BACKGROUND Advanced heart failure and transplant(AHFTC)teams are crucial in the management of patients in cardiogenic shock.We sought to explore the impact of AHFTC physicians on outcomes in patients receiving extracorporeal membrane oxygenation(ECMO)support.AIM To determine whether outcomes differ in the care of ECMO patients when AHFTC physicians serve in a primary vs consultative role.METHODS We conducted a retrospective cohort study of 51 patients placed on veno-venous(VV)and veno-arterial(VA)ECMO between January 2015 and February 2023 at our institution.We compared ECMO outcomes between teams managed primarily by intensivists vs teams where AHFTC physicians played a direct role in ECMO management,including patient selection.Our primary outcome measure was survival to 30 days post hospital discharge.RESULTS For combined VA and VV ECMO patients,survival to 30 days post discharge in the AHFTC cohort was significantly higher(67%vs 30%,P=0.01),largely driven by a significantly increased 30-day post discharge survival in VA ECMO patients in the AHFTC group(64%vs 20%,P=0.05).CONCLUSION This study suggests that patients in shock requiring VA ECMO support may have improved survival 30 days after hospital discharge when an AHFTC team serves in a direct role in the selection and management of patients.Further studies are needed to validate this impact. 展开更多

The key determinants of cloud computing provide a convincing argument for HEIs and its stakeholders to adopt the innovation. These benefits reflect the essential quality characteristics of the cloud, such as Broad network Access;Measured Service;On-demand Self-Service;Rapid Elasticity;and Resource Pooling. However, there are also risks associated with the cloud, leading to non-adoption, such as Confidence, Privacy, Security, Surety and Trust. Understanding the impact of these factors can support multiple stakeholders, such as students, lecturers, senior managers and admins in their adoptive decision of CC in their respected institutions. Using the Multiview 3 (MV3) methodology, a research model was proposed to explore the key qualities and risks that determine the adoption or non-adoption of CC by UK HEIs from multiple perspectives. An exploratory qualitative study was conducted on 32 University stakeholders across 2 UK Universities. The findings suggest that security, privacy and trust are the key determinants to non-adoption as participants felt that the cloud cannot fully guarantee the safeguarding of sensitive information. Determinants to cloud adoption include improving relationships between students and teachers via collaborative tools, in addition to proposing cloud apps for mobile devices for accessing virtual learning materials and email securely off-campus. In conclusion, University stakeholders are still at a cross-road when it comes to cloud adoption, but future advances of the cloud may help to steer their decision to adopt this innovative technology given its overwhelming potential. 展开更多

MoBShield:A Novel XML Approach for Securing Mobile Banking

Mobile banking security has witnessed significant R&D attention from both financial institutions and academia.This is due to the growing number of mobile baking applications and their reachability and usefulness to society.However,these applications are also attractive prey for cybercriminals,who use a variety of malware to steal personal banking information.Related literature in mobile banking security requiresmany permissions that are not necessary for the application’s intended security functionality.In this context,this paper presents a novel efficient permission identification approach for securing mobile banking(MoBShield)to detect and prevent malware.A permission-based dataset is generated for mobile banking malware detection that consists large number of malicious adware apps and benign apps to use as training datasets.The dataset is generated from 1650 malicious banking apps of the Canadian Institute of Cybersecurity,University of New Brunswick and benign apps from Google Play.A machine learning algorithm is used to determine whether amobile banking application ismalicious based on its permission requests.Further,an eXplainable machine learning(XML)approach is developed to improve trust by explaining the reasoning behind the algorithm’s behaviour.Performance evaluation tests that the approach can effectively and practically identify mobile banking malware with high precision and reduced false positives.Specifically,the adapted artificial neural networks(ANN),convolutional neural networks(CNN)and XML approaches achieve a higher accuracy of 99.7%and the adapted deep neural networks(DNN)approach achieves 99.6%accuracy in comparison with the state-of-the-art approaches.These promising results position the proposed approach as a potential tool for real-world scenarios,offering a robustmeans of identifying and thwarting malware inmobile-based banking applications.Consequently,MoBShield has the potential to significantly enhance the security and trustworthiness of mobile banking platforms,mitigating the risks posed by cyber threats and ensuring a safer user experience. 展开更多

