Randomized controlled trials(RCTs) regarding ARTs combining with CT for HF were eligible. The primary outcomes were changes in heart function classification(HFC) according to New York Heart Association class and left ventricular ejection fraction(LVEF). Risk of bias assessment was conducted by two independent authors.Pairwise and network meta-analyses were performed using STATA 13.0 and WinBUGS 1.4.3 software. Results: A total of 26 RCTs were enrolled for analyses, with 5 kinds of ARTs and 2116 patients in all. Pairwise meta-analyses showed that acupoint application(OR: 3.28, 95%CI[2.26, 4.76]), acupuncture(OR: 2.78, 95%CI[1.21, 6.41]), acupoint injection(OR: 3.33, 95%CI[1.85, 6.00]) and moxibustion(OR: 2.51,95%CI[1.02, 6.21]) could significantly improve HFC when they were used as add-on to CT. Acupoint application(MD: 3.57, 95%CI[ 1.45, 5.70]),acupuncture(MD: 7.75, 95%CI[2.33. 13.17]). acupoint injection(MD:4.81, 95%CI[2.99, 6.63]) and moxibustion(MD: 6.99, 95%CI[3.62, 10.36]) were significantly beneficial in improving LVEF. Network meta-analyses showed that acupoint injection(SUCRA = 70.0%) and acupuncture(SUCRA = 90.4%) respectively had the greatest probability in improving HFC and LVEF.Conclusion: Most of the included ARTs add-on to CT was effective in improving HFC and LVEF. Acupoint injection and acupuncture may respectively have better effect than others for HFC and LVEF. However, due to the small sample size and poor quality of the included studies, hence well-designed RCTs are needed to confirm our findings. This study was registered in PROSPERO, CRD42018087700.展开更多

METHODS:Pub Med,EMBASE,the Cochrane Library, and Web of Science was systematically searched to identify randomized controlled trials(RCTs) comparing degarelix(240/80 mg vs 240/160 mg) to the gonadotropin-releasing hormone agonists, goserelin and leuprolide, for the treatment of PCa. Two independent reviewers screened putative studies, assessed the risk of bias, and then extracted pertinent data. Analyses were performed using Review Manager 5.2. RESULTS:Seven papers from six RCTs,involving 1204 patients, were identified. The present meta-analysis showed that treatment with 240/160 mg degarelix is more effective and has fewer adverse events(AEs) relative to conventional 240/80 mg regimen. Degarelix significantly decreased International Prostate Symptom Scores [standardized mean differences(SMD) =-0.32,95%CI:-0.51 to-0.12,P = 0.02] and caused fewer AEs(SMD =-0.28,95%CI:-0.48 to-0.07,P = 0.008) than goserelin. Degarelix suppressed testosterone and prostate-specific antigen significantly faster than leuprolide. CONCLUSION:Degarelix is a useful option in the treatment of advanced PCa. Degarelix 240/160 mgregimen was superior to a 240/80 mg regimen. More rigorously designed RCTs are urgently needed to confirm the efficacy of degarelix.展开更多

Degarelix suppressed testosterone and prostate-specific antigen significantly faster than leuprolide. CONCLUSION:Degarelix is a useful option in the treatment of advanced PCa. Degarelix 240/160 mgregimen was superior to a 240/80 mg regimen. More rigorously designed RCTs are urgently needed to confirm the efficacy of degarelix.
文摘The study aimed to analyze the reporting and methodological quality of systematic reviews(SRs)/meta-analyses(MAs)of animal models to provide references for later studies and avoid the waste of medical resources.EMBASE and MEDLINE databases were searched from inception to November 2017,with no language restriction. Two reviewers selected inclusion dependently and extracted the basic characteristics.Review Manager 5.3,stata 12.0,and SPSS 21 software were used to conduct analyses.A total of 46 SRs/MAs were included. The results showed that the English databases with high retrieval frequency are PubMed/MEDLINE,EMBASE,and Web of Science.67.31%(31/46)of the articles reported the search strategy in the full text or the appendix.65.22%(30/46)reported the literature screening flow diagram, and only 19.57%(9/46)reported the number of works of literature retrieved in each database.60.87%(28/46)illustrated supplement retrieval.Through 2 subgroup analyses,it was found that there were no significant differences in the quality of reports of PRISMA items. But referring to the methodological quality or reporting of PRESS items,SCI was better than that of non-SCI,while there seemed a source of funding to have no significant impact on the methodological quality or the items of PRESS.The results of PRESS,AMSTAR 2,and PRISMA were correlated, and the correlation between PRISMA and AMSTAR 2 was strong. These results demonstrated that search strategies of animal model SRs/MAs are still not enough comprehensive, report specification and methodological quality still need to be ameliorated. To show users the scientificity and rigor of the study, future research should focus on these various guidelines like PRESS,PRISMA,and AMSTAR 2 checklists that have been issued,it can help to increase the value of research and improve the utilization of medical resources.
文摘Background:Irritable bowel syndrome(IBS)is a chronic functional bowel disorder characterized by abdominal pain and altered bowel function. The TongXieYaoFang(TXYF)is a Chinese herbal formula with a long history for the management of chronic diarrhea accompanied by abdominal pain, and it is always used to relieve symptoms of IBS.Although there are some studies were published, there is no consensus on its effects. Moreover, new relevant study was published recently. Therefore,a systematic review and meta-analysis is needed to assess the safety and eff ectiveness of the Chinese herbal medicine TongXieYaoFang for IBS.Methods and analysis:PubMed,EMBASE,the Cochrane library,CNKI,WanFang,VIP and CBM databases will be searched without search date and language restrictions. We will include randomized controlled trial(RCT)comparing TXYF with another therapy method for IBS.The quality of included studies will be independently assessed using Risk of bias tool in the Cochrane Handbook 5.1.0,and be categorized as being at low, unclear or high risk of bias. The primary outcome is the disappearance and significant improvement in global symptoms, and the secondary outcomes including visual analog scale(VAS)score, adverse event, rehospitalization rate and the quality of life. The Meta-analysis will be performed using the Cochrane Collection’s tool RevMan 5.3 software. Results:This study is ongoing and will be submitted to a peerreviewed journal for publication. Conclusion:This study will provide a confirmed evidence on the safety and eff ectiveness of TXYF for IBS.Ethics and dissemination:Ethical approval and patient consent are not required because of the study is a systematic review and meta-analysis based on literatures. The results of this study will be submitted to a peer-reviewed journal for publication.
