摘要
目的分析心力衰竭(HF)患者外周中心静脉导管(PICC)置管期间发生血液感染的危险因素及干预对策。方法以南阳医学高等专科学校第一附属医院2022年1月—2023年12月HF患者PICC置管期间发生血液感染的78例为观察组,并选取同期HF患者PICC置管期间无血液感染的78例为对照组。比较两组一般资料,分析HF患者PICC置管期间发生血液感染的相关危险因素,探讨干预对策。结果观察组年龄较对照组大,白细胞计数水平、异体输血占比、住院时长>14 d占比、低蛋白血症占比、置管时间>7 d占比、导管维护频率延迟占比较对照组高(P<0.05);Logistic回归分析显示,年龄大、白细胞计数升高、异体输血、低蛋白血症、置管时间>7 d、导管维护延迟均为HF患者PICC置管期间发生血液感染的危险因素(P<0.05);似然比χ2检验表明模型建立差异具有统计学意义,Waldχ2检验证实模型构建有效,拟合优度检验提示模型拟合效果较好。结论年龄、置管时间、异体输血等多种因素可对HF患者PICC置管期间发生血液感染产生影响,构建Logistic回归模型对HF患者PICC置管期间发生血液感染具有良好的预测价值,临床可据此实施针对性干预,促进康复。
Objective To analyze the risk factors and intervention measures of blood infection in patients with heart failure(HF)during peripherally inserted central catheter(PICC)catheterization.Methods 78 HF patients with blood infection during PICC catheterization in the First Affiliated Hospital of Nanyang Medical College from January 2022 to December 2023 were selected as the observation group,and 78 HF patients without blood infection during PICC catheterization in the same period were selected as the control group.The general data of the two groups were compared,and the related risk factors of blood infection in patients with HF during PICC catheterization were analyzed,and the intervention countermeasures were discussed.Results The age of the observation group was older than that of the control group,and white blood cell count,the proportion of allogeneic blood transfusion,the proportion of hospitalization time>14 days,the proportion of hypoproteinemia,the proportion of catheterization time>7 days,and the proportion of delayed catheter maintenance frequency in the observation group were higher than those in the control group(P<0.05).Logistic regression analysis showed that older age,increased white blood cell count,allogeneic blood transfusion,hypoproteinemia,catheterization time>7 days,and catheter maintenance delay were risk factors for blood infection during PICC catheterization in HF patients(P<0.05).The likelihood ratio chi-square test indicated that the differences in model establishment were statistically significant,the Wald chi-square test confirmed that the model construction was effective,and the goodness-of-fit test suggested that the model fitting effect was good.Conclusion Age,catheterization time,allogeneic blood transfusion and other factors can affect the occurrence of blood infection in patients with HF during PICC catheterization.The construction of Logistic regression model has good predictive value for blood infection in patients with HF during PICC catheterization,which can be used to implement targeted intervention and promote rehabilitation.
