Effects of bone marrow mesenchymal stem cell combined with platelet-rich plasma treatment of bone nonunion after long bone fracture surgery on bone metabolism and cytokines

Objective: To study the effects of bone marrow mesenchymal stem cell combined with platelet-rich plasma treatment of bone nonunion after long bone fracture surgery on bone metabolism and cytokines. Methods: Patients who were treated in our hospital due to bone nonunion after long bone fracture surgery between March 2011 and October 2017 were selected and randomly divided into two groups, combined group received bone marrow mesenchymal stem cell combined with platelet-rich plasma therapy, and control group received bone marrow mesenchymal stem cell therapy. The levels of bone metabolism markers and growth cytokines in serum as well as the expression of bone metabolism-related signal molecules in peripheral blood were determined before treatment and 1 month after treatment. Results: Compared with those of same group before treatment, serum PINP, OPG, BALP, VEGF, TGF-β1, IGF-I, IGF-II and bFGF levels as well as peripheral blood Runx2, Wnt1, Wnt3a and β-catenin expression intensity of both groups of patients significantly increased whereas serum β-CTX and RANKL levels as well as peripheral blood NOX4 and NF-κB expression intensity significantly decreased after treatment, and serum PINP, OPG, BALP, VEGF, TGF-β1, IGF-I, IGF-II and bFGF levels as well as peripheral blood Runx2, Wnt1, Wnt3a and β-catenin expression intensity of combined group after treatment were higher than those of control group whereas serum β-CTX and RANKL levels as well as peripheral blood NOX4 and NF-κB expression intensity were lower than those of control group. Conclusion: Bone marrow mesenchymal stem cell combined with platelet-rich plasma treatment of bone nonunion after long bone fracture surgery can be more effective than bone marrow mesenchymal stem cell monotherapy to improve the bone metabolism and increase the cytokines. 展开更多

With the continuous development of wearable electronics,wireless sensor networks and other micro-electronic devices,there is an increasingly urgent need for miniature,flexible and efficient nanopower generation technology.Triboelectric nanogenerator(TENG)technology can convert small mechanical energy into electricity,which is expected to address this problem.As the core component of TENG,the choice of electrode materials significantly affects its performance.Traditional metal electrode materials often suffer from problems such as durability,which limits the further application of TENG.Graphene,as a novel electrode material,shows excellent prospects for application in TENG owing to its unique structure and excellent electrical properties.This review systematically summarizes the recent research progress and application prospects of TENGs based on graphene electrodes.Various precision processing methods of graphene electrodes are introduced,and the applications of graphene electrode-based TENGs in various scenarios as well as the enhancement of graphene electrodes for TENG performance are discussed.In addition,the future development of graphene electrode-based TENGs is also prospectively discussed,aiming to promote the continuous advancement of graphene electrode-based TENGs. 展开更多

BACKGROUND Histoplasmosis is a rare infectious condition with mainly pulmonary involvement which is generally self-limiting in immunocompetent individuals.Its manifestation varies and lacks specificity.This study reports a case of primary disseminated liver histoplasmosis in a normal host presenting as liver failure cured by liver transplantation and voriconazole.CASE SUMMARY A 43-year-old Chinese man with intermittent fever,malaise,jaundice and extreme hepatomegaly for more than 40 days was admitted to the Second Xiang-ya Hospital.The patient was immunocompetent and lacked a definitive history of exposure.His condition deteriorated to liver failure,and he promptly underwent liver transplantation to ensure survival.One year later,the patient presented with severe gastrointestinal symptoms,including fever,abdominal pain,and diarrhea.Subsequently,tissue samples acquired via gastrointestinal endoscopy were subjected to pathological examination and next-generation sequencing analysis.Through a comprehensive amalgamation of clinical presentation,biopsy pathology,and next-generation sequencing analysis,the patient was ultimately diagnosed with disseminated hepatic histoplasmosis.The patient achieved complete recovery after 6 months of voriconazole treatment.CONCLUSION In patients with chronic-hepatitis-B having atypical symptoms,histoplasmosis can be a differential diagnosis.Voriconazole is effective in treating histoplasmosis. 展开更多

