摘要
数字智能化(简称:数智化)诊疗正在由“盆景式”试点迈向临床流程标准化、普及化的关键时期。与此同时,在实现数智化为人类健康服务愿景的征程中,数智化诊疗技术在设备-创新驱动下一步步跨越学科交叉鸿沟,实现了新的跃迁。本文详细阐述并剖析了肝胆胰外科如何利用并联合医-理-数优势,实现构建个体化肝脏分段-智能导航-预后预测体系。结合笔者团队前期扎根医理数的经验与成果,成功牵头制定国内首部肝癌数智化诊疗七项团体标准及国际三维可视化共识,这为数智化诊疗技术标准化、同质化奠定了基础。面对多学科耦合难、人才荒、数据共享困难、区域发展失衡、数据壁垒林立、隐私安全、伦理问题和监管力度不足八大瓶颈,本文提出设备-技术-理念三同步、普及化设计、复合人才扩容、政策-资金双支持、标准-共享双运行、相关法律法规建立及推广六大突破策略,以构建“早筛-精诊-优治-智随访”全周期人工智能赋能体系为目标,为《健康中国2030》提供关键科技支撑,引领医学进入“智能-精准”新纪元。
Digital and intelligent diagnosis and treatment is currently transitioning from isolated"bonsai-style"pilot projects to a critical phase of clinical process standardization and widespread adoption.Meanwhile,on the journey toward realizing the vision of digital and intelligent technology serving human health,this field has progressively bridged interdisciplinary gaps,driven by equipment innovation,achieving new leaps forward.This article elaborates on and analyzes how hepatobiliary and pancreatic surgery leverages and integrates the strengths of medicine,basic science,and data science to establish a system encompassing individualized hepatic segmentation,intelligent navigation,and prognosis prediction.Building upon the deep experience and achievements of the author's team in grounding their work firmly in medicine,basic science,and data science,they have successfully led the development of China's first seven group standards for the digital and intelligent diagnosis and treatment of liver cancer,as well as an international consensus on three-dimensional visualization.This has laid the foundation for standardizing and homogenizing digital and intelligent diagnostic and therapeutic technologies.Facing eight major bottlenecks:difficulty in multi-disciplinary coupling,talent shortage,data sharing difficulties,unbalanced regional development,numerous data barriers,privacy and security,ethical issuesand insufficient regulatory efforts:this paper proposes six breakthrough strategies:synchronized advancement of equipment,technology,and concepts;universal design;expansion of interdisciplinary talent;dual support from policies and funding;parallel operation of standards and sharing mechanisms;and the establishment and promotion of relevant laws and regulations.Aiming to build a full-cycle AI-enabled system spanning"early screening,precise diagnosis,optimized treatment,and intelligent follow-up,"these strategies seek to provide crucial technological support for"Healthy China 2030"and lead medicine into a new era of"intelligence and precision."
