摘要
目的观察不同剂量环泊酚用于老年患者全身麻醉诱导的安全性和有效性。方法选取2022年3月至2023年3月河南省第二人民医院全麻手术的老年患者105例,按随机数字表法分为环泊酚低(0.2 mg/kg)、中(0.3 mg/kg)、高(0.4 mg/kg)剂量组各35例,比较麻醉不良反应、麻醉诱导时间、补救镇静率、以及不同时间点血流动力学和脑电双频指数(bispectral index,BIS)变化。结果低剂量组和高剂量组不良事件总发生率高于中剂量组;低剂量组和中剂量组低血压和心动过缓发生率低于高剂量组;中剂量组和高剂量组心动过速发生率和补救性镇静频率低于低剂量组。与T0相比,三组T1至T4 MAP和BIS值均降低,低剂量组全麻诱导后HR降低,气管插管后HR升高,T1至T4期间MAP低剂量组最高,高剂量组最低,HR低剂量组最高,高剂量组最低,中剂量组T1至T4 MAP和HR波动较小。结论0.3 mg/kg剂量的环泊酚诱导老年患者全身麻醉具有良好的安全性和有效性,不良事件发生率低,全麻诱导时血流动力学稳定。
Objective To observe the safety and effectiveness of different doses of ciprofol in inducing general anesthesia in elderly patients.Methods Totally 105 elderly patients undergoing general anesthesia surgery from March 2022 to March 2023 were randomly divided into low(0.2 mg/kg),medium(0.3 mg/kg),and high(0.4 mg/kg)dose ciprofol groups using a random number table.Anesthesia adverse reactions,anesthesia induction time,rescue sedation rate,and changes in hemodynamics and bispectral index(BIS)values at different time points were compared.Results The total incidence of adverse events in the low-dose and high-dose groups was higher than that in the medium dose group;The incidence of hypotension and bradycardia in low-dose and meadium dose groups was lower than that in high dose groups;The incidence of tachycardia and the frequency of remedial sedation in the meadium dose and high dose groups were lower than those in the low dose group.Compared with T0,the MAP and BIS values from T1 to T4 in the three groups decreased.HR in the low-dose group decreased significantly after induction of general anesthesia,and HR increased significantly after tracheal intubation.During the period from T1 to T4,MAP in the low dose group was the highest,while in the high dose group was the lowest;HR in the low-dose group was the highest,while in the high-dose group was the lowest;Fluctuations of MAP and HR were smaller in the medium-dose group.Conclusion The induction of general anesthesia with a dose of 0.3 mg/kg of ciprofol in elderly patients has good safety and effectiveness,low incidence of adverseevents and stable hemodynamics during general anesthesia induction .