3D-programmable streamline guided orientation in composite materials for targeted heat dissipation

Filler-reinforced polymer composites demonstrate pervasive applications due to their strengthened performances,multi-degree tunability,and ease of manufacturing.In thermal management field,polymer composites reinforced with thermally conductive fillers are widely adopted as thermal interface materials(TIMs).However,the three dimensional(3D)-stacked heterogenous integration of electronic devices has posed the problem that high-density heat sources are spatially distributed in the package.This situation puts forward new requirements for TIMs,where efficient heat dissipation channels must be established according to the specific distribution of discrete heat sources.To address this challenge,a 3D printing-assisted streamline orientation(3D-PSO)method was proposed to fabricate composite thermal materials with 3D programmable microstructures and orientations of fillers,which combines the shape-design capability of 3D printing and oriented control ability of fluid.The mechanism of fluid-based filler orientation control along streamlines was revealed by mechanical analysis of fillers in matrix.Thanks to the designed heat dissipation channels,composites showed better thermal and mechanical properties in comparison to random composites.Specifically,the thermal conductivity of 3D mesh-shape polydimethylsiloxane/liquid metal(PDMS/LM)composite was5.8 times that of random PDMS/LM composite under filler loading of 34.8 vol%.The thermal conductivity enhancement efficiency of 3D mesh-shape PDMS/carbon fibers composite reached101.05%under filler loading of 5.2 vol%.In the heat dissipation application of 3D-stacked chips,the highest chip temperature with 3D-PSO composite was 42.14℃lower than that with random composites.This is mainly attributed to the locally aggregated and oriented fillers'microstructure in fluid channels,which contributes to thermal percolation phenomena.The3D-PSO method exhibits excellent programmable design capabilities to adopt versatile distributions of heat sources,paving a new way to solve the complicated heat dissipation issue in 3D-stacked chips integration application. 展开更多

Unsupervised Anomaly Detection in Time Series Data via Enhanced VAE-Transformer Framework

Time series anomaly detection is crucial in finance,healthcare,and industrial monitoring.However,traditional methods often face challenges when handling time series data,such as limited feature extraction capability,poor temporal dependency handling,and suboptimal real-time performance,sometimes even neglecting the temporal relationships between data.To address these issues and improve anomaly detection performance by better capturing temporal dependencies,we propose an unsupervised time series anomaly detection method,VLT-Anomaly.First,we enhance the Variational Autoencoder(VAE)module by redesigning its network structure to better suit anomaly detection through data reconstruction.We introduce hyperparameters to control the weight of the Kullback-Leibler(KL)divergence term in the Evidence Lower Bound(ELBO),thereby improving the encoder module’s decoupling and expressive power in the latent space,which yields more effective latent representations of the data.Next,we incorporate transformer and Long Short-Term Memory(LSTM)modules to estimate the long-term dependencies of the latent representations,capturing both forward and backward temporal relationships and performing time series forecasting.Finally,we compute the reconstruction error by averaging the predicted results and decoder reconstruction and detect anomalies through grid search for optimal threshold values.Experimental results demonstrate that the proposed method performs superior anomaly detection on multiple public time series datasets,effectively extracting complex time-related features and enabling efficient computation and real-time anomaly detection.It improves detection accuracy and robustness while reducing false positives and false negatives. 展开更多

