摘要
目的 观察无结膜瓣的角膜缘切口 ECCE+IOL的临床疗效及其优、缺点。方法 将 16 7例 (175只眼 )白内障患者按手术切口分为两组行白内障囊外摘除人工晶体植入术 (以下简称 ECCE+IOL ) :A组作无结膜瓣的角膜缘切口 ,B组作常规角巩膜缘切口 ;分别比较两组术后 1周、1个月、3个月的视力恢复情况及角膜散光变化。结果 术后 1周、1个月、3个月 ,A组的平均裸眼视力或球镜矫正视力 (以下简称平均视力 )分别是 0 .32± 0 .15、0 .6 1± 0 .2 4、0 .72± 0 .32 ,平均角膜散光 (以下简称平均散光 )分别是 4 .5 2± 1.5 1、2 .99± 1.2 3、2 .4 6± 0 .84 ;B组的平均视力分别为 0 .30± 0 .14、0 .6 2± 0 .2 8、0 .71± 0 .34,平均散光分别是 4 .0 5± 1.4 2、2 .89± 1.33、2 .4 3± 0 .88。虽然术后 1周两组的平均散光比较差异有显著意义 ,但术后 1个月、3个月两组的平均散光以及术后 1周、1个月、3个月两组的平均视力比较差异均无显著意义。结论 无结膜瓣的角膜缘切口 ECCE+IOL与常规角巩膜缘切口 EC-CE+IOL的疗效相同 ,但前者具有操作简便、术中出血少。
Objective To observe the therapeutic effects,advantages and disadvantages of extracapsular cataract extraction with lens intraocular implantation (ECCE+IOL) throuth a corneal limbal incision of no-conjunctival flap.Methods 167 patients(175 eyes) were randomly divided into 2 groups:ECCE+IOL throuth a corneal limbal incision of no-conjunctival flap (group A),ECCE+IOL throuth a routine incision of corneoscleral limbus (group B).The postoperative visual acuity,the changes of corneal astigmatism were observed.Results At postoperative one week,one month,three months,the mean visual acuity with spherical correction or without correction in group A were 0.32±0.15,0.61±0.24,0.72±0.32 and that in group B were 0.30±0.14,0.62±0.28,0.71±0.34 respectively;the mean astigmatism in group A were 4.52±1.51,2.99±1.33,2.46±0.84 and that in group B were 4.05±1.42,2.89±1.33,2.43±0.88.Although there was significant difference between two groups in mean astigmatism at postoperative one week.There were no statistical difference between two groups in the mean acuity with spherical correction or without correction at postoperative one week,one month,three months and in the mean astigmatism at postoperative one month,three months.Conclusion ECCE+IOL throuth a corneal limbal incision of no-conjunctival flap is easy to perform,postoperatively,the reaction is mild,thus the duration for restoration is shortened.
