摘要
目的探讨特发性矮小症(idiopathic short stature,ISS)女患儿血清胰岛素样生长因子-1(insulin-like growth factor-1,IGF-1)与体格发育、生殖系统超声表现的相关性。方法选取2020年6月至2023年10月宜春市人民医院收治的60例ISS女患儿作为ISS组,另选取同期60名健康女童作为健康组。两组均行血清IGF-1检测、体格发育评估及生殖系统超声检查。比较两组血清IGF-1水平、体格发育[身高、体质量、体重指数(body mass index,BMI)、骨龄年龄差(bone age difference,BAD)、骨龄指数(bone age index,BAI)]情况及生殖系统超声表现(子宫体积、卵巢体积),Pearson相关性分析ISS患儿血清IGF-1水平与体格发育指标、生殖系统超声指标的关系。结果ISS组IGF-1水平、BMI、体质量、BAI、BAD均低于健康组,身高矮于健康组,子宫体积、卵巢体积均大于健康组,差异有统计学意义(P<0.05)。Pearson相关性分析结果显示,ISS女患儿血清IGF-1水平与身高、体质量、BMI、BAI、BAD均呈正相关(r>0,P<0.05);与子宫体积、卵巢体积均呈负相关(r<0,P<0.05)。结论ISS女患儿血清IGF-1水平显著低于健康女童,其与体格发育指标呈正相关,与生殖系统超声表现呈负相关,可作为ISS患儿诊断及疗效评估的参考依据。
Objective To investigate the correlation between serum insulin-like growth factor-1(IGF-1)and physical development and ultrasonic manifestations of reproductive system in girls with idiopathic short stature(ISS).Methods 60 ISS girls admitted to Yichun People's Hospital from June 2020 to October 2023 were selected as the ISS group,and 60 healthy girls in the same period were selected as the healthy group.Serum IGF-1 detection,physical development evaluation and reproductive system ultrasonography were performed in both groups.The serum IGF-1 level,physical development(height,weight,body mass index[BMI],bone age difference[BAD],bone age index[BAI])and reproductive system ultrasonography(uterine volume,ovarian volume)were compared between the two groups,Pearson correlation analysis was used to analyze the correlation between serum IGF-1 level and physical development index and reproductive system ultrasonography index in girls with ISS.Results The IGF-1 levels,BMI,body weight,BAI and BAD in the ISS group were lower than those in the healthy control group,height was shorter than that in the healthy control group,and the uterine volume and ovarian volume were greater than those in the healthy group,the differences were statistically significant(P<0.05).Pearson correlation analysis showed that the level of serum IGF-1 of ISS girls was positively correlated with height,weight,BMI,BAI and BAD(r>0,P<0.05),and negatively correlated with uterine volume and ovarian volume(r<0,P<0.05).Conclusion The serum IGF-1 level of ISS girls is significantly lower than that of healthy girls,which is positively correlated with physical development indexes and negatively correlated with the ultrasonic manifestations of reproductive system,it can be used as a reference for the diagnosis and efficacy evaluation of ISS.