Electric tractors(ETs)with mounted implements form operating units.There are significant differences in parameters such as shape,firmness,and moisture content of the soil in contact with the tractor and implements when working in complex terrains such as field stubble,waterlogged silt,and loose/firm terrain.These differentiated dynamics prevent cooperation between ETs and operating implements under independent control,resulting in poor quality operations and low energy efficiency.We propose a control mechanism for ETs and implements to achieve full life cycle management of collaborative control tasks,instantaneous intertask interaction,and a multitask synchronization mechanism.To address the internal redundant communication problems caused by traditional distributed microcontrol units,we break through the underlying technology of unit data processing and interaction and develop an integrated high-performance controller structure with high processing capacity and high-and low-speed communication interfaces.On the basis of hierarchical stepwise control theory,a hierarchical real-time operating system is designed.This system realizes a preemptive kernel response of computational tasks and competitive-collaborative synchronization among tasks;overcomes the low-latency response of collaborative control tasks,instantaneous information interaction,and multitask synchronization problems;and provides system-level support for deep collaborative operation control of units.To demonstrate and validate the proposed collaborative control mechanism,a plowing collaborative operation management strategy is designed and deployed.The experimental results show that the communication delay of collaborative tasks is as low as 83μs,the solution time of complex collaborative equations is as low as 46 ms,the mechanical efficiency of the ET is increased by 9.07%,the efficiency of the drive motor is increased by 9.72%,the stability of the operation speed is increased by 106.25%,and the stability of the plowing depth reaches 94.98%.Our work meets the hardware and software requirements for realizing complex collaborative control of ET units and improves the operational quality and operational energy efficiency in real vehicle demonstrations. 展开更多

A robust method for large-scale route optimization on lunar surface utilizing a multi-level map model

As we look ahead to future lunar exploration missions, such as crewed lunar exploration and establishing lunar scientific research stations, the lunar rovers will need to cover vast distances. These distances could range from kilometers to tens of kilometers, and even hundreds and thousands of kilometers. Therefore, it is crucial to develop effective long-range path planning for lunar rovers to meet the demands of lunar patrol exploration. This paper presents a hierarchical map model path planning method that utilizes the existing high-resolution images, digital elevation models and mineral abundance maps. The objective is to address the issue of the construction of lunar rover travel costs in the absence of large-scale, high-resolution digital elevation models. This method models the reference and semantic layers using the middle- and low-resolution remote sensing data. The multi-scale obstacles on the lunar surface are extracted by combining the deep learning algorithm on the high-resolution image, and the obstacle avoidance layer is modeled. A two-stage exploratory path planning decision is employed for long-distance driving path planning on a global–local scale. The proposed method analyzes the long-distance accessibility of various areas of scientific significance, such as Rima Bode. A high-precision digital elevation model is created using stereo images to validate the method. Based on the findings, it can be observed that the entire route spans a distance of 930.32 km. The route demonstrates an impressive ability to avoid meter-level impact craters and linear structures while maintaining an average slope of less than 8°. This paper explores scientific research by traversing at least seven basalt units, uncovering the secrets of lunar volcanic activities, and establishing ‘golden spike’ reference points for lunar stratigraphy. The final result of path planning can serve as a valuable reference for the design, mission demonstration, and subsequent project implementation of the new manned lunar rover. 展开更多

