Prevalence and Determinants of Erectile Dysfunction in Adult Diabetic Patients in the Southwest Region of Cameroon

Background: Erectile dysfunction (ED) is a common, bothersome and relatively under diagnosed complication of diabetes mellitus. This study was aimed to determine the prevalence of erectile dysfunction and its determinants. Methods: A cross-sectional hospital-based study was carried out in the Diabetology Units of the Buea and Limbe Regional Hospitals involving 332 male patients with diabetes and aged over 21 years. Data was analyzed using Stata and R version 3.5.3. Results: The mean age of the participants was 55years. Most participants (64.46%) were married. About half (50.60%) of the participants actively consumed alcohol, 11.45% were smokers and 57.83% were sedentary. 18 participants (5.42%) recorded high risk sexual behaviour. 54.32% of participants had a comorbidity and 43.90% were overweight. The prevalence of diabetic ED was 78.92%. Age, Fasting Blood Sugar and Glycated hemoglobin were found to be positive determinants of diabetic ED (odds ratio (OR) = 0.77, 95% CI −0.1 - 0.07). Conclusion: The prevalence of diabetic ED in this hospital population study is high, and both physician and patient—initiated measures are needed to reduce this prevalence and improve awareness, recognition and care of this condition. 展开更多

Background: Diabetes mellitus (DM) is one of the leading causes of Erectile Dysfunction (ED) in men of all ages. The unawareness, coupled with common myths surrounding ED, confound the attempts of patients to seek and receive treatment and the attempts of doctors to help them. Objective: The study was aimed to assess the quality of care sought and received by Diabetic patients with ED. Methods: A cross-sectional hospital-based study was carried out in the Diabetic Units of the Limbe and Buea Regional Hospitals involving 322 male diabetic patients and aged over 21 years. Data analysis was done using Stata and R version 3.5.3. Results: The mean age of the participants was 55 years with a prevalence of ED of 78.92%. Only 37.40% of participants with ED sought care for it. Main barriers to care-seeking were health ignorance, health misinformation and fear of stigma. Majority (85.71%) of those who sought care sought medical care. Respondents correctly informed about diabetic ED and those regularly screened by their physician were more likely to seek medical care over non-medical care (p = 0.0021, p = 0.0013). Those who sought medical care reported higher improvement in ED symptoms over those who sought non-medical or combined forms of care (p = 0.0183). Conclusion: Both physician and patient-initiated measures are needed to reduce the prevalence and improve awareness, recognition and medical care of this condition. 展开更多

Penis amputations are rare, they are most often observed in a criminal context or in the context of self-mutilation. We present the case of a partial amputation of the shaft in the context of a domestic accident. This is a 53-year-old patient received in the emergency department for partial amputation of the penis occurred, whose mechanism of occurrence would be the fall of a sheet metal from the roof of his house, with reception on the proximal end of the shaft leading to a partial rupture of it—here, with total urethral section. He has no medical-surgical history. The physical examination finds a good general condition, a partial tearing of the ventral face of the penis associated with a total rupture of the urethra followed by a hemorrhage, the rest of the examination was without particularity. The emergency assessment carried out was without particularity. The treatment consisted initially of catheterizing the urethra by a urinary tube ch 20, then a urethral replacement, followed by a Peno plastie. The surgical suites were enamelled with parietal suppuration plus necrosis at J 14 post-operative. A more bidaily dressing debridement followed with a favorable evolution at 2 months. Subsequently, a penile graft was performed more urethrolastic, with removal of the flap on the inner side of the thigh, the surgical suites were favorable on the sexual plane gradual resumption of night erections. 展开更多

Introduction: Torsion of the spermatic cord is an emergency whose delay in treatment conditions the functional prognosis of the testicle. The aim of this study was to analyze the management of spermatic cord torsion in the Urology Andrology Department of the Ignace Deen National Hospital in Conakry. Material and methods: This was a ten-year retrospective descriptive study from January 1, 2012 to December 31, 2021. It involved all patients admitted for torsion of the spermatic cord, confirmed at surgical exploration. Results: We identified 21 cases of torsion of the spermatic cord. The mean age of the patients was 17.9 ± 4.4 years. The average consultation time was 19.2 ± 21.4 hours with extremes of [2 h and 98 h]. Only 6 patients (28.6%) consulted before the sixth hour. All patients presented with scrotal swelling. At scrototomy, all torsions were intravaginal with two turns of spiral in 13 cases and three turns in 8 cases. Orchiectomy followed by contralateral orchidopexy was performed in 6 cases. In the other cases, bilateral orchidopexy was performed after detorsion. The average hospital stay was 4.5 days. We recorded 4 cases of testicular atrophy after orchidopexy. Conclusion: Spermatic cord torsion is an infrequent emergency in our department. The delay in consultation remains the main predictive factor of testicular necrosis. Emergency exploratory scrotomy should be the rule. 展开更多

Aim: This paper aims to report the second case of mucinous carcinoma of the scrotum revealed by scrotal fistulas listed in the literature and highlight the difficulties in the search and difficulties encountered in the management of urogenital cancers in developing countries. Case presentation: This was a 62-year-old patient who had consulted for fistulized lesions of the scrotum associated with urinary disorders. Physical examination found budding lesions in the scrotum from which mucus was leaking. The diagnostic assessment carried out consisted of a biopsy sample of the scrotal lesions, a blood analysis and medical imaging, which led to the diagnosis. The mutilating nature of the surgery proposed to the patient after multidisciplinary consultation meeting and the expensive cost of chemotherapy drugs constituted the limits of the patient’s therapeutic management. Conclusion: Mucinous carcinoma of the scrotum is rare and primary lesion should always be sought. The presence of budding lesions of the scrotum with discharge of mucus should suggest the diagnosis. 展开更多

They are often secondary, either to an erotic curiosity, <span style="white-space:nowrap;">&#251;</span> or to psychiatric disorders. The diagnosis remains clinically aided by imagery. Treatment is most often endoscopic, but in some cases the use of open surgery is necessary. We reported the case of an 80-year-old man with no history of behavioral disorders;a wooden rod was inserted into the urethra to empty his bladder. The care consisted of a meatoplasty under locoregional anesthesia. Upon discharge, the patient had a good urine flow. 展开更多
