摘要
目的 探讨对影响原发性脑出血临床救治相关因素 ,提高其救治水平 ,最大限度的减少其致残率和死亡率。方法 回顾性分析本院神经内科 1999年元月～ 2 0 0 1年 10月 99例原发性脑出血住院患者临床资料及影响临床救治过程中的相关因素。结果 通过对生存组和死亡组临床资料分析 ,影响救治主要因素 :( 1)出血量多少 :99例脑出血患者出血量 >30 ml者 2 1例 ,死亡 18例 ,生存 3例 ,>10 0 ml者无 1例存活。 ( 2 )特殊并发症发生 :99例脑出血出现 12例呼吸衰竭 ,7例脑疝形成 ,全部死亡。 4 0例死亡组中 ,肾功能不全 2 3例 ,5 9例生存组中肾功能不全者仅 6例 ;死亡组中并发心功能不全、消化道出血各 7例 ,生存组各 3例 ;死亡组中出现高血糖症 2 3例 ,电解质紊乱 2 4例 ,生存组出现高血糖症 19例 ,电解质紊乱 18例。 ( 3)血压情况 :死亡组患高血压 39例 ,收缩压 180～2 80 mm Hg,平均 2 10 .5 mm Hg。舒张压 12 0～ 15 0 mm Hg15例 ,平均 12 6 .6 7mm Hg。结论 对原发性脑出血患者 ,积极的预防和及时的处理可能发生的各种特殊并发症 。
Objective To explore the influence factors of primary cerebral hemorrhage associated with treatment.Methods Jan.The clinic data of 99 cases of prinmary cerebral hemorrhage in patients from 1999.to Octo,2001 were retrospectively analys including the effect of related factors in clinic treatment.Results The important factors which influenced treatment were as follows:①amount of bleeding:among 21 cases whose amount of cerebral hemorrhage>30 ml,18 cases died and 3 cases survived.Patients whose bleeding amount >100 ml all died.②Especially complications:among 99 cases of cerebral hemorrhage,12 cases with respiratory failure and 7 cases with cerebral hernia all died.23 cases developed renal insufficience in 40 dead cases and there were only 6 cases with renel insufficience in 59 cases survival.Seven cases developed cardiac insufficience and 7 cases developed hemorrhage in digestice tract and 23 cases of hyperglycemia,24 cases of electrolyte disorder were found in dead group.Ninteen cases of hyperglycemia,18 cases of electrolyte disorder,each 3 cases of cardiac insufficience and hemorrhage of digestice tract were fome in survial group.③Blood preassure:systolic pressure in 39 cases of hypertension in dead group were 180～280 mmHg and 210.5 mmHg on average,diasolic pressure in 15 cases of 120～150 mmHg and 126.67 mmHg on average.Conclusion It is of significant for the emergency patients with primary cerebral hemorrhage to active precaution and instant treatment the possible occurred complications.