目的探究血小板在COPD患者与健康人之间是否存在差异及血小板相关参数变化的临床意义。方法选择该院122例初次入院被确诊为COPD且排除血液、肿瘤、肺血栓栓塞症等相关疾病的COPD住院患者作为COPD组,以性别、年龄、民族相匹配的该院同期... 目的探究血小板在COPD患者与健康人之间是否存在差异及血小板相关参数变化的临床意义。方法选择该院122例初次入院被确诊为COPD且排除血液、肿瘤、肺血栓栓塞症等相关疾病的COPD住院患者作为COPD组,以性别、年龄、民族相匹配的该院同期健康体检者122例作为对照组,收集两组的临床资料及首次血常规检查结果。结果COPD组血小板计数为(193.85±66.68)×10~9/L、血小板与中性粒细胞比值为(48.97±25.15),对照组分别为(210.45±37.38)×10~9/L、(60.96±16.79),两组比较差异有统计学意义(P<0.05);COPD组平均血小板体积、血小板与淋巴细胞比值、中性粒细胞计数、白细胞计数均高于对照组(P<0.05),而血小板体积分布宽度、淋巴细胞计数、红细胞计数均低于对照组(P<0.05);血小板比容、嗜酸性粒细胞计数两组间差异无统计学意义(P>0.05)。血小板计数与气流受限严重程度不存在相关性(r_s=-0.104,P=0.252);血小板与中性粒细胞比同气流受限严重程度无相关(r_s=-0.070,P=0.444);血小板与淋巴细胞比同气流受限严重程度不存在相关(r_s=0.132,P=0.147);嗜酸性粒细胞计数与气流受限严重程度呈负相关(r_s=-0.267,P=0.003)。结论COPD患者血小板计数、血小板与中性粒细胞比值均降低,血小板计数、血小板与中性粒细胞比值同CODP患者疾病严重程度无关。 展开更多

BACKGROUND Gastric cancer(GC)is the sixth most common cancer and third leading cause of cancer-related deaths worldwide.Current treatments mainly rely on surgery-and chemotherapy-based systemic;however,the prognosis remains poor for advanced disease.Recent studies have suggested that immunotherapy has significant potential in cancer therapy;thus,GC immunotherapy may improve quality of life and survival for patients with this disease.AIM To provide a comprehensive overview of the knowledge structure and research hotspots of GC immunotherapy.METHODS We conducted a bibliometric analysis of publications on immunotherapy related to GC in the Web of Science Core Collection database.We analyzed 2013 publications from 1999 to February 1,2023,using the VOSviewer and CiteSpace software.We assessed publication and citation distributions using the WoS platform and explored research countries,institutions,journals,authors,references,and keywords(co-occurrence,timeline view,and burst analysis).In addition,we examined 228 trials on immunotherapy,137 on adoptive cell therapy,274 on immune checkpoint inhibitors(ICIs),and 23 on vaccines from ClinicalTrials.gov and the International Clinical Trials Registry Platform.The Impact Index Per Article for the top ten high-cited papers collected from Reference Citation Analysis(RCA)are presented.RESULTS Our bibliometric analysis revealed that the study of immunotherapy in GC has developed rapidly in recent years.China accounted for almost half the publications,followed by the United States.The number of publications in recent years has been growing continuously,and most institutions and authors with the most publications are from China.The main keywords or clusters identified were“tumor microenvironment”,“adoptive immunotherapy”,“dendritic therapy”,and“microsatellite instability”.CONCLUSION Our analysis of 2013 publications indicated that immunotherapy for GC has led to several new developments in recent years.Considerable progress has been made in vaccinations,immune checkpoint therapy,and adoptive cellular therapy.In particular,ICIs and chimeric antigen receptor T-cells are novel options for the treatment of GC.We suggest that the combination of ICIs,chemotherapy,targeted therapy,and other immunotherapies should be the primary research direction in the future. 展开更多

The hot deformation behavior and workability of a new reduced activation ferritic/martensitic steel named SIMP steel for accelerator-driven system were studied. The flow curve and its microstructure were studied at 900-1200 ℃ and strain rate range of 0.001-10 s^-1. The results showed that the deformation behavior of the SIMP steel during hot compression could be manifested by the Zener-Hollomon parameter in an exponent-type equation. Based on the obtained constitutive equation, the calculated flow stresses were in agreement with the experimentally measured ones, and the average activity energies Qdrv and QHw for the initiation of dynamic recrystallization and the peak strain were calculated to be 476.1 kJ/mol and 462.7 kJ/mol, respectively. Furthermore, based on the processing maps and microstructure evolution, the optimum processing condition for the SIMP steel was determined to be 1050-1200 ℃/0.001-0.1s^-1. 展开更多