The influence of cooling rate on the microstructure and mechanical properties of two new ultrahigh-strength steels(UHSSs)with different levels of C,Cr and Ni has been evaluated for the as-cooled and untempered condition.One UHSS had higher contents of C and Cr,while the other one had a higher Ni content.On the basis of dilatation curves,microstructures,macrohardness and microhardness,continuous cooling transformation diagrams were constructed as a guide to heat treatment possibilities.Cooling rates(CRs)of 60,1 and 0.01°C/s were selected for more detailed investigations.Microstructural characterization was made by laser scanning confocal microscopy,field emission scanning electron microscopy combined with electron backscatter diffraction,electron probe microanalysis and X-ray diffraction.Mechanical properties were characterized using macrohardness,tensile and Charpy V-notch impact tests.UHSS with the higher C and Cr contents showed lower transformation temperatures and slower bainite formation kinetics than that with the higher Ni content.Higher cooling rates led to lower volume fractions and carbon contents of retained austenite together with finer prior austenite grain size,as well as effective final grain size and lath size.These changes were accompanied by higher yield and tensile strengths.The best combinations of strength and toughness were obtained with martensitic microstructures and by avoiding the formation of granular bainite accompanied by proeutectoid carbides at low CR.For the cooling rates studied,UHSS with the higher C and Cr contents showed the higher hardness and strength but at the cost of toughness. 展开更多

Drought stress(DS)is one of the most critical environmental abiotic stresses for wheat production in the arid environments.Selection of high-yielding genotypes tolerant to DS can play a significant role in mitigation the negative impacts associated with DS.In the present study,generation means analysis(GMA)was used to study the performance of two crosses under well irrigation(WI)and deficit irrigation[cross I(Line 44×Shandweel-1)and cross II(Line 20×Sakha 93)].Significant differences were observed for days to heading(DH),days to maturity(DM),plant height(PH),spike length(SL),number of spikes per plant(NS/P),number of grains per spike(NG/S),thousand-grain weight(TGW),grain yield per plant(GY/P),and proline content(PC)in the six populations of the two crosses within each irrigation level.Cross II had early maturity and the highest PC,NS/P,TGW,and GY/P regardless of the irrigation level.Cross I showed positive significant relative heterosis and heterobeltiosis for GY/P under the two irrigation levels.The inheritance of characters of cross I revealed additive,dominant,and epistatic effects,which varied with trait and stress.Additive genetic effects predominated in DH,SL,and PC,while non-additive were found in DM,NS/P,NG/S,and GY/P.Narrow-sense heritability estimates(h^(2) n)were high for DH and PC,moderate to high for PH and SL,moderate for DM,NG/S,NS/P,and TGW,and low for GY/P.Based on different drought indices the populations BC_(1),BC_(2),F_(1),and P1 of cross II and BC1 of cross I were more tolerant to drought stress.Therefore,PC,TGW and DH can be used as selection indicators to improve wheat for drought tolerance in early generations and other yield components traits in late generations.The second cross(Line 20×Sakha 93)shows promise and is of interest to a drought tolerance breeding program,where wheat breeders can use recombinant breeding strategies to construct desirable drought stress genes.Correlation and path coefficient revealed that TGW and PC were the main contributor in grain yield in both environments. 展开更多

A Systematic Review of Factors Influencing Medication Adherence to Hypertension Treatment in Developing Countries 被引量:1

Background: Coronary heart disease (CHD), a complication of hypertension, is one of the most important and common causes of morbidity, hospitalisation, and mortality among hypertensive population. In recent decades, increased urbanisation and changes to lifestyle, diet and physical activity in developing countries have led to a major increase in the population incidence of chronic diseases including CHD. Poor medication adherence is one of the leading causes of failure to achieve hypertension control. The objective of this systematic review is to describe the prevalence of non-adherence to anti-hypertensive medications among hypertensive population in developing countries and identify factors associated with it. Methods: A literature search was conducted using the following scholarly electronic databases: Proquest, PubMed, JSTOR and Science Direct. The online search engine, Google Scholar was also used to search for and identify relevant papers. Peer-reviewed full-text articles published in English on hypertensive adults in developing countries that measured adherence to antihypertensive medications and their associations with different factors were eligible for inclusion. The review followed the PRISMA reporting and analytical guidelines for systematic reviews. Results: In all, 42 studies conducted across 19 developing countries were selected for the review. The mean prevalence of medication non-adherence (MNA) among the select hypertensive population was 47.34%. Very few studies were conducted in community settings and except for one, no study examined gender differences in MNA factors. The analysis revealed a range of factors that can influence MNA including low household income and socioeconomic status;knowledge and beliefs of hypertension and its management;avoiding side effects of medications;cost of medication;use of herbal preparations;absence of symptoms;irregular follow-up;and dissatisfaction with the treatment and health services provided. There was a general lack of consideration of role of health system in health care delivery, self-efficacy, cultural barriers, perceived individual risk of hypertension complications. There was also a lack of gender-specific research which is necessary at community settings given the social and economic vulnerabilities faced by women in developing countries that may affect adherence to antihypertensive medications. Conclusions: Future research in developing countries should consider individual risk perceptions, cultural barriers, gender and the role of local health system in health care delivery when assessing MNA among hypertensive population at community settings. 展开更多

