摘要
本文基于黄元御《四圣心源》所述“中气”理论探讨针灸治疗桥本甲状腺炎的选穴思路,黄氏中气理论认为天人同气,中气决定了“一气周流”之气机运转,从“中气”理论出发分析该病病因病机,肝木抑郁为阻碍气机循环之关键,肾水虚寒、脾土湿困则中气升降无能,长此以往痰瘀内生为本病基本病机。临床选取人迎、气舍、天突改善局部血流;太冲、合谷、额三针疏肝调情志;阴陵泉、足三里、百会健脾升提;丰隆、筑宾、血海驱邪外出;艾灸神阙培本固元,取得较好临床疗效。
This article explored the thinking of acupuncture point selection for the treatment of Hashimoto's thyroiditis by acupuncture and moxibustion based on"middle qi"theory described in Huang Yuanyu's Si Sheng Xin Yuan.Huang's middle qi theory holds that humans and nature share the same qi,and that the middle qi determines the operation of the qi mechanism of the"circulation of one qi".Starting from the"middle qi"theory,the etiology and pathogenesis of this disease were analyzed.The depression of the liver is the key factor hindering the circulation of the qi mechanism.The kidney yang deficiency and cold coagulation as well as the dampness encumbering the spleen lead to the inability of the middle qi to ascend and descend.Over time,the internal generation of phlegm and blood stasis is the basic pathogenesis of this disease.Clinically,Renying,Qishe,and Tiantu were selected to improve the local blood flow.Taichong,Hegu,and the three needles on the forehead were used to soothe the liver and regulate emotions.Yinlingquan,Zusanli,and Baihui were used to invigorate the spleen and promote the lifting of qi.Fenglong,Zhubin,and Xuehai were used to expel pathogenic qi.Moxibustion at Shenque was used to strengthen the foundation and consolidate the origin,achieving good clinical efficacy.