AIM: To investigate the microRNA(miRNA) expression profile in gastrointestinal stromal tumor(GIST) tissues that could serve as a novel diagnostic biomarker for GIST detection.METHODS: We performed a quantitative real-time quantitative reverse transcriptase polymerase chain reaction assay to analyze the expression of 1888 miRNAs in a sample set that included 54 GIST tissue samples.RESULTS: We found that dysregulation of several miRNAs may be related to the malignant potential of GISTs. Six of these miRNAs, hsa-let-7c, miR-218,miR-488#, miR-4683, miR-34c-5p and miR-4773, were selected as the final list of biomarkers to separate the malignant GISTs(M group) from the benign GISTs(B group). In addition, MiR-29b-2#, hsa-let-7c, miR-891 b, miR-218, miR-204, miR-204-3p, miR-628-5p,miR-744, miR-29c#, miR-625 and miR-196 a were used to distinguish between the borderline(BO group) and M groups. There were 11 common miRNAs selected to separate the benign and borderline(BB) group from the M group, including hsa-let-7c, miR-218, miR-628-5p,miR-204-3p, miR-204, miR-891 b, miR-488#, miR-145,miR-891 a, miR-34c-5p and miR-196 a.CONCLUSION: The identified miRNAs appear tobe novel biomarkers to distinguish malignant from benign GISTs, which may be helpful to understand the mechanisms of GIST oncogenesis and progression,and to further elucidate the characteristics of GIST subtypes. 展开更多

Background Cardiac rehabilitation (CR) protocols have diversified to include home-based cardiac tele-rehabilitation (HBCTR) as an alternative to hospital-based or center-based CR. To adopt the use of home-based cardiac tele-rehabilitation, it is necessary to assess cardiac patients' attitudes towards acceptance of such e-health technology, especially in China where knowledge of such technology is deficient. Methods Interviews were conducted in the First Affiliated Hospital of Shantou University Medical College, Shantou, China. After percutaneous coronary interventional (PCI) surgery, patients completed the survey. Results Among the 150 patients, only 13% had ever heard of HBCTR. After an introduction of our HBCTR program, 60% of patients were willing to participate in the program. From our multivariate analysis of questionnaire data, age (OR: 0.92, 95% CI: 0.864).98; P = 0.007), average family monthly income (OR: 0.13, 95% CI: 0.05-0.341 P 〈 0.001), education level (OR: 0.24, 95% CI: 0.10-0.59; P = 0.002) and physical exercise time (OR: 0.19, 95% CI: 0.06-0.56; P = 0.003) were independent predictors for acceptance of HBCTR. From the reasons for participation, patients selected: enhanced safety and independ ence (28.3%), ability to self-monitor physical conditions daily (25 A%), and having automatic and emergency alert (23.1%). Reasons for refusal were: too cumbersome operation (34.3%) and unnecessary protocol (19.4%). Conclusions Most patients lacked knowledge about HBCTR but volunteered to participate after they have learned about the program. Several personal and life-style factors influenced their acceptance of the program. These indicate that both improvement of technology and better understanding of the program will enhance active participation. 展开更多

Wernicke encephalopathy(WE) is an acute neurological disorder resulting from vitamin B1 deficiency, which is common in chronic alcoholism and is rare in acute liver failure. So far, there are 2 cases of WE reported after liver transplantation. Here, we report a case of a 45-year-old nonalcoholic male patient who developed psychiatric and neurological disturbance 15 d after receiving orthotopic liver transplantation because of hepatitis B-related cirrhosis and portal hypertension. Brain magnetic resonance imaging(MRI) showed symmetric high-signal intensities in the periaqueductal area. The patient was diagnosed with WE and given intravenous high-dose vitamin B1 immediately. His neurological disturbance resolved in 7 d after receiving the vitamin B1. Brain MRI after 5 mo showed nearly complete recovery. Most WE cases may be misdiagnosed in patients after liver transplantation, and we should pay more attention to its onset. 展开更多
