高危型人乳头瘤病毒感染患者阴道免疫微环境与疾病感染状况及疾病转归的关系探究

摘要 目的:探究高危型人乳头瘤病毒(HPV)感染患者阴道免疫微环境与疾病感染状况及疾病转归的关系。方法:选取2023年6月至2024年6月在本院接受治疗的79例高危型HPV患者作为高危组,同期选择HPV阴性组60人作为对照组。对照两组间阴道pH值、乳酸菌、细菌性阴道病(BV)、外阴阴道假丝酵母菌病(VVC)及白细胞介素(IL)-6情况。将高危组患者按疾病转归情况分为进展组(n=49)和无进展组(n=30),对比两组患者阴道pH值、乳酸菌、BV、VVC及IL-6、TNF-α、IFN-γ情况,采用多元线性回归分析或多元Logistic回归模型探讨影响疾病转归的独立因素。结果:与对照组相比,高危组pH值>4.4、乳酸菌阳性、BV阳性人数占比更多,IL-6水平更高。与无进展组患者相比,进展组患者pH值>4.4、乳酸菌阳性、BV阳性占比更多,IL-6水平更高。多因素Logistic回归分析表明,pH值、乳酸菌、BV及IL-6水平是高危型人乳头瘤病毒感染患者疾病转归的独立预测因素。ROC曲线分析结果显示,pH值、乳酸菌、BV及IL-6水平联合具有较高的诊断价值,AUC=0.865,敏感度为86.7%,特异度为77.6%。结论:高危型人乳头瘤病毒感染患者阴道免疫微环境与疾病感染状况及疾病转归存在紧密联系,临床应重视高危HPV感染阴道免疫微环境。 Objective:To explore the relationship between the vaginal immune microenvironment and disease infection status as well as disease progression in patients with high-risk human papillomavirus(HPV)infection.Methods:From June 2023 to June 2024,79 patients with high-risk human papillomavirus(HPV)treated at our hospital were selected to form the high-risk group,and 60 individuals with negative HPV test results during the same period were chosen as the control group.A comparison was conducted between the two groups regarding vaginal pH value,lactobacilli levels,bacterial vaginosis(BV),vulvovaginal candidiasis(VVC),and interleukin IL-6 levels.The high-risk group was further divided into a progression group(n=49)and a non-progression group(n=30)based on disease progression,and the vaginal pH value,lactobacillus,BV,VVC,and IL-6,TNF-α,IFN-γlevels were compared between the two groups.Multiple linear regression analysis or multiple logistic regression models were used to explore the independent factors influencing disease progression.Results:Compared with the control group,the high-risk group had a higher proportion of patients with a pH value>4.4,lactobacillus positivity,BV positivity,and higher IL-6 levels.Compared with the non-progression group,the progression group had a higher proportion of patients with a pH value>4.4,lactobacillus positivity,BV positivity,and higher IL-6 levels.Multivariate logistic regression analysis showed that pH value,lactobacillus,BV,and IL-6 levels were independent predictors of disease progression in patients with high-risk HPV infection.ROC curve analysis results showed that the combination of pH value,lactobacillus,BV,and IL-6 levels had high diagnostic value,with an AUC of 0.865,a sensitivity of 86.7%,and a specificity of 77.6%.Conclusion:There is a close relationship between the vaginal immune microenvironment and disease infection status as well as disease progression in patients with high-risk HPV infection.Clinical attention should be paid to the vaginal immune microenvironment in high-risk HPV infections.
