门脉高压脾机能亢进对根治性治疗肝癌患者预后的影响 被引量:3

目的:探讨不同程度门脉高压脾机能亢进与脾切除对肝癌患者预后影响。方法:回顾性分析2008年1月至2018年1月天津市第三中心医院符合米兰标准并行根治性治疗的403例肝细胞肝癌患者临床资料。依据血小板(platelet,PLT)水平、ALBI分级、APR... 目的:探讨不同程度门脉高压脾机能亢进与脾切除对肝癌患者预后影响。方法:回顾性分析2008年1月至2018年1月天津市第三中心医院符合米兰标准并行根治性治疗的403例肝细胞肝癌患者临床资料。依据血小板(platelet,PLT)水平、ALBI分级、APRI、血小板/脾长径比值(PSR)等进行Cox比例风险回归分析,对比合并严重脾亢肝癌患者中仅行肝癌根治者与联合脾切除肝癌根治者间临床资料,采用Kaplan-Meier法进行生存分析。结果:Cox单因素、多因素分析显示,PLT水平是肝癌患者整体生存(overall survival,OS)和无瘤生存(disease free survival,DFS)的独立危险因素。不同血小板水平肝癌患者多因素OS生存曲线具有显著性差异(P=0.013)。PSR、APRI、ALBI分级等肝硬化门脉高压参数也是肝癌预后危险因素。仅行肝癌根治组与联合脾切除肝癌根治组在门脉高压脾亢程度、肝功能分级、肿瘤分期均无显著性差异(P>0.05),生存分析显示,联合脾切除组具有更长的OS(P=0.025),两组1、3、5年生存率分别为100%、98.2%、68.5%和97.1%、79.4%、56.8%。两组DFS比较无显著性差异(P=0.326)。结论:PLT、PSR、APRI、ALBI等临床参数是合并门脉高压脾亢肝癌患者重要预后影响因素。对合并严重脾亢符合米兰标准的肝癌患者,在肝癌根治同时联合脾切除可提高患者OS。 展开更多

We have designed,assembled,and tested a 4-MA,60-ns fast linear transformer driver(LTD),which is the first operating generator featuring multiple LTD modules connected in parallel.The LTD-based accelerator comprises six modules in parallel,each of which has ten-stage cavities stacked in series.The six LTD modules are connected to a water tank of diameter 6 m via a 3-m-long impedance-matched deionized waterinsulated coaxial transmission line.In the water tank,the electrical pulses are transmitted down by six horizontal tri-plate transmission lines.A 2.1-m-diameter two-level vacuum insulator stack is utilized to separate the deionized water region from the vacuum region.In the vacuum,the currents are further transported downstream by a two-level magnetically insulated transmission-line and then converged through four post-hole convolutes.Plasma radiation loads or bremsstrahlung electron beam diodes serve as loads that are expected to generate intense soft X rays or warm X rays.The machine is 3.2 m in height and 22 m in outer diameter,including support systems such as a high-voltage charge supply,magnetic core reset system,trigger system,and support platform for inner stalk installation and maintenance.A total of 1440 individual±100-kV multi-gap spark switches and 2880 individual 100-kV capacitors are employed in the accelerator.A total of 12 fiberoptic laser-controlled trigger generators combining photoconductive and traditional gas spark switch technologies are used to realize the synchronous discharge of the more than 1000 gas switches.At an LTD charge voltage of±85 kV,the accelerator stores an initial energy of about 300 kJ and is expected to deliver a current of 3–5 MA into various loads.To date,the LTD facility has shot into a thick-walled aluminum liner load and a reflex triode load.With a thick-walled aluminum liner of inductance 1.81 nH,a current with peak up to 4.1 MA and rise time(10%–90%)of about 60 ns has been achieved.The current transport efficiency from the insulator stack to the liner load approaches 100%during peak times.The LTD accelerator has been used to drive reflex triode loads generating warm X rays with high energy fluence and large radiation area.It has been demonstrated that this LTD is a promising and high-efficiency prime pulsed power source suitable for use in constructing the next generation of large-scale accelerators with currents of tens of megaamperes. 展开更多

