摘要
报道弹性纤维假黄瘤1例。患者男,53岁,因面部、颈部泛发淡黄色丘疹4年就诊。无瘙痒、疼痛等不适。其他系统检查无明显异常。皮肤科专科检查:双侧面颊、下颌部及颈部上段可见皮纹增粗,密集分布淡黄色丘疹,沿皮纹分布,大小不一,边界清晰,部分融合增大。触之质软、光滑,皮损呈“拔毛鸡皮样”改变,未见明显皮肤松弛、下垂。右下颌皮损组织病理苏木精-伊红染色示:真皮浅、中层组织疏松,可见较多的粗大、卷曲的嗜伊红纤维,周围可见淋巴细胞、组织细胞浸润;Weigert弹力纤维染色示:真皮浅、中层可见肿胀、断裂的弹力纤维,呈碎羊毛状。诊断结果为弹性纤维假黄瘤。
One case of pseudoxanthoma elasticum(PXE)is reported.A 53-year-old man presented with a 4-year history of asymptomatic yellowish papules on the face and neck.Systemic examination revealed no significant abnormalities.Dermatologic evaluation showed bilateral accentuation of cutaneous folds on the cheeks,jawline,and upper neck,with densely distributed,sharply demarcated yellow papules of varying sizes aligned along skin folds;some papules had coalesced and enlarged.The lesions were soft and smooth,with a characteristic"plucked-chicken skin"appearance,without appreciable cutaneous laxity.Histopathologic examination(HE)of a biopsy from the right mandibular region demonstrated loosening of the superficial to mid dermis with coarse,curled eosinophilic fibers,accompanied by lymphocytic and histiocytic infiltrates.Weigert elastic fiber staining revealed swollen,fragmented elastic fibers with a"crushed wool"appearance in the superficial and mid dermis.These findings were consistent with a diagnosis of PXE.