The incidence and prevalence is unknown, may be due to under reporting or under diagnosis. Gastric volvulus may be transient producing few symptoms. The Borchardt’s Triad may be present only during an acute presentation. Common symptoms may mislead to diagnose a nonsurgical disease if an evaluation is not done, keeping in mind a possibility of gastric volvulus, even if a UGI scopy is normal. Cases may be submerged in the community being undiagnosed. Case Series: CASE 1: 21 yrs old male with intermittent abdominal pain for 1(1/2) yrs with marfanoid habitus, MVP and a normal UGI scopy. BMS revealed an Organo-Axial Volvulus and ligament laxity per-operatively. CASE 2: 65 yrs old diabetic female with vomiting and abdominal pain for 3 months and left sided pneumonitis. UGI scopy showed twisted gastric folds immediately below OGJ and inability to visualise antrum. BMS revealed mixed volvulus with paraesophageal herniation of distal stomach. Per-operatively there was laxity of ligaments with omental content alone within the diaphragmatic rent. Posterior retrocolic sub-mucosal gastrojejunostomy(pexy) was done for all cases. Conclusion: Gastric volvulus should be thought of in a case of chronic intermittent abdominal pain with normal baseline evaluation. A Chest X-ray and BMS should be done, at the time of symptoms. 展开更多

Increasing demands for ultrahigh-strength steels in commercial as well as military applications have raised interest in finding alternatives to the high-cost high-alloyed steel and super-alloys currently used, e.g. the use of economic low-alloy compositions processed via low-cost air induction melting and electroslag refining (ESR). In this work the yield of alloying elements and the removal of the impurities nitrogen, sulphur and phosphorus as a result of electroslag refining (ESR) in a newly developed CrNiMoWMnV ultrahigh-strength steel (UHSS) have been studied in relation to their activities in the molten metal pool. Six experimental heats of CrNiMoWMnV UHSS with different chemical compositions were designed, melted in an induction furnace (IF) and refined using ESR. This was followed by hot forging of the ingots at 1100°C to 950°C. ESR using a CaF2-CaO-Al2O3 slag system led to a high yield in Cr, Ni, Mo, W, Mn and V, while the yield of Si is low. The desulphurization of all six UHSS grades was pronounced with most of the sulphur removed either to the slag or by gas reactions. The degree of dephosphorization was only 5% irrespective of the steel composition. On the other hand, denitrification (removal of nitrogen) was achieved. It ranged from 8% to 63% depending on the steel composition. The yield of the alloying elements and removal of impurities from the steel during ESR depends on the chemical and physical properties of the ESR slag and the activity of the elements in the molten state, taking into account elemental interactions. 展开更多

The last decade witnesses a heave use of social media-based information systems in different fields of business such as logistics, procurement, and supply chain management. Managing these types of information systems, could help companies to outsource their supply chain functions in a global scale and enhance their competitive advantages. However, the digital performance of these activities inherent risks of inappropriate supplier selection, lack of trust, bounded rationality and uncertainty about the supply conditions (e.g., pricing, shipping and timing). To address such challenges, this research explains how companies use e-reputation systems and social media to select their global trusted suppliers. Based on two-case evidence from British Diamond and Egyptian Automotive companies, the researchers conducted 20 interviews with purchasing and supply chain professionals. Chen & Lin’s reputation system model has been adopted to explain the process of selecting and evaluating a trusted supplier and to inform our data analysis. Our findings referred to the lack of experience among diamond professionals on how to use e-reputation systems and their lack of understanding to the role of social media-based ratings or referrals during the stages of selection suppliers’ discovery and approval. Though, automotive professionals find e-reputation system a strong tool to build goodwill, tacit credibility, competence and predictable trust. Ironically, both cases confirm that supply chain professional use these systems to re-evaluate and reselect their existing suppliers than to extend new supply networks. 展开更多

In this paper,the generalized dissipative Kawahara equation in the sense of conformable fractional derivative is presented and solved by applying the tanh-coth-expansion and sine-cosine function techniques.The quadratic-case and cubic-case are investigated for the proposed model.Expected solutions are obtained with highlighting to the effect of the presence of the alternative fractional-derivative and the effect of the added dissipation term to the generalized Kawahara equation.Some graphical analysis is presented to support the findings of the paper.Finally,we believe that the obtained results in this work will be important and valuable in nonlinear sciences and ocean engineering. 展开更多

