AIM:To evaluate the differences and consistency of vault measurements obtained by Scheimpflug tomography(Pentacam),anterior segment optical coherence tomography(AS-OCT,CASIA II),and ultrasound biomicroscopy(UBM)follow...AIM:To evaluate the differences and consistency of vault measurements obtained by Scheimpflug tomography(Pentacam),anterior segment optical coherence tomography(AS-OCT,CASIA II),and ultrasound biomicroscopy(UBM)following implantable collamer lens(ICL)V4c implantation.METHODS:Vault measurements were acquired using three modalities:Pentacam,CASIA II AS-OCT,and UBM.Repeated-measures analysis of variance was used to compare the vault values obtained by the three devices.The correlation and consistency of measurements among the three instruments were assessed using the Pearson correlation coefficient,intraclass correlation coefficient(ICC),and Bland-Altman plots.RESULTS:This retrospective study enrolled 210 myopic eyes of 210 patients(158 women and 52 men)who underwent ICL implantation:108 eyes had a myopic ICL V4c implanted,and 102 eyes had a toric ICL V4c implanted.The mean vault values measured by Pentacam,CASIA II,and UBM were 452.64±204.20μm,538.57±203.54μm,and 560.95±227.54μm,respectively,with statistically significant differences among the three groups(P<0.05).Pearson correlation analysis showed strong positive correlations between vault values measured by different instruments(all P<0.001).ICC results indicated good consistency among the three measurement modalities(all P<0.001).Stratified analysis revealed that when the vault value was≤250μm,the correlation and consistency of measurements across the three instruments were lower than those in the medium and high vault subgroups.CONCLUSION:Vault values measured by Pentacam are lower than those obtained by CASIA II and UBM,with UBM yielding the highest mean vault values.Measurements from the three instruments are not interchangeable but can serve as mutual references due to their significant correlation and good overall consistency.Pentacam and CASIA II demonstrate the highest consistency in vault measurement.Notably,when the vault value is≤250μm,the consistency between Pentacam and the other two instruments decreases significantly.展开更多
Objective:To evaluate the predictive values of gene expressions of ribonucleotide reductase M1(RRM1) and breast cancer susceptibility gene 1(BRCA1) in peripheral blood from Chinese patients with non-small-cell lung ca...Objective:To evaluate the predictive values of gene expressions of ribonucleotide reductase M1(RRM1) and breast cancer susceptibility gene 1(BRCA1) in peripheral blood from Chinese patients with non-small-cell lung cancer(NSCLC) treated with gemcitabine plus platinum.Methods:Forty Chinese patients with advanced NSCLC were recruited and received gemcitabine 1200 mg/m 2 on Days 1 and 8 plus carboplatin AUC 5 on Day 1.RRM1 and BRCA1 expression levels in peripheral blood were detected by quantitative reverse transcription-polymerase chain reaction(RT-PCR) .Kaplan-Meier survival curve and log-rank test were performed to evaluate the correlation between gene expression and overall survival for these subjects.Results:No correlation was observed between gene expression of RRM1 and that of BRCA1(P>0.05) ,but there was a strong correlation between the expression of RRM1 and the response to chemotherapy(P=0.003) .Subjects with low RRM1 expression levels in peripheral blood had longer sur-vival time than those with high RRM1 expression levels(16.95 vs.12.76 months,log-rank 3.989,P=0.046) .However,no significant association between BRCA1 expression levels and survival time was found(16.80 vs.13.77 months,log-rank 0.830,P=0.362) .Conclusions:Patients with low RRM1 expression levels in peripheral blood have a greater response to chemotherapy and longer survival time.Advanced NSCLC patients with low RRM1 expression levels may benefit from gemcitabine plus platinum therapy.RRM1 mRNA expression in peripheral blood could be used to predict the prognosis of NSCLC treated by gemcitabine and platinum.展开更多
