目的比较分析中国版(2020)和美国放射学会(American College of Radiology,ACR)版(2017)甲状腺影像报告和数据系统(Thyroid Imaging Reporting and Data System,TIRADS)对甲状腺结节恶性风险分层评估的诊断效能,验证中国版TIRADS的价值...目的比较分析中国版(2020)和美国放射学会(American College of Radiology,ACR)版(2017)甲状腺影像报告和数据系统(Thyroid Imaging Reporting and Data System,TIRADS)对甲状腺结节恶性风险分层评估的诊断效能,验证中国版TIRADS的价值。方法以组织病理学结果为金标准,分别采用中华医学会超声医学分会浅表器官和血管学组与中国甲状腺与乳腺超声人工智能联盟(China,C)制定的C-TIRADS和ACR-TIRADS对2015年1月至2021年3月海南医学院第一附属医院的1306例患者的1389个甲状腺局灶性病变的恶性风险度进行回顾性评估,分析两种TIRADS诊断效能及相关影响因素。结果1389个甲状腺局灶性病变中,良性结节973个,恶性结节416个,C-TIRADS 4C和ACR-TIRADS 5准确度最高。以C-TIRADS 4C和ACR-TIRADS 5为最佳截断值对甲状腺结节评估效能进行分析,其敏感性、特异性、阳性预测值、阴性预测值和ROC曲线下面积(AUC)分别为87.39%、89.92%、75.00%、95.38%、0.89和85.58%、91.88%、81.84%、93.71%、0.89,差异无统计学意义(均P>0.05)。结论C-TIRADS和ACT-TIRADS对甲状腺结节恶性风险分层评估有很好的诊断效能;C-TIRADS 4C的诊断效能与ACR-TIRADS 5的诊断效能相当。展开更多
