BACKGROUND Intramural pregnancy is a rare type of ectopic pregnancy,which is diagnosed by transvaginal ultrasound and magnetic resonance imaging.Management strategies vary depending on the site of the pregnancy,the ge...BACKGROUND Intramural pregnancy is a rare type of ectopic pregnancy,which is diagnosed by transvaginal ultrasound and magnetic resonance imaging.Management strategies vary depending on the site of the pregnancy,the gestational age and the desire to maintain fertility.The incidence of intramural pregnancy in assisted reproductive technology is higher than that in natural pregnancy.CASE SUMMARY We present a case of intramural pregnancy after in vitro fertilization and elective single embryo transfer following salpingectomy.The patient was completely asymptomatic and her serumβ-human chorionic gonadotropin level increased from 290 m IU/m L to 1759 m IU/m L.Three-dimensional transvaginal ultrasound indicated a heterogeneous echogenic mass arising from the uterine fundus which was surrounded by myometrium and a slender and extremely hypoechoic area stretching to the uterine cavity which was thought to be a fistulous tract.Therefore,we considered a diagnosis of intramural pregnancy and laparoscopic surgery was conducted at 7 wk gestation.CONCLUSION Early diagnosis and treatment of intramural pregnancy is significant for maintaining fertility.展开更多

It was found that 14c, 14d, 14f, 14i, 14j and 14k showed more potent cytotoxic activities than control 5-fluorouracil. Drilling costs of ultra-deepwell is the significant part of development investment,and accurate prediction of drilling costs plays an important role in reasonable budgeting and overall control of development cost.In o...Drilling costs of ultra-deepwell is the significant part of development investment,and accurate prediction of drilling costs plays an important role in reasonable budgeting and overall control of development cost.In order to improve the prediction accuracy of ultra-deep well drilling costs,the item and the dominant factors of drilling costs in Tarim oilfield are analyzed.Then,those factors of drilling costs are separated into categorical variables and numerous variables.Finally,a BP neural networkmodel with drilling costs as the output is established,and hyper-parameters(initial weights and bias)of the BP neural network is optimized by genetic algorithm(GA).Through training and validation of themodel,a reliable prediction model of ultra-deep well drilling costs is achieved.The average relative error between prediction and actual values is 3.26%.Compared with other models,the root mean square error is reduced by 25.38%.The prediction results of the proposed model are reliable,and the model is efficient,which can provide supporting for the drilling costs control and budget planning of ultra-deep wells.展开更多

Although bupivacaine-induced toxicity is mainly caused by substantial blockade of voltage-gated sodium channels (VGSCs), how these hydrophobic molecules interact with the receptor sites to which they bind remains unclear. Navl.5 is the dominant isoform of VGSCs expressed in cardiac myocytes, and its dysfunction may be the cause of bupivacaine- triggered arrhythmia. Here, we investigated the effect of bupivacaine on Navl.5 within the clinical concentration range. The electrophysiological measurements on Navl.5 expressed in Xenopus oocytes showed that bupivacaine induced a voltage- and concentration-dependent blockade on the peak of/Na and the half-maximal inhibitory dose was 4.51 pmol/L. Consistent with other local anesthetics, bupivacaine also induced a use-dependent blockade on Navl.5 currents. The underlying mechanisms of this blockade may contribute to the fact that bupivacaine not only dose-dependently affected the gating kinetics of Nay1.5 but also accelerated the development of its open-state slow inactivation. These results extend our knowledge of the action of bupivacaine on cardiac sodium channels, and therefore contribute to the safer and more efficient clinical use of bupivacaine.展开更多

[Objectives]To explore the effects of Guizhi Shaoyao Zhimu Decoction on laboratory indicators and immune functions of patients with rheumatoid arthritis.[Methods]A total of 110 patients with rheumatoid arthritis admit...[Objectives]To explore the effects of Guizhi Shaoyao Zhimu Decoction on laboratory indicators and immune functions of patients with rheumatoid arthritis.[Methods]A total of 110 patients with rheumatoid arthritis admitted to the Second Department of Orthopedics,the Affiliated Chinese and Western Medicine Hospital of Nanjing,Nanjing University of Chinese Medicine from December 2014 to December 2016 were selected as the research objects and divided into a control group and an observation group using the random number table method,with 55 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with Guizhi Shaoyao Zhimu Decoction on the basis of the control group.Both groups were treated for one month.The clinical effects,improvement of clinical symptoms,laboratory indicators and immune function indicator levels between the two groups were compared;the incidence of adverse reactions in the two groups was counted.[Results]The total effective rate of the observation group was 92.73%,higher than that of the control group(78.18%)(P<0.05).Compared with that before treatment,after one month of treatment,the grip strength of the two groups increased,and the observation group was greater than the control group(P<0.05);the morning stiffness time and 20 m walking time of both groups were shortened,and the observation group was shorter than the control group(P<0.05);the number of joint tenderness,joint swelling,ESR,MPV,PDW,serum CRP,plasma IgA,IgG,IgM,RF levels in both groups decreased,and the observation group was lower than the control group(P<0.05);the serum C3 levels in both groups increased,and the observation group was higher than the control group(P<0.05).During the treatment,the incidence of adverse reactions in the observation group was 0.00%,which was lower than that in the control group(10.91%,P<0.05).[Conclusions]Using Guizhi Shaoyao Zhimu Decoction to treat rheumatoid arthritis can improve the laboratory indicators of patients,and enhance their immune functions,improve the curative effect,and the safety is high.展开更多

