摘要
目的:研究兔的颈动脉缺血模型在麻醉和气道开放时,保持一定的二氧化碳分压是否有利于缺血区脑循环供血和脑细胞的自我保护。方法:将20只日本大耳白兔随机分为A,B两组,制作颈动脉闭塞模型,气管切开插管。A组动物吸入100mL/LCO2,900mL/LO2混合气体,B组动物吸入100mL/LN2,900mL/LO2混合气体。使用激光多普勒血流成像仪测量额叶皮质血流量。实验动物缺血1h,再灌注30min后,分别测量乳酸含量和染缺血灶。结果:颈动脉闭塞后的局部脑血流量A组缺血30min时为(2.40±0.32)m/s,缺血60min时为(2.31±0.74)m/s,高于B组犤(2.21±0.26),(1.97±0.09)m/s犦;A组的乳酸水平犤(1.07±0.55)mmol/(L·g)低于B组(2.46±0.54)mmol/(L·g)犦;A组的未缺血区(0.43±0.28)cm2大于B组(0.28±0.10)cm2,P<0.05。结论:保持一定二氧化碳分压有利于缺血区的脑血流量维持和脑细胞自我保护,这为颈动脉缺血患者的临床治疗提供了一个很好的线索。
AIM:To observe whether a comparatively high arterial carbon dioxide pressur(PaCO2) would benefit regional cerebral blood flow(rCBF) in ischemic region and self-protection of brain tissue in carotid artery ischemia models of rabbits under anesthesia with the airway open. METHODS:Twenty Japan white rabbits were randomly divided into two groups: group A and group B.The rabbits in the group A inhaled mixed gas consisting of 100 mL/LCO2 and 900 mL/L O2;while those in the group B inhaled mixed gas of 100 mL/L N2 and 900 mL/L O2.The carotid artery occlusion models of rabbits were established with endotracheal intubation. rCBF in the cortex of frontal lobe was measured using laser Doppler perfusion imager,and then lactic content and focal ischemic region were detected after bilateral artery occlusion for 1 followed by 30 min reperfusion. RESULTS:After carotid artery occlusion,rCBF were 2.4±0.32 and 2.31±0.74 in the group A followed by ischemia for 30 min and 60 min respectively,markedly higher than in the group B(2.21±0.26, 1.97±0.09). The level of lactic acid was (1.07±0.55) mmol/(L·g) in the group A lower than in the group B [(2.46±0.54) mmol/(L·g)](P< 0.01). Smaller non-ischemic region existed in the group B [(0.28±0.10) cm2],compared with in the group A [(0.43±0.28) cm2](P< 0.05). CONCLUSION:S A comparatively high PaCO2 is of great benefit to maintaining rCBF in the ischemic region and the self-protection of brain tissue, which provides a good therapy for patients with carotid artery ischemia in clinic.