AIM To assess the impact of hepatitis B surface(HBs Ag) seroclearance on survival outcomes in hepatitis B-related primary liver cancer.METHODS Information from patients with hepatitis B-related liver cancer admitted in our hospital from 2008-2017 was retrieved. Cases diagnosed with HBs Ag(-) and HBc Ab(+) liver cancer were included in the HBs Ag seroclearance(SC) group. HBs Ag(+) liver cancer patients strictly matched for liver cancer stage(AJCC staging system, 8 th edition), Child-Pugh score, and first diagnosis/treatment method(surgery, ablation and TACE) were assigned to the HBsA g non-seroclearance(NSC) group. Then, clinical, pathological and survival data in both groups were assessed.RESULTS The SC and NSC groups comprised of 72 and 216 patients, respectively. Patient age(P < 0.001) and platelet count(P = 0.001) in the SC group were significantly higher than those of the NSC group. SC group patients who underwent surgery had more intrahepatic cholangiocarcinoma(ICC) and combined HCC-CC(CHC) cases than the NSC group, but no significant differences in tumor cell differentiation and history of liver cirrhosis were found between the two groups. The numbers of interventional treatments were similar in both groups(4.57 vs 5.07, P > 0.05). Overall survival was lower in the SC group than the NSC group(P = 0.019), with 1-,3-, and 5-year survival rates of 82.1% vs 85.1%, 43.2%vs 56.8%, and 27.0% vs 45.2%, respectively. Survival of patients with AJCC stage Ⅰ disease in the SC group was lower than that of the NSC group(P = 0.029).CONCLUSION Seroclearance in patients with hepatitis B-related primary liver cancer has protective effects with respect to tumorigenesis, cirrhosis, and portal hypertension but confers worse prognosis, which may be due to the frequent occurrence of highly malignant ICC and CHC. 展开更多

针对目前波纹阻火盘采用人工取点的方式对波高进行测量引起的效率低、误差大等问题,提出一种基于机器视觉的波纹阻火盘表面波高测量方法。该方法先利用线阵CCD相机配合二维运动平台扫描的方式获取图像,再利用改进的FLANN自适应特征点匹... 针对目前波纹阻火盘采用人工取点的方式对波高进行测量引起的效率低、误差大等问题,提出一种基于机器视觉的波纹阻火盘表面波高测量方法。该方法先利用线阵CCD相机配合二维运动平台扫描的方式获取图像,再利用改进的FLANN自适应特征点匹配筛选算法优化拼接过程,提高线阵图像配准准确度,然后采用基于Zernike矩的亚像素边缘检测算法准确获取边缘定位,并结合该文提出的波高尺寸测量原理以及最小二乘法,确定最大波高对应的两个像素点的坐标,最后根据线阵相机标定结果计算得到实际波高尺寸。利用波纹阻火盘线阵图像进行实验,并与影像仪法测量结果进行对比,实验结果表明该测量方法测量误差在±0.02 mm以内,满足实际工业生产需要。 展开更多

AIM:To investigate the methylation status of secreted protein acidic and rich in cysteine(SPARC) in human hepatocellular carcinoma(HCC) and evaluate its clinical implication.METHODS:The methylation status of SPARC was analyzed in one HCC cell line(SMMC-7721) and 60 pairs of HCC and corresponding nontumorous tissues by methylation-specific polymerase chain reaction and bisulfite sequencing.The expression of SPARC mRNA and protein were examined by reverse transcription polymerase chain reaction and immunohistochemistry,respectively.The correlations between the methylation status and the gene expression,the clinicopathological parameters,as well as the prognosis after surgery were analyzed.RESULTS:In the SMMC-7721 cell line,the loss of SPARC expression was correlated with the aberrant methylation and could be reactivated by the demethylating agent 5-aza-2'-deoxycytidine.Methylation frequency of SPARC in HCC was significantly higher than that in the corresponding nontumorous tissues(45/60 vs 7/60,P < 0.001),and it was correlated with the pathological classification(P = 0.019).The downregulation of the SPARC mRNA expression in HCC was correlated with the SPARC methylation(P = 0.040).The patients with methylated SPARC had a poorer overall survival than those without methylated SPARC(28.0 mo vs 41.0 mo,P = 0.043).CONCLUSION:Aberrant methylation is an important mechanism for SPARC inactivation in HCC and SPARC methylation may be a promising biomarker for the diagnosis and prognosis of HCC. 展开更多

Innovative deep learning method for predicting the state of health of lithium-ion batteries based on electrochemical impedance spectroscopy and attention mechanisms

Electrochemical impedance spectroscopy plays a crucial role in monitoring the state of health of lithium-ion batteries.However,effective feature extraction often relies on limited information and prior knowledge.To add-ress this issue,this paper presents an innovative approach that utilizes the gramian angular field method to transform raw electrochemical impedance spectroscopy data into image data that is easily recognizable by convolutional neural networks.Subsequently,the convolutional block attention module is integrated with bidirectional gated recurrent unit for state of health prediction.First,convolu-tional block attention module is applied to the electro-chemical impedance spectroscopy image data to enhance key features while suppressing redundant information,thereby effectively extracting representative battery state features.Subsequently,the extracted features are fed into a bidirectional gated recurrent unit network for time series modeling to capture the dynamic changes in battery state of health.Experimental results show a significant im-provement in the accuracy of state of health predictions,highlighting the effectiveness of convolutional block atten-tion module in feature extraction and the advantages of bidirectional gated recurrent unit in time series forecasting.This research provides an attention mechanism-based feature extraction solution for lithium-ion battery health management,demonstrating the extensive application potential of deep learning in battery state monitoring. 展开更多
