某院重点监控药品全程管理的实践与成效分析 被引量:3

目的 促进医院重点监控药品的管控及合理用药。方法 参考《第二批国家重点监控合理用药药品目录》制订医院第二批国家重点监控合理用药药品目录(化药及生物制品);通过健全药事管理体系,进行前置审方系统的信息化干预、强化处方/医嘱审... 目的 促进医院重点监控药品的管控及合理用药。方法 参考《第二批国家重点监控合理用药药品目录》制订医院第二批国家重点监控合理用药药品目录(化药及生物制品);通过健全药事管理体系,进行前置审方系统的信息化干预、强化处方/医嘱审核和点评,结合合理用药管理的绩效考核、预警控量、指标公示、宣教培训等行政干预手段,采取全程管理、分类管理,并应用多维工具实施监控。利用医院信息系统(HIS)对2022年8月至12月(干预前)和2023年2月至6月(干预后)统计重点监控药品的使用情况,分析干预前后的销售金额占比、处方合理率。结果 干预后,处方/医嘱合理率由干预前的93.80%升至96.07%(P=0.002)。药品消耗总金额占比整体呈下降趋势,干预后2023年6月最低,为6.65%。结论 药师参与重点监控药品用药管理,有助于降低药品费用,促进合理用药。 展开更多

目的总结穴位疗法在上消化道内镜诊疗中的应用。方法整理针刺、经皮穴位电刺激、艾灸、穴位按摩、穴位注射等穴位疗法对上消化道的作用机理,对各类穴位疗法在上消化道内镜检查中的应用情况进行综述。结果穴位疗法在上消化道内镜检查中... 目的总结穴位疗法在上消化道内镜诊疗中的应用。方法整理针刺、经皮穴位电刺激、艾灸、穴位按摩、穴位注射等穴位疗法对上消化道的作用机理,对各类穴位疗法在上消化道内镜检查中的应用情况进行综述。结果穴位疗法在上消化道内镜检查中主要有以下作用,其一,通过调节神经及胃肠道电活动缓解焦虑情绪及胃肠道不良反应;其二,减少麻醉药物用量及药物麻醉苏醒时间。结论穴位疗法作为一种安全的中医特色疗法,在上消化道内镜检查中有着良好的辅助作用,但目前开展的临床和基础研究仍然较为局限,需要进一步深入探索和研究。 展开更多

目的:探究自拟疏风散热化痰汤治疗小儿上呼吸道感染的临床疗效及对炎性因子的影响。方法:选择本院在2020年9月—2022年8月诊治的89例上呼吸道感染风热夹痰证患儿,根据治疗方法分为对照组和观察组,对照组(42例)给予西医常规对症治疗,观察... 目的:探究自拟疏风散热化痰汤治疗小儿上呼吸道感染的临床疗效及对炎性因子的影响。方法:选择本院在2020年9月—2022年8月诊治的89例上呼吸道感染风热夹痰证患儿,根据治疗方法分为对照组和观察组,对照组(42例)给予西医常规对症治疗,观察组(47例)在对照组的基础上给予自拟疏风散热化痰汤进行治疗,3 d后评价两组患儿的临床疗效;治疗前及治疗3 d比较中医症状评分及炎性因子(IL-6、TNF-α、hs-CRP)水平变化,记录临床症状消失的时间。结果:治疗3 d观察组总有效率高于对照组(P<0.05);与治疗前比较,治疗后两组患儿发热、头痛、鼻塞流浊涕、咽红、咳嗽、喉中痰鸣等症状评分均降低(P<0.05),观察组降低更明显(P<0.05);与治疗前比较,治疗3 d后两组患儿血清IL-6、TNF-α、hs-CRP水平降低(P<0.05),观察组降低更明显(P<0.05);观察组患儿发热、头痛、咳嗽及鼻塞流浊涕等症状消失时间均短于对照组(P均<0.05)。结论:自拟疏风散热化痰汤治疗小儿上呼吸道感染疗效显著,可加快症状缓解、抑制炎症反应。 展开更多

The E (envelope) protein is the smallest structural protein in all coronaviruses and is the only viral structural protein in which no variation has been detected. The E (envelope) protein is the smallest structural protein in all coronaviruses and is the only viral structural protein in which no variation has been detected. We conducted genome sequencing and phylogenetic analyses of SARS-CoV. Based on genome sequencing, we predicted the E protein is a transmembrane (TM) protein characterized by a TM region with strong hydrophobicity and α-helix conformation. We identified a segment (NH2-_L-Cys-A-Y-Cys-Cys-N_-COOH) in the carboxyl-terminal region of the E protein that appears to form three disulfide bonds with another segment of corresponding cysteines in the carboxyl-terminus of the S (spike) protein. These bonds point to a possible structural association between the E and S proteins. Our phylogenetic analyses of the E protein sequences in all published coronaviruses place SARS-CoV in an independent group in Coronaviridae and suggest a non-human animal origin. 展开更多
