血与尿液平均红细胞体积变化对血尿诊断的价值 被引量:2

摘要 ①目的 评价血、尿液平均红细胞体积变化对血尿诊断的价值。②方法 采用CoulterJT IR全血细胞分析仪测定 5 3例各种类型肾小球性疾病和 71例非肾小球性疾病病人的外周血红细胞平均体积 (MCVB)和尿中红细胞平均体积 (MCVU) ,以 16fL为界值判定血尿来源。③结果 以MCVB -MCVU的平均值诊断肾小球性疾病血尿分别为 98.1%和 97.2 % ,诊断非肾小球性疾病血尿的灵敏度和特异度分别为 97.2 %和 98.1%。肾小球性疾病病人MCVB -MCVU的平均值与非肾小球性疾病病人比较 ,差异有显著意义 (t=3 .760 ,P <0 .0 1)。④结论MCVB -MCVU的平均值在诊断血尿来源上 ,具有无损伤、客观、准确的优点 ,是一种简便。 Objective To evaluate the value of volumetric analysis of urinary and blood erythrocyte in localizing the source of hematuria. Methods Mean corpuscular volume in peripheral blood (MCVB) and mean corpuscular volume of urine erythrocyte (MCVU) of 124 patients with glomerular or nonglomerular hematuria were detected by Culter JT-IR whole blood analyzer. Then mean MCVB-MCVU was observed and calculated for two groups. With bounds of 16 fL, the specificity and sensitivity were calculated. Results Mean MCVB-MCVU was significantly different between glomerular and nonglomerular sources ( t= 3.760, P<0.01). With the bounds of 16 fL, specificity and sensitivity were 98.1% and 97.2% for glomerular and nonglomerular hematuria, respectively. Conclusion Mean MCVB-MCVU was a noninvasive, objective and accurate method to locate the sources of hematuria.