Increasing evidence indicates that oligodendrocyte(OL)numbers and myelin as a dynamic cellular compartment perform a key role in the maintenance of neuronal function.Inhibiting white matter(WM)demyelination or promoti...Increasing evidence indicates that oligodendrocyte(OL)numbers and myelin as a dynamic cellular compartment perform a key role in the maintenance of neuronal function.Inhibiting white matter(WM)demyelination or promoting remyelination has garnered interest for its potential therapeutic strategy against ischemic stroke.Our previous work has shown that low-intensity pulsed ultrasound(LIPUS)could improve stroke recovery.However,it is unclear whether LIPUS can maintain WM integrity early after stroke or promote late WM repair.This study evaluated the efficacy of LIPUS on WM repair and long-term neurologic recovery after stroke.Male adult C57BL/6 mice underwent a focal cerebral ischemia model and were randomized to receive ultrasound stimulation(30 min once daily for 14 days).The effect of LIPUS on sensorimotor function was assessed by modified neurological severity score,rotarod test,grip strength test,and gait analysis up to 28 days after stroke.We found that ischemic stroke-induced WM damage was severe on day 7 and partially recovered on day 28.LIPUS prevented neuronal and oligodendrocyte progenitor cell(OPC)death during the acute phase of stroke(d7),protected WM integrity,and reduced brain atrophy and tissue damage during the recovery phase(d28).To further confirm the effect of LIPUS on remyelination,we assessed the proliferation and differentiation of OPCs.We found that LIPUS did not increase the number of OPCs(PDGFRα^(+)or NG2^(+)),but markedly increased the number of newly produced mature OLs(APC^(+))and myelin protein levels.Mechanistically,LIPUS may promote OL maturation and remyelination by down-regulating the interleukin-17A/Notch1 signaling pathway.In summary,LIPUS can protect OLs and neurons early after stroke and promote long-term WM repair and functional recovery.LIPUS will be a viable strategy for the treatment of ischemic stroke in the future.展开更多

文摘Increasing evidence indicates that oligodendrocyte(OL)numbers and myelin as a dynamic cellular compartment perform a key role in the maintenance of neuronal function.Inhibiting white matter(WM)demyelination or promoting remyelination has garnered interest for its potential therapeutic strategy against ischemic stroke.Our previous work has shown that low-intensity pulsed ultrasound(LIPUS)could improve stroke recovery.However,it is unclear whether LIPUS can maintain WM integrity early after stroke or promote late WM repair.This study evaluated the efficacy of LIPUS on WM repair and long-term neurologic recovery after stroke.Male adult C57BL/6 mice underwent a focal cerebral ischemia model and were randomized to receive ultrasound stimulation(30 min once daily for 14 days).The effect of LIPUS on sensorimotor function was assessed by modified neurological severity score,rotarod test,grip strength test,and gait analysis up to 28 days after stroke.We found that ischemic stroke-induced WM damage was severe on day 7 and partially recovered on day 28.LIPUS prevented neuronal and oligodendrocyte progenitor cell(OPC)death during the acute phase of stroke(d7),protected WM integrity,and reduced brain atrophy and tissue damage during the recovery phase(d28).To further confirm the effect of LIPUS on remyelination,we assessed the proliferation and differentiation of OPCs.We found that LIPUS did not increase the number of OPCs(PDGFRα^(+)or NG2^(+)),but markedly increased the number of newly produced mature OLs(APC^(+))and myelin protein levels.Mechanistically,LIPUS may promote OL maturation and remyelination by down-regulating the interleukin-17A/Notch1 signaling pathway.In summary,LIPUS can protect OLs and neurons early after stroke and promote long-term WM repair and functional recovery.LIPUS will be a viable strategy for the treatment of ischemic stroke in the future.

文摘Several viral infections are often associated with fetal brain structural anomalies,including white matter lesions,cerebellar hypoplasia,temporal lobe lesions,ventricular dilatation,intraventricular septa,ependymal cysts,and microcephaly,et al.Ultrasound(US)is the first modality of choice to evaluate fetal brain structural anomalies,however,Ultrasound is usually effected by maternal and fetal factors such as obesity,oligohydramnios and fetal head positions.Fetal MRI is not susceptible to the limitations as ultrasound with higher contrast resolution than prenatal ultrasound,directly visualize certain structures,such as developing brain parenchyma and further confirming US diagnosis and adding additional diagnostic information.This article reviews prenatal magnetic resonance imaging(MRI)diagnosis of fetal brain structural anomalies associated with viral infection.