The use of nanotechnology in nanoparticle-based cancer therapeutics is gaining impetus due to the unique biophysical properties of nanoparticles at the quantum level. Silver nanoparticles (AgNPs) have been reported as one type of potent therapeutic nanoparticles. The present study is aimed to determine the effect of AgNPs in arresting the growth of a murine fibrosarcoma by a reductive mechanism. Initially, a bioavailability study showed that mouse serum albumin (MSA)-coated AgNPs have enhanced uptake; therefore, toxicity studies of AgNP-MSA at 10 different doses (1-10 mg/kg b.w.) were performed in LACA mice by measuring the complete blood count, lipid profile and histological parameters. The complete blood count, lipid profile and histological parameter results showed that the doses from 2 to 8 mg (IC50: 6.15 mg/kg b.w.) sequentially increased the count of leukocytes, lymphocytes and granulocytes, whereas the 9- and 10-mg doses showed conclusive toxicity. In an antitumor study, the incidence and size of fibrosarcoma were reduced or delayed when murine fibrosarcoma groups were treated by AgNP-MSA. Transmission electron micrographs showed that considerable uptake of AgNP-MSA by the sentinel immune cells associated with tumor tissue and a morphologically buckled structure of the immune cells containing AgNP-MSA. Because the toxicity studies revealed a relationship between AgNPs and immune function, the protumorigenic cytokines TNF-α, IL-6 and IL-1β were also assayed in AgNP-MSA-treated and non-treated fibrosarcoma groups, and these cytokines were found to be downregulated after treatment with AgNP-MSA. 展开更多

Purpose: Tension band wiring is commonly used for fixation of simple transverse fractures. The popular configuration is parallel Kirschner wires (K-wires) and a stainless steel wire loop placed in a vertically oriented figure-of-8. Methods: We used a wooden model of a patella with a midway transverse fracture and compared four different types of fixation. The first construct had a vertical figure-of-8 with one twist of wire. The second contained a vertical figure-of-8 with two twists of wire. The third was a vertical figure-of-8 with two twists of wire placed at adjacent corners while the last one had a horizontal figure-of-8 with two twists of wire placed at adjacent corners, lnterfragmentary compression at the point of wire breakage was measured for each construct as well as permanent displacement on cyclic loading. Results: Placement of the figure-of-eight in a horizontal orientation with two wire twists at the corner improved interfragmentary compression by 63% (p 〈 0.05, Tukey post ]hoc test). On cyclic loading, all the constructs with vertical figure-of-eight but none with a horizontal construct failed (p = 0.01; Fisher's exact test). Permanent fracture displacement after cyclic loading was ,57% lower with horizontal figure- of-eight constructs (p 〈 0.05; t test). Conclusion: Placing wire twists at the corner and a horizontal placement of figure-of-8 improves stability of the construct. 展开更多

In this paper,new explicit unidirectional wave solutions for the modified-mixed KdV equation and bidi-rectional waves for the Benjamin Ono equation are studied.New extension of the rational sine-cosine and sinh-cosh methods are used.In addition,an extension of the recent polynomial function method is applied as well.Different types of topological and non-topological soliton solutions are extracted to the proposed models and categorized by providing 2D and 3D graphs.Finally,some physical properties of the new bidirectional waves solutions to the Benjamin Ono model are discussed. 展开更多

Natural antioxidants play a significant role to prevent cell breaking and regenerate cells.Three plants native to India named dwarf morning-glory(Evolvulus alsinoides),Indian banyan(Ficus bengalensis)and chicory(Chicorium intybus)are used as medicine in India traditionally to treat diseases.The aim of this study is to evaluate and compare the antioxidant activity as well as to compare phytochemical profiles of the aqueous extracts of the selected three plants.The dried plant powder was extracted with ethanol individually by using the Soxhlet method for eight hours and the solvent was evaporated under reduced pressure.Similarly,the air-dried powder was ex-tracted with water,and the water was evaporated.The thin layer chromatography(TLC)profile of the extracts was determined by using TLC densitometer and the antioxidant activity was deter-mined by the conventional 2,2-diphenyl-1-picrylhydrazyl(DPPH)method.The TLC profile shows that there are no similarities of retention factor(R f)values among the plant extracts which means all three plants contain different chemical compounds.The antioxidant activities of the aqueous extracts at different concentrations show different antioxidant activities.Percentage inhibition is dose dependent,as doses are increasing then percentage inhibition is also increasing for all three plant extracts.Among the selected plants,the highest activity was found in C.intybus and the lowest was in E.alsinoides.The order of the antioxidant activity among the selected plants was C.intybus<F.bengalensis<E.alsinoides.Based on the TLC profile and antioxidant activities,the highest activity of the selected plant species was evaluated. 展开更多