AIM:To assess the efficacy of tenofovir disoproxil fumarate(TDF) in lamivudine(LAM)-resistant patients with a suboptimal response to LAM plus adefovir(ADV).METHODS:We retrospectively analyzed the efficacy of switching...AIM:To assess the efficacy of tenofovir disoproxil fumarate(TDF) in lamivudine(LAM)-resistant patients with a suboptimal response to LAM plus adefovir(ADV).METHODS:We retrospectively analyzed the efficacy of switching to tenofovir disoproxil fumarate in suboptimal responders to lamivudine plus adefovir.Charts were reviewed for LAM-resistant chronic hepatitis B(CHB) patients who visited the Zhejiang Province People's Hospital and The First Affiliated Hospital,College of Medicine,Zhejiang University,from June 2009 to May 2013.Patients whose serum hepatitis B virus(HBV) DNA remained detectable despite at least 6 mo of LAM plus ADV combination therapy were included.Patients with a suboptimal response to LAM plus ADV were randomized to switch to TDF monotherapy(300 mg/d orally; TDF group) or to continuation with LAM(100 mg/d orally) plus ADV(10 mg/d orally; LAM plus ADV group) and were followed for 48 wk.Serum HBV DNA was determined at baseline and weeks 4,12,24,36,and 48.HBV serological markers and biochemistry were assessed at baseline and weeks 12,24,and 48.Resistance surveillance and side effects were monitored during therapy.RESULTS:Fifty-nine patient were randomized to switch to TDF(n =28) or continuation with LAM plus ADV(n =31).No significant differences were found between the groups at baseline.Prior to TDF therapy,all patients had been exposed to LAM plus ADV for a median of 11 mo(range:6-24 mo).No difference was seen in baseline serum HBV DNA between the two groups [5.13 ± 1.08 log10 copies/m L(TDF) vs 5.04 ± 31.16 log10 copies/m L(LAM +ADV),P =0.639].There was no significant difference in the rates of achieving complete virological response(CVR) at week 4 between the TDF and LAM +ADV groups(17.86% vs 6.45%,P =0.24).The rate of achieving CVR in the TDF and LAM plus ADV groups was 75% vs 16.13% at week 12,82.14% vs 22.58% at week 24,89.29% vs 25.81% at week 36,and 96.43% vs 29.03% at week 48,respectively(P < 0.001).The rate of alanine aminotransferase normalization was significantly higher in the TDF than in the LAM plus ADV group at week 12(75% vs17.86%,P < 0.001),but not at week 24(78.57% vs 54.84%,P =0.097) or 48(89.26% vs 67.74%,P =0.062).Patients were hepatitis B e antigen(HBe Ag) positive at baseline.There was no significant difference in HBe Ag negativity between the TDF and LAM plus ADV groups at week 48(4% vs 0%,P =0.481).There were no drug-related adverse effects at week 48 in either group.CONCLUSION:Switching to TDF monotherapy was superior to continuous add-on therapy in patients with LAM-resistant CHB with a suboptimal response to LAM plus ADV.展开更多
Climate change has significantly altered the carbon and water cycles of terrestrial ecosystems.Climate change and the extended vegetation growing season enhanced ecosystem gross primary production(GPP).However,the rel...Climate change has significantly altered the carbon and water cycles of terrestrial ecosystems.Climate change and the extended vegetation growing season enhanced ecosystem gross primary production(GPP).However,the relative contributions of climate drivers and vegetation phenology to GPP remain unclear.Based on satellite-derived vegetation phenology and GPP datasets from 1982 to 2018,we investigated the spatiotemporal patterns of GPP and its drivers in the Jinsha River watershed.We found that the growing season GPP significantly increased from 1982 to 2018,which was primarily attributed to changes in the growing season length(GSL)and temperature.The effect of GSL on GPP was the highest(r=0.34),with its effect being larger than that of temperature,precipitation and radiation in 41%of the watershed.Importantly,the area where GPP was predominantly influenced by GSL increased by 12%in grid cells during the period of 2001-2018 compared with 1982-2000,indicating that GSL was playing an increasingly important role in driving GPP.Our findings highlight the dynamic responses of GPP to climate change and the associated phenological variations,which are crucial for improving the understanding of the terrestrial carbon balance.展开更多
