free volume and density versus temperature simulation data based on density and free volume theory which is agree well with the experimental data. In addition, the analyses of mechanical properties results indicate that the values of Young’s modulus(E), bulk modulus(K), and shear modulus(G) decrease with the addition of DOP, demonstrating that the rigidity of material is weakened and the ductility is improved. The mechanical properties can also be effectively improved by increasing the temperature, which may provide a more flexible mixture, with lower E, K, G but an increased ductility.

文摘The novel severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)is the cause of a rapidly spreading illness,coronavirus disease 2019(COVID-19),affecting more than seventeen million people around the world.Diagnosis and treatment guidelines for clinicians caring for patients are needed.In the early stage,we have issued"A rapid advice guideline for the diagnosis and treatment of 2019 novel coronavirus(2019-nCoV)infected pneumonia(standard version)";now there are many direct evidences emerged and may change some of previous recommendations and it is ripe for develop an evidence-based guideline.We formed a working group of clinical experts and methodologists.The steering group members proposed 29 questions that are relevant to the management of COVID-19 covering the following areas:chemoprophylaxis,diagnosis,treatments,and discharge management.We searched the literature for direct evidence on the management of COVID-19,and assessed its certainty generated recommendations using the Grading of Recommendations,Assessment,Development and Evaluation(GRADE)approach.Recommendations were either strong or weak,or in the form of ungraded consensus-based statement.Finally,we issued 34 statements.Among them,6 were strong recommendations for,14 were weak recommendations for,3 were weak recommendations against and 11 were ungraded consensus-based statement.They covered topics of chemoprophylaxis(including agents and Traditional Chinese Medicine(TCM)agents),diagnosis(including clinical manifestations,reverse transcription-polymerase chain reaction(RT-PCR),respiratory tract specimens,IgM and IgG antibody tests,chest computed tomography,chest X-ray,and CT features of asymptomatic infections),treatments(including lopinavirritonavir,umifenovir,favipiravir,interferon,remdesivir,combination of antiviral drugs,hydroxychloroquine/chloroquine,interleukin-6 inhibitors,interleukin-1 inhibitors,glucocorticoid,qingfei paidu decoction,lianhua qingwen granules/capsules,convalescent plasma,lung transplantation,invasive or noninvasive ventilation,and extracorporeal membrane oxygenation(ECMO)),and discharge management(including discharge criteria and management plan in patients whose RT-PCR retesting shows SARS-CoV-2 positive after discharge).We also created two figures of these recommendations for the implementation purpose.We hope these recommendations can help support healthcare workers caring for COVID-19 patients.
文摘BACKGROUND:Biliary complications are a serious problem in patients after liver transplantation and often require reoperation.This study was conducted to summarize the endoscopic diagnosis and management of biliary complications after orthotopic liver transplantation(OLT).METHODS:From December 2000 to November 2003,twelve endoscopic retrograde cholangiopancreatographies(ERCPs)were performed in 7 patients after OLT at Digestive Endoscopic Center of Changhai Hospital in Shanghai,China.The therapeutic maneuvers included endoscopic sphincterotomy(EST),biliary stent placement,balloon and basket extraction,irrigation,and nasobiliary tube placement.A retrospective study was made to determine the types of biliary tract complications after OLT.The success of ERCP and therapeutic maneuvers was also evaluated.RESULTS:Biliary tract complications including biliary stricture,biliary leak,biliary sludge,and stump leak of the cyst duct were treated respectively by endoscopic sphincterotomy with sludge extraction,stricture dilation or endoscopic retrograde biliary drainage.Two of the 3 patients with proximal common bile duct stricture were successfully treated with ERCP and stent placement.Four patients with anastomotic stricture and/without bile leak were treated successfully by dilation and stent placement or endoscopic nosobiliary drainage.No severe ERCP-related complications occurred.CONCLUSIONS:ERCP is an effective and accurate approach for the diagnosis of biliary tract complications after OLT,and placement of a stent is a safe initial treatment for biliary complications after liver transplantation.
