基于免疫炎症的肺动脉高压发病机制研究进展与治疗策略

肺动脉高压(pulmonary hypertension,PH)是一种恶性的心血管疾病,特征是肺动脉血压升高和血管重塑,导致右心室功能受损和生活质量下降。尽管有多种治疗策略,但PH的病理机制仍需进一步研究。免疫炎症与PH的发生和进展密切相关,在血管重... 肺动脉高压(pulmonary hypertension,PH)是一种恶性的心血管疾病,特征是肺动脉血压升高和血管重塑,导致右心室功能受损和生活质量下降。尽管有多种治疗策略,但PH的病理机制仍需进一步研究。免疫炎症与PH的发生和进展密切相关,在血管重塑过程中起关键作用。研究表明,免疫细胞在肺血管壁积聚,释放细胞因子激活炎症信号通路,影响PH的进展和临床结局。因此,深入探寻免疫炎症相关机制或许在PH治疗中具备重要意义。近年来,针对免疫炎症的PH研究进展包括免疫细胞和炎症因子研究、抗炎药物的联合使用和剂型改造、免疫抑制剂、中药单体研究和潜在靶点研究。基于免疫炎症的PH治疗策略有望改善患者预后和生活质量,但需要进一步研究以揭示免疫炎症的确切作用机制,并验证治疗的安全性和有效性。本文综述了免疫细胞、细胞因子和趋化因子在PH发展中的作用机制,着重探讨了抗炎和免疫抑制治疗策略及临床应用。 展开更多

目的:探讨重复经颅直流电刺激(tDCS)联合康复治疗对意识障碍患者促醒和改善痉挛的疗效。方法:回顾性分析2017年4月—2021年4月在兰州大学第二医院康复医学科入院的42例意识功能障碍患者,根据是否给予患者tDCS治疗分为观察组和对照组,每... 目的:探讨重复经颅直流电刺激(tDCS)联合康复治疗对意识障碍患者促醒和改善痉挛的疗效。方法:回顾性分析2017年4月—2021年4月在兰州大学第二医院康复医学科入院的42例意识功能障碍患者,根据是否给予患者tDCS治疗分为观察组和对照组,每组21例。对照组给予常规康复治疗,观察组在常规康复治疗的基础上加用tDCS,阳极置于左侧初级运动皮质区(M1),阴极置于右肩部,刺激电流强度2 mA,20 min/次。2组均每日治疗1次,每周5 d,共治疗6周。2组治疗前后采用改良昏迷恢复量表(CRS-R)和格拉斯哥昏迷量表(GCS)评估患者的意识水平,应用改良Ashworth量表(MAS)评估患者右侧屈肘肌肌张力的改变。结果:治疗前2组CRS-R和GCS评分差异无统计学意义(P>0.05);治疗后2组评分均较治疗前改善,而且观察组CRS-R和GCS评分均优于对照组,差异具有统计学意义(P<0.05);在临床疗效方面,观察组反应者为13例(61.90%),无反应者为8例(38.10%);对照组反应者为2例(9.52%),无反应者为19例(90.48%),2组比较差异有统计学意义(P<0.05)。治疗前2组患者右侧屈肘肌MAS评分差异无统计学意义(P>0.05);对照组治疗前后屈肘肌的MAS评分差异无统计学意义(P>0.05),观察组治疗前后屈肘肌的MAS评分差异有统计学意义(P<0.05);组间比较,治疗后观察组屈肘肌MAS评分优于对照组,差异有统计学意义(P<0.05)。结论:重复经颅直流电刺激联合康复治疗能够有效改善意识障碍患者的意识水平和痉挛状态。 展开更多

The information management system is developing towards the trend of networking, intelligence and integration, which leads to the change of management system and management mode. The visual information management system has created a new direction of enterprise information management. Through the network, the construction site and functional departments are combined with each other to timely reflect the actual information of each project, support decision-making, assist managers in supervision and control, so as to effectively allocate enterprise resources, which plays an important role in solving field problems, strengthening data collection and analysis, reducing decision-making errors and reducing enterprise risks. The promotion and application of visual information management system will certainly become an important part of enterprise information construction. This paper also puts forward the overall design of the examination and training system based on Wechat public platform in combination with the actual training needs of enterprises. The system is composed of six modules: Examination question management, article management, foreground user management, background user management, menu authority management and system configuration. The application of the system can further strengthen the training of the enterprise to the grass-roots management personnel, stimulate the employees learning enthusiasm and internal potential, improve the training effect, promote the employees to master the work skills and apply them to the actual work posts, improve the work efficiency, reduce the training cost, and lay a solid foundation for the development of the enterprise. 展开更多
